The clinical manifestations of atypical hemolytic uremic syndrome (aHUS) vary depending on the genetic background. A 19-year-old man with the C3 p.Asp1115Asn variant experienced 2 episodes of recurrent aHUS following respiratory tract infections caused by influenza and COVID-19. The patient's symptoms spontaneously remitted without plasma exchange or eculizumab treatment in both episodes. This is the first case of aHUS in the p.Asp1115Asn variant in Japan, highlighting that influenza and COVID-19 infections can be an environmental trigger to develop aHUS in patients harboring the C3 p.Asp1115Asn mutation and that aHUS associated with this mutation may not always be severe.
