    REPORT on CASES of BIRD FLU until April 2010

    Throughout February up to April 2010 there are two additional positive cases of bird flu (H5N1).


    First case is, HA (F, 45 yr), housewife, from Malang. Case was found by routine inspection
    sentinel surveillance Influenza Like Illness (ILI) in the center city.
    Patient came to the Health Center on February 23, 2010 with complaints ILI. Because the Health Center

        Avian influenza ? situation in Indonesia
        6 May 2010 -- The Ministry of Health of Indonesia has announced two new confirmed cases of human infection with the H5N1 avian influenza virus. A 45-year-old female from the city of Malang in East Java province developed symptoms on 22 February. She recovered and is in a healthy condition.           LAPORAN KASUS FLU BURUNG SAMPAI JUNI 2010

          Sepanjang bulan Mei s.d. Juni 2010 terdapat tambahan satu kasus positif flu burung (H5N1), atas nama TFA (34 tahun, P), warga DKI Jakarta. Kasus dilaporkan positif terinfeksi H5N1 berdasarkan hasil pemeriksaan Laboratorium Pusat Penelitian Biomedis dan Farmasi Badan Litbangkes tanggal 4 Juni 2010.
          Penderita mulai sakit tanggal 25 Mei 2010 dengan gejala demam, batuk, sesak mual, dan nyeri otot. Penderita berobat tanggal 27 Mei ke praktek swasta.

          Sehari kemudian berobat ke RS swasta di Tangerang. Selama dirawat di RS, kondisi pasien makin memburuk dengan gejala pneumonia berat. Tanggal 1 Juni pasien dirujuk ke RSUD Tangerang dan meninggal pada hari yang sama.
          Faktor risiko, sebelum sakit TFA sering merawat tanaman hias di samping rumah. Selain itu TFA juga sering berkunjung di rumah mertuanya di Kota Tangerang yang merupakan daerah endemis Flu Burung pada unggas.

          Berdasarkan observasi, tidak diketemukan kasus influenza like illness (ILI) pada semua kontak dengan kasus baik kontak serumah, tetangga dan tenaga medis.
          Dengan tambahan satu kasus ini, jumlah kumulatif Flu Burung di Indonesia sejak tahun 2005 sampai dengan Juni 2010 adalah 166 kasus dengan 137 kematian.

                  Pada tanggal 24 Agustus penderita didiagnosis suspek flu burung dan langsung diambil spesimen usap hidung dan tenggorok. Namun kondisi pasien makin memburuk dengan gejala pneumonia berat. Tanggal 27 Agustus 2010 penderita meninggal dunia.

                  LH (P, 40 tahun) ibu rumah tangga, mulai sakit 9 September 2010 dengan gejala demam, batuk, dan pilek. Pada tanggal 12 September penderita baru berobat ke UGD di sebuah RS di Jakarta Timur. Esok harinya penderita dirawat di RS lain. Tiga hari dirawat, keadaan penderita belum membaik, bahkan muncul sesak. Hasil rontgen paru menunjukkan adanya gambaran pneumonia. Dokter yang merawat kemudian mendiagnosis suspek flu burung dan merujuknya ke RS Persahabatan pada tanggal 16 Oktober 2010.

                  Di RS rujukan Flu Burung ini, penderita langsung dirawat di ruang isolasi dan diberikan terapi sesuai standar. Penderita juga diambil spesimen usap tenggorokan dan hidung. Hasil rontgen paru menunjukkan adanya pneumonia yang bertambah luas dan perburukan. Akhirnya penderita meninggal pada tanggal 17 September 2010.

                  Dirjen Pengendalian Penyakit dan Penyehatan Lingkungan Kemenkes selaku focal point IHR (International Health Reagulation) telah menginformasikan tentang kasus ini ke WHO.

                    There is obviously a mistake in this announcement from DEPKES. The woman could not have been referred to the hospital a month after she died. Most likely she was referred to the hospital on September 16 and died the following day.

                            1 confirmed H5N1 case , who entered the hospital as a suspect case. May be she still is a suspect, the press release is not clear to me.
                            On the other hand: reporting a suspect H5N1 case is not what you would expect the Ministry of Health in Indonesia would do.
