[There is no indication in this report if these JE virus infections
have been laboratory confirmed. This area is located in the northern
area of the Ganga River plain, where JE virus probably remains
endemic. There have been significant JE outbreaks in Nepal in the
past, with 24 districts having been classified as endemic previously
(see ProMED archive no. 20060831.2475). The 1st outbreak was reported
in 1978, and there have been sporadic outbreaks since.

JE vaccine being administered in capital

Kathmandu, July 19 - District Public Health Office started administering Japanese Encephalitis (JE) vaccine in the urban areas of Kathmandu just four days before the fiscal year 2009/10 was to end.

Similarly, the DPHO Sarlahi had also started administering the vaccine in some areas of the district .

However, DPHO Makwanpur failed to administer the vaccine in the district as per its plan when the Child Health Division of the Department of Health Services did not send the vaccine to it.

The government last year allocated budget to distribute the JE vaccines in Kathmandu Sarlahi and Makawanpur districts.

Child Health Division under Department of Health Services said that it would launch the vaccine programme in Makwanpur next year.

The DPHO admitted that the programme could not succeed due to lack of adequate publicity of the programme and the delay in the dispatch of the vaccines.

Bishworam Shrestha, chief of the DPHO Kathmandu, said that the vaccine programme was delayed as the vaccines did not arrive in time.

Due to the lack of publicity fewer people participated in the vaccination programme this year.

"Had we got the vaccine in time, we could have administered the vaccine in time," Shrestha said.

Child Health Division of the Department of Health Services was designated to deliver the vaccine to the concerned DPHOs.

People aged over one year except pregnant women and those suffering from chronic diseases can take vaccine. It can shield the Japanese Encephalitis through out the life.

"If we do not administer the vaccine this time, we will not get the budget next year," he said adding, "It will invite controversies as it did last year if we fail to provide fresh vaccine this time."

Controversies were erupted last year when the vaccines were distributed a few days before their expiry.

Around 70,000 vile vaccines had to be destroyed due to the controversy.

The government had brought about 600,000 vile vaccines last year as per the schedule.

Only the Chinese Company produced the JE vaccine till now.

It takes around six months to import JE vaccine, DPHO sources said.

"We have failed to bring the vaccine as per the need," vaccine division chief of Child Health Division under Department of Health Services Krishna Bahadur Chand said.

A total of 65,000 vile vaccines were kept for Kathmandu and 35,000 vile were dispatched for Sarlahi this time, according to the Division sources.

Due the lack of proper plan, the vaccines were wasted last year, this year, there was lack of vaccine, sources said.

However, the coverage was less than 50 per cent, DPHO said.

DPHO Kathmandu had a plan to administer the vaccine to around 1,000,000 people.

Considering the low coverage, the DPHO has again enlarged the programme schedule. According to Shrestha, every person can take the vaccine within seven days in different 22 Ward clinic of Kathmandu Metropolis.

"If Nepal Army, Armed Police Force and Nepal Police personnel and the students want to take vaccine, we will ready to administer the vaccine to them," he said.

JE has emerged as a new problem in Kathmandu. A total of 74 JE cases were found in Kathmandu in 2008 and 54 cases in 2009. This year, four cases were confirmed while 70 cases are yet to be tested in the central lab.

Every one should pay around Rs. 3,200 for a vaccine in any private institution.

"When we are providing it free of cost, the people are not taking it," said Shrestha.

"It is all due to lack of publicity and public support to the laudable programme."

The government had started administering JE vaccine from Kailali and Kanchanpur districts since 2000.
