29 city schools hit with swine flu

All four boards lack plan to deal with pandemic

As the number of Ottawa schools with suspected H1N1 flu cases doubles to 29, the city's four school boards -- with more than 120,000 students on their rolls -- have no plan of their own to respond to a flu pandemic, the Citizen has learned.

The boards say they will rely on the city's pandemic plan if disaster strikes, even though that plan specifically asks individual groups and agencies to draw up their own complementary plans.

"Every organization, agency, or group needs to create a pandemic plan that considers the impact of an influenza pandemic on its organization," the city plan says under a section titled "Organization-Specific Pandemic Plans."

Unlike Ottawa, the Toronto District School Board, the largest in the province, has its own detailed blueprint to deal with a flu pandemic.

Sherry Nigro, the manager responsible for pandemic planning at Ottawa Public Health, said Friday that "evidence we have today is that 29 schools have been affected by the H1N1 virus since we became aware of it."

That is up from 13 earlier this week.

Nigro also said the virus is now spreading here and officials aren't able to trace it to another infected person, a development British experts, who have noted the trend in their country, say is a sign of an impending pandemic.

To date, at least 37 confirmed cases of the H1N1 virus have been discovered in Ottawa. Of that number, 17 have been found among children, including two babies and one child who were hospitalized within the last two weeks.

"We are seeing cases in Ottawa with no specific link to another person. It has been happening in Ontario. That is what we call community spread," Nigro said. "We know that it is fairly transmittable and we know that we are not going to prevent it so what we have to do is minimize it."

Dr. Isra Levy, Ottawa's medical officer of health, alluded to the need for continued vigilance in a recent Citizen interview.

"The commonest misperception that I've run into in the last few weeks is the notion that this problem has gone away and we don't have to worry about it, which I think is perhaps more the problem than some of the early concern," he said.

As unease about the flu virus among public health officials grows, Ottawa school boards remain unruffled about their own lack of a pandemic response plan.

The city's plan emphasizes the importance of advance planning so that when disaster strikes, systems in which everyone knows what to do kick into action smoothly. It is why institutions like the Ottawa police, airport authority and hospitals all have emergency response plans.

But the school boards say since they are educators and not health experts, it is better to rely on the expertise of the Ottawa's Public Health Department and the city's pandemic plan. The boards said they've been following the department's advice in telling parents how to cope with the outbreak, and cautioning children to wash their hands.

"Our plan is their plan. Our plan and the Catholic board's and the French public board and (French) Catholic board, all our plans when it comes to issues like this, is what we are directed by the health unit," said Sharlene Hunter, a spokeswoman for the Ottawa-Carleton District School Board, the largest in the city with more than 72,000 students in 150 schools.

"What they ask us to do is what we do because we feel they are experts in that area."

Mardi de Camp, spokeswoman for the Ottawa Catholic School Board -- with 39,000 students in 80 schools -- agreed, saying she has never seen a separate pandemic plan for school boards. "On all matters having to do with flu-like illnesses in the community, our touchstone is the Ottawa public health," she said. "Our response plan is done in conjunction with Ottawa public health."

However, French public school board spokeswoman Andr?e Myette said their crisis prevention and intervention plan has a section that includes guidelines on how to deal with a potential pandemic. The document is now being updated, she said, to bring it in line with the city's plan.

The approach adopted by the Toronto District School Board, which has about 270,000 students, is completely different.

Chris Broadbent, the Toronto board's manager of health and safety, said because its priorities are different from the city's, the board drew up a separate plan to deal with the needs of students and their parents. It lays out in detail what responsibilities school board employees have during a pandemic, who their leaders would be, what parents should expect, and how things like home instruction work.

"Our plan is based on the fact that we are trying to deliver education to students. It is really designed to let our parents know how we will deal with our students in a pandemic, and ensuring continuity of education for the students," Broadbent said. "Our plan dovetails with the city's. There are components that are common, but it is a separate school board plan."

Preparations for a flu pandemic stepped on the national stage Friday as federal Minister of Health Leona Aglukkaq announced $10.8 million in funding over three years for a national influenza research network.

The funds will support the work of 80 scientists from 30 research and public-health institutions across Canada who will test the safety and effectiveness of new vaccines against the H1N1 flu virus, Aglukkaq said.

"Thanks to far-sighted investments and lessons learned from the SARS crisis, Canada's research community is well-prepared to deal with this issue," she said during an announcement at Toronto's Mount Sinai Hospital.

Led by Dr. Scott Halperin, director of the Canadian Centre for Vaccinology in Halifax, the research network is a partnership between the Canadian Institutes of Health Research and the Public Health Agency of Canada.

As of Wednesday, a total of 1,795 laboratory-confirmed cases of H1N1 flu virus have been reported in nine provinces and three territories in Canada. Three deaths -- two in Ontario and one in northern Alberta -- have been linked to swine flu.

Funding for the research network comes from a $1-billion commitment in the 2006 federal budget for pandemic preparedness.

Globally, 21,940 people are infected with the H1N1 flu virus and 125 have died, according to the World Health Organization. On Friday, the agency's pandemic alert level remained at five, its second-highest level on a scale of one to six.

H1N1 hits local elementary school
Parents are being warned to keep their children at home if they have flu-like symptoms following two confirmed H1N1 cases at St. Clare Catholic School and high absentee rates at four other Ottawa schools.
Dr. Isra Levy, the city?s medical officer of health, said St. Clare was one of the first schools in Ottawa where the higher number of missing students was partly H1N1-related. As such, students were tested for the virus.

As of Tuesday, June 2, four other schools have also reported higher-than-usual absentee rates. Given information found at St. Clare, it is ?safe to assume? the other schools are also being impacted by the flu virus, Levy said.

Public Health is working closely with the school boards to monitor and manage the presence of H1N1, originally referred to as swine flu. While Public Health usually asks institutions to report absentee rates of 15 per cent and higher, Levy said they asked schools, nursing homes and other institutions to report rates over 10 per cent as part of its ongoing surveillance for H1N1.

While exact numbers were not immediately available, Ottawa Catholic School Board spokesperson Mardi de Kemp indicated the absentee rate at St. Clare is higher than normal.

She noted health officials have been out speaking to school staff and information has been sent to parents.

Levy said where absentee rates are high, health officials are working with principals and posting signs in schools to ensure information is getting out. A letter has already been sent out reminding parents children exhibiting flu-like symptoms should stay home. Those with severe symptoms, like high fever and difficulty breathing, should seek medical attention.

According to Levy, there have been 40 confirmed cases of H1N1 in the area. Most of the cases across Ontario have been mild, while a handful have required hospitalization, Levy said. For the last three to four weeks those diagnosed with the virus have no history of travel, he continued, explaining that means it is in the community.

While there?s no suggestion the spread of the virus is abating, Levy indicated it?s hoped the spread of H1N1 will slow in summer when influenza tends to quiet down anyway.

In the meantime, residents are being reminded of proper hygiene and Public Health continues to monitor the situation.

The number of confirmed cases of swine flu in Ottawa is up to 45 and climbing, with no signs of it going away "in the forseeable future, if ever," Dr. Isra Levy, Ottawa's chief medical officer, said yesterday.
Children from 20 different Ottawa schools have been infected, Levy said. This week, Levy sent letters home with students reminding parents of basic flu prevention techniques because, he cautioned, "if it isn't in all the schools, there's a good chance it will be."
The identities of the schools with the confirmed cases have not been released, but officials from all four Ottawa school boards said they will inform every parent from the schools where a child has been infected.
The public health unit began monitoring schools for the H1N1 virus, commonly known as swine flu, about six weeks ago when it asked principals to alert them to high rates of absenteeism. Many of the schools are now free of the virus and the infected students have returned to class.

Flu cases at city schools continue to climb

OTTAWA-With the world teetering on the edge of the declaration of a flu pandemic, the city?s public health unit now says 37 schools have reported suspected cases of H1N1, up from 29 schools last Thursday.

As well, 107 people had tested positive for H1N1, almost double the 56 people who tested positive as of last Thursday. About half were under the age of 16.

The World Health Organization said Tuesday that the world was getting ?very, very close? to a swine flu pandemic. Under WHO guidelines, a key criteria for declaring a pandemic is community spread outside the first region in which the disease was reported.

In this case, that is outside the Americas. So far, 1,211 cases have been recorded, for example, in Australia. In all, 26,563 infections including 140 deaths have been reported from 73 countries.

?One of the critical issues is that we do not want people to panic if they hear that we are in a pandemic situation,? Feiji Fukuda, WHO?s acting assistant director-general, said on Tuesday.

As of Monday, a total of 2,446 laboratory-confirmed cases of H1N1 flu virus have been reported in all provinces and territories in Canada except Newfoundland.

The Ottawa Public Health Unit says officials are now unable, in some cases, to trace the spread of the virus from one person to another, a development that experts say is a sign of an impending pandemic.

?We are seeing cases in Ottawa with no specific link to another person. It has been happening in Ontario. That is what we call community spread,? Sherry Nigro, the manager responsible for pandemic planning at Ottawa Public Health, said last week. ?We know that it is fairly transmittable and we know that we are not going to prevent it so what we have to do is minimize it.?

The growth in the number of cases, and concerns about the spread of the illness in the region, has prompted CHEO to issue a set of guidelines for parents rattled by reports of H1N1 in schools and who are contemplating a trip to the emergency room.

CHEO?s emergency room has been busier-than-expected with an increased number of patients with flu-like illness, said Dr. Ken Farion, associate medical director of CHEO?s emergency department.

Just because a child has flu-like symptoms, however, doesn?t mean they have H1N1. Usually the flu season has tapered off by now, but this year there are a number of flu strains making the rounds, said Farion.

?If you?re sick, stay at home to prevent the spread of all infections ? not just this one,? he said.

?Most of what we?re seeing is milder disease. They have a fever and they have a cough, but they?re still able to get around and eat and drink. If this is the case, they should stay at home.?

Farion said parents should be concerned if their child has difficulty breathing, there are concerns that the child has become dehydrated because of lack of fluids, if the child is extremely listless or if there is an underlying health condition such as immune system weakness or a respiratory condition.

Parents should also be concerned if a child with mild to moderate symptoms appears to get better, then gets worse. The flu may have caused a weakening of the immune system, leading to a secondary infection such as bacterial pneumonia.

Many parents are bringing their child to the emergency room to seek testing, but this is usually only done if the child has an underlying condition.

?Showing up doesn?t mean you?re going to get tested,? said Farion

?Because we?re not testing the vast majority of patients, whether they have Flu 1 or Flu 2 isn?t really relevant. The focus should be on symptom management.?

The vast majority of CHEO patients with flu-like symptoms are being discharged to home with the instructions that they may have H1N1 or another flu. CHEO has only admitted about one or two flu patients a day, and these are often children with underlying medical conditions.

Farion said most healthy children with the flu are ill for four or five days and are significantly improved within a week. For infants and toddlers and children with underlying conditions, it may take longer to recover.

Children and families who arrive at the CHEO emergency department are screened for febrile respiratory illness. Those who have symptoms will be directed to a separate isolation area to see a nurse wearing full protective equipment. These extra measures mean it takes staff longer to see patients, and the sickest patients will be seen first.

Farion advises parents who decide to visit the emergency room to ensure healthy siblings stay at home.

?We don?t need the volume and congestion,? he said.

Meanwhile, the emergency room at the Queensway-Carleton Hospital has had a ?record week? ? but only a tiny proportion of the patients have been ill with the flu and the hospital is still puzzling over what caused the bump in numbers, said chief of emergency Dr. John Earle.

One day last week, for example, the emergency department logged 230 visits when a normal flow would be 185 patients.

?It?s usually what you would see on July 1 or Boxing Day,? he said.
