Second confirmed human case of avian influenza A(H7N9) in Hong Kong under CHP investigation

The patient will be transferred to Princess Margaret Hospital for isolation. His current condition is stable.

The Centre for Health Protection (CHP) of the Department of Health (DH) today (December 8) provided an update on the second confirmed human case of avian influenza A(H7N9) in Hong Kong.

"The epidemiological investigation, enhanced disease surveillance, port health measures and health education are proceeding," a spokesman for the DH remarked.

The 80-year-old male patient lives in Fu Yong Xin He District in Shenzhen.

As of 4pm today, no additional close contacts have been identified.

The number of close contacts remains at 19 while there are over 140 other contacts.

Details are as follows:

• (A) The 19 close contacts remain under quarantine for 10 days since their last contact with the patient.
  • Their specimens all tested negative for the avian influenza A(H7N9) virus upon preliminary laboratory testing by the Public Health Laboratory Services Branch of the CHP.
  • They were prescribed with the antiviral Tamiflu prophylaxis.
  • During isolation, if their health conditions change, further testing and surveillance will be conducted.
  • They include:
    • 1. Thirteen patients who had stayed in the same cubicle with the confirmed patient in Tuen Mun Hospital (TMH).
      • One of them has newly developed fever and another specimen collected from the patient today tested negative for the avian influenza A(H7N9) virus;
    • 2. His five family members; and
    • 3. The taxi driver who had taken the patient and his family members from Shenzhen Bay Port Border Control Point (SBP BCP) to TMH on December 3.
• (B) Over 140 other contacts, including other relatives of the patient, health-care workers (HCWs) of TMH and the ambulance service, relevant hospital visitors as well as an immigration control officer at SBP BCP, have been put under medical surveillance and offered Tamiflu prophylaxis.
  • Among them, one HCW of TMH newly developed non-specific symptoms whose specimen collected today has been tested negative for the avian influenza A(H7N9) virus.

Regarding the first confirmed case affecting the 36-year-old female patient, as of 4pm today, no additional close contacts of this case have been identified.

The number of close contacts remains at 17 while there are now 230 other contacts under medical surveillance.

Of the 17 close contacts, 11 have completed quarantine and will continue to be put under medical surveillance for 10 more days.

The remaining six close contacts remain under quarantine for 10 days since their last contact with the patient.

During isolation, if their health conditions change, further testing and surveillance will be conducted.

The Lady MacLehose Holiday Village in Sai Kung under the Leisure and Cultural Services Department has been converted as quarantine centre. Currently, seven asymptomatic close contacts of the second case are under quarantine in the centre.

The epidemiological investigation into the two cases by the CHP, including contact tracing and tracing the source of infection, is ongoing.

Locally, enhanced surveillance over suspected cases in public and private hospitals is under way. The CHP will continue to maintain liaison with the World Health Organization (WHO), the Mainland and overseas health authorities to monitor the latest developments and obtain timely and accurate information. Local surveillance activities will be modified according to the WHO's recommendations.

"All border control points (BCPs) have implemented disease prevention and control measures. Thermal imaging systems are in place at BCPs for body temperature checks of inbound travellers. The DH has liaised with the Auxiliary Medical Service and the Civil Aid Service to deploy additional manpower at BCPs to conduct random temperature checks using handheld devices. Suspected cases will be immediately referred to public hospitals for follow-up investigation," the spokesman said.

Regarding health education for travellers at BCPs, the distribution of health education pamphlets, display of posters on avian influenza A(H7N9) in departure and arrival halls, in-flight public announcements, environmental health inspection and the provision of regular updates to travel industries via meetings and correspondence have all been escalated. The DH will keep a close eye on the latest developments and adopt corresponding port health measures.

"We have enhanced our publicity and health education on the prevention of avian influenza. The CHP has also sent letters to government departments and related organisations to reinforce our health advice on the prevention of avian influenza," the spokesman added.

The CHP hotline (2125 1111) has been set up for public enquiries. As of 4pm today, 72 calls had been received.

"Travellers, especially those returning from avian influenza A(H7N9)-affected areas and provinces, with fever or respiratory symptoms are reminded to immediately wear facial masks, seek medical attention and reveal their travel history to doctors. Health-care professionals should also pay special attention to patients who might have had contact with birds, poultry or their droppings in affected areas and provinces," the spokesman advised.

The spokesman also urged travellers not to visit live poultry markets in the affected areas and provinces and avoid direct contact with poultry, birds and their droppings. If contact has been made, they should thoroughly wash their hands with soap and water.

Members of the public should remain vigilant and are reminded to take heed of the following preventive advice against avian influenza:

• Poultry and eggs should be thoroughly cooked before eating;
• Wash hands frequently with soap, especially before touching the mouth, nose or eyes, handling food or eating; after going to the toilet or touching public installations or equipment such as escalator handrails, elevator control panels or door knobs; or when hands are dirtied by respiratory secretions after coughing or sneezing;
• Cover the nose and mouth while sneezing or coughing, and hold the spit with a tissue and put it into a covered dustbin;
• Avoid crowded places and contact with fever patients; and
• Wear a mask when respiratory symptoms develop or when taking care of fever patients.

"The epidemiological investigations, enhanced disease surveillance, port health measures and health education are proceeding," a spokesman for the DH remarked.

The male patient aged 80 living in Fu Yong Xin He District in Shenzhen.

As of 4pm today, no additional close contacts have been identified.

The number of close contacts remains at 19 while there are over 140 other contacts.

Details are as follows:

• (A) The 19 close contacts remain under quarantine for ten days since their last contact with the patient.
  • Their specimens all tested negative for the avian influenza A(H7N9) virus upon preliminary laboratory testing by the Public Health Laboratory Services Branch (PHLSB) of the CHP.
  • They were prescribed with the antiviral Tamiflu for prophylaxis.
  • During isolation, if their health conditions change, further testing and surveillance will be conducted.
    • They include:
      • 1. Thirteen patients who had stayed in the same cubicle with the confirmed patient in Tuen Mun Hospital (TMH);
      • 2. His five family members; and
      • 3. The taxi driver who had taken the patient and his family members from Shenzhen Bay Port Border Control Point (SBP BCP) to TMH on December 3.
• (B) Over 140 other contacts, including other relatives of the patient, healthcare workers (HCWs) of TMH and the ambulance service, relevant hospital visitors as well as an immigration control officer at SBP BCP, have been put under medical surveillance and offered Tamiflu prophylaxis.
  • Among them, one HCW of TMH newly developed non-specific symptoms whose specimen collected today has been tested negative for the avian influenza A(H7N9) virus by the PHLSB.

Regarding the first confirmed case affecting a female patient aged 36, as of 4pm today, no additional close contacts have been identified.

The number of close contacts remains at 17 while there are now 230 other contacts under medical surveillance.

Of the 17 close contacts, 12 have completed quarantine and will be continued to be put under medical surveillance for ten more days.

The remaining five close contacts remain under quarantine for ten days since their last contact with the patient.

During isolation, if their health conditions change, further testing and surveillance will be conducted.

Among the 230 other contacts, one HCW of the Queen Mary Hospital newly developed non-specific symptoms whose specimen collected today has been tested negative for the avian influenza A(H7N9) virus by the PHLSB.

In addition, a client of Dr Wong Chun-yan whom the patient consulted on November 26 presented with non-specific symptoms today and has been referred to Princess Margaret Hospital (PMH) for isolation treatment. Specimen will be collected and sent for testing.

Lady MacLehose Holiday Village in Sai Kung under the Leisure and Cultural Services Department has been converted as a quarantine centre.

Currently, six asymptomatic close contacts of the second case are under quarantine.

An asymptomatic close contact of the same case who has been under quarantine in the centre has been transferred to PMH for isolation due to other health problems.

The epidemiological investigations into the two cases by the CHP, including contact tracing, are ongoing.

Locally, enhanced surveillance over suspected cases in public and private hospitals is underway. The CHP will continue to maintain liaison with the World Health Organization (WHO), the Mainland and overseas health authorities to monitor the latest developments and obtain timely and accurate information. Steering Committee on Preparedness Plan for Influenza Pandemic holds meeting


The Secretary for Food and Health, Dr Ko Wing-man, today (December 9) convened a meeting of the Steering Committee on Serious Response Level under the Preparedness Plan for Influenza Pandemic to discuss and follow up on the latest development of the two confirmed human cases of avian influenza A(H7N9) in Hong Kong and relevant contingency measures with the departments concerned.

The Hospital Authority (HA) briefed the Steering Committee on the condition of the two patients.

The 36-year-old female patient, who was confirmed of being infected with avian influenza A(H7N9) on December 2, is still under quarantine treatment at the intensive care unit of the Queen Mary Hospital.

Her condition has improved from "critical" to "serious" last night.

She no longer has fever and does not need the Extracorporeal Membrane Oxygenation machine for respiration.

As for the case confirmed on December 6 involving an 80-year-old male patient, the patient is in stable condition with slight fever. He will continue to receive treatment under isolation in the Princess Margaret Hospital and further testing will be conducted.

Epidemiological investigation by the Centre for Health Protection (CHP) of the Department of Health is still proceeding, including tracing close contacts and the source of infection.

Regarding the 36-year-old female patient, the number of close contacts remains at 17. Twelve of them completed quarantine. Among these 12 close contacts, 11 have returned home and are under medical surveillance for another 10 days, while one remains in hospital for treatment of personal health problems. The remaining five close contacts remain under quarantine for 10 days.

Counting from the day when they had last contact with the patient, today is the last day of their quarantine. Meanwhile, there are about 230 other contacts under medical surveillance.

For the 80-year-old male patient, the specimens of his 19 close contacts are all tested negative for the avian influenza A(H7N9) virus.

They will be put under quarantine for 10 days since their last contact with the patient. About 140 other contacts are under medical surveillance.

Lady MacLehose Holiday Village in Sai Kung under the Leisure and Cultural Services Department (LCSD) is still being used as a quarantine centre. Currently, six asymptomatic close contacts of the second case are under quarantine in the centre.

According to the latest information available, the Steering Committee maintains their view that the two cases are imported sporadic cases.

In addition to notifying the relevant Mainland authorities of the two confirmed cases, CHP and HA sent officers to visit Shenzhen this morning to meet with officials from the Health and Family Planning Commission and the Center for Disease Control and Prevention of Guangdong Province and Shenzhen City respectively, and clinical experts, etc.. Both sides exchanged the latest findings on the epidemiological investigations of the two confirmed cases.

According to the current investigation result, the patient of the first confirmed case might have visited a residential unit in Nanwan Subdistrict of Longgang District in Shenzhen and handled a live chicken. However, the patient was unable to speak clearly the source of the chicken.

For the second case, family members of the patient bought and cooked a slaughtered chicken from a market in Fuyong Town in Shenzhen. However, the patient did not visit live poultry market nor had contact with live poultry directly.

The Mainland authorities have started the relevant investigations and taken samples for testing.

So far, no positive testing results were found.

Both Guangdong and Hong Kong are of the view that the source of infection of the two cases is yet to be confirmed. Both sides have agreed to maintain close liaison and continue to exchange the latest information.

With regard to imported live poultry, Hong Kong has suspended import of live poultry from the registered poultry farms in Shenzhen since December 3 as an urgent and special precautionary measure. The suspension will remain in effect. Once more information on the source of infection comes to light, the Government will review and adjust the arrangement with the Shenzhen inspection and quarantine authority according to the latest situation.

Regarding local live poultry retail outlets, poultry farms and wholesale market, the Food and Environmental Hygiene Department (FEHD) and the Agriculture, Fisheries and Conservation Department (AFCD) will continue to conduct inspections, step up cleansing and disinfection efforts as well as collect samples from poultry and wild bird environment for testing of avian influenza. The two departments have been working and co-operating closely with the Customs and Excise Department to check travellers' baggage at boundary control points with the assistance of detector dogs to combat illegal import of live poultry/poultry meat into Hong Kong. The Government will also enhance publicity and education programmes to remind the public not to import live poultry/ poultry meat illegally.

The Government will continue to closely monitor the situation and maintain transparency in disseminating information to the public. Relevant departments will stay vigilant to ensure various contingency measures are implemented properly and make adjustments in a timely manner.

The Government reminds the public again to maintain good personal and environmental hygiene; wash hands frequently; and avoid contact with wild birds and animals. While travelling abroad, the public should avoid contact with birds and poultry to minimise the risk of infections.

The meeting today comprises representatives from CHP of the Department of Health, HA, FEHD, AFCD, the Education Bureau, the Commerce and Economic Development Bureau, Tourism Commission, the Home Affairs Department, the Information Services Department, the Social Welfare Department and the LCSD.

Two people are negative to the H7N9 virus. The protection center continues the epidemiological survey of these two diagnosis cases, including tracing contact. Human infection with avian influenza A(H7N9) virus ? update

On 28 November and 6 December 2013, the National Health and Family Planning Commission, China notified WHO of two new laboratory-confirmed cases of human infection with avian influenza A(H7N9) virus.

The first patient is a 57-year-old man from Zhejiang Province. He became ill on 20 November 2013 and was admitted to hospital on 25 November 2013. He is currently in critical condition.

The second patient is a 30-year-old man from Zhejiang Province. He became ill on 29 November 2013 and was admitted to hospital on 5 December 2013. He is currently in critical condition. He is the son-in-law of the 57-year-old male from Zhejiang Province with onset on 20 November 2013, reported above.

On 2 December and 6 December 2013, the Centre for Health Protection (CHP), China, Hong Kong SAR, notified WHO of two laboratory-confirmed cases of human infection with avian influenza A(H7N9) virus.

The first patient is a 36-year-old woman who became ill on 21 November 2013. She was admitted to hospital on 27 November 2013 and transferred to an intensive care unit on 29 November 2013. She is currently in critical condition. She had visited Shenzhen, Guangdong province, China on 17 November 2013 and had slaughtered live chicken for cooking and consuming. The investigation is ongoing; close contacts, including family contacts, have so far tested negative for avian influenza A(H7N9).

The second patient is an 80 year old man with underlying chronic illness who lives with his family in Shenzhen and travelled from Shenzhen to China, Hong Kong SAR, on 3 December 2013 for management of his medical condition. He was admitted to a hospital in China, Hong Kong SAR, on 3 December 2013. He was afebrile on admission and developed fever on 6 December 2013. His condition is currently stable. The investigation is ongoing; close contacts, including family contacts, have so far tested negative for avian influenza A(H7N9). No epidemiological link was identified between the two cases in China, Hong Kong SAR.

So far, there is no evidence of sustained human-to-human transmission.

WHO does not advise special screening at points of entry with regard to this event, nor does it currently recommend any travel or trade restrictions.
