Discussion - Thoughts on a global outbreak of monkeypox

A reply to that thread indicates this is very mild. Probably better to have than herpes, since herpes is a forever disease that can cause serious disability or death.

An ID doc told me it’s like the very beginning of Covid when the process for testing was long and burdensome (reports to CDC, paperwork etc.). Also, cases they are seeing are mild with one or two lesions, therefore often mistaken for herpes.

If 2021_MD means the Maryland case from 2021, it had travel history to Nigeria and resulted in no onward transmission in the USA. No word on what animal or human cases it might have had contact with in Nigeria.

The Maryland Department of Health, in collaboration with the U.S. Centers for Disease Control and Prevention (CDC), today confirmed a single case of monkeypox virus infection in a Maryland resident who recently returned from Nigeria. The individual presented with mild symptoms, is currently recovering in isolation and is not hospitalized.

and as I expected is an independent import from Nigeria. It is of interest as it form a separate branch intermediary between the non-African cluster in 2018 and the current outbreak and implies evolution of this branch going on in West Africa. Now we need to find out if this is a new host species forcing faster evolution and if that species is us. It would be useful to know where in Nigeria both this case and the recent UK case were before they traveled.

As global monkeypox cases continue to rise, public-health officials and researchers are questioning whether the current outbreaks can be contained. The World Health Organization has said that the situation is unlikely to escalate into a full-blown pandemic. But more than 1,000 people have now been confirmed to have been infected with the virus in nearly 30 countries where outbreaks do not usually occur (see ‘Unusual spread’).

But there are unknowns and challenges that come with using this strategy for monkeypox, says Natalie Dean, a biostatistician at Emory University in Atlanta, Georgia. Although the vaccines are considered to be safe and effective for use in people with smallpox infection, the vaccines have had limited testing against monkeypox. The strategy also relies on highly rigorous contact tracing, which might not be implemented in every country, and people must also agree to be inoculated with vaccines that can carry rare, but serious, side effects.

Ring vaccination can be a powerful tool, Dean says, but to be effective it needs to be used early — while case numbers are still manageable. “As the numbers crop up and you think about the number of contacts each individual has, the logistics just become more complicated,” she says, adding that there is a narrowing window of opportunity to prevent the virus from gaining a more permanent foothold in human or animal populations in countries where the global outbreaks are occurring.

At the moment, the risk posed by monkeypox to the general public isn’t high enough to warrant mass-vaccination, given the side effects and availability issues, says Daniel Bausch, the director of emerging threats and global health security at the Foundation for Innovative New Diagnostics in Geneva, Switzerland. But if the virus starts spreading in vulnerable populations such as pregnant people or children, or if it turns out to have a higher fatality rate than expected, that risk-benefit calculation could change.

Genetic data suggest multiple monkeypox outbreaks, CDC says

“As the monkeypox outbreak has continued, our laboratory scientists have been working to better understand how the U.S. cases might be linked to [cases in] Europe or other places,” Jennifer McQuiston, DVM, MS, deputy director of the CDC’s Division of High Consequence Pathogens and Pathology, said during a press briefing. “New analysis resulted in sequencing uploaded publicly today showing that in the U.S., we have seen at least two genetically distinct variants circulating.”

As of Friday, McQuiston said 21 cases have been confirmed in the U.S. The cases are all of the West African clade of monkeypox, which is the less severe of the two clades.

According to McQuiston, most U.S. cases have been linked to the large outbreak that began in Europe. Those cases are also similar to one of the imported U.S. cases reported in 2021 through a traveler from Nigeria, who ended up in Maryland.

“It’s been circulating since that time at least,” she said.


Two other monkeypox cases sequenced were similar to another imported case in 2021 that was reported in Texas. McQuiston explained that these cases were reported in a woman who had traveled to West Africa and a man who traveled to the Middle East and Africa. She added that these cases are all similar to each other but are not linked. In contrast, the ones associated with Europe are “very tightly linked to each other.”

“It’s not clear what all of this means, but it’s likely that within the last couple of years, there have been at least instances where monkeypox virus spilled over to people in Nigeria from the animal that maintains it and that virus likely began to spread [from] person to person through close contact and possibly sexual contact,” she said.

The Texas case, along with two others in VA & FL, cluster more closely with a number of 2017 Nigerian cases all from River State (south central coastal region) and are not on the tree I posted previously but just off it and can been seen on the larger tree at the link in post #72. Both branches seem to be active in Nigeria and well adapted to humans.
The 2018/19 sequences from Israel, Singapore and the UK cluster with a Beyelsa State sequence (adjacent to and just West of River State).

What is the situation with monkeypox?

There have been monkeypox cases identified in several countries throughout and beyond Europe. Although anyone can become infected with monkeypox, most cases are currently being detected in gay and bisexual men and other men who have sex with men. Information about this outbreak of monkeypox is still emerging and we are continuously learning more. Stay up-to-date and stick to reliable sources of information (see section ‘Stay calm and stay informed’ below).

How is monkeypox passed on?

Monkeypox can be transmitted through close physical contact (skin-to-skin), including during sex. It can also spread through respiratory droplets in prolonged face-to-face contact, and through contaminated objects (such as clothing, bedding and sex toys).

Common symptoms of monkeypox and what to do if you develop symptoms

Monkeypox can take the form of rashes, spots, ulcers or blisters anywhere on the body, but often in the genital area. It may also cause swollen and painful lymph glands, fever, headache and muscle aches; chills or exhaustion. Most of those affected by the current outbreak will probably just need a few weeks of recovery at home, but a few people have been hospitalised due to their monkeypox symptoms or subsequent skin infections. ...

Monkeypox is not a ‘gay disease’ and gay men are not to be blamed or shamed for the outbreak. Stigma is counterproductive. Monkeypox is nothing to be ashamed of and is not associated with a specific sexual orientation. Do not let stigma prevent you from accessing the healthcare you need. You can help by combatting stigma, sharing information, paying attention to symptoms, and if diagnosed, by following the recommendations of your healthcare provider (e.g. by self-isolating and helping health authorities to notify close contacts). ...

... By early June, the family physician and his colleagues had treated around 15 patients with confirmed infections, out of the roughly 100 lab-confirmed cases reported so far in Quebec. People with suspected infections soon started showing up to the clinic on a near-daily basis.

Cloutier-Blais began to notice some interesting trends.

The pox lesions, he found, weren't presenting exactly like what he'd seen in photos circulating online of people infected in parts of Africa, where the virus had been found for decades.

"Lesions are much smaller and usually very much localized," he said, adding there's also a "very wide spectrum of different kinds of presentations."

In some cases, lesions are showing up on or inside various bodily areas, including patients' mouths, genitals or anal region, sometimes spreading to the limbs or torso or popping up across the entire body.

But for other patients, visible symptoms have been far more subtle — even just a single mark on the skin.

Monkeypox Testing Shows the US Learned Little From the Covid-19 Pandemic

(Bloomberg) -- US testing for monkeypox is insufficient to determine how widespread the virus is and where new cases are cropping up, according to infectious disease experts and advocates concerned about a sluggish response to the outbreak that’s already hit 32 countries.

While government labs have the capacity to test as many as 8,000 samples a week, they’re only using 2% of that capability, suggesting that about 23 monkeypox tests are being performed a day, said James Krellenstein, the cofounder of PrEP4All, an HIV advocacy group that widened its focus during the pandemic. Much more testing is needed to find out where the pathogen is and how fast it’s moving, he said.

The situation echoes the early days of Covid-19, when flawed tests from the Centers for Disease Control and Prevention allowed the pandemic to spread undetected in the US, Krellenstein said.

“It’s concerning that the CDC isn’t pushing for this themselves, especially after Covid,” he said in an interview. “This is a critical period: Outbreaks become harder to control as time goes on.”

Testing is so limited that it’s impossible to tell how much of the virus circulating in the US is being picked up, said Ranu Dhillon, an infectious disease doctor at Harvard Medical School and Brigham and Women’s Hospital in Boston. It could be 90% or just 10%, he said.

Carl Martel was infected with monkeypox. After days of hell and hospitalization, he decided to grant an interview to try to eliminate the taboos surrounding this virus.

It all started when Mr. Martel realized he had a small pimple on his face. Thinking it was just a bump, he decided to press it and put some rubbing alcohol on it.

"The next morning, I had four pimples," recalls Mr. Martel. He went to a pharmacy, where he was reassured and given an antibiotic ointment.

It was a friend of his who put the flea in his ear that it could perhaps be monkeypox.

Mr. Martel therefore went to the Clinique l'Actuel in Montreal to see a doctor. Two specialists confirmed to him that it was indeed the virus.

Worse and worse

A few days after his diagnosis, Mr. Martel went to the hospital because his condition had deteriorated. They promptly returned him home, telling him that the situation would resolve itself.

Seeing that his condition was not taken seriously, he went to the CHUM, where he was quickly admitted and placed in isolation.

His condition declined dramatically.

“In the space of 2-3 days, I was equipped, I had them everywhere. I was on solute for days without being able to eat,” he explains. He was in pain.

“It burns us in the face, it's like pustules and it ends like a volcano,” recalls Mr. Martel.

He admits to having feared for his life, because in addition to the simian pox, he had a streptococcus in the throat, an abscess and swollen tonsils. The rest of his body was also dotted with pustules.

“I was looking for my air, I was running out of air, I was stuck because of pimples in my nose,” he recalls. Doctors had to drain the pus that was blocking her throat and nose.

Doctors offered him treatment for monkeypox, and his condition began to improve.

“I have often been told that I was one of the worst cases. I know what I have endured,” emphasizes Carl Martel.

He was hospitalized for 12 days, in addition to 6 days at home recovering from medication.

He believes that it is a man met on a dating site who transmitted the virus to him, but he is not able to be absolutely certain.

Raise awarness

Carl Martel admits to being afraid that monkeypox, which mostly strikes men who have sex with other men, will further stigmatize the community.

For him, it was important to grant an interview to prevent other people from living the same nightmare as him.

"I don't even want my worst enemy to go through what I went through. It's atrocious the suffering, it's 24 hours a day, ”he summarizes.

He encourages people who are particularly at risk of contracting monkeypox to get vaccinated.

“There is a vaccine, go for it! Go get it, don't be afraid,” he says.

Carl Martel is adamant he will now be more careful with the people he meets, and he wants his story to resonate with other people.

“I hope it will make you think,” he wishes.

It's good of him to share that he was unusually vulnerable to monkeypox. I wish he had a broader range of suggestions than getting the smallpox vaccine. Earlier antiviral treatment than he had might help. Might be a good time for older men to catch up on recommended cancer screening, too. The fist sign of advanced cancer can be increased susceptability to infectious diseases.

The paradigm shift in monkeypox epidemiology

Why do we have outbreaks in multiple countries (more than 23 countries at last count)? Why do these cases occur, mainly outside endemic West and Central Africa? Was the monkeypox disease spreading undetected in the west, and could this explain why, so far, none of the current cases has been traced to an endemic country? Why is there a higher incidence in gay and bisexual populations? Answers to these questions and many more will help epidemiologists ,infectious disease experts and clinicians find a solution to a suspected shift in the epidemiology of the disease.

The natural reservoir of monkeypox remains unknown. However, African rodents and non-human primates (such as monkeys) can harbor the virus and infect humans3. For many years, monkeypox was endemic only in Africa, until the first case outside of Africa was reported in 2003ref.4,5. Since then, there have been pockets of intermittent outbreaks in America, Europe, and Asia, most of which can be traced back to African origins, either through travel or importing animals6. The latest outbreak, however, is not following the same pattern – with epidemiologists suspecting transfer to people who have not been to Africa.

A large number of these infections have occurred in bisexuals and men who have sex with men6, suggesting the possibility of sexual transmission. This suggestion cannot be dismissed, even if it does not support the already known epidemiology of the disease. This may involve new knowledge and previously unknown facts. Examples of emerging and re-emerging infectious diseases that were later found to be potentially sexually transmitted were the Zika and Ebola virus diseases. These viruses were later isolated from semen, vaginal fluids, and breast milk to confirm the possibility of their sexual and postnatal mother-to-child transmission8-12.

With the ongoing outbreaks in non-endemic countries, evidence suggesting sexual transmission, and incidents outside of endemic geographic areas, such as those seen recently in Nigeria, there may be an ongoing change in monkeypox epidemiology.

The first case of monkeypox in a human was reported in 1970 in the Democratic Republic of the Congo. Since then, there have been many monkeypox outbreaks, but they have been self-limiting, with chains of human transmission ending without establishing epidemics. The current outbreak, however, is different. There is more human-to-human transmission, and it is over a much broader geographical area.

In mere weeks, monkeypox has spread to 37 non-endemic countries, with over 2,600 cases. So, what is likely to happen to monkeypox in the following weeks and months?

There are huge gaps in what we know about monkeypox, but combining what we do know with a history of other infectious diseases makes it possible to analyse likely future scenarios.

The four scenarios below are based on the following knowledge: the average number of people an infected person is likely to infect (assuming they have not been vaccinated against the virus or have had the disease before) is 2.13. This is called the basic reproduction number, or R. Herd immunity – the point at which enough people have immunity such that disease transmission can’t be sustained – is 53% (corresponding to this value of R). And the incubation period, the time from catching the virus to the appearance of symptoms, is between five and 21 days.

Scenario 1: Self-limiting outbreak

The 2022 epidemic appears to have started as a super-spreader event involving a network of predominantly men who have sex with men.

But until the current outbreak it was assumed that the relatively low human-to-human transmissibility of the virus makes it unlikely for the virus to spread outside the initial community.

In this scenario, the outbreak ends quickly once the population at risk becomes immune and herd immunity is reached locally. In the past, many people had some immunity (called “cross-immunity”) from the smallpox mass vaccination programmes of the late 20th century. So the effective reproduction number, R, can be close to or even lower than one, and the transmission will soon stop.

Behavioural changes can reduce the number R even more. For example, the ring vaccination can form a “firebreak”, further reducing the susceptible population. Similar previous epidemics include the Sars outbreak in 2002-04, when a quick intervention stopped the disease from spreading.


Scenario 2: All population

The continuing spread of monkeypox in May and June 2022 suggests that the virus is moving beyond the original network.

The size of the outbreak is already well beyond the most prominent 2017-19 outbreak in the Democratic Republic of Congo (760). It is possible that large gatherings, including raves and festivals, have created new transmission clusters.

Scenario 2 assumes that everybody below the age of 50 is susceptible to the infection, reflecting the end of compulsory smallpox vaccination in the 1970-80s. The virus will continue spreading, effectively searching for pockets of high-risk and non-immune communities.

Unless a combination of contact tracing and ring vaccination stops the spread, the monkeypox will continue spreading. But, given the low transmissibility of monkeypox, the epidemic may fizzle out before reaching the herd immunity threshold of 50% of the population.


Scenario 3: Becoming endemic

Complete eradication is impossible because monkeypox exists in a wide range of animal hosts. The low transmissibility also means it can survive at low levels in the population. In addition, the long incubation period and variable symptoms allow it to avoid detection. Therefore, monkeypox may have already been spreading for a long time.

In scenario 3, following the large outbreak, the disease will settle on a long-term, relatively constant level. Similar to the pre-vaccination smallpox or chickenpox.

The influx of susceptible people through birth or migration will keep the virus in the population. Mass vaccination programmes might be needed to eradicate the disease. But relatively low monkeypox transmissibility means such programmes are likely to be highly effective.


Scenario 4: Recurrent large epidemics

The current epidemic might be the first instance of a series of outbreaks. In the long term (scenario 4), we should expect a return of monkeypox caused by future “zoonotic events” where the disease jumps from animal hosts to humans. As the cross-immunity from smallpox vaccines wanes, the epidemics can become even more substantial.

Little is known about the potential of monkeypox to mutate. Still, there is potential for it to evolve into a more rapidly spreading variant.

Effective vaccines for monkeypox exist and are about 85% effective. Although there are currently not enough doses to vaccinate everyone, there is no need for a mass-vaccination programme given monkeypox’s low transmissibility. Instead, vaccines should be offered to those most at risk, including communities in Africa most in contact with the wild animals that carry the virus.

In late May, not long after he'd gotten through a mild bout of COVID-19, Peter Kelly spiked a sudden fever. He quickly realized it wasn't the tail end of a COVID infection.

Over a period of several days, the Toronto resident became exhausted, and his muscles began to ache. His temperature oscillated between chills and night sweats. Then, strange sores started to appear on different parts of his body — eventually around two dozen that he could see, mostly on his legs, and painful ones hidden from view in his rectum.

As a professional dancer, Kelly is used to pain. He's been injured a lot — most recently, a broken rib that's still healing — and has serious eczema, a skin condition that can cause an itchy or burning sensation.

But Kelly had never experienced something as excruciating as the unexplained lesions emerging on sensitive areas of his body.

"This was on another level," he later recalled. "You can't control it. It feels like razor blades in a way, shocking you constantly."

What followed was nearly a month of tests, three emergency room trips, one infected sore, and finally, a lab-confirmed diagnosis: Monkeypox virus or, as it's known throughout the scientific community, MPXV...

U.S. CDC confirms evidence of local monkeypox transmission

June 23 (Reuters) - The U.S. Centers for Disease Control and Prevention (CDC) said there was evidence of local transmission of monkeypox, in addition to reports of cases where people had traveled abroad.

The cases are mainly occurring in men who have sex with men, but women are also getting infected, CDC staff member Dr. Agam Rao said at a panel meeting on Thursday.

The CDC said had also been reports of transmission among family members and close contacts.

"We also heard worldwide about close contacts like close household members who through example shared bedding, towels, have acquired infection," Rao said. "So it is not just through close intimate contact that this is being spread."
