Confusing situation again. Treyfish's post above (#16) suggests that the bus driver was not working and home on sick leave for a sprained foot for 5 days before onset. He would have had limited exposure from other bus riders if this is the case. Importantly though, if he had a sprained foot he probably would not have been exercising outdoors where he might have been exposed to H5N1 from infected migratory birds.

I agree. It is an inconsistent story.

Problem is he may have stepped out onto stoop, balcony or whatever to smoke a cigarette, put his hand on the railing then wiped his mouth, nose or eyes. If an infected bird left a dropping on the railing he inadvirtantly infects himself. Of course, he may have also visited the wet market while being off work due to the sprain. We just don't know at this point. I can't rule either scenario out until we see the clade or if others are infected.

It is also worth noting that the incubation period can exceed 5 days so many potential sources of infection are still viable; however, peak viral shedding before symptom onset is the last 24 - 48 hours so providing the information provided is accurate, exposure potential to bus passengers should be low.

I agree with Shannon on this one; the key here will be if there are other suspect cases in this area, or if there have been other cases that predate this one of which we are unaware.

The question here is what is the water source that they believe may be contaminated? If it is in the general water supply we are talking about large scale potential exposure; if we are talking about water in a rice paddy or similar it is a different matter. Hopefully there will be clarification on this.

I am going to be blunt here. The message from the Chinese government is 'Don't worry, be happy":

1) He got it from wild birds. It is NOT a case of human to human transmission.
2) He did not give H5N1 to anyone since he was inside for a long period of time. It will Not be a case of human to human transmission.
3) He did not get it from poultry since no poultry in China have H5N1. It is not a case of chicken to human transmission.
3) Oh wait...he got it from H5N1 contaminated poop in the water but if you boil the water, it is ok. It is Not a case of human to human transmission.

If we look at the news reports that we have collected (post 18 ) it appears that there have been several H5N1 drills for poultry infections in Guangdong province. Also some pigs have exhibited a SARS type illness.

And...there are protests there. More documented than anywhere else in China.

For me the question is:

Will the government use this single admitted human H5N1 case to quarantine parts of Guangdong province where there are mass demonstrations? Does anyone think this is the only H5N1 human case in China this year?

A threat to the public water supply will justify drastic moves by the government that will act to shut down human rights demonstrations.
