Your paper on autoimmune epilepsy sounds interesting and I hope you'll post it here, Anne. I know there is a lot of research going on in France into these disorders.

I just noticed there are two recent cases of NMDA receptor encephalitis in very young children described by a mother in France here:

The diagnosis of drug-resistant epilepsy of autoimmune origin are becoming more frequent. It is reported more and more of limbic encephalopathy paraneoplastic origin not from the five to ten years. Three antibodies are now described: antibody anticanaux dependent potassium voltage (voltage-gated potassium channels (VGKC)), the antacid glutamic decarboxylase (GAD) and antirécepteur N-methyl-D-aspartate (NMDA). We review the clinical syndromes in association with the imaging findings and the laboratory. Note that most reported cases involve the adult population, although some cases are also seen in adolescents and children. Early recognition of such a syndrome is very important as an early immunomodulatory therapy appears to favorably modify the clinical course.

One third of patients with epilepsy will develop into a drug resistance (that is: a lack of adequate clinical response after the trial of two or more antiépilepeptiques). There are several reasons for this. Surgical treatment can be proposed that some of these patients. The origins of autoimmune increasingly recognized since the last five to ten years and should not be missed, since their detection may potentially lead to effective treatment. The limbic encephalitis is typically associated with temporal lobe seizures and is the most common cause of late-onset temporal lobe seizures (after twenty years) .1
Conclusion

There are more and more cases of epilepsy known as autoimmune although their study is still in its infancy. Most studies including a large group of patients have been published over the past five years and are mainly adults but also children.

Given the large number and rate of discovery of different antibodies, we expect the fact that more and more patients are diagnosed with epilepsy of autoimmune origin, in the coming years. Other autoantibodies are still discovered. For example, anti-AMPA were found in ten patients with limbic encephalitis (nine women, including seven with a tumor) .24 At the same time, new treatments have become available immunomodulators in the last decade as rituximab.Diagnostic tools show significant progress but it still lacks major systematic studies of different immunological treatments.

The evidence is increasingly clear that a good symptomatic recovery is related to early treatment. This is all the more important it may be young or very young patients.

Practical implications

> An autoimmune balance should be considered for epilepsy late, severe epileptic encephalopathy or to the presence of other neurological symptoms such as memory impairment, ataxia, rigidity, involuntary movements, psychiatric disorders, coma and rarely as status epilepticus

Abstract

BACKGROUND:

Antibody mediated anti-N-methyl-d-aspartate (NMDA) receptor encephalitis is a recently reported diagnosis of clinical importance. Recognition of the syndrome, especially in pediatric populations, is difficult and often undiagnosed and/or confused with neurological disorders with similar clinical features.
RESULTS:

We report a case of an 11 year old female with explosive-onset epilepsy and predominantly unilateral frontal lobe abnormalities on magnetic resonance imaging (MRI) and fluorodeoxyglucose positron emission tomography (FDG-PET) neuroimaging. A diagnosis of Rasmussen syndrome (RS) was considered. Cerebrospinal fluid analysis revealed strong positivity for NMDA receptor antibodies. Screening for occult ovarian teratoma with computed tomography (CT) and MRI initially did not demonstrate associated tumor. Treatment with steroids and plasma exchange improved her clinical course and subsequent MRI showed resolution.
CONCLUSION:

NMDA receptor encephalitis has variable neuroimaging manifestations, and can mimic other entities. We emphasize the clinical syndrome of NMDA receptor encephalitis and consideration of the diagnosis in evaluating a child with explosive-onset epilepsy, unilateral imaging abnormalities, and neurocognitive decline.

Abstract

We present a clinicopathological report of a recent fatal case of a 27-year-old woman whom we consider to have had encephalitis lethargica. Clinical features of note were a presentation with vertigo, persistent vomiting and sleep disturbance including marked daytime somnolence and vivid nightmares. On examination, she had impaired slow pursuit vertical eye movements, dysarthria, an expressionless face and slow tongue movements. She went on to develop gross supranuclear gaze palsy, neck rigidity, bradykinesia, blepharospasm, profound somnolence and anarthria but no tremor, weakness or impairment of cognition. She died after an illness lasting 12 months. On investigation, the cerebrospinal fluid was found to contain a very high level of IgG with oligoclonal bands but no cells. Post-mortem examination revealed an active encephalitis, mainly centered on the upper brainstem and diencephalon with extensive Purkinje cell loss and marked plasma cell infiltrates and morula cells. No virus was recovered.

Autoimmunity and the basal ganglia: new insights into old diseases

Sydenham's chorea (SC) occurs weeks or months after Group A streptococcal infection, and is characterized by involuntary, purposeless movements of the limbs, in addition to behavioural alteration. There is a body of evidence which suggests that SC is an immune‐mediated brain disorder with regional localization to the basal ganglia. Recent reports have suggested that the spectrum of post‐streptococcal CNS disease is broader than chorea alone, and includes other hyperkinetic movement disorders (tics, dystonia and myoclonus). In addition, there are high rates of behavioural sequelae, particularly emotional disorders such as obsessive‐compulsive disorder, anxiety and depression. These findings have lead to the hypothesis that similar immune‐mediated basal ganglia processes may be operating in common neuropsychiatric disease such as tic disorders, Tourette syndrome and obsessive‐compulsive disorder. This review analyses the historical aspects of post‐streptococcal CNS disease, and the recent immunological studies which have addressed the hypothesis that common neuropsychiatric disorders may be secondary to basal ganglia autoimmunity.

Abstract

AIM:

We report four cases of acquired severe encephalopathy with massive hyperkinesia, marked neurological and cognitive regression, sleep disturbance, prolonged mutism, and a remarkably delayed recovery (time to full recovery between 5 and 18mo) with an overall good outcome, and its association with anti-N-methyl-d-aspartate (anti-NMDA) receptor antibodies.
METHOD:

We reviewed the four cases retrospectively and we also reviewed the literature.
RESULTS:

Anti-NMDA receptor antibodies (without ovarian teratoma detected so far) were found in the two children tested in this study.
INTERPRETATION:

The clinical features are similar to those first reported in 1992 by Sebire et al.,(1) and rarely recognized since. Sleep disturbance was not emphasized as part of the disorder, but appears to be an important feature, whereas coma is less certain and difficult to evaluate in this setting. The combination of symptoms, evolution (mainly seizures at onset), severity, paucity of abnormal laboratory findings, very slow recovery, and difficult management justify its recognition as a specific entity. The neuropathological substrate may be anatomically close to that involved in encephalitis lethargica, in which the same target functions (sleep and movement) are affected but in reverse, with hypersomnolence and bradykinesia. This syndrome closely resembles anti-NMDA receptor encephalitis, which has been reported in adults and is often paraneoplastic.

The term "basal ganglia" refers to a group of several structures in the brain: the caudate nucleus, the putamen, the globus pallidus, and the subthalamic nucleus. The substantia nigra, a midbrain structure that has many interconnections with the basal ganglia, is not actually part of this grouping but is often associated with it.

The basal ganglia are involved in a complex loop that connects them to various areas of the cortex. The information from the frontal, prefrontal, and parietal areas of the cortex passes through the basal ganglia, then returns to the supplementary motor area via the thalamus. The basal ganglia are thus thought to facilitate movement by channelling information from various regions of the cortex to the SMA. The basal ganglia may also act as a filter, blocking the execution of movements that are unsuited to the situation
Not all of the circuits involving the basal ganglia are motor circuits, however. Many are instead involved in memorizing and in cognitive and emotional processing. A great deal about the basal ganglia remains unknown. They seem to play a far larger role than just their contribution to motor control.

I agree that is very interesting that Sydenham's chorea could be autoimmune in origin, Anne. Maybe research will lead to treatment that might provide quicker recovery, and/or less relapses.

Abstract

Limbic encephalitis, first described in the 1960s as a paraneoplastic condition, has emerged as an autoimmune condition, occurring often without evidence of an underlying tumor. Many novel autoantibodies have been identified, and this diagnosis is increasingly being made in the pediatric population. This article reports the case of a 16-year-old boy who presented following gastrointestinal illness with subacute evolution of neuropsychiatric symptoms. Brain magnetic resonance imaging revealed progressive hippocampal signal abnormality and swelling. N-methyl-D-aspartate (NMDA) receptor antibody was detected in serum. The patient responded well to pharmacological immunotherapy but has residual cognitive deficits. The available literature on this condition is reviewed. Limbic encephalitis should be considered in the differential diagnosis in children presenting with encephalopathy, particularly with neuropsychiatric manifestations. Long-term surveillance and close follow-up are required to accurately clarify tumor risk and natural history of this condition in children and balance these factors with risks of radiation exposure through imaging.

Abstract

In the majority of cases, anti-NMDA (N-methyl-D-aspartate) receptor encephalitis is a severe, but treatable disorder, therefore early diagnosis and adequate therapy are very important. It should be suspected in children and young women, who develop acute psychiatric symptoms and seizures. During the course of the disease severe encephalopathy, agitation, hallucinations, orofacial dyskinesias, prolonged cognitive disturbance, autonomic symptoms can be observed and akinetic mutism develops. EEG shows diffuse slowing. Brain MRI is normal or unspecific. Elevated protein, pleiocytosis and oligoclonal bands can be present in the CSF Detection of NMDA-receptor antibodies in sera or CSF confirms diagnosis. We present the case of a 15-year-old girl, who fully recovered within two months after steroid treatment and repeated plasma exchange. Ovarian teratoma has not been detected.

Abstract: The field of autoimmune encephalopathies has expanded rapidly in the last few years. It is now well-established that a substantial proportion of encephalitides are associated with autoantibodies directed against the extracellular domains of cell-surface proteins which are critical in the regulation of neuronal excitability. These include LGI1, CASPR2, contactin-2 (VGKC-complex antibodies), and the NMDA, AMPA, and GABAB receptors. The clinical importance of these conditions lies in their frequent immunotherapy-response and, less commonly, their association with distinctive tumors. Studies which have examined cohorts of patients defined by these serum antibodies have identified a number of clinical features that have helped understand the core phenotypes of these conditions. In addition, sensitive antibody assays have allowed the expansion of the phenotypes to include a minority of patients with isolated epilepsies or psychoses. There is also evidence that autoimmune encephalitis may progress to adult-onset hippocampal sclerosis. Clinical, and accumulating scientific, data strongly suggest direct pathogenicity of these autoantibodies. The generation of the autoantibody, in some patients, can be explained by the presence of tumors which express their antigenic target. Serum antibody levels are higher than their levels in CSF in the vast majority of cases. However, the majority of patients do not harbor a tumor and the etiology of the disease in these patients is less clear. Below, we suggest models for the etiology and pathogenic mechanisms of these autoantibodies by incorporating concepts such as serum generation of the autoantibodies, the blood-brain barrier, intrathecal antibody production, and prodromal infections.

Abstract

A large proportion of "encephalitis" is caused by unknown agents. Of late, a new category of disorders, "autoimmune encephalitis," has been described, which present with features similar to viral encephalitides. A well-delineated and common entity among this group is the recently described anti-NMDAR encephalitis (NMDARE). Although this entity was initially described in young women harboring ovarian teratomas, it is now characterised as well in children and men. Approximately 60% of the patients have an underlying tumor, usually an ovarian teratoma. In 40% of the patients, no cause can be found (idiopathic NMDARE). NMDARE typically presents with psychiatric features followed by altered level of consciousness, severe dysautonomia, hyperkinetic movement disorders, seizures and central hypoventilation. Orofacial dyskinesias resulting in lip and tongue mutilation are quite common. Seizures, are common and may be difficult to treat. The disease can be confirmed by serum and cerebrospinal fluid anti-NMDAR antibodies. Titers of these antibodies can also guide response to treatment. Tumor removal is necessary if identified, followed by immunological treatment. Intravenous methylprednisolone and immunoglobulins aim to suppress/modulate immune response while plasma exchange attempts to remove antibodies and other inflammatory cytokines. Rituximab and cyclophosphamide aim to suppress antibody production. Recovery is slow and often with neurological deficits if treatment is delayed. With many distinctive clinical features, a specific antibody that aids diagnosis, and early effective treatment with commonly available drugs leading to good outcomes, NMDARE is a diagnosis that should be considered early in any case of "unexplained encephalitis."

The last sentence is very interesting:

The similarity of this illness to "encephalitis lethargica" or Japanese B encephalitis [JE] is striking, and presumed JE cases with negative serology should be further investigated to rule out NMDARE.

The article also notes that the first cases of NMDARE were described in Japan. It made me wonder if most cases are appearing in the summer when mosquitoes are out. I'm sure that would have been noticed by now, though.

It looks interesting and supports the similarities between the current NMDARE cases and the early 20th century EL epidemic. They mention just cases in young children of both sexes in the EL epidemic, so that leaves out the NMDARE cases predominantly in teen girls and young women today, (many of those having teratomas.)

Perhaps there were less teratomas back in the early 1900's, or there was a bias for diagnosing young women with the symptoms as having strictly non-medical psychiatric problems rather than underlying neurological problems.

To the Editor: Anti?N-methyl-D-aspartate (NMDA) receptor encephalitis, although rare, is an emerging cause of catatonia.1 We describe a 22-year-old male patient whose akinetic catatonia responded to intravenous immunoglobulin therapy and steroid therapy after his cerebrospinal fluid and serum samples tested positive for the anti-NMDA antibody.
