              on March 30 2007

                The 86th AI Positive Case
                20 Mar 2007
                Specimen observation in laboratory of National Institute of Health Research and Development (Balitbangkes) and Eijkman Molecular Biological Institute, Jakarta mentions that BAJ (m, 32 years), Jakarta resident, is positive of Avian Influenza (AI). Confirmation from second laboratory was received on Friday afternoon, March 16, 2007. Up to today, number of AI case increases to 86, with 65 were dead. Case Fatality Rate (CFR) on human in Indonesia is 75.6%.
                It is a latest report (March 16, 2007) from AI Post of DG of Disease Control and Environmental Health MOHRI.
                On March 7, 2007, BAJ, who lives in Duren Sawit, East Jakarta , went to doctor for his high temperature. On March 13, he was referred to Emergency Unit of Harum Hospital and diagnosed as AI suspect. The day after, at 01.30 a.m, he got difficulty in breathing that he was referred to Persahabatan Hospital . Unfortunately, he died at 08.30 a.m. on the same day.
                BAJ is the 22 nd AI case in DKI Jakarta. Therefore, in Indonesia there are 86 people infected by H5N1 with 65 people dead. Distribution of AI confirmed cases is: West Java (28 cases, 22 dead), DKI Jakarta (22 cases, 19 dead), Banten (12 cases, 10 dead), North Sumatera (7 cases, 6 dead), East Java (6 cases, 3 dead), Central Java (5 cases, 4 dead), Lampung (3 cases, alive), South Sulawesi (1 case, dead) and West Sumatera (2 cases, alive).
                For interest of AI research and control, on March 14, 2007, surveillants (from Duren Sawit health center) and doctors (from Pondok Kelapa health center) had investigated the case. Though there is one parrot in his house, there is no dead avian and influenza-like illnesses ( ILI ) are found there.
                Public health office and health center have implemented ILI monitoring/surveillance in BAJ neighboring and are planning to do health promotion. Moreover, today, team of DG of Disease Control and Environmental Health MOHRI continue the investigation toward the case, his family, neighboring and employee in his working place. Farming office also does the investigation. The direct contact with avian has not been known yet. Further investigation is still managed.

                  S (P, 23 th) the resident Cempaka White Jakarta the Centre, began to be sick (onset) on March 28 2007 with the sign of the fever, the cough, and diarrhoea.

                    I'm sorry to ask but we are in the midst of our hospital changing over in ownership right now and I'm not getting time to read here as I usually do. Was the Doctor (I think from Indonesia) that was being treated found to be positive for AI? He was suspected last I read. And I think some other hospital employees were tested at the same time. Our hospital really depends on news we get from Flu Trackers, so once again...Thanks for all you guys do to keep everyone informed and prepared.

                      The doctor - Sandra - was tested negative and went home today.

                        The case of Indonesian Bird Flu Most in the World

                        on April 09 2007

                        RJ (in the news of the previous press was acknowledged as RY), the girl was 15 years old the Jakarta resident the positive Centre was infected H5N1, died on Thursday, on April 5 2007, struck 17.10 WIB. this SLTP Student began to show the sign was sick on March 28 2007 and began to be treated in the Hospital St Carolus Jakarta the Centre on March 30 2007.

                        With 94 cases of Bird Flu, currently Indonesia has the number of cases of most Bird Flu in the world, exceeded Vietnam that had 93 cases of Bird Flu.

                        Bird flu began to infect the Vietnamese resident since 2003, but the Vietnamese Government had succeeded in stopping the increase in the number of cases of Bird Flu in this socialist country since the end of November 2005.

                        Compared CFR, Indonesia then sat in the highest level with the Case Death Rate reached 78.72% temporary Vietnamese of 45.16%.

                        Acknowledged by the expert in the world that strain the Bird Flu virus in Indonesia was indeed more dangerous than strain the Vietnamese virus.

                        The president in fact reduced 7 instructions in order to control Bird Flu all over the Indonesian territory.

                        However, the appeal and the central government regulation and this area apparently not yet fully willingly was supported by the community.

                        In fact the key to the control was the co-operation government-community to maintain the cleanliness of the environment and himself respectively the resident as well as carried out matters that indeed were needed to prevent infection by Bird Flu.

                        Government efforts will not go smooth without the full support of the community.

                        In order to protect himself so that not infected by Bird Flu, preferably the community:

                        1.
                        Do not touch the poultry that was sick or died.
                        If already, immediately cleaned the body with soap.
                        Immediately reported this incident to RT/RW or the Village Head,
                        2.
                        Used water and soap to wash the hands and the cooking utensils.
                        Cooked the poultry and the poultry egg through to ripe,
                        3.
                        Separated the poultry from humankind.
                        Separate the new poultry from the long poultry for 3 weeks,
                        4.
                        Let investigate himself to the Community Health Centre or the hospital (especially the hospital of government reconciliation) if experiencing the sign of flu and the fever, especially after being adjacent to the poultry.

                          Riau, the 11th AI Infected Province
                          08 May 2007
                          Riau is the 11th province infected by avian influenza (AI). A (f, 29) who lived in Nangka Permai Residence, Pekanbaru, Riau, was positive of AI virus, H5N1, based on RT � PCR (Real Time Polimerase Chain Reaction) Test in Laboratory of National Institute of Health Research and Development MOHRI at Jakarta on 5 May 2007 and Laboratory of Eijkman Biological Molecular Institute at Jakarta on 7 May 2007.

                          A, a merchant, had her onset on 25 April 2007, and was hospitalized in Awal Bross Hospital , Pekanbaru on 27 April 2007. On 1 May 2007, A continued her treatment in Gleneagles Hospital , Medan , North Sumatera Utara. Two days later, 3 May 2007, at 11.30 p.m. she died at the hospital.
                          Until now, the risk factors are still unknown. A MOHRI united team will visit her residence for epidemiological investigation. The team consists of personnel from Surveillance Sub Directorate and Zoonosis Sub Directorate, both from DG of Disease Control and Environmental Health, cooperate with personnel from National Institute of Health Research and Development MOHRI. Team of Riau Provincial Health Office has started investigation earlier. Until today, the risk factors are not concluded yet in her residence.

                          Since H5N1 infects human in Indonesia, in July 2005, there are 5 provinces in Sumatera infected by disease, which infects among animal and has distributed in Sumatera throughout. From 10 provinces in Sumatera, there are still 5 province not infected yet, those are Nanggroe Aceh Darussalam, Jambi, Bengkulu, Bangka-Belitung and Riau Islands.

                          Since the last case dead on 5 April, number of infected AI cases in Indonesia reaches 95 people, with 75 people dead. The Case Fatality Rate (CFR) is 78.95%. Total distribution of H5N1 positive cases among human is:

                          The widening infected area of AI among human in Sumatera is supposed to motivate local government to be more aware of sick or sudden dead avian. Local govt. should be more comprehensive in monitoring avian trading traffic and avian trading in traditional market.

                          The health personnel should be more aware of symptoms, such as fever, high temperature and cough. According to review meeting of avian influenza surveillance training, held by DG of Disease Control and Environmental Health, on last cases, there are also stomach ache and diarrhea. When these symptoms occur, the sufferer should be immediately brought to health center to get first aid of Tamiflu. Tamiflu is effective if it is given before 2 days after the symptoms occur on AI infected people.
                          People should keep self-cleanliness and washing hands with soap, especially after having contact with avian. Avian should be placed separately from people residence, then cleaning it at least once a week, not to mention old avian from the new ones at least for 3 weeks. They should always consume good and well-cooked avian meat and eggs. When they find sick or sudden dead avian, they must report it immediately to head of village or local farming office.

                            Another AI sufferer from North Sumatera died. NS (f, 26), who lived in Medan Estate Village, Percut Sei Tuan Subdistrict, Deli Serdang, North Sumatera, was stated positive of H5N1 based on RT � PCR (Real Time Polimerase Chain Reaction) Test of Laboratories of National Institute of Health Research and Development MOHRI and Eijkman Biological Molecular Institute, on 12 May 2007.
                            This information is received by Center for Public Communication from AI Post, DG of Disease Control and Environmental Health MOHRI, on Monday, 13 May 2007.
                            NS (f, 26) showed symptoms of high temperature and difficulty in breathing on 3 May 2007. Previously, there were 5 chickens suddenly dead around her house, and she was expected consuming it. On 8 May 2007, she was hospitalized in RS Haji, Medan , and referred to RS Adam Malik, Medan, on 9 May 2007. On Saturday, 12 May 2007 at 9.30 p.m., she died.
                            With this new case, NS is the 8th case of H5N1 positive in North Sumatera. While in Indonesia, it is the 96 th case of H5N1 positive, with 76 people died. The Case Fatality Rate (CFR) reaches 79.17&#37;. Distribution of AI confirmed cases can be seen in the following table.

                            The local government must be more concern on sick and sudden dead avian and immediately take more efforts to control and prevent the transmission. Research or investigation has to be socialized to get public approval to gain information which is important for AI study.
                            Further knowledge on AI will help this communicable disease control and be expected to prevent human-to-human transmission.
                            Symptoms such as fever, high temperature and cough should be more concerned by local health personnel. When a person shows one or more of the symptoms, he/she should be immediately brought to health center to get primary help of Tamiflu. Tamiflu is effective when given before 2 days after the symptoms of AI occur.
                            For general population, they should keep their self-cleanliness and washing hand with soap, especially after having contact with avian.                               Seventh AI Sufferer in Central Java

                              23 May 2007

                              SI (f, 5 years) died on May 17, 2007 for avian influenza (AI). Laboratory results of National Institute of Health Research and Development MOHRI (Balitbangkes) and Eijkman Institute Jakarta on May 23, 2007 proved that this little girl was positively infected by H5N1 virus, which has infected Central Java.

                              From report compilation of Central Java health office, SI had shown symptoms of fever and cough since May 8, 2007. She went to private practical doctor and then to a clinic in Eromoko Subdistrict, Wonogiri District, Central Java . After getting sick for a week, on May 15, 2007, she was referred to Dr. Oen Hospital at Solo Municipality, Central Java, and the next day to Dr. Moewardi Hospital, Solo. The symptoms covered fever, cough and difficulty in breathing. Laboratory result showed that SI had leukopenia, thrombocytopenia and pneumonia. On Thursday, May 17, 2007, she died at 05.55 a.m.

                              Dr. Moewardi Hospital sent her specimen to Balitbangkes Laboratory on May 22, 2007, in the same day it sent to Eijkman Molecular Biological Institute. Still in the same day, result of serologic and RT-PCR Test of Balitbangkes showed that SI was positively infected by H5N1 virus, the AI virus among human. It was then emphasized by result of Eijkman Institute Laboratory in the next day.

                              So far, the risk factor found by investigation team of Central Java is about 20 dead chickens in her neighboring. The advance epidemiological investigation has been doing by a team, consists of personnel from DG of Disease Control and Environmental Health, Balitbangkes, health offices of Central Java and Wonogiri District, supported by health center and local personnel.

                              Therefore, in Indonesia AI has infected 97 people, and SI becomes the 77 th dead sufferer. The Fatality Rate (CFR) of AI reaches 79.38&#37;, which means 4 of 5 infected people are dead. Distribution of AI cumulative cases is in the following table.

                              Controlling AI distribution, every individual is supposed to concern and be aware of environmental health and AI symptoms. One way is self-cleanliness by often washing hands with soap, especially after having contact with avian. All families should consume healthy and well-cooked avian meat and eggs. When finding sick or sudden dead chicken, people should report it to head of village or local farming office. When someone has symptoms of fever and cough, he/she does not have to wait until it becomes difficulty in breathing or worse but immediately go to health center or local govt. hospital. Tamiflu effectively prevents virus growth only about 2 x 24 hours or 2 days after the symptoms occur.

                              When growing avian, people must register it to local farming office and check it whether potentially transmit H5N1 or not. They should always clean the cage at least once a week and place separately avian from residence. In addition, old avian should be also placed separately from the new ones for at least 3 weeks.

                              MOHRI suggests all health personnel to be more aware of similar AI symptoms, though it is almost the same with other diseases. Contact with sick or dead chicken in neighboring area can be an indication to immediately refer patient to local govt. hospital, which is AI referral center.
