Isolation of DENV serotype 4 in Roraima / Brazil

On July 30, 2010, the Secretary of Health of Roraima (SES / RR) notified the Secretariat of Health Surveillance / SVS a suspected case of dengue serotype by DENV 4 virus. The case is indigenous to Boa Vista, state capital, and was detected in a Unity Sentinel monitoring viral. The initial diagnosis was made by the RT-PCR and Virus Isolation, processed at the Central Laboratory of Roraima.
Immediately after the knowledge of this result, the Departments of Health
Roraima / SESAU-RR and Boa Vista (SEM / BV) implemented the measures recommended epidemiological surveillance and vector control in order to prevent a possible spread of virus. Among the cast of measurements made there are the following:
- Active search of cases with suspected dengue, house to house in an area of nine blocks around the index case;
- Collect samples from patients with febrile syndrome in areas near the index case for identify other suspected cases;
- Application of insecticide and costal space using equipment attached to vehicles in order to decrease the density of adult Aedes aegypti;
- Intensification of mechanical removal of the breeding and use of larvicides
chemicals to remove larval forms of Aedes aegypti.
Because the only record of the four DENV serotypes have occurred in Brazil 28 years ago, the detection of a suspected case requires laboratory confirmation needed for different techniques: molecular biology, achievement of the isolates in C6/36 cell cultures (Gold standard technique), followed by sequencing of the viral genome to determine the phylogenetic relationship and origin of the lineage of DENV 4 Boa Vista in stock. These tests are being conducted by the National Reference Laboratory and the International on Arboviruses, Instituto Evandro Chagas (IEC), the Secretariat of Health Surveillance of Ministry of Health

Importantly all cases, including possible future cases, should await outcome - definitive - in all the necessary laboratory techniques, described above.
Despite the apparent decline in the number of dengue cases in Roraima,
epidemic in recent weeks, this Technical Note, is intended to alert the
Surveillance of the State Health Department from all over Brazil, to the intensification of efforts to control dengue. Special attention should be given to actions relating to isolation of serotype.
Additionally, in observing the International Health Regulations, the
SVS / MS has been in PAHO and WHO informed of all steps that the Government Brazil (Federal, State and Municipal) has taken.
The SVS / MS ratifies all proactive measures to contain the SES / RR
and SMS / BV has been adopting since the time of suspicion of the index case.

Two laboratory techniques were used in all samples: the first survey of dengue virus genome by RT-PCR complemented by conducting research of virus inoculated into C6/36 cells, and latter technique considered the gold standard for confirmation of serotype DENV 4.
II - Summary of confirmed cases of DENV 4 with a positive result by the technique of cell isolation C3/36
The epidemiological investigation of the trajectory of the three confirmed cases of DENV 4 autochthony confirmed infection in Boa Vista. III - Summary of suspected cases with four DENV positive by RT-PCR
The epidemiological investigation of the trajectory of the nine suspected cases confirmed the four DENV autochthony Infection 7 cases in Boa Vista. Investigations to define autochthony infection and Sing in the towns of
Normandy is underway. The median age of cases was 23 years (10-55), all being classified clinically as dengue fever. The evolution was to cure without any admission of cases.
