Suspect bird flu patients continue to arrive at RS Sanglah

Indosiar. com, Denpasar- the number of victims of the virulent bird flu virus in Bali keeps growing. In the last two days the hospital again received 4 new suspect bird flu patients, two children and two adults.

Two children, namely Gusty Ayu Nabila Putri and Ni Kadek Tiwi.

Two adults, namely Sri Wayan Inu and I Nyoman Marta.

They were brought to the hospital because of high fever, crowded breast and having contact with birds that died suddenly.

The patients were put in a individual room in the special bird flu isolation space.

At the moment the hospital is treating 7 suspect patients, 4 of them are children.

Local authorities were desinfecting pens etc, vaccinating birds and also checked the health of residents.

Also called a baby sling in western countries. The babies carried this way have much better body tone and are not floppy. It means they are a more 'mobile' at a younger age when put down on the ground.

Off topic, but wanted to throw this out there...
I used the practice known as Kangaroo Care (almost unheard of in our locale) after the birth of our 1st baby who was 10 weeks premature. Had great success and would highly recommend it. Daddys can do it as well (without the breastfeeding part of course :D)

Researchers have found that the close physical contact with the parent can help to stabilize the preterm infant's heartbeat, temperature, and breathing. Preterm infants often have difficulty coordinating their breathing and heart rate. Researchers also have found that mothers who use kangaroo care often have more success with breastfeeding and improve their milk supply. Further, researchers have found that preterm infants who experience kangaroo care have longer periods of sleep, gain more weight, decrease their crying, have longer periods of alertness, and earlier hospital discharge.
Sorry for the thread drift :oem. Now we return to the regularly schedule program....

Dutchy, your input certainly clarifies how local customs might be responsible for infant infection. I can also see that the government needs to start a huge campaign targeting hand-washing as essential.

His source of exposure is currently under investigation.

Unless I'm mistaken, some articles stated that he bought the chicken on August 25th. Some articles stated that he began symptoms on August 25th. Could not have been the chicken he purchased. I did make a reference to this somewhere? Either here or on another forum.

The Ministry of Health published this:

The investigation indicates GS culled a chicken he bought from the market near his house.

Some news sources mentioned the date august 25 for buying the chicken, you mentioned it.

Furthermore there were reports talking about a dead pig near the house of the man and a dead dog, the dog supposed to have died after digging up some chicken bones, if I am not mistaken.

The reports were nor very clear, may be we should have a second or third look at them.

Dutchie,
Yes, I have to find the articles that mentioned the actual purchase on the 25th. And I did post the article on the pig dying. Now there are two or three articles that state this.

This said he felt the symptoms three days after and entered Santa Maria hospital on the 28th.

And I think there is where I misinterpreted it:

This show?s my error in thinking. I?ll change the onset of symptoms to the 28th.

Interesting quote in this report found yesterday by Commonground, from Sumatra, Riau.

Monday, september 10

Watch out for bird flu in this rainy season

Bird flu in Riau is a horrible disease at the moment. Right now there were 23 bird flu cases (grand total, starting from 2005) 3 of them died. In the rainy season we have at the moment, the bird flu virus will circulate for three more months. The health service asked the whole society to watch out and be very carefull, especially the bird breeders.

Then the "sub head" of the health service and a someone from the Riau Food Authority say this: " There were extremely many bird flu cases in Riau, not only the 23. We will not know how much, because the cases were in the interior, dying from high fever is regarded as normal by the public" .

" We reported 23 cases, our take is there could be as many identical cases in the disctricts that were not reported". "Because of this we ask the whole community to watch out for this virus".

Then the report goes on: people should go to a clinic or to the Health Service if they display symptoms especially sudden high fever, and get medication. The dedicated bird flu hospitals are: RSUD Arafin Ahmad, RSUD Dumai and RSUD Tembilahan.

They express their concern, because they expect the virus to circulate for the next 3 months.

Note: this story is adapted from a press release.

I believe WHO has only confirmed four human cases from Riau, three have died. So, where is the information on the other 19 individuals since 2005 that have contracted H5N1? Does this seem to be speculation? The number seems very specific and the implication is that all of these other individuals survived. If this is true, that would put the CFR in Riau Province at 13%.

23 is the total number of suspected cases from Riau, isn't it?

In the report the officials did not discriminate between suspect and confirmed cases. It seems customary for regions and hospitals to mention the total number of (suspect) cases from the start in 2005.

We know the discussions about false negatives, etc, etc. It hard to draw conclusions frome here. One things strikes me, in a lot of reports, if a patient is tested negative for bird flu: most of the time there is no mentioning of a diagnosis. The patient was very ill, did not have bird flu, but ....?????
If local officials do not make a difference between suspected and confirmed cases, may be it means something.

You are right about the CFR: official CFR is around 80%, seems very high?

Indonesian officials talking about underreporting, because they want to warn the public, kudo´s indeed!

Re: INDONESIA - Human cases - Sept 3 +

Re: INDONESIA - Human cases - Sept 3 +

I was just....assuming....the 23 was all suspected, including negatives. But I agree with you about that statement in that article I found....kudos to the sub-head of health in Riau and also the person from the Riau Food Authority. I probably should have posted it like this:

Then the "sub head" of the health service and a someone from the Riau Food Authority say this: " There were extremely many bird flu cases in Riau, not only the 23. We will not know how much, because the cases were in the interior, dying from high fever is regarded as normal by the public" .

I tried something:

Denpasar 12.09.2007

Tested negative , patient returned

Sanglah; After stated negative infected by bird flu, Mertha, 75, patient from Karangasem, finally was permitted to go home from the hospital RSUP Sanglah, on wednesday (12 / 9).

Doctor Ken Wirasandhi affirmed that Mertha, who was a patient that started treatment in RSUP Sanglah since saturday (8 / 9 / 07), was stated negative according to bird flu test of the blood blood in body by the laboratory of Department of Health in Jakarata.

- snip –
-
Whereas two other patients that also were treated since saturday (8 / 9) to the day still attended in RSUP Sanglah. They are Tiwi, 2 yo, from Bongkasa Abiansemal Badung, and Nadila, 1 year and seven months, from Denpasar. The blood test of these two patients are still in process in the laboratory of the Ministry Helalth (depkes balitbangkes) in Jakarta.

According to dokter Ken, the expenses of the treatment of suspect bird flu patients will be payed for by the governement , until a negative test returns from balitbankes (lab in Jakarta).

However after that, the expenses of the continued treatment equal with type disease “become patient bersangkutan dependant/ relevant/involved (??)”

Not sure, it looks like the expenses of the hospital are paid for by the government, but should the patient stay in hospital after a negative test for bird flu, he starts paying himself (or his insurance).
