Secretary for Food and Health  confirmed on Hong Kong's first human infection with avian influenza A (H7N9) cases
***************************** *
The following is the Secretary for Food and Health KO presided yesterday (December 2) night Influenza Pandemic Contingency Plan Serious Response Level Steering Committee met with the press conference transcript:

Reporter: Are there risks of large-scale community outbreak?

Secretary for Food and Health: For this case, we certainly still need further investigation, for example for people who have close contact with the patient (check). I just said that some of them (household contacts) mild symptoms, we will quickly test for these people as soon as possible.Centre for Health Protection before further investigation, we do not quite determine the risk to human transmission of the current extent, being only one of these cases. As I said earlier, because the patient had a clear history that ever went to Shenzhen, slaughtering chickens, so it may be an imported case.

Reporter: Before Dongguan farm H7 virus found, in fact, at present the patient had traveled to Shenzhen, but only suspended three farms of chickens to Hong Kong, Shenzhen, is it enough? Other chickens from the Mainland market situation is normal? Patients had to Shenzhen which markets?

Secretary for Food and Health: the moment we have yet been able to grasp clearly the information displayed in Shenzhen, where the patient had been to this reason, we pause for chickens to Hong Kong, Shenzhen within three games of chickens to Hong Kong is actually a preventive measure.

Reporter: (about tracking contacts with patients who have a condition)

Secretary for Food and Health: I would like to add some, I hope you understand that we have today (December 2) 8:00 pm - 9:00 when diagnosed (the case ), but we very much hope that the transparency of information as far as possible, so we quickly hold this meeting and then announce to you as soon as possible. But in this case, I hope you understand that some of you are asking the question, we may not be able to reply immediately. Just ask some questions about the center CHP will be a lot of public health and epidemiological investigations, for example, another in Shenzhen, together with the patient whereabouts of persons; we need to identify the patient in the past several days have contacts who have visited the place, (and prepare) list. We can tell you that when there is further information, we'll inform you as soon as possible.

Reporter: Which patients visited private clinics?

Secretary for Food and Health: As I mentioned earlier, in fact, initially (patients) may not suffer from the disease of consciousness is so serious, so Xianxiang private doctor. The private clinics, as well as patients who had contact with the patient, but also CHP who need to make some contacts to track.

Reporter: Who is that four family members?

Secretary for Food and Health: temporarily only say that they are members of the family (patient services), until we grasp the details and then tell you.

Reporter: What are their symptoms?

Secretary for Food and Health: upper respiratory system symptoms.

Reporter: (patient of that name) friend is Indonesian nationals or Hong Kong?

Secretary for Food and Health: As I explained earlier, we are still looking for some information, if you need detailed information on every aspect, we may have to wait for further investigation, the outcome will be announced as soon as possible.

Reporter: Why are they the first two tests were negative, symptoms will appear hidden problem?

Secretary for Food and Health: Not so, any pathology, especially bacteria or virology samples, we can not say that all the samples taken from the patient will certainly be positive, because the patient will depend on the stage of illness, and then his possession the amount of virus, etc., so this is a normal situation, not necessarily each test can check out.

Reporter: After the patients had symptoms for some time been to different clinics, and even in hospital, but did not find that there may be some risks, such as health care workers infected?

Secretary for Food and Health: Contact Center CHP conducted tracking included everyone, that is, this time, from the patient before disease onset or her contact with people who have been to places she visited, including clinics and hospitals that may come into contact with people, we also need to be included in the list of contact tracing. In this Incidentally, do not rule out that some people worry that such patients over this period of time to which hospital or clinic, some people may be left at that time went to those places, will have a chance (of infection). If they want to get further information, we will arrange hotline colleagues will later provide to us (hotline) number. If the public on the matter in doubt, we will provide a hotline, when he provided us with information, we will contact with him, and for him to analyze the situation.

Reporter: (circumstances of the patients)

Secretary for Food and Health: I have already made it clear that the patient is in critical condition.

Reporter: (circumstances of the patients)

Secretary for Food and Health: I have said very clearly in my earlier speech, the patient appeared in the November 27 shortness of breath, and then admitted to Tuen Mun Hospital. I believe that the information provided to you tonight, I hope you understand that we want to let everyone know that there is some information as soon as possible, and we really need, whether clinical or epidemiological aspects may also want to investigate further. the CHP and the hospital has a lot of work behind the scenes, if more further news, we come to that announcement. I believe the explanation today, we have tried to be exhaustive.

Reporter: her family has revealed why she suddenly to Shenzhen?

Secretary for Food and Health: Unknown.

Reporter: At present three stop live chickens to Hong Kong farms, the number of chickens to Hong Kong's chicken stalls input?

Secretary for Food and Health: I believe that these data can be provided to you later.

Reporter: epidemiological tracking needs to be long?

Secretary for Food and Health: This is a question we can not answer immediately, how many days you want, depending on how many people had contact with the patient, because if she had contact with many people, we also track longer required. 2 December 2013

Preparedness Plan for Influenza Pandemic

Clip:
The Government’s Response Systems - Serious Response Level

Serious Response Level
9. Serious Response Level corresponds to a situation where the risk of a novel influenza virus causing new and serious impact to human health in Hong Kong is moderate. Generally, it depicts a moderate risk of serious human infections caused by the novel influenza virus in Hong Kong, but serious infections are not expected to be widespread in the short term. Illustrative scenarios are shown in Box 2.

Box 2: Illustrative scenarios of Serious Response Level Examples of scenarios under this level include the following –
(a) confirmation of HPAI outbreaks in Hong Kong in the environment of oramong poultry population in retail markets, the Cheung Sha WanTemporary Wholesale Poultry Market (the Wholesale Poultry Market) orpoultry farms in Hong Kong, due to a strain with known human health impact;

(b) confirmation of outbreak of novel influenza in Hong Kong among otherfood animals in farms, due to a strain known to have caused human infection with serious health impact; or

(c) confirmation of sporadic or small clusters of human case(s) of HPAI ornovel influenza, known to have caused human infection with serious health outcome in Hong Kong.

Under these scenarios, human-to-human transmission has not been sufficient to sustain community level outbreaks.
Other examples include the confirmation of a novel influenza virus, capable of efficient human-to-human transmission, spreading in an area that has insignificant trade and travel relationship with Hong Kong; or a sporadic or small cluster of human case(s) of novel influenza that may have been historically known to be of mild nature but is capable of efficient human-to-human transmission in Hong Kong.

Re: Hong Kong confirms first human case of H7N9 (RTHK, December 2 2013)

Despite the 'surprise' transpiring from media outlets after the confirmation of this birdflu case, it worths remember that Guangdong recorded human cases as well recently and the spillover in Hong Kong was quite anticipated by this spring events analysis performed by ECDC and other health agencies.

Since H7N9 is currently circulating in poultry in China, other human cases, clusters are likely to emerge in the next weeks in Mainland and neighboring countries.

Without tracing poultry outbreaks it is impossible to avoid human cases and to slow virus evolution toward an efficient and sustained transmission in community. Further reassortments of internal genes are also possible as the seasonal winter outbreaks are coming to northern hemisphere.

Only a strong commitment of all stakeholders in China and neighboring regions may alter the course of the events. GM
