Oregon Teen Loses Legs to Mystery Illness

By LAUREN BROWNE, M.D.
Dec. 7, 2012

An Oregon teen has lost both her legs to a mysterious infection, leaving doctors searching for answers.

Tabitha Schulke, an 18-year-old woman who wanted to devote her future to helping others as a missionary, is now fighting for her life in the critical care unit of a Portland Hospital.

The teen first started feeling ill on Thanksgiving morning, when she came down with flu-like symptoms, but in a strange turn of events, she quickly developed gangrene on her feet.
...
Doctors still do not know what exactly has transformed Schulke from a healthy-appearing young teen into a critically ill patient fighting for her life.

Signs point to toxic shock syndrome, a disease with a 50 percent chance of survival caused by certain types of Staphylococcus bacteria. The syndrome can often initially look like a flu infection, but can quickly worsen with high fevers, dangerously low blood pressure and organ failure.

The rare condition is sometimes seen after superficial skin wounds become infected or when cloth is packed in the nose to stop a common nosebleed. And about half of all cases occur in menstruating women or women using barrier contraceptive devices, according to the New York City Bureau of Communicable Disease.

"I recommend that women use pads," said Dr. Philip Tierno, an expert on toxic shock syndrome and director of clinical microbiology and immunology at New York University's Langone Medical Center. "Or use tampons from the health food stores that are 100 percent pure cotton."

Tabitha Schulke, 18, first exhibited flu-like symptoms on Thanksgiving morning. In the days that followed, a swift-moving infection left her body swollen with fluid and sent her into shock...

...Schulke has suffered damage to her brain, liver and spleen and only has use of her left arm, the family said...

...Exactly what the illness is or what caused it is unknown, even three weeks after it started...

Teen Tabitha Schulke Nearly Dies from Mysterious Illness, Loses Legs

By Ashley Davis, Thu, February 28, 2013

A mother who thought her 18-year-old daughter had the flu, was shocked to find she had a mysterious, deadly illness after she started developing gangrene on her feet.
[snip]
Donna was forced to make the decision to cut her daughter?s legs off alone, as she is a single mother. She wondered if it would be best to let Tabitha go, as her body was so damaged from the mysterious infection that she had no idea if she would suffer brain damage or go into a coma.

She decided to have them amputated.

Doctors said it was unlikely Tabitha would survive the illness, as they felt there was little they could do to help her.

But thanks to a specialized emergency team at Legacy Emanuel, they were able to stabilize her with ?extracorporeal membrane oxygenation,? a technique that gives support to patients with severely damaged hearts and lungs.

Donna said she told her daughter goodbye at least 12 times, as Tabitha suffered from multiple organ failure.

Andrew Michaels performed the surgery, which lasted 8 hours, but saved her life.

?To save her life, we committed her to a lifetime of disability. Had we done the right thing?? he said. He told Tabitha the news when she woke up as he ?wanted her forgiveness.?
[snip]
Despite the shock and upset, Tabitha said her mother made the right decision to amputate her legs.

Tabitha spent 82 days in the hospital and was allowed to go home last week, where she stays in a hospital bed in the living room.

I don't think the cause of Tabitha's illness has been identified, but there is recent news of her progress and in August there will be a benefit concert to help pay for wheelchair equipped van for her:

This article was about an earlier benefit held for her:

Two days before Thanksgiving, Tabitha Schulke was an energetic and happy 18 year old girl volunteering her time putting together food boxes and then delivering them in the driving rain to hungry Dream Center families. - See more at: http://wsfc.org/home/tabitha-schulke-benefit-concert/#sthash.MRQAAyRw.dpuf
