Experts have warned doctors and parents of dengue fever patients to be careful not to assume a drastic decrease in fever indicates recovery, because it could be the calm before the real storm.

The shock syndrome occurs because the blood's plasma liquid seeps through the blood vessels and spreads into the whole body, leaving the blood cells to coagulate inside the vessels.

Psychologist Tika Bisono, who in July lost her 10-year-old daughter Janika to dengue fever, said, "We must change our assumptions regarding high fever decline. Sometimes it means recovery. Other times, it can mean otherwise."

Alan said most people still think death will come only to patients experiencing massive bleeding, particularly intestinal bleeding, indicated by the dark color of their feces.

"This is incorrect. Some 30 percent of dengue patients do not show even the slightest sign of mild bleeding in their skin pores and nose," he said.

Both Alan and Tika spoke at a media conference organized by pharmaceutical firm GlaxoSmithKline, which produces a fever relief drug under the label Panadol.

Alan said it was imperative for dengue patients with a fever of 40 degrees Celsius to drink up to two liters of water a day to recharge their body fluids.

"Taking a fever relief drug is an option when the fever reaches as high as 38.5 degrees Celsius, or if the patient has a history of convulsions due to high fever."

However, he emphasized that easing a dengue patient's fever was not the same as curing them.

Alan exhorted people to be more careful in choosing safe medicines for relieving fever.

GlaxoSmithKline said some fever relievers contain drug substances, like ibuprofen and acetylsalicylic acid, which have side effects that can harm the patient by decreasing blood platelets or causing stomach bleeding.

"Dengue fever is still unpredictable, and we need to constantly be alert to each of its symptoms by counting the days," Alan said.

"We must not only observe the level of platelets in the blood, but most importantly, and unfortunately sometimes overlooked in diagnosis, is to observe the hematocrit or measures of the proportion of blood volume that is occupied by red blood cells."

In cases of dengue fever, in which a full blood count is performed daily, a high hematocrit count indicates an increased risk of dengue shock syndrome.

Tika said both doctors and patients needed to understand how to read laboratory results.

"Sad to say, not all doctors understand how to read the lab results. Caretakers of dengue patients must proactively ask when they don't understand how to read the results."

The highest number of dengue fever cases reported in the capital this year was in April, with 5,133.
