Dengue fever blunts Brazil's tourism boom

By Los Angeles Times

Brazil is booming. The currency is soaring, people are buying houses and cars at a record pace, global financiers are keen to invest and tourists keep arriving. The country seems poised to acquire official First World status.

But residents of Rio de Janeiro, Brazil's self-proclaimed "city of wonders," are worried and angry about a Third World affliction ? dengue fever, the tropical disease spreading in epidemic fashion here.

This year, dengue has killed about 90 people in the state of Rio de Janeiro and sickened more than 93,000. Most cases occurred in the city, Brazil's principal tourist attraction. Dengue has become an annual scourge in a broad swath of tropical Brazil, but this year's epidemic seems likely to be Rio's deadliest in recent history.

Rather than blaming the stripe-bellied Aedes aegypti mosquito that spreads the ailment, many cariocas, as Rio residents are known, are lashing out at what they call a confused and belated government response. Critics say officials were slow to fumigate and take other action during the Southern Hemisphere's summer, when heavy rains created optimum mosquito breeding grounds.

With the number of cases soaring and clinics overwhelmed, officials this month belatedly directed more than 1,000 soldiers to join firefighters, volunteers and others patrolling streets and inspecting tens of thousands of homes in a much-publicized anti-dengue offensive. The teams fumigate and destroy stands of still water where mosquitoes breed.

The dengue epidemic, like the gunbattles pitting police against traffickers in the city's slums, or favelas, has become a nasty blemish on Rio's glamorous image. Some accuse the government of trying to keep a lid on dengue to avoid scaring off tourists.

There have been scattered reports of reduced hotel occupancy rates, but no mass cancellations. Indeed, life continues at a hedonistic pace along chic beachfronts such as Copacabana and Ipanema. Few signs warn visitors of the dengue threat at major hotels, the international airport or at approaches to tourist icons such as Corcovado and Pao de Acucar (Sugar Loaf) mountains.

But word is out. The U.S. and other embassies have advised visitors to take precautions, such as wearing long-sleeved shirts and avoiding short pants.

Dengue fever, a viral disease transmitted by infected mosquitoes, is a flulike ailment that causes headaches and elevated temperatures but is usually not fatal. However, physicians worry that a more deadly variant may be spreading here and in neighboring Paraguay, which also faces a dengue crisis.

There is no vaccine for dengue. Treatment usually involves rest and increased intake of fluids.

Minister of Health says that Brazil lost the war against dengue

F?BIO GUIBU
Agency Leaf, in Maceio

The Minister of Health, Jose Gomes Tempor?o, said yesterday in Miami, that Brazil "has lost the war" against dengue in the last ten years and, now, will have to live with the epidemic of the disease by "many, many years" .

According to Tempor?o, in 2009, the Ministry of Health works with the possibility to occur in the Northeast, North and the South East, the situation as critical as currently found in the River "Certainly [the epidemic] does not end so soon," said the minister. "It's a question of many, many years that Brazil vai have to live with dengue and with the situation."

"In 1990 we had 1,700 municipalities with the presence of the mosquito [Aedes aegypti] today, we have 4,000," said Tempor?o. "We, in ten years, we lose the war. Now, we have to work very hard, betting including the emergence of a vaccine."

To the minister, only the discovery of a vaccine will enable the effective control of the disease in the country. "Between five and ten years it is possible that we have this vaccine," estimated. "There would be a permanent solution." While the drug is discovered, Tempor?o said that the government will work to "minimize the deaths and the number of cases."

The minister also complained of alleged lack of commitment of the population in the fight against dengue. "You are missing mobilization, raising the ass of the chair and work," he said. The election year is another issue that worries. "There is a risk of disengagement by political issues," he said.

Figures

The Ministry of Health also reported yesterday updated figures on dengue in the country. There were 230,829 cases reported from January to April this year, before 258,795 cases in the same period of 2007 - a drop of 10.8%. Rio starts lifting of possible outbreaks of dengue outbreak in 2009

LUISA BELCHIOR
Collaboration for IRIN, in Rio

Firemen of Rio started this Monday a survey to diagnose the main points of the state with the potential for outbreaks of dengue. Call of Lira (Removal of the Index of Fast Infestation), the study is usually done across the country in October by the Ministry of Health, but this year was anticipated because of municipal elections and the epidemic of dengue recorded in Rio since the beginning of year.

In total, 400 firefighters and 175 health workers began to learn to collect, store and identify possible larvae of the Aedes aegypti. They will be training until the next day 30 and then will go to the streets collecting data for the survey.
The action, according to the State Secretariat of Health of the River, is already part of the programme to prevent a new epidemic of dengue in 2009.

Also new this year, according to the government of the state, the result of the mapping of the most likely outbreaks of epidemic of dengue will be distributed to the population. Previously, state and local governments across just delivered the results to the Ministry of Health carry out the national survey.

The Lira 2008 indicates that at least 16 Brazilian states, plus the Federal District, have high risk of developing an epidemic of dengue. They are: Rio Grande do Sul, Santa Catarina, Sao Paulo, Rio de Janeiro, Minas Gerais, Goias, Bahia, Pernambuco, Rio Grande do Norte, Ceara, Piaui, Maranhao, Para, Amapa, Amazonas and Roraima, plus the Federal District. The ministry reported that the notice states.

To make the diagnosis, the Lira takes into account factors such as movement of types of virus in the country, information provided by municipal officers and state surveys in the homes, density and mobility of the population and climatic conditions, urban infrastructure, quantity of mosquito and the prevention of work of each state.

In Rio, there are 109 deaths from dengue and 145,350 confirmed cases of the disease this year alone,
according to the latest assessment of the State Secretariat of Health of the River, published in last Thursday (15).

Dengue epidemic in Rio easing but 10.635 cases in May

The dengue epidemic in the Brazilian state and city of Rio do Janeiro added another 10.635 cases last week and totaled 110 deaths since the beginning of the year according to the latest report from the city?s sanitary authorities.

The city?s Health Secretariat said in a release that another 124 deaths are been investigated to confirm if they were caused by the viral disease transmitted by mosquitoes.

Since January the number of dengue cases reported in Rio totals 155.985, which is almost half the national total for the Brazilian population of 180 million. Of those, 84.131 are concentrated in the famous city of Rio do Janeiro followed by the world famous seaside resort of Angra dos Reis with 10.652 cases.

However sanitary authorities also pointed out that the peak of cases was last March with 63.196 cases and has since steadily decreased with 40.364 in March and 12.256 so far in May.

The incidence of the epidemics is significantly diminishing, as had been anticipated by sanitary authorities, because of the fall and lower temperatures which are not favorable for the reproduction of the ?aedes aegypi? mosquito.

Besides last March the Brazilian three services were ordered into the city with camp hospitals to help with the mounting number of cases which had overwhelmed the precarious municipal hospital system.

According to the US Center for Disease Control and Prevention, dengue and dengue hemorrhagic fever are caused by one of four closely related, but antigenically distinct, virus serotypes (DEN-1, DEN-2, DEN-3, and DEN-4), of the genus Flavivirus.

DF and DHF are primarily diseases of tropical and sub tropical areas, and the four different dengue serotypes are maintained in a cycle that involves humans and the Aedes mosquito. However, Aedes aegypti, a domestic, day-biting mosquito that prefers to feed on humans, is the most common Aedes species. Infections produce a spectrum of clinical illness ranging from a nonspecific viral syndrome to severe and fatal hemorrhagic disease.

Important risk factors for DHF include the strain of the infecting virus, as well as the age, and especially the prior dengue infection history of the patient.
