فى ظل التطور السريع لظهور حالات أنفلونزا الخنازير فى مصر، والجارى علاجها بمستشفى حميات العباسية، انفرد اليوم السابع بالحصول على قائمة بمستشفيات الحميات المجهزة على مستوى الجمهورية لاستقبال الحالات المصابة، والتى من المقرر أن تعلنها وزارة الصحة فى الأيام القليلة الماضية مع الانتهاء من إعدادها.

القائمة تحتوى على 99 مستشفى ما بين مستشفيات متخصصة فى الحميات وأقسام للحميات فى المستشفيات العامة والمركزية فى جميع المحافظات، والتى جارٍ حاليا إمدادها بالمساكات الواقية والكمامات والتاميفلوا كاستعدادات أولية لمواجهة الوباء فى جميع المحافظات المصرية من القاهرة إلى الإسكندرية.

يأتى هذا بعد قرار وزارة الصحة بإغلاق 38 مستشفى للحميات بصفة نهائية بمختلف المحافظات ودمج 54 مستشفى أخرى كأقسام حضرية فى المستشفيات المركزية، ليصل عدد المستشفيات فى مصر من 108 إلى 30 فقط، مما استوجب على الوزارة إعادة فتح بعضها وتأهيل بعضها لمواجهة الوباء.

بذلك تقتصر خريطة مستشفيات الحميات المؤهلة حالياً فى مصر على 30 مستشفى فقط، أهمها مستشفى حميات العباسية ومنشية البكرى.

وناشدت وزارة الصحة على لسان وزير الصحة الدكتور حاتم الجبلى، المواطن إذا شعر بأى أعراض، بضرورة التوجه إلى أحد مستشفيات الحميات المخصصة فى هذه المحافظات أو إلى قسم الحميات بالمستشفى العام للمحافظة.

More than just medicine

As the Abassiya Fever Hospital, continues to host swine flu cases, Lina Attalah spends the day with its Doctors and patients

?I have been working for more than 19 hours a day nonstop,�? said, Ahmad Uthman, while putting down his face mask and wiping the sweat away from his forehead. He finally manages to sit down for a few minutes to rest. For the past two weeks, the outbreak of swine flu cases in Cairo translated into a lot more work for him and his fellow medics at the Abassiya Fever Hospital.

This was the second national crisis he had to deal with throughout his three-year professional age at the hospital, the first being a series of measles cases last summer. Around the hospital, medics and nurses have been moving around with masks covering their faces and signs of exhaustion nevertheless showing. Uthman?s office was painted with faint yellow and looked as old as the incumbent Fever Hospital, one of Egypt?s oldest public health spaces dating back to 1893.

While the hospital has been busy with the treatment and testing of A/H1N1 cases, complaints about the level of the hospital?s preparedness have been loud. These complaints reintroduced the continuing lack of resources in the country?s fever hospitals, symptomatic of fallouts in healthcare provisions.

A young American student at the American University in Cairo spoke late last week to al-Masry al-Youm about his experience with getting an A/H1N1 test at the Abassiya Fever Hospital. He referenced the poor hygienic conditions in the hospital in general and the room where he was kept in particular.

While Uthman said that the basic state of hygiene in the hospital was ?good�?, he admitted that ?it could be better.�? ?I need resources,�? he added.
For Emad Eldin al-Hashemy, director of the Abassiya Fever Hospital, the Ministry of Health?s support during the swine flu crisis has been unconditional. ?The ministry?s perception to the hospital has changed in the wake of the crisis,�? he said. ?That does not mean the hospital used to be neglected, but the care towards it has intensified with the outbreak of swine flu.�?
According to him, the ministry?s support translated into constant monitoring and some expenditure allocated to bonuses to medics working over time as well as more Tamiflu portions and other preventive tools? stocks. ?The hospital was also equipped with a new x-ray machine, as the one we had before was not strong enough to cope with a crisis,�? al-Hashemy said. He also said that just yesterday a delegation from the Ministry of Health was at the premises of the hospital to study the expansion of the reception area to better cope with emergencies. ?This was a showcase of responsiveness, without resorting to the usual routine of submitting requests and awaiting reactions. I appreciate a lot this language. It helps us a lot as executives.�?

?I can go on as much as I can offer something,�? said Uthman, for whom, those were critical moments at the hospital. Al-Hashemy prided his hospital for not having acquired any additional staff since the crisis began. ?We have actually dispensed 31 of our medics to help in the airport?s contingency operation as a first line of defense against the disease. We managed to compensate this shortage through increasing the number of working hours and shifts. We are very proud of that.�?
The first few cases in Egypt?s swine flu record which has hit 22 people until today were Americans. This entailed more responsibility for al-Hashemy and his team because it involved reporting to foreign bodies. ?Our patients now have embassies and consulates supporting them. So our job here has become more than just medicine. Now it involves diplomacy, tourism and media,�? he said. ?We got a lot of thank you messages from embassies and from the American University in Cairo,�? he added, the university being involved because the earliest infected cases were from its students? body.

Meanwhile, the Abassiya Fever Hospital has not been certified by the Quality Department, an operation within the ministry developed in 1999 to support public health care establishments to improve the quality of their services. A series of internationally recognized clinical criteria are requested to be implemented by hospitals for the certification to be pursued and only a few hospitals have managed to be certified. None of those was a fever hospital.

The swine flu uproar still managed to bring back to mind the problems associated with the country?s 130 fever hospitals, where little attention has been accorded from the government and mounting complaints have been expressed about their being a financial burden for the health sector.
Alaa Ghanam, Health and Human Rights Program Director at the Egyptian Initiative for Personal Rights, reminded how central fever hospitals are to public health care provisions, yet how this has not been reflected in public policy making. Recently, the Ministry of Health has been working on annexing small-scale fever hospitals to public hospitals for cost containment, especially that those hospitals occupy a large surface area. ?There was no projection of the disease burden in Egypt when this decision was taken,�? Ghanam said. The country has been a fertile environment for both communicable diseases associated with developing nations, such as flu and non-communicable diseases, such as diabetes and heart problems. With this health background in the picture, fever hospitals become instrumental.

The health sector in general in Egypt has been suffering from declining public expenditure. According to the 2008-2009 fiscal year, public expenditure on health consisted of 3.6 percent of the overall state budget. According to Ghanam, the international standard average for public health expenditure should be between 9 and 14 percent of the state budget. Moreover, of this cost, only 9 percent consist of preventive expenditure, while 57 percent is allocated for curative expenditure, a critical policy amidst epidemic and pandemic outbreaks.

Ghanam praised the National Pandemic Preparedness Plan developed by the government through external funding support, for it tackled main issues concerning communication, awareness and inter-ministerial cooperation. ?But the problem lies in the implementation. There is no capacity for implementation. What sort of resources allocation has been taking place since the crisis started?�? he asked.

?Fever hospitals are at the heart of the health culture and health history amongst Egyptians. It has been the first place entrusted by Egyptians when they fall sick, especially for those who simply understand illness as fever,�? Ghanam added, reminding how elementary it is for the government to support those establishments. The Abassiya Fever Hospital has been part of this health culture heritage with a long history of combating major disease outbreaks, such as cholera in 1947, meningitis in 1977 and the Rift Valley Fever in 1977.

ناقش مجلس محلى مركز دمياط برئاسة المهندس محمد خشبة، تقرير لجنة السياسات الطبية المعد برئاسة محمد شبانة، حيث كشف التقرير عن عدم توافر خطة واضحة تكاملية للخدمات الصحية المقدمة من المستشفيات والمراكز الطبية المختلفة فى دمياط.

كما طالب التقرير بسرعة الانتهاء من أعمال التجديد فى مستشفى دمياط العام والتخصصى مع الالتزام بجدول محدد لذلك، وكذلك تشغيل وحدة القىء الدموى المعطلة بالمستشفى التخصصى بدمياط، رغم توافر أجهزة المناظير والكوادر الطبية لتشغيلها تخفيفاً عن مركز الكبد بدمياط.

وطالب التقرير بزيادة أعداد الأطباء البيطريين المعينيين والمتعاقدين لسد احتياجات الفترة الحالية مع تشديد الرقابة على الأسواق لمنع بيع الطيور الحية، مع تحصين الطيور المنزلية وتشديد الإجراءات الوقائية والصحية بمجازر اللحوم والطيور.

كما أوصى التقرير، أن تواصل أجهزة مديرية الصحة تنفيذ خطة الوزارة بخصوص الأنفلونزا الطيور والخنازير بكل حزم ودقة وزيادة الرقابة على المستشفيات الخاصة للتأكد من تطبيقها معايير مكافحة العدوى أسوة بالمستشفيات العامة، مع زيادة عدد المحارق للنفايات الخطرة وتفعيل دور السيارات الخاصة لنقل النفايات لكونها لم تعمل حتى الآن مع تشديد الرقابة على نقل المخلفات من الوحدات الصحية والمستشفيات إلى المحارق مع إلزام المنشآت الطبية بوضع ختم لها أو علامة مميزة على الأكياس الخطرة الحمراء.

أنهت مديرية الصحة بأسيوط، التعاقد مع الخبرات السابقة من الأطباء والإداريين ممن أحيلوا إلى المعاش وذلك لمواجهة أنفلونزا الطيور.

وأشار الدكتور أحمد عبد المنعم وكيل وزارة الصحة بأسيوط، أن هذه القيادات السابقة لديها الخبرات المتراكمة للتعامل مع هذا المرض، وتمثل قوة مضافة للمديرية، مشيراً إلى أن هذه التعاقدات الجديدة تأتى تنفيذاً لتوجيهات الدكتور حاتم الجبلى وزير الصحة بالاستعانة بالقدرات البشرية الملائمة لسرعة تشخيص المرض والتعامل معه.

يذكر أن أسيوط من المحافظات المتميزة فى إطار مكافحة أنفلونزا الطيور، حيث لم تشهد سوى إصابتين فقط وذلك بسبب الإجراءات المشددة التى اتخذتها المحافظة فى مراقبة الكتاكيت القادمة من المحافظات المجاورة، فضلاً عن تحصين كامل لمزارع الدواجن وعددها 832 مزرعة بالمحافظة، كما تشهد المستشفيات حالة من الاستنفار العام لمواجهة أية إصابات وسرعة التعامل معها وإرسال العينات للمعامل المركزية بالقاهرة.

تبدأ اللجنة المكلفة باختيار المستشفيات التى تستقبل حالات أنفلونزا الخنازير على مستوى الجمهورية اجتماعاً الخميس المقبل مع مديرى المستشفيات لتحديد معاير استقبال المرضى، على أن يتم توفير وحدات للرعاية المركزة فى كل المستشفيات كإجراء احترازى، رغم عدم حاجة الحالات المصابة بأنفلونزا الخنازير لاحتجازها بوحدات الرعاية المركزة على الإطلاق.

يأتى هذا بعد اختيار الإدارة المركزية للمستشفيات العامة بوزارة الصحة 9 مستشفيات عامة ومركزية بالقاهرة والمحافظات لاستقبال المرضى بأنفلونزا الخنازير بواقع 2 مستشفى من مستشفيات وزارة الصحة فى كل محافظة لاستقبال مرضى أنفلونزا الخنازير والطيور مع الفصل الكامل بينهما خوفاً من تحور الفيروس.

وقال هشام شيحة مدير عام مستشفيات وزارة الصحة، إن قرار الفصل يستهدف عدم التزاوج بين فيروسى أنفلونزا الطيور والخنازير، موضحاً أن قائمة المستشفيات سيتم إعلانها يوم الاثنين القادم، وأكد شيحة أنه لا حاجة لاتخاذ أية إجراءات استثنائية.

اتخذت محافظة أسيوط تدابير وإجراءات صحية واحتياطية للتصدى لمرض أنفلونزا الخنازير بعدما أعلنت منظمة الصحة العالمية عن تحوله إلى وباء، تنويها إلى التنسيق مع مطار أسيوط الدولى لمراقبة وفحص جميع القادمين عبر الحجر الصحى للتأكد من خلوهم لأية أمراض وبائية.

كما تم تجهيز غرفة عمليات مركزية تم تشكيلها لإدارة هذه النوعية من الأزمات منذ فترة طويلة تعود إلى بداية مواجهة أنفلونزا الطيور، وتم عقد برنامج تدريبى عن وباء الأنفلونزا العالمى لمديرى المستشفيات العامة والتأمين الصحى والشئون الوقائية.

وأكد الدكتور أحمد عبد المنعم وكيل وزارة الصحة بأسيوط عدم وجود حالات مصابة أو اشتباه حتى الآن بنطاق المحافظة، لافتاً تفعيل نظام الترصد النشط لاكتشاف أية حالات اشتباه والتعامل معها.

ونوه وكيل وزارة الصحة عن أن المديرية جهزت مستشفيات الحميات وعدد من أقسام الحميات بالمستشفيات المركزية والعامة بالمحافظة لاستقبال أية حالات يشتبه فى تعرضها للمرض.

فيما أوضح الدكتور محمد مرزوق مدير مديرية الطب البيطرى، أن المحافظة خالية تماماً من الخنازير، وأن المديرية قامت بالتفتيش فى جميع القرى والنجوع للتأكد من خلوها من مزارع الخنازير، فضلاً عن مراقبة جميع مداخل المحافظة لمنع دخول الخنازير إليها أو تداولها حفاظاً على الصحة العامة للمواطنين
