and could be produced at scale using mRNA platforms if H5N1 begins spreading in humans140.
The severity of a future H5N1 pandemic remains unclear. Recent human infections with H5N1
2.3.4.4b viruses have a substantially lower case fatality rate compared to prior H5N1 outbreaks
in Asia, where half of people with reported infections died14. The milder symptoms in US
farmers have been attributed to the route of infection through the eye65 and absence of viral
pneumonia in the lung. Whether B3.13 viruses cause less severe disease in humans or whether
mild cases are simply under-detected in Asia is unclear
due to case ascertainment bias142.
Older people appear to have partial immunity to H5N1 due to childhood exposure (“imprinting”)
to seasonal H1N1 and H2N2 viruses, whereas younger people born since the 1968 H3N2
pandemic may be more susceptible to severe disease in a H5N1 pandemic143. Some degree of
cross-reactivity between H5N1 and the avian-origin N1 neuraminidase that has circulated in
humans since the 2009 pandemic may also provide partial protection144. At the same time,
symptoms and disease severity could change if B3.13 viruses further adapt to infect the
respiratory tract.


Going forward, we know more about H5N1’s global distribution (Figure 1), non-human
host range (Figure 2), and genetic diversity (Figure 3) than virtually any other zoonotic
pathogen. Still, most H5N1 testing is conducted in dead or severely ill animals. One lesson from
the COVID-19 pandemic is that symptomatic cases that result in severe disease are clinically
important, but unobserved subclinical infections can be important in transmission and fuel
epidemics at a population level
. The H5N1 panzootic has been defined by powerful visuals of
beaches littered with sea lion carcasses or barns of ill dairy cows wasting away after going off
feed. But what keeps scientists up at night is the possibility of unseen chains of transmission
silently spreading through farm worker barracks, swine barns, or developing countries, evolving
under the radar because testing criteria are narrow, government authorities are feared, or
resources are thin.
A second lesson from the COVID-19 pandemic is not to underestimate the
importance of human behavior, culture, and economic context. New technologies like mRNA
vaccines, next-generation sequencing, and CRISPR-Cas diagnostics provide rapid, flexible
tools for outbreak response, but are of little use when they are not allowed on the farm.​

A Bird Flu Vaccine Might Come Too Late to Save Us from H5N1

If the influenza virus infecting cattle workers starts a pandemic, help in the form of a vaccine is months away

H5N1 bird flu is here. It’s moving from animals to people in ways not seen before. It’s spreading to new species and new places, and this spread is largely happening under the radar.

So far 36 human cases have been reported in six U.S. states: California, Colorado, Michigan, Missouri, Texas and Washington. Those are just the cases that health officials know about. Not all states are testing people or animals. The tests for the virus are flawed and in short supply.

We’ve somehow not learned this valuable lesson to prepare early for a bird flu pandemic, even though this particular influenza virus has been scaring doctors, scientists and public health experts for far longer. H5N1 is a well-known and well-characterized virus, and in theory there could be a large bank of vaccines ready to go should it acquire the ability to spread easily from person to person.

But there isn’t. The U.S. currently has fewer than five million vaccines matched to the H5N1 strain that is circulating among cattle and occasionally infecting people right now. The federal government has contracts out that will stock up a supply of 10 million filled syringes, but not until spring of 2025. And because it will take two doses to protect, that’s enough to fully vaccinate just five million people. That’s less than 2 percent of the U.S. population—to say nothing of the rest of the world.

On Tuesday, Oct. 29, the USDA National Veterinary Services Laboratories also confirmed one of the farm’s five pigs to be infected with H5N1, marking the first detection of H5N1 in swine in the United States.

The livestock and poultry on this farm shared water sources, housing, and equipment; in other states, this combination has enabled transmission between species. Although the swine did not display signs of illness, the Oregon Department of Health and USDA tested the five swine for H5N1 out of an abundance of caution and because of the presence of H5N1 in other animals on the premises. The swine were euthanized to facilitate additional diagnostic analysis. Test results were negative for two of the pigs, and test results are still pending for two others.

I really hesitated to post this unsubstantiated report, however, I think the post raises important discussion topics even if it is considered as a hypothetical case.

H5N1 in BC

I wanted to share with this group since Covid conscious folks will actually care. According to a BCCH connected person I know, the patient is in intensive care. The patient has also been taken off of airborne precaution isolation (respirator masks required) and downgraded to droplet (surgical masks) despite there being no definitive research that is it now spread via aerosols. Very concerning.

USDA's Dereliction in Containing Bird Flu Could Cause a Calamitous Pandemic (Part 1)

An inherent conflict of interest -- USDA both regulating and promoting livestock industries -- prevents appropriate responses to outbreaks of infectious disease​.


When dairy cows in Texas started falling ill last Spring, alarm bells started to ring. Veterinarians had found feverish cows struggling to breathe and yielding barely any milk alongside rotting dead barn pigeons and dead cats that had eaten the pigeons or drunk contaminated milk. This nightmarish scene led veterinarians to suspect an unprecedented outbreak of H5N1 (bird flu) in cattle.

One veterinarian sent an urgent email in mid-March to the National Veterinary Services Laboratories (NVSL) in Ames, Iowa. Days went by with no response. Finally, on March 25, USDA’s lab confirmed that the H5N1 strain of avian influenza virus had indeed infected dairy cows in Texas and Kansas.

This revelation sent shockwaves through the scientific and agricultural communities. While H5N1 bird flu had affected various species globally — ranging from skunks and bears to sea lions — this marked the first known occurrence in cattle.

The stakes are huge: Not only could an outbreak threaten the dairy and beef industries, but unchecked it could spread the virus among cows with the potential for transmission to pigs –– a species in which different viruses are able to mix and form new variants during simultaneous infection, creating an unpredictable and potentially catastrophic situation.

The H5N1, a deadly strain of flu virus with the potential to mutate and spark a pandemic among humans, has been on epidemiologists’ radar for nearly two decades. Virologists have long warned that if bird and human flu viruses recombine during simultaneous infection in animals like pigs or cattle, it could produce the worst-case scenario: a new virus capable of human-to-human transmission, and which could spread readily because humans would have little or no immunity to it. The possible impacts of such an event are dire; the Gates Foundation’s Institute for Disease Modeling estimates that a global flu pandemic could claim up to 33 million lives within six months. Veterinarians assumed that once the virus was detected, federal agencies would immediately spring into action with interventions such as extensive testing and restrictions on the movement of cows, as they had in similar previous outbreaks. But that didn’t happen.

“Nobody came,” said one Western state veterinarian. “The government … didn’t know what to do so they did nothing.”

The government’s inaction has allowed H5N1 to spread with remarkably little attention. The virus has now affected at least 446 dairy herds in 15 statesand more than100 million birds, mostly commercial poultry, in addition to the documented human cases.

A report from the CDC and Michigan and Colorado public health officials released on November 7 found that a significant percentage of human H5N1 infections in Michigan and Colorado from June to August this year went undetected in dairy workers who worked on farms with cows that were confirmed positive for the virus. Among 115 farm workers who underwent blood tests in the two states, eight had evidence of recent infection in the form of antibodies — but only half could recall having symptoms. All eight had either been milking cows or cleaning the milking mechanisms," according to officials.

This, from a November 8 article in Fortune, summarizes the current state of the outbreak in humans:
n the U.S., human H5N1 cases in California and Washington are on the rise. Nationally, 46 human cases have been documented and confirmedduring the 2024 outbreak, including a person in Missouri with no known exposure to either cattle or poultry, the two primary sources of exposure so far. All of the individuals have experienced only mild symptoms, such as conjunctivitis or cough, and none have been hospitalized.

Why the slow response?


Usually, the USDA’s responsibility to protect the public aligns with its mandate to advocate for the livestock industry. Not this time. Secretary of Agriculture Thomas Vilsack, who previously served as president and CEO of the U.S. Dairy Export Council, has been notably reluctant to publicly address the bird flu crisis.

Raising additional concerns about potential conflicting loyalties, during the World Dairy Expo, Vilsack sidestepped a reporter’s question about whether he would return to the dairy industry after his tenure at USDA. That fueled speculation about his and his department’s priorities. The overall situation has the stench of a conflict of interest if not outright malfeasance by a Cabinet secretary.

Adding to the complexity of the situation and other possible conflicts is the Biden administration’s newly established Office of Pandemic Preparedness and Response Policy (OPPR). Intended to coordinate swift responses to future outbreaks, the office struggled to manage an unexpected interspecies outbreak during an election year.

High pathogenicity avian influenza suspected on sub-Antarctic Marion Island

High pathogenicity avian influenza (HPAI) virus is suspected to have reached Marion Island, one of South Africa’s two sub-Antarctic islands in the southern Indian Ocean and is possibly responsible for causing mortalities in at least three breeding seabird species.

After an initial suspected case in a Brown Skua in mid-September 2024, another five suspected cases were found in early November 2024, involving three Wandering Albatross chicks and two Southern Giant Petrel adults. The virus can be transported long distances by migrating birds, and this is likely how the virus arrived on Marion Island.

Having spread across the globe since 2021, HPAI (H5N1) was detected in seabirds and marine mammals in South Georgia, southeast of South America, in October 2023. It reached Antarctica in February 2024 and suspected cases were reported from Southern Elephant Seals at Possession Island in the Crozet Archipelago, east of Marion Island, on 21 October 2024.

The Prince Edward Islands, comprising Marion Island and Prince Edward Island, are breeding and moulting sites for millions of seabirds, including almost half of the world’s Wandering Albatrosses and hundreds​…

Abstract

H5N1 high pathogenicity avian influenza virus has killed thousands of marine mammals in South America since 2022. Here we report epidemiological data and full genome characterization of clade 2.3.4.4b H5N1 HPAI viruses associated with a massive outbreak in southern elephant seals (Mirounga leonina) at Península Valdés, Argentina, in October 2023. We also report on H5N1 viruses in concurrently dead terns. Our genomic analysis shows that viruses from pinnipeds and terns in Argentina form a distinct clade with marine mammal viruses from Peru, Chile, Brazil and Uruguay. Additionally, these marine mammal clade viruses share an identical set of mammalian adaptation mutations which were also present in tern viruses. Our combined ecological and phylogenetic data support mammal-to-mammal transmission and occasional mammal-to-bird spillover and suggest multinational transmission of H5N1 viruses in mammals. We reflect that H5N1 viruses becoming more evolutionary flexible and adapting to mammals in new ways could have global consequences for wildlife, humans, and/or livestock.

The implications of sustained mammal-to-mammal transmission of H5N1 HPAI viruses could be far-reaching, both from a conservation and a public health perspective. From the standpoint of wildlife conservation, this is particularly concerning for endangered pinnipeds with limited geographic distribution such as Caspian seals (Pusa caspica) and Hawaiian monk seals (Neomonachus schauinslandi), among others80.

From a public health perspective, mammal-to-mammal transmission could be a stepping-stone in the evolutionary pathway for these viruses to become capable of human-to-human transmission87. As mentioned previously, some of the mutations found in the strains of the marine mammal clade are already known to be of concern. In particular, the mutation D701N in PB2 has been shown to compensate for the lack of the E627K mutation in PB2 in terms of improved viral growth in mammalian cells and enhanced aerosol transmission of H3N2 and H5N1 viruses88. The fact that the H5N1 HPAI virus detected in a human case in Chile belongs to the marine mammal clade and is transmissible among ferrets55,89 highlights the potential risk to public health. In addition, the possibility of zoonotic strains resulting from the reassortment between HPAI viruses and other IAV strains infecting pinnipeds is also of concern43,90, especially since pinnipeds are known to occasionally host human-like IAV strains45,46,47. However, the phenotypic effects of mutations in other gene segments found in the H5 viruses from our study (Supplementary Table 4) are not yet known, and further research using in vivo mammalian models is needed to determine whether they can enhance virulence and/or transmission.

In conclusion, the world has seen a concerning increase in the number of H5N1 HPAI detections in mammals since 2023, including notable outbreaks such as the one reported here. Amidst growing evidence that mammal-to-mammal transmission played a role in H5N1 HPAI outbreaks in dairy cows in North America57,91 and in fur farms in Europe92,93, the outbreak among elephant seals in Península Valdés represents another case where mammal-to-mammal transmission was potentially involved in the spread of H5N1 HPAI infections, this time in free-ranging wildlife. Genetic drift and shift in IAVs are stochastically-driven phenomena94, and mutations that increase transmissibility between mammals are more likely to occur in mammals than birds95,96. Therefore, the recent increase in H5N1 HPAI circulation in mammals is a warning that should not be ignored. Moving forward, HPAI management requires holistic strategies that recognize the interconnectedness of human, animal, and environmental health as well as safeguard biodiversity, promote sustainable practices, and enhance resilience globally to emerging infectious diseases.

A teen in British Columbia remains in critical condition in what it's believed to be Canada's first human case of the avian flu, Dr. Bonnie Henry says.

Henry provided an updated Tuesday (Nov. 12) after the province said on Saturday that a teen was the first presumptive positive case of H5 avian influenza detected in B.C.

She said it's still being called a "presumptive positive" case until it can be confirmed at the country's national laboratory, but her office has confidence that it is a bird flu case. Confirmation is expected in the next few hours, she added.

The teen, who is from the Fraser Health region, was admitted to BC Children's Hospital on Friday.

Henry said the initial symptoms started with conjunctivitis.

The public health agency said it was notified by B.C. health authorities on Saturday that the teenager tested presumptive positive for the avian flu, and Tam said field epidemiologists are looking at potential sources for the teen’s exposure….

“We are unable to speak to the patient because the patient is quite sick,” Tam said. “And so, I think some of the information is necessarily second-hand from the family members.

how long did he have conjunctivitis?
did you touch any birds?
did you touch any animals ?
have you been around anyone sick?
did a bird crap on you ?
did you eat any raw eggs or milk?
where have you been exactly?
he was sick and didn’t go to school.. and wasn’t tested till he was way too sick and already at the hospital.

I don’t think we are getting the whole story if it just came out he can’t communicate, no wonder they say we may never know where they got it.. because prognosis is very poor. If already has conjunctivitis, we getting close to the brain. Can’t communicate at all and they are saying they are sending prayers.

Speaking of risk I looked at the #H5N1 D1.1 wild bird seqs from BC they are wild type for S247 N1 comes low path wild birds (with N1) closest match to the D1.1 is an H1N1 low path from BC A/Mallard/BC/AIVPHL-2360/2024 collected 8/20/24 WITH S247N H5N1 D1.1 may be mixture​

While there is an ongoing H5N1 outbreak in dairy cattle in the United States, no cases of avian influenza have been reported in dairy cattle in Canada, and there has been no evidence of bird flu virus in milk samples, reports Xinhua news agency.

Furthermore, the clade of H5N1 virus detected in dairy cattle in the United States differed from that found in the human case reported in British Columbia, PHAC said.
