Muarojambi regency hit by chikungunya

Muarojambi regency in Jambi has experienced an outbreak of a dengue fever-like disease that has affected at least 23 people over the past 10 days.


Chikungunya is a mosquito-borne disease with symptoms similar to those of dengue fever, including high fever and severe joint pain.
Because of the high number of people affected, officials in the regency have declared it a "special incident".

"The declaration of a special incident was based on the number of people infected over such a short time period," said Aspan Effendi, head of the Muarojambi Health Office.

According to data from the health office, 23 people are currently being treated for chikungunya at Raden Mattaher Hospital in Jambi city.
Most of those affected are residents of the Puri Masurai II housing complex in Jambi Luar Kota district.

One person, identified as Syurono, 32, has reportedly died of the disease. However, the health office is still running tests to determine the exact cause of death.

First recognized in its epidemic form in East Africa in 1952, chikungunya has continued to cause major epidemics in Africa, India and Southeast Asia. Also known as epidemic polyarthritis and rash, or buggy creek virus, the first case reported in Indonesia was in Samarinda, East Kalimantan, in 1973.

There were reported cases of chikungunya in Yogyakarta in 1983 and then the disease seemingly disappeared from the country for almost 20 years until 2001, when outbreaks were reported in Muara Enim in South Sumatra, Aceh and Bogor in West Java. In 2002, cases were reported in Bekasi in West Java, and Purworejo and Klaten in Central Java.

Chikungunya is caused by a self-limiting febrile virus that is transmitted through the bite of the Aedes aegypti or Aedes africanus mosquito.
The illness usually lasts for three to 10 days, though the joint pain can remain a problem for weeks to several months after the initial phase. While painful, chikungunya rarely results in death.

Since there are currently no antivirals available, prevention of the disease focuses on controlling mosquito populations and avoiding mosquito bites. Those who do contract the disease are advised to take plenty of rest and eat properly.

Aspan said his office was spraying for mosquitoes in those areas affected by the outbreak.

One of my worst fears may have just been realized.

I have always been concerned that once Chikungunya reached Indonesia it would become almost impossible to effectivly track H5N1 cases due to the problems associated with handling just Chikungunya.

It should be remembered that Chikungunya presents like Dengue and H5N1 can present like Dengue, therefore H5N1 can present like Chikungunya.

All efforts, and I mean ALL efforts should be put into place to attempt to control the Chikungunya outbreak, otherwise it will be almost impossible to maintain a handle on the H5N1 situation.

Unfortunalty we have not been given much by those who have done the sequencing on this virus. All that has been said is that there was one, potentially two mutations. The first mutation allowed the virus to be transmitted by more than one type of mosquito. The second mutation that was mentioned was said to have allowed the virus to be transmitted from a mother to child, possibly through either childbirth or breastfeeding (I can't recall the exact event). But very little information has been provided about either of these mutations, only that there has been mutations.

I find this to be rather amazing considering the speed and spread of this disease.

If I had a suggestion it would be that any doctors or those in the medical profession push their government to provide them full disclosure on this virus.

You can start by requesting the information from the French government because they have all the data that exists from the La Reunion outbreak earlier in the year.

Genome Microevolution of Chikungunya Viruses Causing the Indian Ocean Outbreak

Background

A chikungunya virus outbreak of unprecedented magnitude is currently ongoing in Indian Ocean territories. In R?union Island, this alphavirus has already infected about one-third of the human population. The main clinical symptom of the disease is a painful and invalidating poly-arthralgia. Besides the arthralgic form, 123 patients with a confirmed chikungunya infection have developed severe clinical signs, i.e., neurological signs or fulminant hepatitis.
Methods and Findings

We report the nearly complete genome sequence of six selected viral isolates (isolated from five sera and one cerebrospinal fluid), along with partial sequences of glycoprotein E1 from a total of 127 patients from R?union, Seychelles, Mauritius, Madagascar, and Mayotte islands. Our results indicate that the outbreak was initiated by a strain related to East-African isolates, from which viral variants have evolved following a traceable microevolution history. Unique molecular features of the outbreak isolates were identified. Notably, in the region coding for the non-structural proteins, ten amino acid changes were found, four of which were located in alphavirus-conserved positions of nsP2 (which contains helicase, protease, and RNA triphosphatase activities) and of the polymerase nsP4. The sole isolate obtained from the cerebrospinal fluid showed unique changes in nsP1 (T301I), nsP2 (Y642N), and nsP3 (E460 deletion), not obtained from isolates from sera. In the structural proteins region, two noteworthy changes (A226V and D284E) were observed in the membrane fusion glycoprotein E1. Homology 3D modelling allowed mapping of these two changes to regions that are important for membrane fusion and virion assembly. Change E1-A226V was absent in the initial strains but was observed in >90% of subsequent viral sequences from R?union, denoting evolutionary success possibly due to adaptation to the mosquito vector.

Conclusions

The unique molecular features of the analyzed Indian Ocean isolates of chikungunya virus demonstrate their high evolutionary potential and suggest possible clues for understanding the atypical magnitude and virulence of this outbreak.

Add this to the already confusing situation in the "earthquake zone" and we have one big invisible mess.

It will be a miricle if we see a meaningful outbreak until it's "out of the box".

PS - back from a glorious time volunteering my time at a sleep away camp. Almost forgot about BF - NOT!

Have they released any information on the second mutation that went from mother to child?
