The reason for the increase was the appearance of a new virus enterovirus 71 type B2, said Giang.

Cases of hand, foot and mouth disease (HFMD) increased sharply in Ho
Chi Minh City between April and May 2011. Samples from 174 HFMD cases
were obtained from Children's Hospitals No. 1 and No. 2 in Ho Chi Minh
City (HCMC) and from several southern provinces, including Dong Thap,
Dong Nai, Binh Duong and Tien Giang provinces and were tested in the
Laboratory of Enteric Viruses of the Pasteur Institute in HCMC. Data
from RT-PCR assays showed the presence of human enterovirus 71 (HEV
71) in 43/174 (25 percent) of cases and other enteroviruses in 91/174
(52 percent) of cases. HEV 71 was identified in 5 fatal cases of HFMD
from specimens that included CSF and throat swabs.

In 9 cases, HEV 71 was isolated in cell culture, and the sub-genotype
was identified by nucleotide sequencing. These data showed that 8
patients (including 2 fatal cases from Ho Chi Minh City) were infected
with HEV 71 sub-genotype C4, and a single HFMD case from Dong Thap
province was infected with HEV71 sub-genotype C5. According to
surveillance data obtained from the Pasteur Institute in Ho Chi Minh
City, HEV 71 C4 and C5 strains have been prevalent in southern Viet
Nam since 2003.

[ProMED-mail welcomes receipt of this authoritative report.

It is clear that, as elsewhere, the epidemiology of HFMD in Viet Nam
is complex and that human enterovirus 71 plays a major role in the
causation of this disease. As commented previously, a great diversity
of human enterovirus 71 strains circulate globally, and no particular
genogroup has been specifically associated with severe disease.
Different human enteroviruses co-circulate in the community annually.

According to a recent review (Lee, T.C. et al., Pediatr Infect Dis J.
2009 Oct;28(10):904-10), the emergence in recent years of the human
enterovirus 71 epidemic in the Asia Pacific region has been associated
with the circulation of 5 genetic lineages (genotypes B3, B4, C1, C2,
C4) that appear to be undergoing rapid evolutionary changes. More
recent reports indicate that sub-genotype C4 predominates in China at
the present time (Mao LX, et al., J Clin Virol. 2010 Oct;49(2):100-4),
and this, on the basis of the above report, appears to be the
situation in Viet Nam also. The identification of a B2 sub-genotype
virus in the preceding ProMED report from Viet Nam [Hand, foot & mouth
disease - Viet Nam (02): (QN, HCM) 20110604.1712] appears not to be
indicative of the current epidemiological situation in this part of
South East Asia.

13-year-old dies from hand, foot and mouth disease
Last updated: 6/9/2011 14:00

A 13-year-old boy died from hand, foot and mouth disease last weekend, becoming the 12th HFMD fatality in Ho Chi Minh City this year.

The boy fell sick several days prior to his death and was admitted to hospital when his conditions worsened, according to HCMC Children?s Hospital 2.

His parents thought he just caught the normal flu and it was rather late when they brought him to hospital. The boy had serious complications, which led to his death.

He is a rare case because the HFMD usually attacks children under five, doctors said.

Dong Nai reports seven dead from hand, foot and mouth disease
Last updated: 6/8/2011 14:10

A two-year-old boy in the southern province of Dong Nai died from hand, foot and mouth disease (HFMD) on Tuesday, bringing the HFMD fatalities in the province to seven this year.

Nguyen Minh Vao of Long Thanh District was admitted to hospital a day earlier with high fever and respiratory disorder.

Doctor Nguyen Le Da Ha of Dong Nai Children?s Hospital said the boy contracted the HFMD two days before being taken to hospital and later had complications.

The hospital is providing treatment to around 210 HFMD patients.

Ha advised families of HFMD-infected children to bring them to the hospital quickly because youth patients can have serious complications.

Apart from Dong Nai, Ho Chi Minh City has so far reported 11 deaths from HFMD and Quang Ngai Province has reported three deaths.

Dengue fever, hand-foot-mouth disease still rampant
The Ministry of Health stated on July 3 that dengue fever and hand-foot-mouth disease (HFMD) was still rampant in many provinces nationwide.

Nguyen Van Binh, head of the Preventive Medicine Department, said the two diseases are rapidly spreading in the southern and central provinces. Ninh Binh in the North has reported the most number of cases with over 300 patients, 60 percent of who contracted the disease through the lethal virus strain EV 71.

The month of June alone recorded more than 1,758 dengue fever cases in 30 provinces and cities with three fatalities, bringing the total toll in the country to 17,510 with 14 deaths. Despite the high figure, the number has decreased 2.9 percent.

Medical workers fret about the hand-foot-mouth disease because of 40 related deaths in the first six months of the year. Yet the department said the South and Central provinces still reported younger patients were contracting the disease.

The department blamed parents and district medical workers of low awareness of HFMD which was causing these severe outbreaks.

Health official warns HFMD outbreak might get worse
Last updated: 7/29/2011 14:00

...

Nguyen Van Binh, VAPM director general, spoke to Thanh Nien about the development of the disease and measures taken to prevent and treat it.

Is the HFMD a new endemic in Vietnam?

In Vietnam, HFMD cases with serious complications have been reported since 2003. That year, several HFMD-related acute encephalitis cases were recorded in Ho Chi Minh City.

In the following years, the number of HFMD cases has kept increasing, with more than 10,000 cases reported in 2008, prompting the Ministry of Health to issue guidelines for the diagnosis and treatment of the disease.

Between 2008 and 2010, Vietnam recorded an estimated 10,000-15,000 HFMD cases, including 20-30 fatalities each year. The Health Ministry has included HFMD in the list of endemics that must be reported on weekly.

This year, there has been a dramatic increase in the number of HFMD cases with up to 2,300 cases reported in a peak week. The number has decreased over the last few weeks to around 1,800-1,900 cases per week.

...

The new guidelines for the diagnosis and treatment of HFMD, issued by the Health Ministry on July 19 only focus on child patients, while there are some sources saying the disease have recently attacked adults, killing a 21-year-old patient in An Giang Province. Is it abnormal to have senior HFMD patients?

Reports say most HMFD cases and fatalities are children. So far, 96 percent of fatalities have been of children aged below five. That’s why the new guidelines only focus on child patients.

HFMD adult cases, in fact, have been reported before, but very few. As of July 28, we have not received official report of any HFMD adult fatality.

...

Do you have any predictions on the disease's development in the coming months?

Reports from previous years show that the disease usually peaks between September and November. The new school year is coming, which means the disease could spread widely. The Health Ministry is working with the Ministry of Education and Training to take more preventive measures. We’re concerned over the rising fatalities over the last few weeks.

...

There are opinions that the viruses causing the HFMD have mutated. Has the Health Ministry conducted any research on this yet?

Until now, the viruses which cause the HFMD are known ones (Enterovirus 71 and Coxsackie A).

The Pasteur Institute and other epidemic prevention centers are taking the samples of severe HFMD to test for any mutation, So far, they have yet to detect any.

Hand, foot and mouth disease - DREF operation n?
MDRVN008, 5 August 2011

CHF 127,221 has been allocated from the IFRC?s Disaster Relief Emergency Fund (DREF) to support
the Viet Nam Red Cross Society in delivering immediate assistance to 113,625 beneficiaries over a
period of four months from August to November 2011. Un-earmarked funds to repay DREF are
encouraged.
Summary: Cases of hand, foot and mouth disease have increased significantly in the last two months
in Viet Nam, affecting people in 49 provinces, mostly from the southern part of the country. As of 1
August, 25,918 cases and 74 deaths have been reported to the Ministry of Health (MOH). Children
under five years of age are the most vulnerable. Big cities are among the most affected areas and the
Ministry is leading public health interventions to reduce infection and deaths.
As a member of the National Steering Committee on Infectious Diseases and Pandemics, Viet Nam
Red Cross (VNRC) has participated in regular coordination meetings led by the Ministry of Health to
keep a close watch on the situation. In July, guidance to reduce risks of HFMD was provided by Viet
Nam Red Cross to its members, staff and volunteers in all 63 provinces. Provincial chapters,
particularly the most affected regions in the South have mobilized volunteers to conduct community
awareness-raising and education campaigns in support to HFMD prevention and control efforts of
health authorities. Red Cross staff and volunteers have also strengthened cooperation with health
authorities and other partner organizations in carrying out these efforts.
With DREF support, VNRC seeks to assist the most vulnerable communities in the five most
severely-affected provinces of Viet Nam. Target beneficiaries are parents and caregivers of children
under five years of age and those in primary school as well as their teachers. The DREF operation will
focus on the communication of risks including measures to mitigate them, to prevent further
transmission and to provide care to children who are infected by HFMD.
VNRC will consult national and local health authorities in further defining its interventions, as well as
carry out a rapid needs assessment in selected operation areas through those trained in pandemic
preparedness programmes held previously before beginning the DREF operation. Existing materials
on good hygiene such as hand-washing and coughing etiquette, home care and physical isolation of
the sick (which were developed through the pandemic preparedness programme) will be adapted and
utilized for this operation.
This operation is to be implemented for four months from August 2011, and will be completed by 30
November 2011; a final report will be made available three months after the end of the operation by
31 March 2012.
