EVENT DESCRIPTION

A monkeypox outbreak was reported in Cameroon on 15 December
2021 following confirmation of one case from Ayos health district,
in the Centre region. As of 17 February 2022, a total of 25 suspected
cases with three confirmed and two deaths (CFR =8.0%) have been
reported. Cases were reported from four health districts, namely
Ayos, Benakuma, Djoungolo, and Kumba across three regions of
Centre, North-West and South-West. The monkeypox virus was
detected in three (25.0%) of the 12 laboratory samples that have
been collected and analysed.

Among the four affected health districts, two (Ayos and Djoungolo)
are from the Centre region, with two suspected and one confirmed
case coming from Ayos health district and four suspected cases
and zero confirmed case from Djoungolo health district. The last
case was reported on 15 February 2022 in the Centre region from
Djoungolo health district. No death has been reported so far in this
Centre region.

In the North-West region, only Benakuma health district is affected,
with the highest number of cases, 10 (40%) out of the 25 total cases
reported. One death has been recorded in this region (CFR=10%)
and the last case was reported on 3 February 2022.

The South-West region is the second in number of cases reported
to-date, with nine cases out of the 25 (36.0%) and one death
recorded (CFR= 11.1%). Of the nine suspected cases, two were
confirmed. All cases reported in this region are from Kumba health
district, with the last case reported on 29 December 2021.

PUBLIC HEATH ACTIONS

The Ministry of health has conducted advocacy for resource
mobilization for response and prevention activities

Rapid case investigations have been conducted in the affected
areas with sample collection for laboratory confirmation and
case management of the reported cases

In-depth case investigation protocol has been drafted
Intensification of event-based surveillance for early case
detection in the areas with security challenges is ongoing.

SITUATION INTERPRETATION

Monkeypox is a rare zoonosis which occurs sporadically in
forested areas of Central and West Africa. Monkeypox cases
have been regularly reported in Cameroon since the 1970s.
Between 2020 and 2022, more than half of the regions in the
country reported at least one case of monkeypox. The majority
of cases recorded for the ongoing outbreak have been reported
in the North-West and South-West regions that are currently
characterized by insecurity and humanitarian crisis.

PROPOSED ACTIONS

Identified gaps so far should be covered in order to optimize
response to this outbreak especially in the areas with difficult
access due to security challenges.

Risk communication activities and community mobilization
to reduce risk of animal-to-human and human-to-human
transmissions should be intensified

Active case finding, contact tracing and case management
should be also intensified.

Cameroon has reported 28 suspected cases with two deaths from four districts across three regions since the beginning of 2022. Three cases have been laboratory confirmed from Kumba Health District in the South-West (2) and Ayos Health District in the Centre Region (1).

Several cases of monkey pox have been identified in Cameroon.

In the Centre, North-West and South-West regions of Cameroon, five cases of monkey pox have been identified. These figures were revealed on the occasion of the 2nd Zoonoses Day organized in Yaoundé on the theme “Monkey pox in Cameroon”.

According to the Deputy Director in charge of the fight against epidemics and pandemics at the Ministry of Health, Cameroon is one of the endemic countries for monkeypox. In 2022, more than 30 suspected cases notified by the health system, five came back positive.

In this context, the health authorities have reinforced surveillance for the rapid detection of cases and their management.

As of 5 August 2022, Cameroon has notified 34 suspected cases of monkeypox from six districts across three regions, since the beginning of 2022, including two deaths (CFR 5.9%). Thirteen human samples have been collected and seven cases have been laboratory-confirmed from Ayos Health District (4) in the Centre Region, Kumba Health District (2) in the South-West Region and Benakuma Health District in the North-West Region (1). Males and females are equally affected and the median age is 17.3 years (range 1-36 years).

A monkeypox outbreak has been confirmed in the town of Mbonge, health authorities in Cameroon say, but armed separatists are preventing workers from investigating suspected cases.

Cameroon government officials say health workers have been deployed to the districts of Kumba and Mbonge to communicate to hundreds of civilians with suspected monkeypox infections to immediately isolate and avoid contact with other people and animals, including pets.

Kumba and Mbonge are districts located in Cameroon's English-speaking southwest region near the border with Nigeria.

Emmanuel Lenya Nefenda, the highest ranking Cameroon public health official in Kumba, said civilians are being educated after a suspected monkeypox infection was confirmed in Kumba. From 16 September to 18 October 2022, two new suspected cases of Monkeypox have been reported, each in Kumba and Mbonge districts of South-West region.
Between 1 January and 18 October 2022, the country has notified 48 suspected cases of monkeypox from seven districts across five regions, including two deaths (CFR 4.2%). A total of 35 human samples have been collected and ten cases have been laboratory-confirmed from Centre (4), South-West (2), North-West (2), Littoral (1) and South (1) regions. Males are slightly more affected than females (sex ratio M/F 1.4). Ages range from six months to 52 years with a median of 11 years; the under-five represent the most affected age group (n=15, 31.2%), followed by the 21-30 years (n=14, 29.2%).

From 19 October to 6 December, 58 new suspected cases of Mpox have been reported from South-West (40), North-West (7), Littoral (4), Centre (3), East (3), and Far-North (1) regions. Between 1 January and 6 December 2022, the country has notified 106 suspected cases of Mpox from 22 districts across seven regions, including three deaths (CFR 2.8%). A total of 77 human samples have been collected and 18 cases have been laboratory-confirmed from South-West (9), Centre (4), North-West (4), and South (1) regions. Males are slightly more affected than females (sex ratio M/F 3/2). Ages range from six months to 53 years with a median of 12.5 years; the under-five represent the most affected age group (n=21; 19.8%), followed by the 21-30 years (n=17; 16.0%).
