    H5N1 Spread To Rangpur Bangladesh

Fresh outbreak of poultry AI was confirmed on 2 December 2008 in northern district of Rangpur. On 28 November 2008, farmers reported unusual poultry mortality to the District Veterinary Officer.

The above comments are from a WHO situation update for Bangladesh. Although the update did not yet have the sub-type, the unusual mortality, positive influenza A (H5?), location (see updated map), and date of outbreak strongly suggests these are confirmed H5N1 outbreaks (number culled farms for Rangpur was increased from 7 on March 10, 2008 to 9 on December 3, 2008).

These outbreaks are about 100 miles from the closest outbreak in Assam and support the comments from Meghalaya on additional outbreaks in Bangladesh and rumored outbreaks in West Bengal.

The outbreaks in Bangladesh are also near the rumored outbreaks in West Bengal and close to the western border of Meghalaya.

Thus, the outbreaks in Bangladesh are not a surprise, and more outbreaks in Bangladesh and West Bengal are expected.

      Prevent bird flu in time

THE age-old practice of rearing poultry, at the household level has of late started changing. It is due to increase of demand for poultry, products. The demand for poultry is on the rise for a number of reasons, the foremost one is its being the cheapest source of animal protein. In response, efforts for producing poultry on commercial basis have increased. Besides meeting local demand some entrepreneurs have also started exporting poultry products.

Poultry production has thus come out of the family-venture type and trnsformed into small-scale economic units of production and marketing. This economic sub-sector has, of course, faced different problems including inadequate support of financial institutions and official agencies. The sub-sector faces the threat of bird-flu that often spreads to the poultry farms. As reported in newspapers, bird-flu has again started being noticed in farms at different locations of Bangladesh. It poses a threat to the sector.

Measures for prevention and spread of bird-flu and ultimate control of the disease have to be taken forthwith. It is to be noted that the growth of the poultry sub-sector has led to the creation of job opportunities for millions of youths. If the poultry sector is not protected with supportive measures, the after-effects will be disastrous. It is worth noting that the supply of poultry products remains commendable. The price of poultry at the current rate of Taka 80 (eighty) per kilogram is very attractive for consumers. It has given relief to consumers in the context of the comparative high prices of fish, beef and mutton. Official agencies have to take all measures for combating and controlling bird flu without loss of time to protect the sector.

Although the number of outbreaks only increased by 1 (289 to 290) and the number of farms culled increased by 1 (549 to 550), I see three increases in farms (the total does add up to 550, so the Dec 3 report should have said 547 instead of 549):

Narsingdhu increased from 66 to 67
Pallabi (1) listed for first time
Dhamrai (1) listed for first time

Three Dhaka Bangladesh H5N1 Outbreaks Cause Concern
                    Recombinomics Commentary 08:08
                    December 15, 2008

Three recent H5N1 outbreaks in and around Dhaka, Bangladesh were reported today in the update by the Ministries of Fisheries and Livestock. Details on the size of the outbreaks are lacking and they have not yet been described in the Bangladesh situation update by WHO South-East Asia Regional Office. The most recent outbreak describe was confirmed December 2 and was in the northern district of Rangpur. The three recent outbreaks were in Narsingdhu, Dhamrai, and Pallabi (see updated map).

The Pallabi outbreak is adjacent to the National Zoo in Dhaka and a few miles from the Zia International Airport (see Dhaka map).

The three outbreaks close to Dhaka suggests there are more unreported outbreaks in the area. These three outbreaks are reported days after the outbreak in the north and raise concerns that the spread in Bangladesh will be similar to the rapid spread in Assam. Moreover, the explosion of outbreaks in Malda, West Bengal has produced panic there.

The relationship between symptomatic cases in Assam to the rapidly spreading outbreaks in poultry remains unclear. Most have been identified by survey, and the extent of testing remains unclear. Samples were collected from the symptomatic cullers in Chirang, but India has never reported a human H5N1 case, or anything other than dead or dying poultry, so testing remains suspect.

Independent testing of symptomatic patients in Assam, as well as detail on the outbreaks in or near Dhaka would be useful, as would sequence data on the H5N1 isolates from India and Bangladesh.
