This is a 56 page document published May 2006, referenced yesterday in a ProMed posting. In my review I found little that was targeted specifically for humanitarian agencies but the document itself is a valuable resource for any health or social service agency.

A few highlights:
The pdf file page numbers differ from the document page numbers. I am using the pdf page number first with document page number in parenthesis.

Page 24 ? Plan of action during phase 6
Page 33 ? Annexes begin
Page 40 (34) Annex 2b ? provides social mobilization actions for phase 3-5.
Page 46 (40) Annex 5 ? table providing recipes for disinfectant solutions
Page 47 (41) ? sample calculations for pre-positioning supplies.I highly recommend the document for assistance in preparation.
