SASKATOON—Saskatchewan health officials say a man who recently travelled to western Africa is seriously ill in hospital and one of the possible diagnoses they are considering is Ebola hemorrhagic fever.

Dr. Denise Werker, deputy chief medical health officer, said there is fear an outbreak of the Ebola virus has spread to Liberia, where the man was travelling.

“All we know at this point is that we have a person who is critically ill who travelled from a country where these diseases occur,” she said.

A person returning to Canada from western Africa has been hospitalised after displaying symptoms consistent with those of the Ebola virus that has killed 59 people in Guinea.
A health official said the person is critically ill.
"All we know at this point is that we have a person who is critically ill who traveled from a country where these diseases occur," Denise Werker, joint director of health in Saskatchewan province in western Canada, told reporters.
She said the casualty had been in Liberia and had developed the symptoms after landing in Canada.
He or she would not have been contagious when travelling and was now in isolation, pending test results.
Aid workers and health officials in Guinea are battling to contain west Africa's first outbreak of the deadly Ebola virus as neighbouring Liberia reported its first suspected victims.
The Liberian cases, if confirmed, would mark the first spread of the highly contagious pathogen into another country.

Tests Say No Ebola for Woman in Canada

But doctors at a Canadian hospital said at a news conference today that preliminary tests on a 32-year-old woman in their care did not have the hemorrhagic virus.

"The Ebola testing to date has been negative," Dr. Mark Loeb, a disease specialist at Henderson General Hospital, said.

The unidentified woman, who had arrived form Congo this weekend, had been exhibiting symptoms "possibly compatible" with diseases like Ebola.

"This lady came into the hospital with symptoms of a serious infectious disease and was recently arrived from Africa," Dr. Ron St. John, executive director of the Center for Emergency Preparedness and Response, told ABCNEWS, "[but], there is a long list of possible diseases, both African and non-African ? that might be the cause."

A man who recently travelled to Western Africa is in critical condition in hospital in Saskatoon, according to provincial health officials.

The nature of the illness was not released, but the man has a high fever, in addition to other symptoms. Officials say a diagnosis has not been confirmed.

Saskatchewan's deputy medical health officer Dr. Denise Werker said on Monday [24 Mar 2014] that the patient -- who had visited Liberia -- was being examined for a suspected case of viral hemorrhagic fever.

[Clearly, more information from reliable sources would be greatly appreciated, including locations of travel within Liberia as well as more information on the clinical presentation and laboratory results.

The details provided in the flurry of media reports that have accompanied this announcement do not provide information on the clinical picture of the case other than "high fever and other symptoms" and that the man was in Liberia, and that viral hemorrhagic fever (VHF) is being considered in the differential diagnosis.

Given the recent announcement of the ebolavirus outbreak in Guinea and the suspected geographic spread into neighboring Liberia, any person returning from this general area with a high fever and signs of a serious illness should be evaluated for possible viral hemorrhagic fever.

Ebolavirus disease has not been previously identified in Liberia, and the reports of suspected cases there (in Liberia) related to the confirmed outbreak in neighboring Guinea have not as yet been confirmed. Other possible VHFs known to occur in Liberia include Lassa fever and yellow fever.

While current reports state that Ebolavirus disease has not been reported in Liberia, a review of the history of Ebolavirus disease in the ProMED-mail archives revealed that, in 1994, there was a single case of Ebolavirus disease confirmed in Cote d'Ivoire, which involved the isolation of a new species of the virus called Cote d'Ivoire Ebola virus, in a woman who had done a necropsy on a chimp that had died of Ebolavirus disease (see http://www.eurosurveillance.org/ViewArticle.aspx?ArticleId=342 for more details). In addition, in 1994, there was a WHO report posted on ProMED-mail of a 25-year-old male from a village in Liberia who was admitted to hospital in the Tabou Prefecture (south of Tai National Park) in whom a diagnosis of Ebolavirus infection was confirmed by serological tests done at the Institute Pasteur in Paris (see Ebola - Cote d'Ivoire/Liberia 19951219.1294).

It would be interesting to know whether the patient had measles... in addition to a detailed picture of symptoms.

There is nothing to strange to elucidate the clinical presentation of the illness affecting this traveler, or not?

Other possible ailments include malaria, dengue fever, typhoid, pneumonia, meningitis, and other infections. It will be Friday at the earliest before test results are available, and if the diagnosis doesn't present a public health hazard, medical health officers may not share that diagnosis publicly.

I don't think it's malaria or dengue; that would be easy to determine in this setting. I think it's going to be something considerably rarer or more difficult to detect. Also, the article indicates he took proper anti-malarial drugs prior to his trip.

The fact that meningitis, pneumonia, and typhoid (and at one point Ebola) are all in the list of diagnoses under investigation suggests to me that there is some kind of disseminated infection in his body, whatever its cause. I would assume that by "meningitis", they would be suspecting meningococcal menignitis, which is common in West Africa.

There are two diagnoses that come to mind that not in the list (perhaps because they have been excluded?) in the above article. One is influenza. The picture above does seem like a late-stage presentation of a severe case of influenza, and it is still flu season in the Northern Hemisphere. The other thing I wonder about is leptospirosis; his job as a geologist brings him in contact with soil, rocks, and other such things that could easily be contaminated with that bacterium.

Remote lay diagnosis of a single case of illness like this is exceedingly difficult; if his doctors, who have specialized training, laboratory tests, and access to the patient have not been able to diagnose his illness yet, we have even less of a chance.

I totally agree that there is no way remote diagnosis can be done. I just mentioned the dengue thrombocytopenia research with Vit E to get the word out on that. I'd like to see that research progress and hope it applies to other diseases and even to ITP.

But I'll admit that I suspect dengue since the geologist has had a previous infection so now he could have 'secondary dengue.' I think that can be worse than the first infection due to original antigenic sin kicking in in some cases and/or an overly robust immune response.

Our latest article says they only tested for the for Ebola, Lassa, Crimean-Congo and Marburg at the time it went to print.
Not all hemorrhagic illnesses have been ruled out ? just the four most serious ones.

We may not find out if this is secondary dengue since there is not a transmissible disease threat. But I did read that dengue is an
under-recognized emerging threat in parts of Africa.

Whatever the patient has, I hope he can heal from it.

Condition of ill Sask. man appears to be worsening after Africa trip, friend says

A friend of Rod Ogilvie says it appears the man’s condition is worsening at a Saskatoon hospital after his trip to Africa.

Ogilvie, president of Global Geological Services in Saskatoon, remains in hospital after recently returning from a trip to Liberia.

Last week, health officials said he does not have hemorrhagic fevers caused by the Lassa, Marburg or Crimean Congo viruses, though they hadn’t diagnosed the ailment that has him in critical condition.

George Sharpe, also a business partner of Ogilvie, visited him on Monday and said he remains in intensive care.

“They’re only letting close friends like me and his family in, and he doesn’t look good at all,” Sharpe said.

“Whatever it is, it’s something that is going to take him a long time to get through. Because he’s quite sick right now and it’s about as bad as it can get.”

Ogilvie is on a respirator and unable to communicate, Sharpe said.

The Saskatoon Health Region said it doesn’t have authorization to release information on any patient.

[This is now highly unusual. This man has been in a Canadian hospital for 10 days, remains critically ill, and it appears no one has any idea what is making him sick. I think we would have heard of this case by now, even without the spurious Ebola connection. I think we have to assume that whatever is making him sick isn't contagious, at least not as a severe illness, because he has been in Canada for perhaps two weeks without any contacts becoming seriously ill. I suspect this is going to be something extremely rare. It is not clear from the brief description whether this is still suspected to be infectious or not. - alert]
