GENEVA/BEIJING, April 17 (Reuters) - The World Health Organization said on Wednesday that a number of people who have tested positive for a new strain of bird flu in China have had no history of contact with poultry, adding to the mystery about the virus that has killed 16 people to date.

WHO spokesman Gregory Hartl confirmed that "there are people who have no history of contact with poultry", after a top Chinese scientist was quoted as saying that about 40 percent of those with the H7N9 virus had had no contact with fowl.

"This is one of the puzzles still (to) be solved and therefore argues for a wide investigation net," Hartl said in emailed comments, though he added he did not know the exact percentage.

Re: WHO says no poultry contact in some China bird flu cases

It was usually difficult in China to connect human cases of avian influenza virus (H5N1) infections to direct exposure to poultry. So it is no surprising that this is also true for H7N9.

But see also there April 16 press release:

Retrospectively diagnosed five people, two people have been killed and three others have been cured.

The same day, Zhejiang, Jiangsu and have new outbreaks have been reported, human infection with the H7N9 avian influenza confirmed cases increased by 8 cases. In addition to a 21-year-old young women, and the remaining seven patients were in the elderly over the age of 50. 8 patients diagnosed serious condition.

Yesterday morning, the national guard Planning Commission joint prevention and control mechanism for the recent epidemic situation, the Group carried out a risk assessment.

Experts believe that human infection with the H7N9 avian influenza virus from infected poultry or contaminated environment, contact with infected poultry or exposure to live poultry market is an important risk factor for infection.

Before the source of infection has not been effectively controlled, the number of cases is still possible to continue to increase.

Evolving epidemic, as well as the possible presence of potentially infectious, CDC is also assessing the need to expand the range of active surveillance for H7N9.

Does not appear in epidemic areas, the scope of monitoring in accordance with the existing prevention and control programs, all unexplained pneumonia cases. Confirmed cases in the region, monitoring should be extended to all patients with flu-like.

■ around the epidemic

Shanghai retrospectively diagnosed cases of 5

Shanghai Municipal Health and Family Planning Commission informed the 16th Retrospective 5 cases of confirmed cases of human infection with the H7N9 avian influenza.

It is understood, April 15, the Shanghai Municipal Health and Family Planning Commission pick national CDC us inform in late March, Shanghai Shanghai Fifth People's Hospital of the sample of cases submitted to the national CDC detected four cases human infection with the H7N9 avian influenza virus positive cases. ■ interviews

Into avian flu did not come into contact with poultry

The experts suggested that the four provinces in East China rural poultry and migratory birds should prevent contact

Beijing News (Reporter Rao Pei), chief scientist of China's Center for Disease Control and Prevention, Epidemiology, Zeng Guang, Chaoyang District CDC expert advisory experts said yesterday that there are now nearly 40% of patients with H7N9 avian influenza is no clear history of poultry exposure urgent need to find the source of infection.

Reporter learned from the Chaoyang District Center for Disease Control, from the situation of the confirmed cases, cases for more than 50 years of age accounted for 78%, over the age of 60 accounted for 71% of the cases in the elderly population is susceptible groups.

Recommend screening healthy people carry the virus

Chaoyang District, discovered the first case of healthy people carry the H7N9 avian influenza virus, this is very important. "Zeng said," Now, nearly 40% of patients do not have a clear history of poultry exposure, these people in the end is how infection? Was a mystery. "

He suggested that the screening to see healthy people carry the H7N9 avian influenza virus is isolated phenomenon or a common phenomenon in other regions. If healthy people can be widely carrying the H7N9 avian influenza virus, it is necessary to appropriate prevention and control measures.

Detoxification of units that can be purchased with the H7N9 avian flu
Zeng Guang, patients with H7N9 avian influenza virus may still exist a detox period, "He did not spread to close contacts does not mean not to detox." He further explained that this is because only a handful of avian influenza receptor, allowing the virus to enter the body.

"We prevention and control of the patient how long it takes?" He said, such studies need to be strengthened, and equally important to study the virus variability.

Zeng Guang, also acknowledged the urgent need to clarify the natural history of H7N9, it would be difficult to say that the strategy of preventive or wrong. For example, the disease incubation period how long? Say seven days, but this inference is based on other avian influenza is not detected according to the disease itself. "

H7N9 vaccine of public concern, Zeng Guang, or virus during the study period, should avoid the virus did not study on the vaccine.

The East four provinces should prevent infection of poultry in rural areas
Speculated that the H7N9 virus East, both poultry and wildfowl, with poultry infected and carry into the ecological chain - feeding, trafficking, slaughter, and then entered the city.

"We are concerned about the city, but four provinces in East China to pay more attention to rural areas." Zeng Guang, Beijing is concerned, it should not importation of poultry. Also attach great importance to migratory birds, avoid migratory birds contact with poultry. "Overall, Beijing is still very safe."

■ treatment

The Farmer Participation infection medical treatment expenses may be reimbursed

Beijing News (Reporter Wei Ming words) in the future, Farmer Participation infected with H7N9 avian influenza, medical costs included in the new rural cooperative medical reimbursement, outpatient and inpatient medical costs charged pro rata reimbursement. Yesterday, national guard Planning Commission issued a notice on good new rural cooperative medical support for people infected with H7N9 avian influenza prevention and control work.

Taking into account the infection of H7N9 avian influenza patients are treated in large hospitals in the provincial and municipal, "notification" Special requests can assume human infection with the H7N9 avian influenza diagnosis and treatment of patients with severe medical institutions, as the new rural cooperative medical institutions designated to treat. When the patient is discharged from the hospital, the new rural cooperative medical reimbursement of medical expenses part directly advanced by the medical establishment, patients pay only own expense. The national guard Planning Commission requirements, never allowed to be affected due to cost issues and give up the treatment of the phenomenon.

Re: WHO says no poultry contact in some China bird flu cases

they could show the (anonymized) questionnaires of the cases

so we can compare with H5N1 , in China,Indonesia,Egypt,India,

they should have done such a thing with seasonal flu long ago

make infection-certificates in computer-readable form
where they think they may have got it from

location,weather,travel history,job,#contacts,symptoms,dates,
...

I never saw this. We should have at least some hundred such
case-certificates per flu-wave, per WHO-region

Re: WHO says no poultry contact in some China bird flu cases

I think that Beijing has started doing serology testing of human contacts; would be good to see the same testing of known case contacts in Shanghai (for example). If there were low level infections or asymptomatic carriers early on in the outbreak it should show up in antibody tests by now. They need to rule out subclinical H2H, and since Shanghai seems to be the earliest primary outbreak area, its the place to look.

If such testing is negative... Cats, dogs, rodents? I seem to remember one paper that suggested that H5N1 could be transmitted by flies??
see http://www.flutrackers.com/forum/showthread.php?t=194043&highlight=h5n1+flies

Perhaps time to start thinking laterally.

Re: WHO says no poultry contact in some China bird flu cases

how many flies would you test ...
if flies were involved, we should know it by now.
What we had already were pikas, but they had strange sequences,
with recombinations. And there are presumably no pikas in Shanghai ?!
http://jvi.asm.org/content/83/17/8957.abstract

Then we had tree-sparrows, also with strange sequences

the slaughtering and wetmarket ban should show a decrease in cases
