CDC immunization expert in-depth city Wenchuan County, including more than 20 residents gathered at schools and institutions insecticidal disinfection units, at the same time drinking water, food and other epidemiological investigation. Experts found that many settlements flies, mosquitoes Man Tianfei, no one dealing with the trash, while a small number of residents in the phenomenon of diarrhea, epidemic prevention situation is not optimistic. Immunization experts said the earthquake disaster urgent need attention to disinfection, insecticide, to prevent the spread of infectious diseases.Live flies point in time are Gan Buzou

Concerns growing over quake-related disease in China

Masaru Kawanishi and Yoshiaki Takeuchi / Yomiuri Shimbun Staff Writers
OSAKA--Emergency disaster-relief teams to be sent by the government to areas hit by the earthquake in Sichuan Province, China, are expected to have to treat people with life-threatening illnesses and conditions such as pneumonia and strokes, which often occur in the harsh living conditions that follow major earthquakes.
Kobe University and other organizations considering sending medical relief teams to China are also making preparations to improve sanitation and provide mental health care under the severe conditions.
Shortages of toilets, water and food are serious problems at evacuation centers, which are so overwhelmed they cannot shelter all the victims.
"The number of patients suffering from infectious diseases usually increases from the second and third days after a disaster due to water shortages, malnutrition and harsh living conditions," said Shigeki Asahi, director of emergency operations at the International Medical Center of Japan's Kohnodai Hospital in Chiba Prefecture.
The diseases that cause the greatest concern are respiratory tract infections, such as pneumonia.
Pneumonia was the most common condition among those hospitalized after the Great Hanshin Earthquake in 1995.
A peak in pneumonia patients occurred a week after the Hanshin disaster because elderly people and other evacuees living communally in poor conditions became less able to resist disease.
Incidences of cerebral vascular disease and ischemic heart disease were higher in areas that suffered the greatest damage, indicating that mental and physical exhaustion caused by drastic changes in living environments were causes of the diseases.
Kazuhiko Moji, a professor at the Research Institute for Humanity and Nature in Kyoto, said outbreaks of diarrhea and colds at evacuation centers are a concern. "Poor hygiene makes it worse. To prevent this, toilets should be cleaned and there should be no dumping of human waste nearby," said Moji, who surveyed the 2004 Indian Ocean tsunami.
Kobe University Hospital is preparing to dispatch a medical team. A hospital official said: "It seems we've already missed the period for acute treatment, but medical treatments in fields such as public hygiene and mental health care are still necessary. We'd like to contribute the knowledge and experience in emergency medical treatment we obtained through our activities in the Great Hanshin Earthquake and disaster sites in other countries."

Truckloads of China's dead seek final resting place

GEOFFREY YORK
From Tuesday's Globe and Mail
May 19, 2008 at 7:50 PM EDT

MIANZHU, CHINA ? With more than 50,000 dead, there is little time for niceties in the disposal of the victims of China's massive earthquake.
At the Mianzhu funeral home, bloated corpses in body bags are sprayed with disinfectant and tossed into a truck like sacks of potatoes. The employees are doing their best to be respectful, but they are overwhelmed by the sheer scale of the disaster.
All around the earthquake zone, mortuaries are so overcrowded that the dead are often trucked more than 200 kilometres away. In a few cases, soldiers have dug mass graves to dispose of decaying bodies, for fear that they could spread disease if not buried quickly.
The catastrophe has reached such magnitude that it is exhausting the guardians of the dead. ?We urgently need more charcoal-filter masks, rubber gloves and disinfecting fluids,? says Yang Honggang, director of the Mianzhu funeral home, on the outskirts of one of the worst-hit cities.

Enlarge Image Disinfectant is sprayed on body bags containing recovered corpses as search and rescue operations are underway in Beichuan on May 19, 2008 in southwest China's quake-stricken Sichuan province. (FREDERIC J. BROWN/AFP/Getty Images)

?The bodies have decayed so badly that it's hard to lift them up to wrap them, and the smell is very strong,? he said.
Mr. Yang was forced to suspend all cremations at his funeral home because the earthquake shattered his tall brick chimney and knocked out his electricity and water supply. ?We urgently need a crane to fix the chimney, but all the cranes were sent to the front,? he said.
Unable to cremate the bodies, all he can do is disinfect them, collect them and send them on to another funeral home, hundreds of kilometres away.
When the latest truckload of bodies is ready to be sent off, an employee lists the contents. ?There are 18 bodies, including four unidentified bodies and one left foot,? she reports to her colleagues.
Rescue workers have taken DNA samples from the nameless bodies and body parts, in the hopes of identifying them later. But the funeral homes can scarcely cope with the deluge of bodies.
At a funeral home near the quake-hit city of Deyang, as many as 110 bodies a day are being processed ? almost 10 times the normal number ? and the crematorium is working 24 hours a day to burn them.
?Some people have lost almost everything, so we waive our normal fees for transportation and cremation, and we give them a free urn for the ashes,? said Ren Shigao, the home's director.
?We have 70 staff working in shifts, around the clock, seven days a week. But we urgently need more disinfectant, charcoal-filter masks, rubber gloves and rubber gowns. They're almost all gone, and we have nowhere to buy any more, because everything has been donated to the front. We received some supplies from the government, but it's far from enough.?
China Monday began a three-day official mourning period for the victims of the earthquake. All over the country, people stood at attention for three minutes of silence at 2:28 p.m., exactly one week after the quake hit. Air-raid sirens wailed, motorists blared their horns and flags were lowered to half-mast.
On an Air China passenger jet waiting for departure at Chengdu airport, the crew asked the passengers to rise to their feet in the aisles at 2:28. The passengers bowed their heads for three minutes. Then, in an emotional voice on the public address system, a crew member declared: ?After the storm, we will see the rainbow. ? Pray for the Chinese people and pray for China.?
The official mourning period is being so strictly enforced that cinemas have gone dark for three days, music has been prohibited at bars, karaoke clubs have been temporarily closed and entertainment shows on television have been cancelled. The entertainment and game portions of all Chinese websites have been shut down, and the Olympic torch relay has been suspended for three days.
It is the first official mourning period in China since the death of Mao Zedong in 1976.
The Chinese government has said that it expects the earthquake death toll to climb beyond 50,000. On Monday the official death toll reached more than 34,000. But in Sichuan province, a provincial leader said the toll did not include more than 9,500 people who are still buried in the rubble and a further 29,400 who are missing. This suggested that the final death toll could reach 73,000 or more. More than 245,000 have been injured, while a further 4.8 million have been left homeless.
Among the dead are 158 relief workers who were buried over the past three days as they tried to repair roads in remote parts of the quake-hit region.:tiphat: http://www.theglobeandmail.com/servlet/story/RTGAM.20080519.wchina20/BNStory/International/home

As of May 18, the sixth day after the earthquake, more and more earthquake victims have diarrhea, they're catching colds and getting bitten by dogs in Wenchuan County. Fortunately, everyone may receive treatment in various field hospitals set up by Army Hospital relief workers. Disease prevention teams have been sent to the quake-hit areas from the Sichuan Provincial Center for Disease Control and Prevention (Sichuan CDC).
Mad dogs bite people
The inner side of Liu Guijun's knee was injured due to a dog bite. He received treatment in a field hospital set up by the Beijing Army General Hospital.
On the morning of May 14, when Liu Guijun finished his porridge in a tent with another villager, a black and yellow dog rushed up to him and bit him randomly. Because he was busy doing reconstruction work, Liu didn't think he had been seriously injured so he did not seek any treatment until May 18.
On May 17 medical staff of the Hainan Frontier Garrison Hospital for the Armed Police said that they had treated many victims bitten by dogs after they arrived at Longchi Town, Dujiangyan on May 14. Due to limited medical supplies they could only clean the victims' wounds.
Doctor Li Yunjun, working for Beijing Army General Hospital, cleaned Liu Guijun's wound but lacked any rabies vaccine on May 18. He advised Liu to go to Dujiangyan to get the rabies vaccine as quickly as possible. Doctor Li explained that a dog becomes more aggressive after losing his master and/or experiencing an earthquake. He also suggested that victims carry sticks to ward off vicious dogs.
Unclean drinking water results in diarrhea
Unclean drinking water results in diarrhea
Many field hospitals set up by the Beijing Army General Hospital and the Hainan Frontier Garrison Hospital for the Armed Police have treated several hundred patients with cold symptoms and diarrhea as of the afternoon of May 18.
Ms Li from Wayao village, Xuankou Town did not have any medicine to combat her sore throat for six days; afterwards she received treatment in a newly established field hospital. Another six-year-old boy from the same village had almost endless diarrhea until treated. Many similar illnesses are prevalent in this town, Beijing News reported on May 18.
According to the Beijing Army General Hospital, their field hospital treated 200 patients on May 17, of whom over 30 percent suffer from colds and diarrhea, caused, according to medical personnel, by unclean water. Disease prevention staff from Chengdu repeatedly sprayed disinfectant around living quarters, toilets and throughways. A staff member from the Chengdu Wuhou district CDC who is working in a living area in Xuankou Town said disaster area's disease prevention work has been effective because no cases of plague have materialized to date.
The same situation exists in some towns around Qingchuan County, according to a China News Service report; Increasingly people are getting diarrhea and catching colds. Because of the earthquake, water sources may be contaminated. Water purification efforts have not been totally effective under current conditions.
Currently, disease prevention is the top priority for all medical staff. More sanitation and disinfectant campaigns are needed.

The Qingchuan county is at Qiao Zhuang the town, the running water pollution is also unable to withstand, the locality presents the intestinal tract disease's patient also suddenly to increase. In addition, many villagers seek after conveniently, throws conveniently the domestic animal skeleton to inside the open land, the reservoir and the rivers. East Mianzhu the northern town Guang Hecun horse's tail section of river, in the river is floating the dead pig, the chicken duck, has been soaked to swells rottenly, spreads the intermittent odor. The horse's tail rivers after the Mianzhu town center, flow to cities again and so on Deyang, Guanghan. City of Deyang's house document ?illness controls the group work bulletin? to point out that the Mianzhu animal corpse float drainage, the ruins, not yet carry on disinfection processing, brings the very major difficulty to the guard. However to comfort the will of the people, the authority emphasizes the regional drinking waters unceasingly ?the present security available?. {The authorities tells them the water is good anyway:( }:tiphat: http://svsurl.systransoft.com/?t=ou...k=11d1def534ea1be0--4482064e-119fe7a2513-2b5f

At the same time, a military medical team specializing in the prevention of epidemic diseases that affect humans and animals left Changchun, capital of northeast China's Jilin Province, aboard a passenger plane on Wednesday morning, heading for Chengdu, capital of southwest China's Sichuan Province.

Its members, all with doctoral degrees, are from the Military Veterinary Institute of the Military Academy of the Chinese People's Liberation Army (PLA).

Their job is to sterilize the carcasses of dead animals that might harbor diseases that are infectious to humans and animals, said Gao Hongwei, head of the institute.

As earthquake relief campaign enters the 10th day, a top priority for the authorities and rescuers has become the prevention of infectious diseases in the quake areas, where numerous human and animal bodies are decomposing beneath the ruins, a hotbed for dangerous infectious diseases.

"It is not sheer alarmism," said Jiang Tianjun, a doctor with the PLA's No. 302 Hospital in Beijing, referring to the possibility of an outbreak of serious epidemic diseases in the areas stricken by the devastating quake.

The expert urged people in the affected areas to be vigilant against rats, mosquitoes, flies and bugs. Contaminated water should be cleared away from residential areas and pesticide sprayed to kill mosquitoes hiding underneath the ruins and other shelters, he said.

The expert advised people to sleep in mosquito nets, cover up drinking water and wear long-sleeved coats and long trousers. He also suggested that local residents get hepatitis-B vaccinations as soon as possible.

Recently, the China Center for the Control of Animal Diseases published a pamphlet on the prevention of animal diseases, advising people not to cut, eat, sell or transport dead animals. All animal carcasses must be sterilized and buried at least 2 meters deep, it said.

It also advised local authorities to closely monitor the situation, strictly control and supervise animal breeding, butchering, processing, transport and storage, and severely punish the trading of dead animals.

In the chatrooms of many Chinese websites, netizens are calling for the government to take immediate action to prevent possible epidemic diseases, while persuading people, mostly volunteers, to stop flooding into the affected areas from other parts of the country.

To make way for the upcoming large-scale disease-prevention efforts, military rescuers have began relocating local residents, mostly villagers, away from their homes in the worst-hit areas.

On Wednesday, a 3,200-person detachment of the Chinese People's Armed Police resettled 4,149 villagers from the remote areas of Wenchuan. According to military sources, soldiers searched the villagers' homes door to door, to ensure that no one was left behind in the affected villages, which will be completely sterilized in the coming days.

As of noon of Wednesday, the death toll from the quake had reached 41,353 nationwide, while 274,683 people were injured and 32,666 still missing.

It was even more powerful than the Tangshan earthquake in 1976 in terms of scope and intensity, Chinese Premier Wen Jiabao said during his inspection of the quake-hit areas in Sichuan.

Measuring 7.8 on the Richter scale, the catastrophe in Tangshan of north China's Hebei Province claimed about 240,000. :tiphat:

Beijing/Manila, 22 May 2008 ?The World Health Organization (WHO) is rushing additional medical supplies to the People's Republic of China. The supplies will be able to treat 130 000 people. WHO is also sending an expert team to collaborate with the government on rebuilding its health infrastructure following the devastating May 12 earthquake.
WHO will provide equipment to ensure clean drinking water and proper sanitation, which is vital for controlling any outbreak and spread of communicable diseases such as diarrhoea. This includes 5 000 chlorine disinfection tablets, drinking-water treatment units and mobile toilets.
Four Interagency Emergency Health Kits are now being sent to the quake-affected areas of Sichuan Province by WHO. Each kit provides sufficient essential medicines and supplies to treat 30 000 people for one month. Contained in the kits are medicines for common diseases and medical devices for primary health care workers.
A WHO team of experts will be dispatched shortly to China to support government efforts to reconstruct health care services severely impaired and damaged by the 8.0 magnitude earthquake.
"WHO has already identified that the key health issue in the earthquake's aftermath is to prevent and control communicable disease outbreaks," said Dr Eric Laroche, Assistant Director-General for WHO's Health Action in Crises cluster. "But the longer-term challenge is how to best rebuild its damaged health infrastructure."
WHO promotes the construction of hospitals and health facilities that can survive the impact of natural disasters, including high-intensity earthquakes. In most cases, a very small increase in construction costs is sufficient to enable health facilities to withstand such natural disasters, at a time when their services and staff are most needed.
Dr Hans Troedsson, WHO's Representative to China, said the expert team would include a specialist in constructing appropriate health systems, as well as an epidemiologist to assist in disease surveillance control.
"We know health facilities in and around the epicentre of the earthquake were severely affected by the disaster," Dr Troedsson said. "WHO wants to support Chinese efforts to restore fully its hospitals, clinics and other health facilities to ensure they are strong enough to withstand future natural disasters and provide healthcare in the wake of others."
The United Nations has contributed US$ 8 million from its Central Emergency Relief Fund to the Chinese government, and UN agencies are intensifying support to authorities there to mobilize relief efforts. WHO and other UN agencies are preparing to send further medical supplies and emergency equipment requested by the Government. WHO has already acquired emergency medical supplies locally in China and given them to local authorities to assist in the provision of health care for survivors in the affected area. These include rapid water quality testing instruments, chlorine disinfectant equipment, drinking water treatment equipment and mobile sanitation facilities.
WHO has been providing the Chinese Ministry of Health and the Center for Disease Control with ongoing technical assistance and materials on health emergency management and preventing the spread of communicable diseases.

According to sources at the Chinese Center for Disease Control and
Prevention (CCDCP) 30 patients in Sichuan University's Huaxi Hospital
have been infected with gas gangrene
. An isolation ward has been set
up to prevent the disease spreading. All patients injured in the
quake arriving at Huaxi Hospital will be sent to the isolation ward
for diagnosis on arrival.

Five patients diagnosed with gas gangrene disease on [14 May 2008]
are in a stable condition, according to Shi Yingkang, the dean of
Huaxi Hospital.

Gas gangrene is a bacterial infection that produces gas within
gangrenous tissue. People with open fractures are particularly
susceptible. It is caused by the clostridium bacteria and if left
untreated is usually fatal within 12 hours.
Isolation of patients and
effective disinfection can prevent cross infection. It takes about 5
minutes to diagnose the disease by testing wound secretions.

There have been no cases of cross infection in Huaxi Hospital so far.
But rescuers in the quake-hit region have been warned not to make
direct contact with survivors' skin, and to follow effective
disinfection procedures.

Experts from CCDCP say isolation wards should be set up in all
hospitals receiving patients from the quake-hit area. All injured
persons transferred from the quake area should be first sent to
isolation wards to be checked for gas gangrene. If gas gangrene is
suspected, treatment must begin immediately. Thoroughly cleaning
wounds, removing foreign objects and dead tissue is the best way to
prevent clostridium infection.

[Another news release states that as of 19 May 2008, 58 cases of gas
gangrene had been reported by several major facilities treating
survivors of the earthquake in Sichuan, China. Of these, about 30
will require amputation
(see
http://en.ce.cn/National/Politics/200805/21/t20080521_15553120.shtml).

Gas gangrene is a rapidly progressive, frequently fatal, necrotizing
infection of muscle
that is usually caused by _Clostridium
perfringens_, and thus is also called clostridial myonecrosis. Gas
gangrene usually involves traumatic civilian and war wounds, but can
also involve surgical wounds, particularly after colon or biliary
surgery. Clostridial uterine myonecrosis can complicate a septic abortion.
