Discussion thread VII - COVID-19: Endemic Stage

>gs I would argue that the premise in both of the linked papers are
>interesting but inconclusive. I think the pre-Omicron variants are
>just pre-existing variants which were not fit enough to become
>dominant until the mass vaccination campaign, and weight of prior
>infections, changed the population immune profile to their advantage.
>The vaccines became available at the end of 2020 and ramped up
>through 2021. Alpha showed up at about the same time and was globally
>dominant from March to June 2021 and Delta dominated from July to Dec.
>The other VOC were much more regional and never gained global dominance
>(see https://covariants.org/per-variant ).

but how likely is it that one of the rare ancient viruses acquires mutations
and becomes dominant rather than one of the widespread alrady more
transmissable viruses ?

>They each had a little under 25 NS mutations about 10 in S and the rest
>distributed across the other proteins (relative to the ancestral strain)
>however Alpha has only 3 S and 1 other change from its direct ancestor.
>Omicron is quantitatively different having 36 NS in S and a further 26
>elsewhere it is also throwing up a lot of minor variants
>plus being phylogenitically rooted in an older dormant lineage

alpha : 7*S+ 6*sy+ 4*o1+4*N+3=24
beta : 7*S+ 3*sy+ 4*o1+1*N+3=18
gamma : 2*S+ 7*sy+ 8*o1+3*N+2=22
delta : 7*S+ 3*sy+10*o1+4*N+4=28
BA.1 : 25*S+ 7*sy+ 7*o1+3*N+4=46
BA.1.1: 26*S+ 8*sy+ 7*o1+3*N+4=48
BA.2 : 27*S+12*sy+12*o1+4*N+5=60
BA.3 : 21*S+ 9*sy+ 7*o1+4*N+4=45

>all of which make me think it is not just a variant that has been
>undetected while it slowly morphed into its epidemic form but gained them
>rapidly in an immunocompromised host or in an un-sampled non human population.
>The rate of change, now that it is in a normal human immune environment,
>would imply rapid adjustment to that system.

probably not just one host, there is diversity, see BA.2 ,
Other than with alpha. Well, for delta we had also 2 sublineages,
but closer.

>re. your earlier question "The problem was mainly Wuhan and Hubei,
>in other provinces it was quickly controlled. So, why couldn'r we
>do that ?"
>We probably could, in practical terms, but it was not politically
>possible. Look at the current Canadian situation and all they are
>being asked to do is get vaccinated not having entire cities locked
>own tight and guarded to enforce compliance.

so it's our political system ...but USA could react to Pearl Harbour,
or 9/11 it's motivation. We need emergency laws that enables the government
to react in a crisis quickly.

"but how likely is it that one of the rare ancient viruses acquires mutations

I do not know as we do not have data to go on but I would guess it is low probability which is why I suspect a different mechanism for Omicron. There are a number of possible reasons/confounding factors.

As you know exactly where you add a new sequence into a phylogenetic tree is dependent on the algorithm used and becomes increasingly dodgy with large jumps in the sequence so the current rooting may just be the best fit to the previously sequenced samples and not the actual ancestry.

This pandemic has occurred at a time where the technology has advanced to a point that we can sequence and observe change in a way we could not before so we have never been able to observe evolution in real time like this previously. In addition medical technology has allowed unprecedented numbers of immunocompromised individuals to live amongst us who would not have survived had they caught HIV or needed a transplant in 1918.

The enormous number of viral replications occurring when you have daily new cases measured in millions makes the mutation permutations staggering. This also makes me wonder about molecular clock calculations which are showing very high rates for SARS-CoV-2 but how much of this is due to the exceptionally high propagation rate in the middle of a pandemic wave rather than an intrinsic property of an endemic virus?

Re. The BA1/2 difference I suspect one source but two introductions.

Re. The political problem. I think the answer is probably in education of the public. The data is clear enough but we are not acting on it. Is this due to people not being able to understand relative risk calculations?
In terms of US deaths Pearl Harbour 2k, WW2 418k, 9/11 3k, 1918 flu 675k, COVID 918k and counting. It is fairly apparent where the danger lies but this is not reflected in the preparedness investment.

I would recommend this Mayo clinic video which concisely lays out the US data.

I think he is wrong in his comment about the flu vaccine H1N1 component and disagree with part of his take on routes to endemicity but otherwise I agree - some of his conclusions will not go down well with the anti-mandaters.

Liberal MP Joël Lightbound’s full remarks: ‘It's time to choose positive, not coercive methods’

...
Now looking beyond the demonstrations, I’ve heard from hundreds of constituents and citizens who took the time to reach out to me these past weeks, folks who have nothing to do with these demonstrations, who are for the most part vaccinated, who have done everything as they should these last few years. People who have shared with me legitimate concerns about where we are heading collectively.

I think we must hear these concerns, and I think we must respond to them.

I’ve heard from parents worried to see their kids sink into depression and slowly lose their joy of living. I’ve heard from pediatricians in tears telling me about their young patients’ despair, anxiety, isolation, as well as the stunning increase in school dropouts they are observing. I’ve heard from artists are on the brink of mental and financial collapse after two years, barely enough work to get by. I’ve heard from social workers answering suicide hotlines who are overwhelmed by the number of calls they’re receiving. I’ve heard from entrepreneurs and restaurant owners who are contemplating losing but they’ve spent their whole lives building — that’s when it hasn’t happened already.

I’ve heard from fellow Quebecers who are rightfully appalled that in our province 2022 we’re locking up triple vaxxed seniors for days on end. I’ve heard from fellow Quebecers appalled that in Quebec, January 2022, we have locked up kids aged six to 10 years old for up to 10 days in windowless rooms. Kids who tested negative who had no symptoms, who had been in contact with someone, though, who had the virus.

So let me repeat in Quebec in 2022 we locked up kids aged six to 10 for up to 10 days in windowless rooms.

Let that sink in. This was a public-health measure that had been drafted, approved and applied. I’ve heard from people worried that we seem collectively to have forgotten the population’s health is kind of like a pie, and Omicron is but a slice of that pie. Economic health, social health, mental health must be accounted for.

I’ve heard from people worried that those making the decisions seem at times to have been blind to the fact that we’re not all equal for lockdowns that not everyone can earn a living on a Macbook at the cottage. I’ve heard people worried that a few might have lost sight of the quiet and discreet suffering of the many.

I’ve heard people in great pain to see some of their friends whom they love and respect but who’ve decided, for whatever reason, we might very well disagree with, who’ve decided not to get vaccinated and as a consequence, are jobless, selling what they have and moving to the United States, away from their communities, away from their friends and from their families.

I’ve heard from teachers worried to see kids reenacting in the schoolyard the kind of discrimination and segregation we see in our society between vaccinated and unvaccinated. I’ve heard from people worried to see those they care about fighting each other on this issue, tearing some families and some friends apart.

I’ve heard from a lot of people wondering why just a year ago, we were all united, in this together. And now that we have one of the most vaccinated population in the world, we’ve never been so divided. Now these people are increasingly confused, when on the other hand, they hear experts like Dr. Karl Weiss, a renowned Quebec epidemiologist, say last week and I quote “that COVID-19 is here to stay that those at risk will have to be monitored closely when symptoms appear; that we will have to protect our health-care systems; but then we will also have to live with the virus like we do with influenza; that we can’t go back to lockdowns and restrictions not supported by science.”

They’re confused when they hear Dr. (Theresa) Tam (Canada’s chief public health officer) state last week that all existing public-health policies, including vaccine passports, need to be re examined, and that we need to have longer-term sustained approaches and capacity building so we’re not in crisis mode all the time as we fight this virus.

They’re confused when they see the undeniable trend around the world whereby, for instance, the World Health Organization recently recommended dropping or alleviating many border measures, including vaccine requirements, as they’ve proven to be ineffective in fighting the propagation of the Omicron variant. That’s the World Health Organization.

They’re confused when they see countries around the world like Ireland, Sweden, Norway, (inaudible), the Czech Republic, Brazil, Denmark, Spain and the U.K., who have either dropped almost all restrictions or fast moving in that direction. All countries, I note, with lower vaccination rates than us.

Now while folks are hearing and seeing all of this, they’re left rightfully wondering, ‘where the hell are we heading here in Canada?’

I think there lies the frustration. They feel there is no appetite from our government to adapt so as to reflect the changing data and the changing reality of the pandemic of the world. They’re worried that measures which ought to be exceptional and limited in time are being normalized with no end in sight, like vaccine passports, mandates and requirements for travellers.

They’re worried because they feel it is becoming harder and harder to know where public health stops and where politics begins.

Now, I firmly believe governments would do well not to dismiss these legitimate concerns, not to demonize those who voice them.

Public Health Agency of Canada

Remarks from the Minister of Health on COVID-19, Feb 11, 2022

From: Public Health Agency of Canada
Speech

February 11, 2022 | Ottawa, ON | Public Health Agency of Canada

First, I would like to address some of the events that have marked the news this week.

A few days ago, my colleague Joël Lighbound held a press conference in this very room.

I have a lot of friendship, esteem and affection for Joël.

We regularly discuss many issues, including the management of the pandemic.

In a democracy, it is healthy for everyone to be able to express their opinions.

Debate and discussion should be encouraged.

We all know people are eager to be done with the pandemic.

We're all eager to be done with it.

Like Joël, I hear it at home, in Québec City and here, in Ottawa.

We also know that in order to get out of the pandemic phase of COVID, we must continue to work together, in a spirit of benevolence, by joining our forces against the virus.

This is true for vaccination and it is true for adherence to all the public health measures that are still in place.

For two years now, our government’s actions in the fight against COVID-19 have been based on prudence and science.

As we all know, science is continuously evolving and as we have done since the beginning of this pandemic, we will continue to base our decisions on the best available scientific information.

Now let’s be clear: the public health measures currently in place were always meant to be temporary.

Some will be with us for a long time, others will be lifted as soon as the epidemiological situation and science allow it.

That said, it is important to keep in mind that these measures are intended to limit the harm caused by the virus.

All measures are subject to constant re-evaluation and we will continue to adjust them according to science and the evolution of the epidemiological situation.

With the worst of Omicron now behind us, our government is actively reviewing the measures in place at our borders and we should be able to communicate changes on this very soon.

We would all like to know for sure what the next few weeks and months have in store for us, but after two years of this pandemic, we know that this virus will always find a way to surprise us.

We know that the question is not whether there will be other waves or other variants, but rather: when?

We all agree that we have to learn to live with the virus, but concretely, what does it mean to live with the virus?

Let me humbly suggest an answer to this question.

Firstly, I would say that living with the virus means that people have to stop dying from it in such large numbers.

Living with the virus means that the virus and our healthcare system must be able to coexist without the latter being paralyzed.

Without cancer patients being told that their surgeries will have to wait because our hospitals and our healthcare workers are overwhelmed by COVID.

Living with the virus means that we must – individually and collectively – maximize the use of the tools at our disposal to limit its transmission and limit the severity of infections.

Once transmission is under control and the severity of infections is limited, our healthcare system will be able to breathe again and most public health measures can be relaxed.

We are not there yet, but that day is fast approaching.

We must not give up now.

There is hope.

Living with the virus also means that we must be lucid and recognize that it is not only possible, but very likely that the coming months will hold other unpleasant surprises.

Living with the virus means that we must remember that we are not powerless in this situation.

We have power, we can help ourselves, and we must help ourselves.

We have the power to make it so that the public health restrictions are less severe and disrupt our social life and our economy as little as possible.

Our main tool for that remains – still and always: vaccination.

Even if we know that vaccination only partially prevents transmission of the Omicron variant, the science is unequivocal: vaccination DOES prevents severe complications.

If we can prevent severe complications, our hospitals and healthcare workers will not be overwhelmed.

If our hospitals and healthcare workers are not overwhelmed, then in turn, public health restrictions will be less severe.

It is obvious, but it is important to remember it: we have – individually and collectively – the power to change things.

Living with COVID means that we exercise this power.

That’s the only way to reduce the impact of the virus on our daily lives.

Now that the worst of Omicron is behind us, living with the virus means that we should not wait for the next variant or the next wave to restart our vaccination campaigns.

We must continue to administer as many doses as possible, including the first, second and booster doses, so that when the next wave or next variant comes, we are better prepared and more resilient.

50% of eligible Canadians have received their booster dose.

We must continue.

Together, we can do this.

Trudeau plans to invoke Emergencies Act in response to protests: sources

Act grants cabinet ability to take 'special temporary measures that may not be appropriate in normal times'

Catharine Tunney · CBC News · Posted: Feb 14, 2022 7:54 AM ET | Last Updated: 40 minutes ago

Prime Minister Justin Trudeau has told his caucus he will invoke the never-before-used Emergencies Act to give the federal government extra powers to handle anti-vaccine mandate protests across the country, sources say.

Those sources, who were not authorized to speak publicly, said the prime minister informed the premiers of his decision this morning.

The Emergencies Act, which replaced the War Measures Act in the 1980s, defines a national emergency as a temporary "urgent and critical situation" that "seriously endangers the lives, health or safety of Canadians and is of such proportions or nature as to exceed the capacity or authority of a province to deal with it."

Mounties seize guns and arrest 11 protesters at border blockade in Alberta

RCMP confiscate firearms, body armour and a machete

The Canadian Press · Posted: Feb 14, 2022 11:21 AM MT | Last Updated: 26 minutes ago

Alberta RCMP have arrested 11 people after searching three trailers and finding weapons at a protest against pandemic restrictions near the main U.S. border crossing in southern Alberta.

Mounties said in a release Monday that they became aware of a small organized group within the larger protest at Coutts.

They say they had information that the group had access to a cache of firearms and ammunition. Officers seized long guns, handguns, multiple sets of body armour, a machete, a large quantity of ammunition and high-capacity firearm magazines.

... RCMP say a large farm tractor and a semi truck attempted to ram a police vehicle on Sunday evening, providing "an example of the militant mindset of a small segment of the protest," the release said.

Officers were able to get out of the way and avoid a collision.

The driver of the tractor was identified and officers are working to locate him and arrest him, RCMP said.

... Police say a small organized group within the protest was said to have a "willingness to use force against the police if any attempts were made to disrupt the blockade."

Canada's Trudeau invokes emergency powers in bid to end protests - First Time Ever

https://www.reuters.com/world/americ...ed-2022-02-14/

-----------------------------------------------

The Emergencies Act outlines four different types of emergencies: public welfare emergencies, public order emergencies, international emergencies and war emergencies. If the legislation is invoked this week, it will likely be under the 'public order' category. Again, the criteria here is strict - lawful protests do not qualify.

Instead, the situation must be considered a threat to the security of Canada, as defined by the Canadian Security Intelligence Service Act. This law outlines four possible scenarios:
  • Espionage or sabotage
  • Foreign-influenced activities
  • Threats or use of acts of serious violence for political, religious or ideological objectives
  • Covert, unlawful acts intended to undermine or overthrow the constitutionally established government

Since this is the discussion forum I will offer my personal and unscientific observations:

1) Male, 50s, got Delta as vaccines were rolling out (Spring 2020). Had a serious, but treatable, at home case. Received J&J vaccine a few months later. No known side effects to vaccine. Got Omicron in Winter 2021. Very slight case. Not treated. Test positive.

2) Female, wife of above case, 50s, same exact description as case #1 above. Test positive.

3) Male, 20s, got Delta in Summer 2020. Very slight case. Not treated. Received an RNA vaccine in Summer 2021. No known vaccine side effects. Not boosted. Got Omicron in Winter 2021 and was seriously ill for about a week. Self treated at home. Test positive.

4) Male, 50s got Delta in Summer 2020. Medium case. Received RNA vaccine in 2021. No known side effects. Close exposure to case 3 above. Booster status unknown. Either had no COVID or an asymptomatic case as a result of exposure. Test positive.

5) Female, 50s, suspected slight Delta case in Fall 2020. Received RNA vaccine in Fall 2021. Reported fatigue side effect. Not eligible for booster at onset of suspected Omicron. Was medium sick. Headache, sore throat, extreme fatigue. Recovered without treatment in a week.

6) Female, 20s, got Delta in Fall 2020. Was medium sick. Typical symptoms. Recovered without treatment. Received RNA vaccine in Winter 2020. No known side effects. Booster status unknown. Got suspected Omicron Winter 2021. Was medium sick for about a week. Recovered without treatment. Test positive for Delta event.

7) Female, 20s, in household of case 6. Also medium sick with different symptoms. Recovered without treatment. Received RNA vaccine in Fall 2020. No known side effects. Booster status unknown. Suspected Omicron case Winter 2021. Was medium sick for a few days. Recovered without treatment. Test positive.

8) Female, 80s, fully RNA vaccinated plus booster and has not contracted COVID-19 (unless was asymptomatic) despite being in two households with test confirmed cases. Has been wearing N95 masks and limits social contacts. No known side effects of vaccine. Added: Wife of case #17 who died.

9) Female, 80s, fully RNA vaccinated plus booster and has not contracted COVID-19 (unless was asymptomatic). Has been wearing N95 and surgical masks. Limits social contacts. No known side effects of vaccine.

10) Male, 40, school worker with extensive contact to students. Fully RNA vaccinated plus booster. No known side effects to vaccine. Has not contracted COVID-19. Wears at least an N95 mask at work. Sometimes adds a surgical mask to cover a vented N95 mask. Some social limits.

11) Female, 30s, fully RNA vaccinated plus booster. No known side effects to vaccine. Has not contracted COVID-19. Wears N95 mask. Some social limits.

12) Male, 30s, fully RNA vaccinated plus booster. Sore muscle and injection site. Has not contracted COVID-19. Wears N95 mask. Limits social contacts.

13) Female, 60s, fully RNA vaccinated plus booster. Fatigue after each initial shot pairing. Has not contracted COVID-19. Wears N95 mask. Limits social contacts.

14) Male, 60s. fully RNA vaccinated plus booster. No known side effects to vaccine. Has not contracted COVID-19. Wears N95 mask. Limits social contacts.

15) Male, 60s, suspected Delta in Spring 2020 before vaccines available. Slight case. Headache, fatigue, fever. Recovered in a week with home treatment. Fully RNA vaccinated plus booster. No known side effects. No re-occurrence of COVID-19. Many co-morbidities. Wears N95 mask. Limits social contacts.

16) Female, 70s, health care worker working hospital shifts throughout pandemic. Fully RNA vaccinated plus booster. No known side effects. Has not contracted COVID-19. Wears at least N95 mask. Limits social contacts.

17) Male, 80s, never vaccinated. Kidney disease. Hospitalized and died in Fall 2020. Mask use is unknown. Test positive. Added: Husband of case #8.

Observation limits - Asymptomatic cases may have occurred but are unknown. All of the cases above are generally healthy but cases 13 and 14 are at much higher risk for negative COVID-19 outcome.

My analysis of the small observation group above:

1) COVID-19 vaccines are not generating a large amount of side effects in generally healthy people.

2) COVID-19 vaccines may be somewhat protective against hospitalization in known cases.

3) COVID-19 vaccines are not preventing infection.

4) Of the cases without a history of COVID-19 illness - all wear N95 masks.

5) Of the cases without a history of COVID-19 illness - all limit social contacts outside of work requirements.

Also, I did not state this above but anyone in the 80s age range is at high risk for a negative COVID-19 illness outcome.

In the USA, at least, because of FDIC insurance of banks and the fiat currency system, any dollar deposited in an FDIC-insured bank is exactly as safe (and as unsafe) as that dollar in your wallet or mattress. The thing behind it is the full faith and credit of the US government. If the banking system fails, it would take out the FDIC and the value of the dollar, and if the dollar loses its value, it would take down the banking system as well. The two are completely linked.

Attempting to get away from the banking system would also force you to use an alternative currency.

Under pressure some governments are relaxing restrictions for people who are fully vaccinated.

Please remember that people who are vaccinated can still get COVID-19. They may be able to spread it.

Being vaccinated only helps lessen disease severity for the person who is vaccinated. A room of fully vaccinated people does not guarantee anything.

You still need to take of yourself.

Alert, I'm not worried about my bank failing right now. FDIC does have limits, though. https://www.fdic.gov/resources/deposit-insurance/brochures/deposits-at-a-glance/
Not sure how it would all play out in a systemic failure.
What concerns me is the expanding definition of 'terrorist financing' and the government freezing bank accounts. I think you could stockpile US dollars. How to do that safely is the question. US dollars are what I would need living here.

Most of us don't have nearly enough money to worry about FDIC limits; even if you don't do anything unusual, up to $250,000 per person PER BANK is insured. If you really have enough money to worry about that limit, you can just use several different banks or different titling (i.e. beneficiaries) on the accounts at the same bank.

The problem with holding dollars in your mattress (besides the other issues such a robbery risk and civil asset forfeiture issues) is that if the banking system collapses, causing the FDIC to have to step in everywhere, the resulting effective printing of dollars would render the ones you're holding worthless. The crypto people say that they have the solution, but that carries its own risks, which are probably even worse.

I never even thought of civil asset forfeiture until you mentioned that. I have read about abuses of that system, though.

The home invasion scenario is my bugaboo. That's why I wouldn't want a safe. I'd end up tortured because I couldn't remember the combo.

I am asking again that people do not make any political posts. We are not a political group. If this persists I will have to move these types of posts to the archive.

As to the Canada situation - the posts should be from the aspect of de-stabilizing society. This is a real threat to public health. It is not important who the political leaders are specifically. Please keep your comments to the situation and its relationship to public health and violations of human rights.

As an example - I have seen some Canadian legacy media reports about the Ottawa protest today. It appears the police have established a secured area which they have blocked off. Instead of making this move in the mid-week when there are less protestors, they have made a bad choice to take heavy police action on a weekend. Despite the snow storm, it appears many protestors are again going to Ottawa. The legacy media continues paint the protestors as "fringe" and uses other various derogatory words. I am not going to link to the legacy media because I think their reporting of this event is very slanted to the point of being "fake news".

I wish the Canadian people the very best and this includes all police entities too.

Canada is a paradise. Don't ruin it.

Take care everyone.

Covid-19: an explosion of reinfection cases since December 2021

ParisMatch |Published on 02/18/2022 at 3:45 p.m.
Yannick Vely

In its weekly report, Public Health France comes back precisely to reinfections with Covid-19, the number of which has exploded since December 2021.

The numbers are unequivocal. Since December 2021, there has been an explosion of cases of Covid-19 reinfectionsin France. According to Public Health France (SpF), while 416,995 possible cases of reinfection were identified between March 2, 2021 and January 27, 2022, 384,375 have been identified since December 1, 2021, or 92% of the total. And that's mainly due to Omicron: 79% of possible cases of reinfection for which a screening result was available and interpretable for the reinfection episode had a result suggestive of this variant Nearly 3800 people presented three or more episodes. the two episodes of infection was 240 days on average, with a median of 263 days, explains SpF.Also according to the same source, possible cases of reinfection represented 2.8% of all confirmed cases of Covid 19 between March 2, 2021 and January 27, 2022. Until December 5, 2021 (i.e. before the emergence and very rapid dissemination of the Omicron variant in France), this proportion was 0.8%. This proportion increased sharply from December 6, 2021, rising to 3.6% of all confirmed cases of Covid-19, also explains Public Health France.

A majority of asymptomatic reinfection
51.4% of possible cases of reinfection were asymptomatic during the second episode of Covid-19, SpF analysis, which specifies that a clear overrepresentation of health professionals and young adults (18-40 years old) is observed among the possible cases of reinfection, compared to all confirmed cases detected during the study period. Several factors may be involved in this overrepresentation of these two populations, in particular overexposure to infection by Covid-19 due to professional activity, less adherence to barrier measures and social distancing or even a lower vaccination coverage among 18-40 year olds compared to older populations during the study period.
