Fatal H5N1 Confirmed in Riau Indonesia Toddler

          Recombinomics Commentary 14:39
          April 1, 2009

Results of the laboratory test against the sample of blood and SWAP the deceased Wahyu Ibnu Saputra, was two years old nine months, the resident Street Kuantan VII, RT07, RW04, Kelurahan Tanjung Rhu, Kecamatan Lima Puluh positive contained the virus H5N1 or bird flu. These results were raised by Kesehatan Riau Section Head, Dr Mursal Amir through his cellular phone, on Tuesday (31/3)

The above comments cite lab confirmed on H5N1 in the fatal infection of a toddler in Riau, Indonesia (see updated map). This confirmation is cited in multiple local media reports, although it is unclear if the labs in Jakarta have acknowledged the confirmation.

Last year a number of confirmed cases in Riau were reported, but timely reports of cases by the MOH in Indonesia have ceased, and WHO has failed to issue situation updates on lab confirmed cases acknowledged by the Indonesian MOH.

This WHO reporting failure may stem from discrepancies in official reports in Indonesia. In February, a UNICEF report cited four confirmed cases, which was followed by MOH statements in early March, which also cited four cases. However, only three of the four cases matched in the two reports, indicating there were five confirmed cases. Clarification without citing five cases would be problematic for a WHO update, which may have led to the lack of reports by WHO. The last WHO update for Indonesia was January 22, which described cases from 2008. UN members strengthened reporting requirements for diseases that had the potential for spread beyond national borders and H5N1 was added as a notifiable disease that required reporting within 24-48 hours of confirmation.

The failure of WHO to issue updates on any 2009 cases, even after acknowledgement of lab confirmation by the Indonesian MOH, highlights extreme oversight failures, continues to be hazardous to the world’s health.

Toddler confirmed to have died of bird flu

Rizal Harahap , The Jakarta Post , Pekanbaru | Thu, 04/02/2009 3:17 PM | The Archipelago

Blood samples taken from two-and-a-half-year-old Wahyu Ibnu Saputra, who died last week in Pekanbaru, Riau, have tested positive for the bird flu virus, an official said on Wednesday.

Azizman Saad, coordinator of the bird flu prevention team at the Arifin Ahmad hospital in Pekanbaru, said confirmation that Wahyu died from bird flu had been received by the Riau Health Office several days ago.

"The victim is believed to have contracted the virus from dead chickens in his surrounding areas," Azizman told The Jakarta Post.

Indonesia's bird flu death reached 119 as of early this month, according to data from the national bird flu commission. Blood samples of patients believed to have been infected with the virus are usually sent to the Health Research Agency of the Ministry of Health in Jakarta.

Following the confirmation, Azizman urged Wahyu's family members and neighbors in Tanjung Rhu village, Pekanbaru to go to the nearest community health center to have a check-up.

"There has not been any evidence that the bird flu infection can be transmitted from human to human, but the case shows the bird flu outbreak took place in the victim's area. It's better for those who had made contact with the victim to go to the health center even though they do not have any health problems," he said.

Azizman explained that Wahyu was intensively treated for high fever and coughing and breathing difficulties in an isolated room at Arifin Achmad hospital.

"One day before he died, his condition was improving but, finally, his condition dropped and he fell unconscious. Six doctors tried to save his life, but to no avail," he said.

Azizman explained that before the disclosure of the laboratory tests Wahyu was believed to have died from multi-organ failure.

Rini Hermiyati of the Pekanbaru Health Office said she had assigned a team to observe the health condition of the victim's family members and at the same time make an inventory of neighbors who were in contact with the victim.

"No one has been found with symptoms of bird flu, but those having contact with the victims will be closely watched out for 21 days," Rini said.

The Pekanbaru Livestock Husbandry Office made the decision to cull chickens in the victim's surrounding areas to help curb the spread of the virus. "The number of chickens raised here is small," she said.

Meanwhile, Susi Herlinda, a member of the Pekanbaru Legislative Council, criticized the administration for its failure to anticipate the bird flu infection. The death of the victim showed the administration was not aggressive enough in campaign about the danger of bird flu, she said.

"It has become a habit of our people, everybody acts only after somebody falls victim to a certain diseases. The death of Wahyu should have been used as momentum to improve," she said."

Related institutions have to create awareness among the public about the danger of the bird flu," Susi added.

The family members who fell ill are monitored and treated; how many have flu-like symptoms is not clear. At least one parent of the deceased boy is treated in hospital, it is not clear if the grandparents are in hospital too.

The above translation and comments present two very different views of the relatives of the lab confirmed fatal H5N1 infection of a toddler (3M) in Riau (see updated map). The local media notes that the father and two grandparents have had symptoms and have been hospitalized, and the father remains hospitalized. In contrast, the Jakarta Post states that no one has been found with "symptoms of bird flu", suggesting a very narrow definition is being use to make the claim.

This cluster is very similar to other clusters in Indonesia which involve lab confirmed fatal cases. In several instances relatives of the confirmed case died earlier, but were never tested. In other cases, relatives developed symptoms but recovered after treatment with Tamiflu, and were said to be H5N1 negative. In other instances, the earlier contacts were said to have died of other diseases that are associated with bird flu symptoms, such as lung inflammation, thyphus, or dengue fever.

These clusters in Indonesia continue to cause concern. Reporting of confirmed cases by WHO has ceased, as has the flow of sequence data from poultry or patients, even though Indonesia has reported the highest number of fatalities and the case fatality rate remains at 80%.

The inability of WHO to exercise oversight remains a hazard to the world's health.

                                Can I ask a question?

In the past, hasn't there been a question of family transmission? Usually, it seemed to be mother-child or child-mother, with the occasional sibling. (Am I remembering incorrectly?)

This possible child and now father & grandparents is falling out of that pattern, right? Do we know if the grandparents are maternal or paternal? That's something else to know, in considering family transmission, correct?

Sorry, if this is too basic and I'm showing my ignorance in the science part of this ... but it came to mind, this afternoon.

                                Prepping ... yup, I'm decent at that. Science ... . -k
