World Health Organization (WHO) representative in China Hans Troedsson has expressed appreciation for China's quick response to the outbreak of hand-foot-mouth disease (HFMD) and pledged the world body's willingness to support China in its efforts to combat the illness.

During a Monday meeting with Health Minister Chen Zhu, Troedsson said the WHO office in China would continue to actively work with China in coping with the outbreak, provide technical support and share with other countries and regions the experience gained here, the health ministry said on its website on Tuesday.

As of Monday, the number of HFMD cases reported nationwide this year had risen to 11,905 with 26 fatalities, according to a Xinhua count of local official figures.

The worst-hit area, Anhui Province in east China, updated the number of childhood infections to 5,840, including 689 new cases that were recorded on Sunday, according to the provincial health administration.

In Anhui's Fuyang City alone, 22 children have been killed by the epidemic since March 20 out of 4,496 children infected, and 1,314 are still hospitalized.

During the meeting, the minister briefed Troedsson on the epidemic's development in Fuyang and China's measures against the disease.

China had classified HFMD as a C class epidemic that must be reported to the ministry, he said. Previously, local health authorities were not required to report HFMD cases.

The ministry had also upgraded the emergency response scheme of the Anhui public health incident from grade III to grade II, he said.

Meanwhile, expert teams have been sent to Fuyang to help with diagnosis and treatment. Kindergartens in the county were closed to prevent a further spread of the disease and a public awareness campaign was launched.

Chen said patients were being treated and prevention and control measures were being implemented in an orderly, effective manner.

He said that he was heading up a health ministry leading group to combat the disease, which would soon hold a national tele-conference to urge stepped-up nationwide monitoring, reporting, prevention and treatment of the disease.

Meanwhile, release of information about the course of the epidemic would be stepped up to avoid public panic, he said.

The epidemic was first reported on March 20 in Fuyang. Since then, outbreaks have been reported elsewhere in China. So far, all the victims have been children, mostly below 6 years old.

The southern province of Guangdong reported 1,692 cases on Monday, an increase of 767 from Sunday. Three boys have died so far in the outbreak.

Zhejiang Province in east China said on Monday that 1,198 children have been infected this year, and that a 5-year-old boy died on April 6.

In southwestern Chongqing Municipality, 42 sporadic cases have been detected since Thursday.

Beijing recorded at least 1,482 cases as of Monday, of which 818 were reported in kindergartens. The capital's neighboring Hebei Province reported 206 infections.

Cases have also been reported in the provinces of Jiangsu, Hunan, Hubei, Shaanxi, Jiangxi and Henan.

Tang Xiaoping, an epidemic expert and deputy head of the health bureau of Guangzhou, said the drastic rise in cases reflected the order by the Ministry of Health on Friday, which classified HFMD as a C class epidemic that must be reported to the ministry.

According to the Guidelines Regarding Prevention and Control of HFMD published on the Ministry of Health website, HFMD can be caused by a host of intestinal viruses, but EV71 and the Coxsackievirus (Cox A 16) were the most common.

Taiwan: CDC reports 2nd death from enterovirus

TAIPEI, Taiwan -- Public health officials reported the death of a three-year-old boy in southern Kaohsiung yesterday; the second fatal enterovirus case this year. They urged all parents and school teachers to be more vigilant against enterovirus, as temperatures in Taiwan continue to rise into the summer months.

Chou Jih-haw, deputy director of the Center for Disease Control (CDC) under the Department of Health (DOH), said that the latest victim was a boy aged three years and four months who had probably been infected by his five-year-old sister.

On April 11, the boy developed a fever, with rashes and sores in his mouth, and he was checked into the hospital with spasms the following day, Chou said. The boy lapsed into a coma shortly after he was admitted, and died five days later despite efforts to save him, Chou said. An investigation found that the boy's sister also had an enterovirus infection, and had been admitted to the hospital on April 13. But she fully recovered because her case was not that serious.

"Children under three years old are most vulnerable to the disease because they have no anti-bodies, making them a high risk group in serious cases of enterovirus, especially in cross infection situations," Chou said. The first enterovirus death occurred in March in Yunlin County, southwestern Taiwan, the CDC reported.

Health officials at the CDC said that the number of confirmed serious cases of enterovirus has reached 60 this year, and among them there have been two deaths.

Seven other patients remain in intensive care, while one has been hospitalized for about four months due to encephalitis complications.

Enterovirus infections can be deadly for young children. The enterovirus enters the body through the gastrointestinal tract and often moves on to attack the nervous system, leading to symptoms of fever, muscle aches, and aseptic or viral meningitis.

In a massive outbreak in Taiwan in 1998, 405 children were infected and 78 of them eventually died. The most recent outbreak came in 2005, with 145 children infected and 15 dying from the virus.

DOH officials said people should maintain good personal hygiene by frequently washing their hands. Parents and teachers at elementary schools and kindergartens should especially supervise young children to clean their hands in southern Taiwan during warmer weather, the said. Young children should also be kept away from places with big crowds, they advised.

However, the current situation is still normal and people should not panic as there are preventive measures, said Wu.

The number of hand-foot-mouth disease (HFMD) cases reported in China this year rose to 12,164 as of Tuesday, resulting in 26 deaths.

HFMD is caused by more than one type of virus, such as EV71 and Coxsackie A16, but has similar symptoms, and most of the cases involve young children. It can prove fatal, and there is no vaccine.

HONG KONG ? A tight-lipped Chinese government response to a common childhood illness ? albeit a more lethal outbreak than usual ? underscores how China's propaganda machine, geared to controlling political discourse, still struggles to communicate information about disease with an increasingly educated public.

As of Monday, the outbreak of hand, foot and mouth disease, or HFMD, that started in China's eastern Anhui province has affected 8,531 children, most below the age of two, according to the state-run Xinhua news agency.

Twenty-five children, nearly all of whom lived in Fuyang city of Anhui province, have died in the latest outbreak.

While cases of the illness, whose unusually high death rate first caught the attention of international health officials, first showed up in early March, according to an earlier Xinhua report, official word from the government was released only in late April, allowing rumors to simmer for weeks.

The vacuum of information has taken its toll on the local government and health officials involved in the outbreak, as some Chinese media outlets have suggested there was a cover-up. Communist Party officials often instinctively shy away from releasing even relatively benign news for fear of causing greater panic, only to find later that the dearth of information sowed more panic than the information itself probably would have.

An official with the Ministry of Health in Beijing said he thought an active cover-up was unlikely, and that incompetence among inexperienced local health officials was the more likely culprit. "The local doctors did not know how to deal with this," he said. If there was a slow response, it was "because they don't know about this disease...; maybe the technical capacity of the local health workers is not high and needs to be improved."

Hans Troedsson, the World Health Organization's chief representative in China, said that while the death of children for whatever reason will rightly spark concern, this particular virus is a routine and common illness in the region.

"It's nothing to be worried about. This is a common childhood disease. We've seen outbreaks like this in the region," including Singapore, Vietnam and elsewhere in China, Dr. Troedsson said. "We'll see more cases. But there's no reason for alarm."

Singapore has tallied 9,026 cases of the disease so far this year, according to a statement issued by the city-state's Ministry of Health in late April, noting that the disease is "endemic in Singapore and there will be yearly seasonal outbreaks."

Over the past week, officials from the Ministry of Health in Beijing have issued strong statements emphasizing the need to swiftly tackle the spread of the disease. Xinhua reported Monday that 10 doctors and officials in Anhui province had been punished for "their misdeeds in handling the hand-foot-mouth disease," citing unnamed official sources. Two doctors were sanctioned for delaying moving a patient to a larger hospital, and other doctors were punished for profiteering off sick patients.

Hand, foot and mouth disease can affect adults, but young children are most vulnerable. While the mortality rate of the current outbreak of HFMD in China is very high compared with that of previous outbreaks ? raising questions about exactly what mix of factors might be contributing to the deaths ? China's Health Ministry has pinpointed the particular subtype of virus behind the outbreak in Anhui, according to Dr. Troedsson, who met with Chen Zhu, China's health minister, Monday afternoon.

"This is hand, foot and mouth disease, not some severe pneumonia spreading among families," said Yuen Kwok-yung, chairman of microbiology at the University of Hong Kong. "I expect it every year.... It's nothing compared to SARS or bird flu." A local Chinese newspaper had reported that residents of Fuyang were referring to the mystery illness as "Baby SARS."

The northern Inner Mongolia Autonomous Region reported 131 sporadic infections, according to Wang Wenrui, an official with the region's disease control and prevention center.

The local government has ordered shut down of all kindergartens and implementation of morning inspection at primary schools in the worst-hit Dengkou County until the epidemic eases.

Tianjin reported 134 HFMD cases, including 113 children below five years old.

A local health official said the disease occurred annually in the city, usually increasing starting in April and peaking in the summer.

The northwestern Qinghai Province also reported nine HFMD infections, which were not triggered by EV71 on the basis of medical tests.

The nine children were receiving treatment in a hospital in the provincial capital Xining.

Two children died of the enterovirus 71 (EV71) virus, which caused them to develop HFMD, in the southernmost Hainan Province, a provincial health department official said on Thursday. He did not say when they had died.

According to the latest figures from the provincial health department, 180 cases of HFMD were reported by Thursday, of which five were confirmed to be infected with the EV71 virus.

The central province of Hunan and the southwestern Guangxi Zhuang Autonomous Region reported their first and only HFMD deaths on Wednesday.

As a first step, they urge active-duty servicemembers and their families headed to China to stop by a base public health office for a travel medical briefing.

It?s a recommendation they extend to anyone going to a foreign location, not just China, according to Maj. Mahendra Kabbur, public health flight commander at Misawa Air Base.

While it?s still safe to go to China, Kabbur and Lt. Col. Cheryl Lowry, 35th Aerospace Medical Squadron commander at Misawa, said travelers should wash their hands frequently, drink bottled water and eat fruit that can be peeled. Travelers should also avoid salad, ice cubes and ice cream, especially in rural areas. Also, they should reconstitute powdered baby formula with bottled water, Lowry said.

Those precautions apply not to just China but elsewhere throughout Asia and overseas, where water sources might be contaminated.

On Tuesday, the U.S. Embassy in Beijing posted information about the outbreak on its Web site, noting there were no specific precautions to heed other than the general hygiene recommendations for living in China.

The outbreak of enterovirus 71 ? which can cause a severe type of hand, foot and mouth disease ? has sickened more than 5,000 people since late March, mostly in rural eastern China. Fuyang, about 500 miles south of Beijing, is among the hardest hit cities, with 362 new cases reported as of Monday, according to The Associated Press.

The World Health Organization said the virus is unlikely to be a threat to the Beijing Olympics in August, since it is a disease mostly affecting young children, the AP reported.

Enterovirus 71 can cause a number of diseases and is of particular concern for children younger than 5, Lowry said, noting that adults tend to have some immunity. In more serious cases, the virus can cause meningitis, Kabbur said. There is no vaccine.

The number of cases is expected to peak in June or July, according to the World Health Organization.
