Weeks 40 & 41: Low Activity
Influenza A (2009 H1), A (H3), and B have all been identified in the first two weeks of this influenza season. Of 810 specimens tested by the RLN and sentinel labs during Weeks 40 and 41, 8 (1.0%) were positive for influenza A and 1 (0.1%) was influenza B. Most of the positive results have been reported from Southern California.

The California Department of Public Health Viral and Rickettsial Disease Laboratory (CDPH-VRDL) has not tested any specimens for antiviral resistance to date. No California specimens have been strain typed by the Centers for Disease Control and Prevention (CDC) to date.

CDPH has received no reports of laboratory-confirmed influenza-associated deaths among patients under 65 years of age.

There were no reports of acute respiratory outbreaks due to laboratory-confirmed influenza in Weeks 46 and 47.

The percentages of Kaiser hospitalizations for P&I in northern California were similar in Weeks 46 (4.7%) and 47 (5.2%) compared to Week 45 (5.0%). In Southern California, the percentages of Kaiser hospitalizations for P&I increased in Weeks 46 (4.4%) and 47 (4.9%) compared to Week 45 (3.6%)

Circulation of a novel swine-origin triple reassortant H3N2 influenza (S-OtrH3N2) virus has been reported in four states, including Pennsylvania, Indiana, Maine, and Iowa. Of the ten reported cases, all except one have been children; two had underlying chronic medical illness and were briefly hospitalized. While almost all have had either direct or indirect exposure to swine, investigation of a recently reported influenza-like illness (ILI) cluster of three children in Iowa has suggested limited person-to-person transmission.

All of the cases were identified by routine influenza surveillance and testing at a state public health laboratory. In California, the Respiratory Laboratory Network (RLN) provides the state’s front-line for testing for influenza. All RLN laboratories are able to screen for the S-OtrH3N2 influenza virus; to date, the S-OtrH3N2 virus has not been identified in California, but testing has been limited due to the low number of influenza specimens submitted to RLN laboratories. RLN laboratories are encouraged to work with the California Department of Public Health Viral and Rickettsial Disease Laboratory (CDPH-VRDL) and their local partners to perform broad influenza testing in different populations to monitor for the emergence of the S-OtrH3N2 virus in California.

Of 1,012 specimens tested by the RLN and sentinel laboratories during Week 48, 4 (0.4%) were positive for influenza B and 8 (0.8%) were positive for influenza A; 1 (12.5%) influenza A specimen was subtyped as A (H3).
The CDPH-VRDL has not tested any specimens for antiviral resistance to date.
One specimen from a California resident has been strain-typed as A/Perth/16/2009-like, the influenza A (H3N2) component of the 2011-12 influenza vaccine for the Northern Hemisphere.

Fourteen specimens from California residents have been strain-typed this season; all but one matched with components of the 2011-12 influenza vaccine for the Northern Hemisphere.

No suspected or confirmed influenza A (H3N2)v [variant influenza A (H3N2), formerly called swine-origin triple reassortant A (H3N2)] has been detected in California to date.

The proportion of human metapneumovirus (hMPV) detections continued to increase in Week 1 (6.8%, compared to 5.3%, 5.2%, and 2.1% in Weeks 52, 51, and 50, respectively). Likewise, respiratory syncytial virus (RSV) detections increased to 11.3% in Week 1 compared to 9.8% and 6.1% in Weeks 52 and 51, respectively.

CDPH received one report of a laboratory-confirmed influenza outbreak at a skilled nursing facility in the San Francisco Bay Area region. A total of 4 residents were ill; 3 were confirmed positive for influenza B by PCR (1), culture (1) and rapid test (1). No hospitalizations or deaths occurred.

Of 1299 specimens tested by the Respiratory Laboratory Network (RLN) and sentinel laboratories during Week 2, 77 (5.9%) were positive for influenza; of these 12 (15.6%) were influenza B and 65 (84.4%) were influenza A. Of the 65 specimens that tested positive for influenza A, 17 (26.2%) were subtyped as seasonal A (H3), 8 (12.3%) were subtyped as 2009 A (H1), and 40 (61.5%) were not subtyped.
