HEFEI, May 20 (Xinhua) -- Kindergartens in Fuyang City, the worst-hit Chinese city of hand-foot-mouth disease (HFMD), will resume classes on the coming Children's Day on June 1, as the authorities considered the situation has been brought under control.

The viral epidemic mainly caused by intestinal viruses of EV71 and the Coxsackievirus (Cox A16) takes toll on children. Most kindergartens suspended classes after the city, with a population of 9.3 million reported the country's initial HFMD outbreak in April.

As of May 18, 1,116 HFMD patients remained in hospitals in Fuyang, two of whom were still in critical condition, and 17 others are under intensive care, said sources with the city's health bureau.

However, no fatalities of the viral epidemic have been reported for the eighth consecutive day in the city
in east China's Anhui Province, as of Tuesday.

The incidence of HFMD in Fuyang was on decline, with discharged patients outnumbering those checking in to hospitals for the first time on May 9.

The government sources said that 72 new infection cases were reported on May 18, and 88 HFMD patients were discharged on the same day.

SHANGHAI reported 2,138 cases of hand, foot and mouth disease from last Monday to Sunday, a small increase over the previous week, the Shanghai Health Bureau said yesterday. There were no fatalities.

Most of the cases were children between one and six years old. Those living on the outskirts had a higher incidence than children in downtown areas and more migrant children suffered the disease than local children.

The city had three minor outbreaks of the disease at local kindergartens last week and each outbreak involved about 10 cases, officials said.

The Shanghai Center for Disease Control and Prevention conducted checks on 81 HFMD cases detected last week, confirming 27 patients infected with Enterovirus 71, or EV71, and 10 patients with Cox Asckievirus A16, another main intestinal virus leading to HFMD. None of the 37 cases had serious symptoms.

"The city will further enhance control and prevention of HFMD and focus on disease prevention at kindergartens, which are ordered to conduct effective daily checkup and disinfection," said Song Guofan, a health bureau official. "We are concerned that a growing number of children staying at home suffer HFMD, so we will carry out more education in communities. Educating adults is also important, since adults can spread the virus to children."

Local medical experts have been in Anhui Province's Fuyang, the city with the highest number of HFMD cases and largest mortality rate.

Nine pediatricians set up a 50-bed unit at Fuyang No. 2 People's Hospital on May 1.

"They saved seven dying children," Song said.

LHASA, May 20 (Xinhua) -- Two children in Lhasa were thought to have contracted hand-foot-mouth disease (HFMD), said a spokesman of the regional disease prevention and control center.

Symptoms of the contagious disease were displayed by a 5-year-old boy in a kindergarten in Lhasa on Sunday, said Rao Ruodeng, director of the regional disease prevention and control center on Tuesday.

The only information about the other case was the location, in Zhoulin County.

The children were receiving treatment in isolation wards and were both in stable condition.


"The health bureau has sent blood samples to the state disease prevention and control center for diagnosis," Rao said.

The kindergarten, where the 5-year-old had studied, had its classrooms and facilities sterilized to prevent further infection.

On May 16, a 22-month-old girl from Ganzhou, a city in the eastern Jiangxi Province, died of HFMD in a hospital, pushing up the national toll from HFMD to 43.

According to the Ministry of Health, HFMD, an epidemic disease that mainly affects children, can be caused by a host of intestinal viruses, but EV71 and the Coxsackievirus (Cox A16) were the most common.

HFMD usually starts with a slight fever followed by blisters and ulcers in the mouth and rashes on the hands and feet. Those with EV71 often show serious symptoms. It can also lead to meningitis, encephalitis, pulmonary edema and paralysis in some children. There is no vaccine.

Executive Summary

In late March 2008, a hospital clinician was alarmed by the occurrence of 3 consecutive deaths of pre-school children presented with severe pneumonia and rapid deterioration in Fuyang City, Anhui Province.

Up until mid-April, 15 children have died of similar severe illness.

Through an investigation conducted by local and national experts, the disease was confirmed on April 23 as hand, foot and mouth disease (HFMD) caused by enterovirus 71 (EV71) based on epidemiological, clinical, laboratory and pathological evidence.

Retrospective case investigation revealed that a simultaneous outbreak had occurred in the same area with children presenting skin rashes and blisters over hand, foot, and mouth.

Anhui is an inland province located in the central part of Southeastern China.

Fuyang City is located in the Northwest region of Anhui Province and has a total population of 9.76 million.

The city covers a total area of 9,700 km2 with one of the highest population densities in Mainland China (1,000 per square km).

In Fuyang City, from March 1 to May 9, there have been 6,049 reported cases of HFMD of which 353 were severe and 22 were fatal (case fatality rate 0.4%).

Retrospective case-finding identified 302 cases that occurred between March 1 and April 22, and revealed that the first case occurred as early as March 10.

The number of cases according to date of onset began to increase in early April, and peaked on April 28.

The number of reported HFMD cases in Fuyang City decreased after
May 5.

Among the 6,049 reported cases in Fuyang City the gender ratio was 1.9:1. The age range varied between 28 days to 18 years of age, with 78% of the cases being 3 years of age or younger.

All districts/counties in Fuyang City reported HFMD cases, with more than half the cases concentrated in 3 districts (Yingzhou, Yingdong and Yingquan).

Epidemiological investigation revealed no contact between the 22 fatal cases, but environmental investigation of the cases' households revealed poor hygienic and sanitary conditions among these families.

The clinical symptoms of the mild HFMD cases are those typical for the disease: rashes on hands and feet, mouth and buttocks, fever and general malaise.

All fatal cases presented an acute onset of fever and influenza-like-illness without catarrhal syndrome.

The cases' condition rapidly deteriorated, developing tachypnea, cyanosis, and some presented seizures.

All fatal cases died of serious complications such as neurogenic pulmonary oedema due to EV-71 infection.

Testing initial cases for a variety of diseases, including seasonal and avian influenza A/H5N1 and SARS did not reveal any conclusive results.

Subsequently, additional testing by Chinese Center for Disease Control and Prevention (CDC) was performed and several expert consultations were conducted.

On April 23, a variety of specimens such as pharyngeal swabs, lung puncture fluid, lung tissues, and blood from 5 out of 12 fatal cases (42%), tested EV71 nucleic acid positive by RT-PCR. EV71 genotype C4 viruses were isolated from specimens of both mild and fatal cases.

These sequences from mild and fatal cases exhibited high homogeneity.

Isolated EV71 virus strain gene sequences were uploaded to GenBank on May 7.

From January 1 to May 9, 2008, 61,459 HFMD cases and 36 deaths were reported through the disease reporting system in Mainland China.

The number of reported cases increased sharply after the disease became notifiable on May 2, and with cases being reported from nearly all provinces.

The 5 provinces with the highest number of reported cases are Guangdong (11,374), Anhui (9,235), Zhejiang (6,134), Shandong (4,566) and Henan
(3,230).

Because of the change in reporting policy and increased awareness of the general public about the disease, it is expected that there will be an increase in the number of reported HFMD cases in the upcoming weeks and months from Anhui Province and the rest of China.

Non-polio enteroviruses are common and exist worldwide.

Although infection often has no symptoms and goes unnoticed, these viruses are also associated with occasional outbreaks in which a larger-than-usual number of patients develop clinical disease, sometimes with
fatal consequences.

The initial high case fatality rate of the Fuyang City outbreak, 2.9% (18/610) from March 1 to April 23, was likely attributed to the following factors: rapid disease progression, late clinical presentation, and limited local medical capacities.

Although there is no specific treatment for enterovirus infections and a vaccine is not currently available, once the aetiology of the disease was known and early treatment was provided to the severe patients, the case fatality rate decreased considerably to 0.07% (4/5439) from April 24 to May 9, due to enhanced surveillance and implementation of prevention and control measures.

There have been a number of outbreaks of EV71 HFMD in the Asia-Pacific region since 1997.

In China, outbreaks have been reported in Taiwan Province in 1998 (>100,000 cases and 78 deaths) and in Shandong Province in 2007 (38,606 cases and 14 deaths).

The Government of China has shown its strong technical and political commitment to control the disease in Anhui and other provinces. In addition to enhanced surveillance, training was provided to clinicians and public health workers to improve the treatment success rate by increasing early identification and treatment of severe HFMD patients.

Guidelines were written to enable early admission of severe cases to the hospital, and critical cases to the paediatric ICU. After that, the case fatality rate has decreased substantially. In addition, to prevent HFMD, the Ministry of Health started a nationwide health campaign, stressing the need for personal hygiene, in particular hand washing practices.

In the future, China is devoted towards enhancing the above-mentioned measures for the prevention and control of HFMD in China. As part of the efforts to implement the International Health Regulations, IHR(2005), China will further strengthen the early warning system by immediate notification of clustering of clinically abnormal and severe cases, as well as increasing international collaboration and information exchange.

A. Laboratory testing of fatal cases
Between March 31 and April 16, the Anhui CDC tested 53 specimens (including whole blood, blood serum, pharyngeal swab and tissue samples), collected from fatal HFMD cases, and tested negative for the presence of seasonal influenza, avian influenza A/H5N1, Severe Acute Respiratory Syndrome (SARS) .

On April 18, the Institutes of Infectious Diseases and the Viral Diseases Prevention and Control of Chinese CDC received from Anhui CDC the previously collected specimens: pharyngeal swabs, lung puncture fluid, lung tissues, and blood. Laboratory testing for the presence of bacteria and virus was performed immediately on these samples.

Specimens from 5 out of 12 fatal cases (42%) tested EV71 nucleic acid positive by RT-PCR. Of these 5 EV71 positive fatal cases, the lung tissue specimen tested EV71 positive for 1 case, and for another case, the intestinal lymph nodes, cerebrospinal fluid, spleen, thymus, kidney, brain,
heart and lung specimens were EV71 positive.

For the other 3 cases, the pharyngeal swabs were EV71 positive.

Viral isolation was conducted on 6 different types of specimens (including lung tissue, mouth efflux, brain tissue and pharyngeal swab) collected from 3 fatal cases. For all 6 specimens, the nucleotide sequencing and neutral antibody tests revealed the presence of EV71 virus.

B. Autopsy findings
The autopsy on 3 fatal cases performed by the Medical School of Beijing University, found mainly changes in the brain, lung, heart and lymphatic system.

There were cases of
* severe brain oedema;
* lung congestion/pulmonary oedema, no significant lung inflammation, no significant bleeding;
* cardiac enlargement;
* hypertrophy of tonsils, thymus and lymph nodes;
* spleen enlargement; normal kidney, liver and gastrointestinal organs.

The autopsy findings were overall consistent with the clinical manifestations and disease progression and strongly suggest viral infection.

HEFEI, May 23 (Xinhua) -- The outbreak of hand-foot-mouth disease (HFMD) has ebbed in east China's Anhui Province, which was worst-hit by the viral epidemic since April, the provincial department of health said on Friday.

The number of cases reported everyday dropped from a peak of 1,160 on May 1 to 153 on May 21, and no further deaths had occurred over the past 12 days, the department said in a circular.

It said 734 people were still hospitalized in Anhui, including 10 in serious or critical condition.

Doctors successfully treated 10,696 HFMD patients in Anhui, although 26 died in the outbreak.

Chinese officials: child virus outbreak eases


BEIJING (AP) ? The spread of a virus that has sickened more than 24,000 people and killed dozens across China is slowing in the province where the outbreak was first reported, a state-run news agency said Friday.

The health department in eastern Anhui province said no deaths from the hand, foot and mouth disease virus had occurred there in the past 12 days, the Xinhua News Agency reported.

The report said the number of new cases reported daily had dropped from a high of 1,160 on May 1 to 153 on May 21. It said 10 people were still in serious or critical condition.

The World Health Organization could not verify the figures and whether the decline was significant.

Gregory Hartl, a spokesman for WHO in Geneva, said the global health body had received no details on the situation since May 19. He said determining whether a drop in cases was meaningful would be difficult because the disease was not identified until the middle of the outbreak when it was already spreading rapidly.

"It will take some time to iron out the numbers," Hartl said.

He said the picture was further complicated because under normal seasonal patterns the virus usually peaks in the hot months of June and July.


Anhui was the worst-hit province with 26 deaths, and the outbreak was first reported there in March. Most cases of hand, foot and mouth disease in China this year have been blamed on enterovirus 71, which usually infects children.

The virus, a major concern as Chinese authorities prepare for the Aug. 8-24 Olympics, has killed at least 43 people across China, but the state health ministry hasn't updated that number since May 16. The ministry didn't respond to a faxed request this week for an updated number, and a call to the ministry Friday night was not answered.

China Tries to Halt Spread of Hand, Foot and Mouth Disease
The contagious viral malady has left 42 children dead and infected 30,000 across Asia

SHANGHAI?Chinese authorities have launched a nationwide public hygiene campaign in an effort to combat the spread of hand, foot and mouth disease (HFMD), which has killed 42 children and infected at least 30,000 youngsters throughout Asia since the end of April. The disease, caused by an unusually virulent strain of the intestinal flu known as enterovirus 71 (EV71), so far has spread from Mongolia to Vietnam, including most parts of eastern China, and health officials warn that the situation could get worse as the disease usually peaks during the summer heat of June and July. Already, Mongolia has shut all kindergartens and primary schools as a result of the spreading outbreak.

The hardest hit areas in eastern China have been in Anhui Province, especially the city Fuyang, where 23 died, and even Beijing was not spared: two children, one a resident and one a visitor, died from the disease last week, whose symptoms include a mild fever, rash on the hands and feet and open sores in the mouth. In the most severe cases it can lead to a heart and lung infection (pulmonary edema) as well as inflammation of the brain lining (meningitis) or of the brain itself (encephalitis)?conditions that can result in paralysis or death.

Health officials here have ordered residents, schools and business to take steps designed to prevent the disease's spread, including sterilizing furniture and toys with alcohol wipes as well as keeping symptomatic children at home. The national government also ordered kindergarten and primary schools to check kids twice daily for signs of the illness. In fact, health officials have ordered schools to close for two or three weeks if any child attending becomes severely sick or if more than two cases occur in one class.

Shanghai has had nearly 2,000 cases of the disease so far this year. "Until now, the number of deaths caused by hand, foot [and] mouth disease in Shanghai [had been] zero," Fang Weimin, director of the Health Supervision Agency of the Pudong New District of Shanghai, told me through my translator. "The education department should do the educational work to enhance the awareness and alertness of related people" to make sure that parents know precautions to take to prevent the disease and how to recognize symptoms in affected kids.

The disease is typically allowed to run its course and does not usually kill those infected, according to the U.S. Centers for Disease Control (CDC). Those afflicted are most commonly the poorest of the population, such as workers who have emigrated to the booming cities and then must pay hundreds of dollars (thousands of yuan) for treatment or, in the worst cases, funerals.

The English-language newspaper Shanghai Daily reported, for instance, that 31-year-old migrant worker Chen Weiming in Guangzhou, who earns just $145 (1,000 yuan) a month, had to borrow money from his family to scrape up $245 (1,700 yuan) for his four-year-old daughter's funeral. He also cannot afford the $720 (5,000-yuan) treatment that saved his seven-year-old and 18-month-old daughters, Shanghai Daily reported and therefore cannot take custody of them from the hospital until he can pay the fee.

The World Health Organization and the U.S. Department of Health and Human Services are helping the Chinese track the source of this year's unusually deadly outbreak and map its spread. The U.S. was the first country to identify the EV71 virus as one of the culprits behind this disease in 1969. But this outbreak is also being caused, in some cases, by another enterovirus?coxsackievirus A16, according to Shanghai authorities. The CDC considers the A16 virus the most common cause of HFMD.

Chinese authorities are also working on developing a vaccine against it, but do not expect to have one ready anytime soon. "This infectious disease usually starts in May and reaches its peak in July," Sun Shuguang, a mouth disease specialist, or stomatologist, at the Medical Center of Jingan District in Shanghai said through my translator. Six punished for fatal HFMD case in China's Hubei Province

WUHAN, June 6 (Xinhua) -- Authorities in central China's Hubei Province have penalized six doctors and officials for mishandling a hand-foot-mouth disease (HFMD) case in which a child from Xianning City died, official sources said on Thursday.

The Chibi Municipal Health Bureau revoked the license of Huang Yingjun, a doctor with the Quankou Clinic in Guantangyi Town, Chibi city, who was said to have delayed the transfer of the patient to the county hospital.

The patient, surnamed Xie, came to the hospital on May 10 with a fever and cough. The symptoms were diagnosed by Huang as a cold. Xie was treated at the clinic for two days before being referred for further care.

The head of the clinic, Li Gexian, was dismissed for dereliction of duty.


On May 12, when the patient sought treatment at the Xian'an District Women and Children's Hospital and Dingsiqiao Township Hospital, doctors did not take the child's medical history or make an accurate diagnosis, nor did they refer the patient to other hospitals in a timely manner.

Wu Changqing, deputy head of the Xian'an hospital and Zhang Yinghua, deputy head of the Dingsiqiao hospital, were reprimanded for dereliction of duty. Lei Qing, a pediatrician at the Xian'an hospital, was dismissed and Wu Yuhua, a doctor at the Dingsiqiao hospital, was given a demerit.
