Genetic clues to new ferocity of chikungunya - 03.07.06

Research published recently could help explain the recent unprecedented Chikungunya outbreak and how it managed to infect so many people across the region.

The study in PLoS Medicine suggests that chikungunya has mutated in a way that makes it better at infecting the mosquitoes that spread the virus to people.

Last year, the virus began to infect people living in the Comoros Islands before spreading to the rest of the region.

The fever and joint pain it causes are estimated to have infected one-third of R?union's population of 790,000 and, since December 2005, about 180,000 people in southern India.

The authors of the new study, led by Sylvain Brisse of France's Pasteur Institute, identified the entire genetic sequence of virus samples from six patients in R?union and Seychelles.

They also sequenced a viral gene called E1 from samples taken from an additional 127 patients in Madagascar, Mauritius, Mayotte, R?union and Seychelles.

The results show that the chikungunya outbreak began with a strain related to East-African forms of the virus that then developed into several distinct variants.

The strain now dominant differs genetically from those involved in earlier outbreaks and this, say the researchers, could explain why the virus has become more aggressive.

In particular, they say, two changes to the structure of E1 could make the virus more likely to enter mosquito cells and replicate after the insect has fed on the blood of an infected person.

The virus was first recorded in 1952 in Tanzania and gets its name from a Swahili word meaning 'that which bends up' ? a reference to the stooped posture of infected people whose joints swell up.

Since then periodic outbreaks have hit parts of Africa and Asia. The virus is not usually fatal, but deaths have been reported in other countries during the current outbreak, raising concerns that the virus's genetic changes have also made it more deadly.

Experiments are now under way to test which of these features might be responsible for the apparent increase in the virus's ability to infect people and cause disease.

While the numbers of people infected in Reunion's is correct the number of people infected in Southern India is incorrect.

It is rather hard to get an exact figure of those infected in India because the only reports that come through are those from the government hospitals and most people in India are going to private hospitals for Chikungunya.

Based on the government reports I would say that they have reported close to 1 million cases in India across Karnataka, Andrha Pradesh, Tamil Nadu, Kerla, Mahastrata, Jalugupaur and Orissa.

However, in reality if we were to take into account all the patients that are visiting private hospitals I would say we are somewhere in the range of 10 to 20 million people infected in Southern India since December of 2005.

Experiments are now under way to test which of these features might be responsible for the apparent increase in the virus's ability to infect people and cause disease.
It is very, and I mean very, sad that these experiments have not been completed. Here we are 7 months into an outbreak of Pandemic Chikungunya virus and only now they are working on these experiments.

Very sad indeed.

As chikungunya viral fever spreads in Karnataka, health authorities suggest it should be declared an epidemic.

Experts say the disease is not fatal as the virus affected most parts of the state in the past two months.

The disease spreads through mosquito bites with symptoms including moderate to high fever, redness in the eyes and severe joint pain.

In extreme cases the patient finds it difficult to move.

Late at night my son-in-law had high fever and soon two more [members] also had high fever and started shivering with pain in the legs and joints," said Waheeda Bano, a resident of Shimoga.

Over 50,000 people are affected in Gulbarga region, about 36,000 in Tumkur over 40,000 people in Bidar and about 27,000 people in the rural areas surrounding Bangalore.

A study by the Directorate of Health and Family Welfare Services shows that over two lakh sixty thousand people are affected in 167 districts.

Aedes mosquito causes the fever and it breeds in dirty water and experts suggest that water in tanks and coolers should be frequently changed and drains in the house should be clean.

Disinfection of dirty ponds and water bodies in neighbourhoods is essential.


As the virus spreads beyond Karnataka, authorities formed a task force to take up awareness programmes to manage the situation.

Like I said above, there are more than 180,000 cases in Southern India.

This report describes that there are over 260,000 people affected in 167 districts of Karnataka.

Remember, Karnataka is only one of the states in India that is affected.

I am extremely confident that the 260,000+ people infected in Karnataka is majorly underestimating the number of cases.

In case you were wondering, there are currently more cases in Karnataka than there were in La Reunion.

You will see that it was reported back in May that there were more than 500,000 cases in Karnataka.

I always find it interesting when people forget how to count.
