    Dead housemaid tests positive for bird flu

    A housemaid who died from a lung infection at Dr. Soetomo General Hospital in Surabaya over the weekend was infected with Avian flu, the hospital revealed Wednesday.

    Chairman of the Press Forum at state-run hospital Urip Murtedjo said Sunday that a blood sample taken before the victim's death and sent to the Health Ministry had tested positive for Avian Flu.

    The victim, identified only as S, and who is believed to have worked as a housemaid for a family in the city, was transferred from to the hosptical from a private hospital last Friday after suspicions were raised that she was suffering from the fatal disease.

    "Initially, we did not know but remained suspicious of the patient's symptoms. After her death, the hospital's medical team sent a sample of her blood to the Health Ministry and the laboratory test result *confirmed* she was infected with the H5N1 virus," he told The Jakarta Post.

    Urip said the hospital's medical team had doubted she was infected with the disease after learning the patient was not exposed to poultry at her work place or in her home village in Lumajang.

    "This case has received a serious level of attention from the medical team because it is a new phenomenon,
    " Urip said.

    Chairul Anwar Nidom, a bird flu specialist at Airlangga University in Surabaya, said it would be foolish to rule out the possibility of an endemic in the city, citing recent outbreaks in Jakarta, West Java, North Sumatra and Banten.

    He said his team would continue to research the path the virus was taking across the archipelago to determine which areas were vulnerable.

      MAROS, South Sulawesi – Fishery, Marine and Livestock Service Maros culled approximately 79 birds at Pao-Pao, Bajingmangai, Mandai, Maros Subdistrict.

      Head of Health service of Maros, Firman Jaya, announced Ansar as bird flu suspect patient since he develop bird flu like symptoms such as high fever and sore throat.

            - it was exposed to infected poultry droppings inhaled dust at the village streets
            - or she got it directly by air droplets from an infected human

              Under the radar an outbreak of H5N1 in poultry in Yogjakarta (Central Java), 2 "fever patients" mentioned, no details. A snip from a report from jan 4 this year:

                  Actually it is not new, we seem to have problems regarding diagnosing avian influenza in Indonesia for some time, among others:

                  - "Stealth chickens " , a-symptomatic poultry could spread the virus, without triggering the attention of the doctors to bird flu, no chickens which "died suddenly";

                  - The virus could spread in all kinds of ways, especially in cities, even through the air indeed. Live markets, trucks loaded with chickens riding through the streets, etc. Not every (severe) respiratory case seems to be tested for avian influenza.

                  Serosurveillance studies are being carried out, by dr Nidom from Surabaya University and in Tangerang, the Pilot Project.

                  Did not see results from these studies, but we all remember dr Nidom stating that a large percentage (>50%) of the Jakarta inhabitants could have anti-bodies for avian influenza.

                      The above comments describe the latest lab confirmed H5N1 case in Indonesia (see updated map). The absence of a poultry link is cause for concern. The level of H5N1 in poultry in Indonesia is high, but the lack of OIE reports since 2006 from Indonesia limits specifics.

                      Similarly, the delays in reporting confirmed cases by Indonesia and WHO complicates analysis in humans. The WHO has not issued a situation update for confirmed cases, and since Indonesia has imposed a news blackout on cases, the updates lag disease onset dates by weeks or months.

                      Moreover, misdiagnosis of cases in clusters has also contributed to a serious undercount. The Ministry of Health has denied index cases diagnosed as lung inflammation, dengue fever, or typhus, even after family members have been H5N1 confirmed.

                        If the above figured out to be correct, than now the human organism in the endemic avian bird flu virus zones are mainly accustomed to that virus, so the deadly episodes are more sporadic than spreadings (?)
                        apart this, than the new dangers could came mainly from an human form of bird flu which seems/maybe to not exists at the moment
