    ECDC. Swine influenza outbreaks in the US and in Mexico (4/27/2009)
    Swine influenza A(H1N1)

    As of 27 April the media in three European countries (Spain, France, Scotland/UK) report a total of 12 persons under investigation for swine influenza virus infection. All of them have a history of travel to Mexico.


    The patients reported from Scotland are currently hospitalised.

    According to the Pan American Health Organization (PAHO), Mexico reports 1 455 probable cases of influenza as of 26 April.

    The number of confirmed cases in the United States (according to the US Centers of Disease Control and Prevention) has risen to 20, a further six cases were reported from Canada (according to the Public health agency, Canada).

    Further cases under investigation are reported from New Zealand, Israel and Columbia.

    The 1 455 probable cases of influenza with severe pneumonia, including 84 deaths, in Mexico are from 24 of the 32 States in this country.


    The majority occurred in the Federal District, Mexico and San Luis de Potosi States, those affected were mainly young adults previously reported healthy.

    Of the 25 confirmed cases, 17 were reported from the Federal District, six from the State of Mexico, one from Oaxaca and one from Veracruz.

    The United States declared a country-wide public health emergency on 26 April.

    A total of 20 confirmed cases of swine influenza A(H1N1) are reported from the US by the CDC.

    The eight confirmed cases in New York are all students returning from travel to Mexico.

    One case reported from Ohio State also had a history of travel.

    None of the cases reported any exposure to pigs.

    Apart from one person requiring hospitalisation, all cases reported mild clinical symptoms.

    The US CDC is updating their website at 15:00 hours each day (Atlanta time) and ECDC will update its situation reports accordingly.

    Infections of novel influenza virus A(H1N1) level alert raised

    Yesterday, 27 April, the WHO Director-General raised the level of influenza pandemic alert from level 3 to level 4.


    This decision was based primarily on epidemiological data demonstrating human-to-human transmission and the ability of the virus to cause community-level outbreaks. Given the widespread presence of the virus, the WHO Director-General considered that global containment of the outbreak is no longer feasible.

    The current focus should be on mitigation measures.

    As of 28 April, two European countries have reported confirmed cases of novel influenza virus infection: one in Spain and two in the United Kingdom. Both countries have additional persons, nine and 17, respectively, who are still under investigation. All of these confirmed cases and persons under investigation have a history of travel to Mexico.


    In France, four, and in Sweden, five persons who were under investigation have now been confirmed to be negative.


    A further seven European countries (Czech Republic, Denmark, Germany, Greece, Ireland, Italy and Poland) also have persons under investigation.


    The number of countries reporting confirmed cases with novel flu virus infection (human swine influenza H1N1) or persons with influenza-like illness being investigated for such infection is increasing rapidly.

    Outside of the European Union, a total of 79 confirmed cases of novel influenza virus infection have been reported: 40 from the United States (California 7, Kansas 2, New York City 28, Ohio 1, Texas 2), 33 from Mexico and six from Canada.


    Most of these confirmed cases in the US and all of those in Canada have a history of travel to Mexico.

    Fourteen non-EU countries are reporting persons being under investigation.

    Most of these persons, but not all, have a history of travel to Mexico or the US.

    In Mexico, more than 1600 persons are suspected of being infected.


    Mexico is the only country so far reporting deaths (5) among confirmed cases.

    All cases diagnosed outside of Mexico show a mild form of the disease.


    European response

    All EU Member States have implemented appropriate response measures in order to limit the spread of this infection to the continent. These include the provision of information to the general public and to travellers, and raising the alert on this situation among the healthcare workers.

    In addition, several Member States have implemented enhanced surveillance and/or activated their national pandemic preparedness plan.

    On a European level, the implementation of these measures is coordinated by the European Commission.

    ECDC is providing technical support to the Member States and the European Commission with regards to the surveillance of cases reported in the EU and abroad.

    Technical guidance regarding the management of infected persons and their contacts has also been prepared and existing guidance documents on personal protective measures and public health measures to be implemented in case of an influenza epidemic/pandemic are being updated.

    ECDC, in collaboration with the Member States, the European Commission and the World Health Organization, is closely monitoring this rapidly evolving situation, and provides daily updates on its website.

    The next situational update will be published at 10:00 AM CET 29 April 2009.

      ECDC - SITUATION REPORT

      Main highlights in past 24 hours


      • One additional confirmed case in the EU, in Spain, totalling four confirmed cases (2 in Spain and 2 in the UK);
      • In Europe, all investigated cases have a mild course, no deaths reported;
      • Fourteen Member States (MS) have reported persons under investigation for this novel influenza virus;
      • More cases to be expected in the EU as diagnostic capacity is being strengthened;
      • European case definition and travel advice agreed upon with MS and Commission;
      • Outside Europe, confirmed cases reported in Costa Rica, Israel and New Zealand, in addition to USA, Canada and Mexico;

      The number of countries reporting confirmed cases with novel flu virus infection (human swine influenza A(H1N1) or persons with influenza-like illness being investigated for such infection continues to increase. The report below is based on official information provided by the national public health websites, or through other official communication channels. In addition, media reports from credible sources are included.

      An update on the number of confirmed cases and persons under investigation as of today, 29 April, 08:00 hrs, is presented in Table 1.


      European Union

      One new confirmed case has been reported in the EU by Spain within the past 24 hours, totalling 4 confirmed cases (2 from Spain, 2 from United Kingdom). All of these have a history of travel to Mexico.

      Compared to yesterday?s update, 3 additional Member States reported persons being investigated for novel influenza virus A(H1N1) in their country, taking the total to 14 Member States having cumulatively investigated 146 persons.

      Persons investigated and confirmed to be negative are reported in Table 1.


      Outside of European Union

      Outside of the European Union, 124 confirmed cases of novel influenza virus infection were reported. These include 64 cases from the United States, 33 from Mexico and 13 from Canada. Most of these confirmed cases in Infections of novel influenza virus A(H1N1) the US and all of them in Canada have a history of travel to Mexico. In addition, confirmed cases have also been reported by Costa Rica (1), Israel (2), and New Zealand (11). A total of twenty non-EU countries have reported persons under investigation. In Mexico, more than 2500 persons are suspected of being infected.

      Mexico is the only country so far reporting 7 deaths among confirmed cases. Investigations are ongoing to assess reasons for severity of illness in reported cases in the country. Latest reports from the USA identified 5 persons being hospitalised.


      Assessment of situation in the EU

      Even though the present novel influenza virus has an animal origin, it is now clearly spreading among humans. As of today, it still seems that the disease is more virulent in Mexico than in other parts of the world. However, the outbreak should be closely monitored to timely identify changes in the epidemiological characteristics of confirmed cases.

      This novel flu virus has already entered the EU, and it is inevitable that more cases will be detected. So far, all the confirmed cases have acquired their infection outside the EU ? no transmission inside the EU has been reported.

      The current assessment is that the novel flu virus is not very virulent, but that it will certainly continue to spread. However, potential changes in the virus characteristics for e.g. virulence, need to be closely monitored.


      European response

      All EU Member States have implemented appropriate response measures in order to limit the spread of this infection. These include the provision of information to the general public and to travellers, and raising the alert on this situation among health care workers. In addition, several Member States have implemented enhanced surveillance and/or activated their national pandemic preparedness plan. On a European level, the implementation of these measures is coordinated by the European Commission.

      In the past 24 hours, EU Member States, the European Commission and ECDC agreed on a common definition for cases of novel influenza virus A(H1N1) infections which will ensure a co-ordinated reporting in the EU.

      A shared guidance for travellers to Mexico, the only country with known sustained human-to-human transmission, has been finalised and agreed upon.

      A more detailed case-reporting system to support an in-depth EU epidemiological assessment is being prepared in addition to guidance on optimal use of antiviral drugs and vaccines in the EU.

      ECDC, in collaboration with the Member States, the European Commission and the World Health Organisation, continues to closely monitoring this rapidly evolving situation, and provide daily updates on its website.

          Outbreak of influenza A(H1N1) virus


          The World Health Organization raised the level of pandemic alert from phase 4 to phase 5 on 29 April 2009, following sustained human-to-human transmission of the virus in Mexico and the United States.


          Countries around the world should now start implementing the actions foreseen in their national pandemic plans.

          Health experts have anticipated for many years that an influenza pandemic was due.

          Health officials in the European Union and its Member States have been working intensively on pandemic preparedness since 2005.

            ECDC SITUATION REPORT

            Main developments in past 24 hours

            • 12 new cases reported from EU and EEA/EFTA countries;
            • First confirmed case in Denmark, with exposure in New York, and first confirmed cases in France;
            • To date, 39 confirmed and 5 probable cases reported in 10 EU and EEA/EFTA countries;
            • Of these, 4 confirmed cases with no travel history to Mexico or the US;
            • Worldwide, 610 cases reported, 596 of which confirmed ;
            • EU case definition published in Official Journal of the European Union.


            The report below is based on official information provided by the national public health websites, or through other official communication channels. In addition, media reports from credible sources are included when quoting national authorities. An update on the number of probable and confirmed cases as of 2 May, 08:00 hours CEST, is presented in Table 1 and Table 2.


            European Union and EEA/EFTA countries

            Within the last 24 hours, 12 new confirmed cases were reported in the EU and EEA/EFTA countries: 2 cases in Germany, 7 cases in the United Kingdom, as well as the first cases in France (2) and Denmark (1). This brings the total to 39 confirmed cases in the EU and EEA/EFTA countries. Three new occurrences of secondary human-to-human transmission (no travel history to affected areas) occurring in the EU were reported by Germany and the United Kingdom. In Germany, a case was identified in a healthcare worker who had contact with a confirmed case. In the United Kingdom, two cases were reported with no history of travel to Mexico. This raises the total of cases with autochthonous transmission in the EU to 4, including the case previously reported in Spain1. The new confirmed case from Denmark did not travel to Mexico but is suspected to have acquired infection in New York where he travelled.


            Outside of European Union and EEA/EFTA countries

            Outside of the EU and EEA/EFTA countries, the number of confirmed cases increased to 596 influenza A(H1N1) virus infections, reported from 7 countries (Table 2). Since yesterday?s report, an additional 23 confirmed cases were reported by Canada and 32 cases by the US. One case is confirmed by Hong Kong SAR China, and 2 probable cases are reported from El Salvador.


            Assessment of situation for the EU and EEA/EFTA countries

            Two additional Member States reported their first confirmed cases. It is anticipated that other countries will also identify first cases as searching continues by Member States under the efforts to timely diagnose and thus mitigate the impact of introduction and possible spread of infection. To date, three of the 5 WHO regions have reported confirmed cases. However, in order to move to the pandemic preparedness phase 6, the WHO criteria require community-wide outbreaks in at least two WHO Regions. The overall picture for the EU and EEA/EFTA countries is that of identified introductions of infection from Mexico and most probably the US, but no sustained secondary transmission or community outbreaks are identified to date.


            European response

            A number of EU Member States are currently engaged in contact tracing activities when a case is identified, for example if they were symptomatic while travelling back on an airplane from an affected area.

            The emergence of the first confirmed case exposed in New York City should be taken into consideration regarding public health management of suspected cases.

            The effectiveness and feasibility of this measure will decline if and when the epidemics progress.

            The EU Member States ensure a daily update of probable and confirmed cases to ECDC and the European Commission, according to the common EU case definition and through the Early Warning and Response System.

            A specific EWRS data entry module has been activated on 1st May.

                    This report is based on official information provided by the national public health websites, or through other official communication channels. In addition, outside of the EU and EFTA countries, media reports from credible sources are included when quoting national authorities. An update on the number of probable and confirmed cases as of 5 May, 08:00 hours CEST, is presented in Table 1 and Table 2.


                    European Union and EFTA countries

                    Within the last 24 hours, 28 new confirmed cases were reported in the EU and EFTA countries: 13 cases in Spain, nine cases in the United Kingdom, two cases in France, one case in Portugal, one case in Germany and two additional cases in Italy. This brings the total to 107 confirmed cases in the EU and EFTA countries. Currently ten probable cases are reported, from France (3), Sweden (1) and the United Kingdom (6). However, it needs to be noted that the number of probable cases reported changes on a daily basis as some become confirmed or are discarded.

                    There are seven additional cases reported today from the United Kingdom without travel history to affected areas.

                    This increases the total number of reported cases with in-country transmission to 15 (10 from United Kingdom, 3 from Spain and 2 from Germany).


                    Influenza A(H1N1) infection - Update 5 May 2009, 08:00 hours CEST

                    • 28 new confirmed cases reported from EU and EFTA countries, among them 13 from Spain;
                    • To date, 107 confirmed and 10 probable cases reported in 12 EU and EFTA countries;
                    • 15 confirmed cases in EU and EFTA countries were infected through human-to-human transmission in the EU;
                    • No sustained human-to-human transmission has been recorded so far in the EU;
                    • Outside of EU and EFTA countries, 1182 cases are reported, 1162 of which are confirmed;
                    • Human-to-pig infection in the Canadian province of Alberta was determined to be an isolated incident with limited impact on human public health;
                    • 23 EU and EFTA countries are currently able to perform PCR to diagnose influenza A(H1N1) virus infection in humans;


                    Outside of European Union and EFTA countries

                    Outside of the EU and EFTA countries, the number of confirmed cases increased to 1162 influenza A(H1N1) virus infections, reported from 10 countries (Table 2). Since yesterday?s report, an additional 137 confirmed cases were reported by Mexico, 53 new cases by the US and 39 new cases by Canada, one of which is hospitalised. El Salvador and New Zealand confirmed two cases and Israel confirmed one additional case each.

                    During the media briefing on 4 May (http://www.cdc.gov/media/pressrel/2009/a090504.htm), the US CDC reported some early epidemiological findings from confirmed cases, including:
                    • Median age of confirmed cases is 16 years;
                    • 62% of confirmed cases are under 18 years of age;
                    • Currently there are a total of 35 hospitalisations with 1 death.


                    The suspected human-to-pig transmission of influenza A(H1N1) in a swine herd in Alberta, Canada, as reported yesterday, is currently being considered an isolated incident with no significant additional risk for public health by the Canadian authorities. Additional testing is ongoing in this swine herd and a detailed report is expected shortly. However, influenza is not a food-borne disease, and WHO, FAO and the European Commission have all stated that the safety of consuming well-cooked pork is not affected.

                    WHO reported today that initial epidemiological analysis from all reported cases worldwide suggests that the incubation period of influenza A(H1N1) virus infection is up to 8 days, and that sustained community transmission has not been identified outside of Mexico and the US.                     Assessment of situation for the EU and EFTA countries

                    As laboratory capacity in EU and EFTA countries increases in the coming days and weeks, it is to be expected that a substantial increase in case reports from these countries will be seen. This does not necessarily indicate that actual case numbers have increased, but would rather reflect the ability by countries to retrospectively confirm cases that were under investigation. WHO has been facilitating the distribution of real-time PCR kits with primers and probes specific for the influenza A(H1N1) to National Influenza Centre of 23 countries of the EU and EFTA countries
                    (http://www.who.int/entity/csr/resour...a_20090504.pdf).

                    To date, three of the 6 WHO regions have reported confirmed cases.

                    Sustained transmission is only reported in the Americas. In the EU, the reports of human-to-human transmission do not indicate that the transmission is sustained. The contexts in which these cases of human-to-human transmission have been recorded include:
                    • 1) Close household or social contact: 8 confirmed cases (5 in the United Kingdom and 3 in Spain);
                    • 2) Contact in educational facility: 5 children in the United Kingdom attending the same school as a previously confirmed case who had travel history to the US;
                    • 3) Contact in healthcare facility: one nurse and one patient in Germany, both exposed to the same confirmed case with travel history to Mexico.


                    However, it needs to be considered that there may be some bias in the detection and reporting of such transmission. Case-finding is currently mainly concentrating on people with links to affected areas, which may result in an underestimation of human-to-human and especially community transmission. The continuous close monitoring of the reports of transmission in the EU and EFTA countries is important to ensure the appropriate selection of containment versus mitigation measures. In the United States the approach has already moved on from containment to mitigation measures.


                    European response

                    The EU Member States ensure a daily update of probable and confirmed cases to ECDC and the European Commission, according to the common EU case definition and through the Early Warning and Response System.

                    ECDC will conduct bi-weekly (Tuesday and Thursday) web casts in order to cover the updated epidemiological situation as well as educational briefings on the novel virus and the implications on human public health.

                    ECDC, in collaboration with the Member States, the European Commission and the World Health Organisation, continues to closely monitor this rapidly evolving situation, and provide daily updates on its website.

                        This report is based on official information provided by the national public health websites, or through other official communication channels. In addition, outside of the EU and EFTA countries, media reports from credible sources are included when quoting national authorities. An update on the number of probable and confirmed cases as of 6 May, 08:00 hours CEST, is presented in Table 1 and Table 2.


                        European Union and EFTA countries

                        Within the last 24 hours, 18 new confirmed cases were reported in the EU and EFTA countries: 16 cases in Spain, one case in the United Kingdom and one case in Italy. This brings the total to 125 confirmed cases in the EU and EFTA countries. Currently 15 probable cases are reported, from France (8), Sweden (1) and the United Kingdom (6). However, it needs to be noted that the number of probable cases reported changes on a daily basis as some become confirmed or are discarded. Out of the 16 new cases reported by Spain, two did not have a history of travel to affected areas. This increases the total number of reported cases with in-country transmission in the EU to 17.


                        Outside of European Union and EFTA countries

                        Outside of the EU and EFTA countries, the number of confirmed cases increased to 1527 influenza A(H1N1) virus infections, reported from 11 countries (Table 2). Since yesterday?s report, an additional 215 confirmed cases were reported by Mexico, 124 new cases by the US, 25 by Canada, and one additional case by Republic of Korea. The first confirmed case from Guatemala was also reported on 5 May.

                        • 18 new confirmed cases reported from EU and EFTA countries, among them 16 from Spain;
                        • To date, 125 confirmed and 15 probable cases are reported in 12 EU and EFTA countries;
                        • 17 confirmed cases in EU and EFTA countries were infected through human-to-human transmission within the EU;
                        • No sustained human-to-human transmission has been recorded so far in the EU;
                        • Outside of EU and EFTA countries, 1549 cases are reported, 1527 of which are confirmed;
                        • Case-based reporting for confirmed cases in EU countries started on 5 May through the EWRS platform.
                        • On May 4, 2009, the Mexican Secretary of Health mentioned the following during a media briefing (http://portal.salud.gob.mx/contenido...atutina.html):
                          • 51.6% of currently reported cases are under the age of 19 years;
                          • Cases have been confirmed from 26 of the 33 states in Mexico;
                          • Seventeen of the deceased cases for which age information was available were between 21 and 40 years of age.


                        The European Commission received an updated report concerning the infection of a swine herd in Alberta, Canada, from the Canadian Veterinary officers. The report states that the Canadian contractor, who was working with this swine herd, developed influenza-like illness after travel to Mexico, but tested negative for influenza A (H1N1) after recovery. Two weeks after the return of this contractor, the pigs from this particular farm evidenced fever and loss of appetite, without mortality, and tested positive for influenza A with H1 subtype. The Canadian authorities imposed quarantine on this swine farm, and no influenza-like symptoms have been observed in any other Canadian swine herds to present. Influenza is not a food-borne disease, and WHO, FAO and the European Commission have all stated that the safety of consuming well-cooked pork is not affected.


                        The WHO press briefing from 5 May, available from http://www.who.int/mediacentre/multi.../en/index.html, reports on the main points of a technical meeting, to which ECDC participated:
                        • In all affected countries it appears that mainly younger people (under 60 years) are infected with this virus. The reasons for this need to be further investigated;
                        • The proportion of confirmed cases is similar among males and females;
                        • Ongoing investigations suggest that the incubation period is between 1 to 7 days;
                        • Some health care worker infections have been reported, but it needs to be further studied whether their infection occurred in health care settings or in other contexts;
                        • The virus which is being isolated from the various countries in the world at present remains very similar and continues to be sensitive to neuraminidase inhibitors.


                        Assessment of situation for the EU and EFTA countries

                        In-country transmission of influenza A(H1N1) has been reported in 17 confirmed cases in the EU and EFTA countries. The development of these in-country transmissions continues to be carefully monitored in order to rapidly identify the occurrence of sustained transmission with community outbreaks. The first signs of community outbreaks would come from confirmed cases for whom no epidemiological link (e.g. travel to affected areas, or close contact with a confirmed case) could be established.


                        European response

                        The public health authorities in all EU member states are ensuring consistent efforts to coordinate the timely exchange of relevant epidemiological information, allowing for an efficient response to the cases in the EU. As of 5 May, the Early Warning and Response System website allows member states to submit case-based reports in addition to the aggregated reporting already in use. The reporting of detailed information on each confirmed case will enable a more extended analysis at the European level and constitute a strong tool for risk assessment. ECDC, in collaboration with the Member States, the European Commission and the World Health Organisation, continues to closely monitor this rapidly evolving situation, and provide daily updates on its website. An update on the number of probable and confirmed cases as of 7 May, 08:00 hours CEST, is presented in Table 1 and Table 2.


                            European Union and EFTA countries

                            Within the last 24 hours, 17 new confirmed cases were reported in the EU and EFTA countries: eight cases in Spain, four in the United Kingdom, three in France and one case each from Poland and Sweden. This brings the total to 142 confirmed cases in 13 EU and EFTA countries. Currently 4 probable cases are reported, from France (3) and the United Kingdom (1). However, it needs to be noted that the number of probable cases reported changes on a daily basis as some become confirmed or are discarded. For the cases reported from Poland and Sweden, both had travel history to the United States. No new cases of in country transmission in the EU were reported.


                            Outside of European Union and EFTA countries

                            Outside of the EU and EFTA countries, the number of confirmed cases increased to 2075 influenza A(H1N1) virus infections, reported from 11 countries (Table 2). Since yesterday?s report, an additional 342 confirmed cases were reported by the US, 103 (30%) of which were confirmed in the state of Illinois. Mexico confirmed 170 new cases. Forty-four deaths are reported in total outside
                            of the EU and EFTA countries, 42 from Mexico and 2 from the US.


                            Influenza A(H1N1) infection Update 7 May 2009, 08:00 hours CEST

                            • 17 new confirmed cases reported from EU and EFTA countries;
                            • Poland and Sweden reported the first confirmed cases;
                            • To date, 142 confirmed and 4 probable cases are reported in 13 EU and EFTA countries;
                            • No sustained human-to-human transmission has been recorded so far in the EU;
                            • Outside of EU and EFTA countries, 2098 cases are reported, 2075 of which are confirmed;
                            • Mexico has reported over 1000 cases since the start of the outbreak.
                            • The Canadian National Microbiology Laboratory in Winnipeg reported today that they have completed full genetic sequencing on viral isolates from Mexico, Nova Scotia and Ontario (http://www.phac-aspc.gc.ca/media/nrr...6_h1n1-eng.php). The sequences have been submitted to GenBank?, a collection of all publiclyavailable DNA sequences from the National Institutes of Health in the US.
                            • On 5 May 2009, the Standing Committee on the Food Chain and Animal Health of the European Commission madea statement regarding the previously reported human-to-pig transmission in a swine herd in Alberta, Canada:
                              • Based on the evidence, restrictions on trade of pigs and their products would not be justified;
                              • The importance of the pigs in the epidemiology of the novel influenza A(H1N1) virus is still to be fully understood and assessed by the scientific community.


                            Assessment of situation for the EU and EFTA countries

                            With the reported confirmed cases from Sweden and Poland, the number of EU and EFTA countries reporting confirmed cases of influenza A(H1N1) is increasing. However, the suspected source of infection in these new cases continues to be from other affected areas and there is currently no evidence of sustained transmission within the EU.


                            European response
