So a CFR of about 9.4% (counting only hospitalizations and lab-testing).

During the Pandemic years of 1918-1920, how were CFR's determined? ...by their severity and hospitalizations? They would have had no way of knowing if influenza was present if it was mild, or without fever (if truly there were such cases).

The number of deaths as a percentage of hospitalizations has been between 5% and 10% since the beginning. If we see this start to increase beyond 10%, that would be an indicator that something significant has changed, IMHAEO (In My Humble Amatuer Epidemiological Opinion).
