Hyogo Prefecture
A male high school student who had contacted a confirmed patient in the same school had a fever of 37.4 d C Wednesday. He stays home, recovering.
A girl under 10 who had ILI (cough, nasal fluid, nasal congestion) from May 23. On May 25, she had a fever of 38.3 d C and was rapid-tested A type flu positive. First PCR test didn’t give a clear result but the second test confirmed her infection yesterday.
A (high school?) student in late teens was confirmed infected with novel flu. His school had been closed already. He stays home.

School Closure
Senzoku Gakuen high school in Kawasaki of which two female students were infected during their trip to NY to attend a UN session simulation for the young, disclosed that the school received about a hundred mails and telephone calls accusing or bushing the school and the infected students. Some of the bushing on the internet stated “the students didn’t attend the UN session simulation” or “they would have gone to NY just for pleasure.”

Takayoshi Maeda, school principal, stated mental health care will be provided by psychiatrists and school councellors to students who fear accusations and bushing.

But the principal also revealed that the school received from all over Japan about a thousand mails and telephone calls supporting and cheering up the school and the students, which they found very encouraging in the time of exhaustion.

Quarantine
Ms. Kimura, medical officer of the Health ministry, who criticized the ministry in yesterday’s budget committee meeting (see post #121 above) emphasized in an tv interview this morning: “onboard quarantine’s cost-performance is very low”, “the ministry suppressed voices from experts and the frontline of quranantine calling for policy change” and “discrimination (against her) is already felt.”

It was reported that the government’s expert advisory committee held on May 13 had suggested scaling down the quarantine but the suggestion was deleted from the final draft of the committee’s report.

According to the ministry of health, there still are about a hundred doctors are sent from various hospitals to four international airport to help quarantine.

Hyogo Prefecture
A high school student had flu like symptoms (fever 40dC, lethargy) on May 27, staying home, recovering.
A high school sutent had a fever of 38d C on May 28, close contact with a patient in his school, staying home, recovering.
Chairman of Kobe doctors association expressed his objection to the ‘safe for now’ declaration made by tha mayor of Koabe-city yesterday, stating “It’s too early. We still had new confirmed cases on 27th. Mayor shouldn’t do it until Kobe has no new patient for a week.”
Hyogo prefecture announced that the novel flu related expenditures would amount to 8.5 billion Japanese yen (US$89 million), including 4.5 billion Japanese yen (US$47 million) to be born by municipality. The prefecture stated that financial support from the central government is much expected.

Chiba Prefecture (First confirmed case)
A female (19) working as a waitress in a restaurant in the restricted departure area (after immigration) in Narita airport, had flu like symptoms (cough, nasal fluid) on May 28 night, a fever (39d C) on May 29 morning, and was hospitalized in a designated medical institution for infectious disease in Narita.
Chiba prefecture governor, Kensaku Morita, stated that the prefecture would not close public facilities and requested calm response to residents of Chiba.

School Closure
Kobe high school and Hyogo high school of which 17 and 43 students respectively were infected resumed classes today. All the infected students have recovered already.

Two senior high schools in Kobe, Hyogo Prefecture hit hard by new-flu infections reopened Saturday after closing for about two weeks to prevent the spread of the disease. Kobe High School and Hyogo High School scheduled a special school day Saturday to confirm the health conditions of their students. Classes will resume Monday.

The first case of the new H1N1 strain of influenza A in Japan outside airport quarantine was detected at Kobe High School while many students came down with the new flu at Hyogo High School. Most schools in Hyogo Prefecture in western Japan, the epicenter of new-flu infections in the country, resumed classes last Monday after closing for about a week. But Kobe High School and Hyogo High School continued to suspend classes as the number of new-flu infection cases totaled 17 at Kobe High School and 43 at Hyogo High School.

Niigata Prefecture (First confirmed case)
A female student (21) returned from Nevada, USA onUA853 on May 29.
She had a fever of 38.6d C but tested negative at Narita airport, (another false negative case by rapid test)
She had flu-like symptoms on May 30, was confirmed infected with novel flu and hospitaliszed. She is recovering with a fever dropped to 36s.
She took a shared-taxi from Narita to Niigata (approx 400km, 6 hours) with 7 people (a driver and passengers) to which Niigata prefecture requested to stay home and was watching their health conditions.

School Closure
Kansai Okura junior and senior high school of which 101 students were infected, held a meeting today to explain the current situation to parents in view of reopening the school Monday. All the infected students have recovered already and will attend the school tomorrow.


These days, there are less and less news coverages on novel flu in the media.

Identical virus caused group infection
?An identical virus seems to have caused the group infection in Osaka and Hyogo.? National Institute of Technology and Evaluation and National Institute of Infectious Disease announced Friday.

The report came out as a result of the gene analysis of viruses isolated from 9 patients of Osaka and Hyogo. The analysis also determined that these viruses belong to a different strain from the virus isolated from the patient detected at Narita on May 8. NIID also stated that the outbreak in Osaka and Hyogo might have started at the end of April, more than two weeks before the first patient was confirmed on May 16.

The analysis was perfromed on viruses isolated between May 16 and 17 from four patients of Osaka and five patients of Hyogo. Viruses from two of the patients had completely idential genes while viruses from the other seven patients had slight mutations. According to NIID, this possibly indicates that the infection spread rapidly from a single patient

Comparing with the viruses taken from 38 patients worldwide in terms of gene mutations, the Osaka and Hyogo strain can be placed between the strain that initiated the outbreak in Mexico and the strain that caused the group infection in USA and Canada in the end of April.

Daily reports from 2200 drugstores throughout Japan showed sharp increases in prescription of influenza antivirals in Kobe-city on April 28 and in Ikeda and Maikata cities, Osaka on May 1. As seasonal flu was going out of season then, these increases possibly indicate that outbreak of novel flu started then, NIID stated.

The scientific confirmation of the initial worldwide totaly wrong policy of NOT grounding international planes traffic.

If the planes were grounded for 2 weeks from the moment of knowledge (middle/late April) about heavy suspected pneumonia/flu outbreaks in Mexico,
and at the same time the border there cordoned and applying an supported antiviral blanketing with international helps to contain it localy,
this single case could not reach Japan and initiate there the spreadings, and not only there.

Yamanashi Prefecture (First confirmed case)
A female student (20) returned home from N.Y. on May 29 onboard JAL 9, had a fever (37.8d C) from May 31 before dawn, tested negative by a rapid test but PCR test returned a positive result.
Hospitalised in a designated medical institution for infectious disease, prescribed antivirals and fever dropped to 37s with stable condition.

Due to the end of school year in US, new patients these days are students returning from US. It probably indicates a widespread of novel flu in US and more patients returning in weeks to come. Unfortunately, it increasingly become clear that the quarantine at the airports is not effective enough. Nor is rapid tests.


School Closure
Kansai Okura Junior and Senior High School, the hardest-hit school with 103 students and teachers infected, resumed classes Monday after two weeks’ closure. No new patient were confirmed from May 24 and all the patients have recovered.
The school increased the number of school buses, which were determined as the major source of infection by National Institute of Infectious Diseases, from 15 to 20.

These days, there are less and less news coverages on novel flu in the media.

Aichi Prefecture (First confirmed case)
A female (40s) and a female (30s) who traveled to Hawaii from May 20 to 30, returned to Chubu International Airport on May 30 aboard JO83. The first female had a fever (38.8d C) and was tested positive by rapid test for influenza A and confirmed infected with the novel influenza. She was hospitalized in a designated medical institution. The second female had a fever of 38d in Hawaii on May 27 but continued the trip after the fever subsided the following day. Following the first female’s case, the regional healthcare center requested her for a health check. Then she had flu-like symptoms and was tested positive for novel flu on June 1.
A female (20s) who traveled to Orland from May 25 to 30, returned to Chubu International Airport on May 30 aboard NW71.
She had a fever (39.0d C) and was tested positive by rapid test for influenza A and later confirmed infected with the novel influenza. She was hospitalized in a designated medical institution.

Chiba Prefecture
A female (19) working as a waitress in a restaurant (a different restaurant than the first case in Chiba) in the restricted departure area (after immigration) in Narita airport, went to a theater in Tokyo on May 28 together with the first case, had a fever of 38 degrees C on May 31, was tested negative by rapid test but later confirmed infected with novel flu and hospitalized in a designated medical institution for infectious disease in Narita.

Tokyo Metropolis
A female (29) who was studying in NY, returned on May 29 from US, had a fever (37.4d) on May 31, consulted with a fever consultation center and stayed home. Her fever rose on morning June 1, consulted again with the center, visited a fever clinic on the same day. She was tested positive by rapid test and later positive for novel flu, hospitalized in a designated medical institution of Sumida ward.

New patients from North America

New patients of novel flu in Japan have shown a clear change in this week. While until last week, new patients were predominantly high school students in Hyogo and Osaka prefectures, the majority of new patients who were reported in this week by the Ministry of Health Labor and Welfare, are travelers who returned from North America, especially USA.

Of 19 patients reported since Sunday May 31, 10 patients, all Japanese and all females, recently returned from North America, 9 from USA and 1 from Canada. 5 of them returned from their study in USA and Canada; 3 from their trips in Hawaii and Florida; 2 are residents of Conneticut and New Jersey visiting their families in Japan.


LIST OF TRAVELER'S CASES



Four other new patients were considered to have been infected from one of the above students during a wedding party which they participated with the student.

Seems my remark of June 1 is becoming a reality?.

Tokyo metropolis
A girl (4) living in New Jersey, returned to Japan on May 31, had a fever (39.1) on June 2, rapid test positive and PCR positive. Hospitalized.

Saitama prefecture
A female (26) studying in Canada, returned to Japan on May 31, had a fever from June 1, first rapid test negative, second rapid test on the following day returned positive and PCR confirmed her infection. Hospitalized.

Wedding party infection
Two males and in Tokyo and two females in Chiba, all 29 years old (maybe classmates) were considered to have been infected from the patient in Tokyo who was reported in post # above. They all attended a wedding party on May 30 with the patient.

Shizuoka prefecture
A businessman (20s)　was confirmed infected with novel flu today.

Tokushima prefecture (first confirmed case)
A female (20s) returned from her study in USA on June 1 at Kansai International Airport, took a bus to go to Tokushima, had a fever (39.2), rapid test negative but PCR confirmed her infection. Hospitalized.

Yamaguchi prefecture
A female (30s) living in Conneticut, USA, returned to Japan on May 29, had a fever (over 38) in the morning June 1, first rapid test negative, second rapid test on June 6 positive and PCR confirmed her infection. Hospitalized.

Hyogo prefecture governor, Toshizou Ido, made the “declaration of safe Hyogo” today.
According to Kobe shinbun, a newspaper in Kobe, 85 patients, or 43% of 196 patients in total in Hyogo, had onset of the disease before May 16’s detection of the first patient in Hyogo. While Japanese government focused on the onboard quarantine inspection, the infection was already widespread. (Note: This will be another proof that the one-week school closure from May 18 worked very well for stopping the spread of infection.)
The last high school in Hyogo reopened today.

National Institute of Infectious Diseases has strengthened its influenza surveillance thru dragstores. NIID will receive a report of prescriptions of flu antivirals from 3000 dragstores throughout Japan to see whether any unusual increase of prescriptions appears in any part of Japan. NIID previously stated that the novel flu infection in Hyogo and Osaka may have started as early as at the end of April when prescriptions of anti-flu drugs made a sharp increase. (Note: In Japan, hospitals and clinics don’t give you drugs but prescriptions only. You then have to go to a drugstore to buy drugs.)

The number of new cases are increasing for the last 4 days.

Osaka prefecture
A businessman (40) returned from Hawaii on June 2 and had a fever (38d) on June 2.
A student (15) of Kansai Okura High School whose infection was confirmed by Osaka prefecture on May 20, but the prefecture failed to inform him as well as to report to the Health ministry. The student was treated as a negative but fortunately no further infection took place. The prefecture made an official appology.

Tokyo metropolis
A businesswoman (36) went to her business trip to NY, USA from May 27 to June 1, returned on June 2, had a fever (38.3d) from 23:00 on the same day, rapid test negative on June 3, PCR positive. Hospitalized.
A student (25) temporarily returned to Japan from NY, USA on June 2, had a fever (39.1d) on June 3, rapid test positive, PCR positive. Hospitalized.

Kanagawa prefecture
A man (60s) returned from Philippines on May 31, had a fever (37.1d) on June 3, rapid test positive. PCR positive.

Saitama prefecture
A girl (8) living in Ontario, Canada, arrived at Narita on June 2, had a fever on June 3. Rapid test positve, PCR positive. Hospitalized.

Shizuoka prefecture
Two businessman (both 20s) staying in their company’s training center in Shizuoka from May 18 with about 80 colleagues including the patient who was confirmed yesterday, were under medical surveillance since the first case, had a fever (38.9,39.0) and were hospitalized.

Yamaguchi prefecture
A female (50s), family member of the first case of Yamaguchi, had a fever (37.1-37.9) on June 3 but rapid test was negative. PCR test confirmed her infection and she is hospitalized.

Wedding party infections
The wedding party infections reported yesterday developped further today. Three people of Kanagawa prefecture and two people of Chiba prefecture were confirmed infected with novel influenza by the Japanese student returned from her study in NY, USA.
I feel very sorry for her who came back from US to attend her friend’s wedding party but infected eight people in return. I hope no more people are infected.

According to an investigation of 401 patients by the Ministry of Health Labor and Welfare, the patient who had the earliest onset of novel flu was a high school student in Kobe who had onset of the disease on May 5 which was before the first patient was confirmed at Narita airport on May 9. As the student has no record of overseas travel, it suggests that the infection had spread before his infection. May 17 was the peak of onset with 73 new cases but the number of new cases dropped very rapidly after that.
The Ministry investigated 401 patients confirmed before 11 am June 4, excluding 8 people confirmed at Narita. Onset dates of 390 patients could be confirmed by healthcare centers. 358 patients, or 90% of all patients, have fully recovered. No severe case is reported.

Traveler Cases



5 more traveler cases were reported today, 3 from USA, 1 from Canada and 1 from Philippines. The total number of traveler cases in this week reached 15, or 45% of all cases of this week. If the wedding party are count in as secondary infections from a traveler, the percentage reaches 70%.

Chiba prefecture
Two females (21, 23) who work in an airline’s departure lounge in Narita airport, had a fever (38.7d, 38.5d) on June 5; rapid tests both positive on June 6. To be hospitalized.

Kanagawa prefecture
A businessman (27) had a fever (37.5d) on June 5, rapid test positive on June 6. Hospitalized.

Yamaguchi prefecture
A small boy who is of the same family with the previous 3 cases, had a fever (37s) on June 6; rapide test positive.

Fukuoka prefecture (new school infections)
6 pupils of a primary school and 6 students of a junior high school in the same area of Fukuoka have been confirmed infected with novel flu so far. They don’t have record of visiting any foreign country or Kansai area recently. Their schools and another primary school of the same area will be closed until June 14.
A new series of school infections may have started in Fukuoka.
With 50 to 60 other pupils and students having flu-like symptoms on June 6 and 7, the number of confirmed cases is likely to increase.
The first case student was reportedly tested positive for both of novel flu virus and seasonal A(H1N1) virus!! (No reassortment please)

The number of confirmed cases in Japan has increased rapidly for 2 days in a row. There are two clasters of school infection in Chiba and Fukuoka prefectures.

Chiba prefecture
The outbreak in Chiba is mainly within a municipal junior high school whose students went to a trip to Tohoku area from June 3 to 5. An employee of a restaurant where the students took a meal and a tour guide of a bus which students used for the trip have been infected in Iwate and Miyagi prefectures.

Fukuoka prefecture
Students of a junior high school and pupils of a primary school in Itatsuke area are infected. Three schools in the area are closed
It is assumed that the virus originated from the first confirmed case in Fukuoka, an American traveler (38) whose infection was ocnfirmed on May 25, because (1) the gene sequences of viruses isolated from students and the first confirmed case are identical and (2) the first patient took a meal in a restaurant in the area. However, an interval of 12 days between the first case and the students suggests that there should have been one or two infected peolple between them. As none of 15 people who had close contact with the first case were infected, the prefecture is looking for other people transmitted the virus.
