China struggles to contain viral epidemic: state media
BEIJING - DOCTORS in China struggled to contain the spread of an intestinal virus that has infected more than 3,300 children, killing 21 of them so far, state press reported on Friday.
The latest death occurred in the city of Fuyang in Anhui province, the epicentre of the epidemic with 2,946 children infected there as of Friday, Xinhua news agency reported, citing local health officials.

The number of children in Anhui infected with the enterovirus 71, known as EV71, has risen by nearly 500 since Wednesday, the report said.

Up to 340 children have been infected in central Hubei province and five others are sick in the city of Hangzhou in eastern Zhejiang province, Xinhua reported.

Sixteen children have also fallen ill in Henan province, which borders Anhui.

EV71, which causes hand, foot and mouth disease, is highly contagious and is spread through direct contact with the mucus, saliva, or faeces of an infected person. Young children are most susceptible because of lower immune systems.

The disease begins with fever, blisters, mouth ulcers and rashes, and has spread in Anhui since early March, amid accusations by the Chinese media of a government-led cover-up of the epidemic.

Most of those stricken are under the age of six, with 879 children currently being treated in Fuyang hospitals, nine of whom are in critical condition, the report said.

Forty others were in serious condition.

News of the epidemic only surfaced on Monday, when it was reported that 19 children had died.

State television showed video footage of parents from rural areas of Anhui bringing their toddlers to overcrowded hospitals in Fuyang.

Health officials warned that although older people may not show symptoms of the disease, they could be carriers of the virus and could possibly infect others, the report said.

'Fuyang city is strengthening medical emergency treatment and prevention and control measures in every way possible,' Xinhua said, noting that treatment facilities would be expanded.

The World Health Organisation earlier this week expressed concern over the epidemic, which in serious cases can lead to brain, heart and lung damage.

It said in a statement Wednesday that while enteroviruses are found across the world, 'the situation (in Anhui) is still of concern especially because of the current high reported case fatality rate compared to previous years.'

China's health ministry has tried to calm fears, saying early discovery of the disease and better treatment has lowered the mortality rate, but has warned that the disease was likely to spread.

'We estimate that the hand, foot and mouth disease (caused by EV71) in Fuyang city will still continue for some time, the number of cases will continue to increase, and serious and fatal cases might still continue to happen,' the ministry said on Wednesday. -- AFP

Most of the cases in Hubei were isolated, except for 39 children aged three and four reported at three kindergartens in Guanghua district, Qianjiang City since April 29, the Provincial Health Department said in a statement.

Experts from the provincial disease control center collected and tested samples from five children on Thursday morning, determining that they had been infected with EV71, it said.

But no figures were provided on the total number of children in the city and the province at large have been infected with EV71.

According to the statement, 33 children in Qianjiang have recovered or are recovering, and six are being treated as outpatients.

No new cases of hand-foot-mouth disease have been reported in Hubei since April 30, it said, and local health authorities have been ordered to make daily reports on the disease.

Last year in Linyi City, eastern Shandong Province, two boys and a girl, all under the age of two, died from the disease.

It may also cause high fever, meningitis, encephalitis, pulmonary edema and paralysis in a small number of children.

It isnt' ther first time HFMD is detected in Asian countries. Also HK SAR detected cases in recent weeks (see FT thread 'HK Surveillance for EV71'). One may feel a bit reassured by a large resource spent by Chinese health authorities for research in deep these recent outbreaks.
Since no chances was left, after preliminary respiratory pathogens testing inconclusive results, central laboratory was involved and EV71 finally isolated.
As WHO report stated, these viruses remain for long period in the environment, possibly contaminating vegetables and fruits served in kindergartens and schools.
Schools closures is advisable in the perspective of reducing the scope of epidemics and for disinfection of facilities.

A total of 978 children remain in hospital, 48 of them were in critical conditions, Wang said. "Authorities in Anhui and Fuyang are doing utmost to treat the children and strengthen disease control."

Thirty-two medical specialists dispatched by the Health Ministry and the provincial health department have arrived in Fuyang to direct the rescue work, Wang said.

The province has upgraded its response plan for public health emergency from class three to class two, which means the provincial government has the power to quarantine the patients and control the traffic in and out of Fuyang.

A VIRUS has spread rapidly, killing at least 22 children and infecting more than 3,300 others in Fuyang City, local authorities said yesterday.

The situation is so serious that the city now faces quarantine.

Enterovirus 71 began spreading in Fuyang, Anhui Province, in early March and by the early hours of yesterday, 3,321 children had been infected by the virus, up from 2,946 registered a day earlier, said Wang Yan, a government spokesman.

EV 71 cases usually peak from March to July. It began spreading in Fuyang in early March as hospitals in the city started to take in children with fever, blisters, mouth ulcers and rashes on the hands and feet. Some were diagnosed with brain, heart and lung damage.

All cases in Fuyang were children aged below six, most of them under two.

There are about 400,000 children below the age of five in Fuyang.

In a bid to stop the virus spreading, all kindergartens in Fuyang began the three-day May Day holiday two days early.

Hubei reports 340 cases of hand-foot-mouth disease

Central China's Hubei Province has seen 340 cases of hand-foot-mouth
disease this year [2008], but no deaths reported, local health
authorities said on Friday [2 May 2008].

At least 5 cases were caused by an intestinal virus known as human
enterovirus 71 (EV 71) which has left 21 children dead and 2946 ill
in Fuyang City, in neighboring Anhui Province, since early March
[2008].

Most of the cases in Hubei were isolated, except for 39 children aged
3 and 4 reported at 3 kindergartens in Guanghua district, Qianjiang
City since 29 Apr 2008, the Provincial Health Department said in a
statement. Experts from the provincial disease control center
collected and tested samples from 5 children on Thursday morning [1
May 2008], determining that they had been infected with EV71, it
said. But no figures were provided on the total number of children in
the city and the province at large who have been infected with EV71.
According to the statement, 33 children in Qianjiang have recovered
or are recovering, and 6 are being treated as outpatients.

No new cases of hand-foot-mouth disease have been reported in Hubei
since 30 Apr 2008, it said, and local health authorities have been
ordered to make daily reports on the disease. "Experts from the
provincial disease control center are still in Qianjiang to guide the
disease control work," the statement said.

Hand-foot-mouth disease is a common childhood illness that mainly
affects children under 10. Symptoms include fever, sores in the mouth
and a rash with blisters. It often begins with a sore throat.
Moderately contagious, the disease, more common in summer and autumn,
can be transmitted through nose and throat discharges. It can
sometimes be fatal if complications occur.

China 1st reported hand-foot-mouth disease in Shanghai in 1981. An
outbreak of the disease in northern Tianjin in 1983 caused more than
7000 infections. Last year in Linyi City, eastern Shandong Province,
2 boys and a girl, all under the age of 2, died from the disease.
EV71 can cause hand, foot and mouth disease, which usually starts
with a slight fever followed by blisters and ulcers in the mouth and
rashes on hands and feet. It may also cause high fever, meningitis,
encephalitis, pulmonary edema and paralysis in a small number of
children. Infection can lead to high death rates in serious cases and
no vaccine id available.

Practice good hygiene to keep foot and mouth disease at bay

Sarawak is stepping up prevention and control measures for hand foot
and mouth disease (HFMD) following recent outbreaks in Singapore and
China. Deputy Chief Minister Tan Sri Dr. George Chan said the state
health department's routine HFMD surveillance showed that 1729 cases
of the disease had been reported as of April 26 this year [2008],
including 3 small, localised outbreaks.

The 1st occurred at Universiti Malaysia Sarawak (Unimas) on 10-25 Mar
2008 involving 48 adults, the 2nd in Kampung Pichin, Serian (4-22 Apr
2008) involving 24 children and the last at Al Hidayah Abim Nursery
in Sarikei (9-24 Apr 2008) involving 10 children. Dr. Chan said none
of the cases involved enterovirus 71 (EV71), which potentially causes
a more severe form of the disease. "The outbreaks were mild and no
one was hospitalised. All the outbreaks have now been controlled.

"However, because of the increased number of cases in Singapore
including a few with EV71, and also in Anhui, China, where there have
been 20 deaths, this is a good time to warn the public, especially
those looking after young children, to make sure they follow hygienic
practices," he told a press conference at his office here on Friday
[2 May 2008].

He said parents with young children and operators of childcare
centres should take preventive measures such as practising good
hygiene at all times. He also advised childcare centre operators to
make sure their premises were clean and to screen children for
symptoms of HFMD, In addition, parents of young children should take
the necessary precautions, such as washing their hands before eating
and after going to the toilet, covering their mouths and noses when
coughing or sneezing and ensuring cleanliness in their homes,
including their children's toys.

Sarawak last experienced a major outbreak of HFMD in 2006, when more
than 14 000 cases and 13 deaths were reported.

[According to the WHO non-polio enteroviruses are common and
distributed worldwide. Although infection often has no symptoms and
goes unnoticed, these viruses are also associated with occasional
outbreaks in which a larger-than-usual number of patients develop
clinical disease, sometimes with fatal consequences. Currently human
enterovirus 71 is associated with outbreaks of HFMD in the Chinese
provinces of Anhui and Hubei.

Since the preceding report from Anhui dated 1 May 2008 (see: Hand,
foot & mouth disease - China (03): (Anhui), WHO 20080501.1504) the
number of confirmed case of HFMD has increased from 1884 to 2946 and
the number of fatalities has risen from 20 to 21. In Anhui province
the outbreak is centered on the city of Fuyang, whereas a similar
outbreak in the neighboring province of Hubei is more dispersed and
fewer cases and no deaths have been reported so far.

By contrast a contemporaneous outbreak of HFMD in the Sarawak
province of Malaysia, which does not involve human enterovirus 71,
although extensive, is following a milder course.

Fears of virus spread as boy dies in south China: officials

BEIJING (AFP) -

A young boy has died in southern China from what was probably hand, foot and mouth disease, health authorities there said Saturday, raising fears the virus had spread from its epicentre further north.
The 18-month-old died Friday in Foshan city, Guangdong province bordering Hong Kong, the provincial health department said in a statement posted on its website.

It is feared the case was due to an intestinal virus known as enterovirus 71, or EV71, an official with the department told AFP, citing the results of preliminary checks by the provincial disease prevention control centre.

"It's very likely to have been the result of the EV71 virus," said the official, identifying himself by his surname Yu. "The test results are not out yet, but that's what we suspect."

He said another death in the same area reported in late April was less likely to have been caused by EV71.

A further 22 children have already died of the virus in Anhui province in eastern China, and another 3,300 there have been infected.

EV71, which can cause hand, foot and mouth disease, is highly contagious and spread through direct contact with the mucus, saliva or faeces of an infected person. Young children are most susceptible because of lower immune systems.

The disease -- which begins with fever, blisters, mouth ulcers and rashes -- has spread in Anhui since early March, amid accusations by the Chinese media of a government-led cover-up of the epidemic.

Five other cases of EV71 have been confirmed in the city of Hangzhou in eastern Zhejiang province, while 16 children were earlier reported to have fallen ill in Henan province, which borders Anhui.

In central Hubei province, 340 children were confirmed to have hand, foot and mouth disease, and local officials were testing them for EV71, state media said earlier.

China orders nationwide fight against fatal virus

SHANGHAI, May 3 (Reuters) -

China ordered authorities across the nation on Saturday to fight hand, foot and mouth disease, after the death of an 18-month-old boy suggested a virus causing the illness had spread from eastern provinces to the south.
The boy died in the southern province of Guangdong from a suspected case of the disease that was apparently caused by the enterovirus 71, or EV71, the official Xinhua news agency quoted health authorities there as saying.

Twenty-two children have died from hand, foot and mouth disease and over 3,000 people have been infected in Fuyang city, in the eastern province of Anhui, since a mass outbreak of the EV71 intestinal virus began spreading there in early March.

"Local Communist Party and government officials are on high alert. Health authorities are urgently taking measures to prevent the disease and treat seriously ill children," the Health Ministry said in a statement.
It said special equipment was being installed in children's wards of hospitals to treat patients. Local authorities were strengthening the supervision of food safety and water quality in an effort to stop the disease from spreading.
Of the 3,321 people infected, 978 were in hospital and 58 were critically ill or in serious condition, the ministry said.

It ordered health authorities across the country to step up monitoring for cases of hand, foot and mouth disease, requiring all cases to be reported within 24 hours.

Officials were told to go to nurseries and primary schools to educate staff there on hygiene and prevention methods. The ministry said Chinese scientists would step up research into the disease.

REPORTING DELAY
The virus began spreading in Anhui in early March, but a delay in reporting it to the public until last weekend triggered heated discussion and criticism in the Chinese media, which said local government officials should be sacked.

An initial cover-up of the SARS epidemic in 2003 led to the sacking of Beijing's mayor and the health minister.

But health officials say there was no cover-up and the reason for the delay in this case was because medical teams were trying to work out what the illness was.

Xinhua did not say whether authorities believed EV71 had spread from Anhui to Guangdong, but official Chinese media reported earlier that the virus had spread from Anhui to the neighbouring province of Henan.

Hand, foot, and mouth disease, characterised by fever, sores in the mouth and a rash with blisters, is a common illness among infants and children and is usually not fatal, according to the National Center for Infectious Diseases in the United States.

But the fatal cases in Fuyang involved complications due to infection by EV71, the World Health Organization said on its Website (www.who.int).

The number of hospitalised cases in Fuyang has risen sharply since April 19, and public health experts think cases will keeping rising before peaking around June or July, the WHO said.

There is no vaccine or antiviral agent available to treat or prevent EV71. Enteroviruses spread mostly through contact with infected blisters or faeces and can cause high fever, paralysis and swelling of the brain.

Northwest China's Shaanxi Province has seen 118 cases of hand-foot-mouth disease this year, but no deaths have been reported, local health authorities said Saturday.
The Shaanxi Provincial Health Department has begun to monitor kindergartens and schools to prevent the spread of the disease.

Central China's Hubei Province has seen 340 cases of the disease this year, but no deaths have been reported, local health authorities said Friday.

Death toll from a mass intestinal virus infection rose to 22 and 3,321 children had been sickened by the virus in Fuyang city in the eastern Anhui Province since early March.

Hand-foot-mouth disease, also known as coxsackievirus infection, is a common childhood illness that mainly affects children under ten.

Symptoms include fever, sores in the mouth and a rash with blisters. It often begins with a sore throat.
