Two ProMed posts. ProMed is now speculating about the involvement of Nipah virus in some of these cases. Nipah has a very high CFR, higher than 40% in some places. While that agent might be responsible for some of the encephalitis, it is not likely responsible for most of it. Nipah also tends to have some limited H2H transmission - you might read about a dead HCW if there were significant Nipah involvement.

[It is time to change the name of this thread. Since late August 2009
Uttar Pradesh has been reporting acute encephalitis of unidentified
etiology, suspected to include cases of Japanese encephalitis. One
might have expected a definitve lab result from the Mational Institute
of Virology in Pune by now. We wonder if they have excluded Nipah
virus; south UP appears to be just within the range of _Pteropus_, the
fruit bat vector. - Mod.JW]

Another 3 children succumbed to encephalitis [in Gorakhpur, Uttar
Pradesh], taking the toll of the viral disease in eastern Uttar
Pradesh districts to 545 so far this year [2009].

Additional director, health, LP Rawat said that the dead included 2
from Kushinagar and one from Maharajganj. The deaths took place over
the last 2 days. Some 11 persons suffering from the disease have
been admitted to district hospitals.

Out of the 3088 patients admitted to various hospitals with Japanese
encephalitis and acute encephalitis syndrome, 545 have died. Japanese
encephalitis is transmitted by mosquitoes and causes inflammation of
the membranes around the brain.

[Japanese encephalitis virus (JEV) and the acute encephalitis syndrome
of unspecified etiology, perhaps enteroviruses, continue to claim
victims in Uttar Pradesh. No mention was made in this report about
ongoing vaccination for JEV.

The eastern districts of Uttar Pradesh have been affected by
encephalitis and the death toll has risen to 552 here. During the last
24 hours, 2 more deaths were reported at the BRD Medical College
Hospital in Gorakhpur. Also, 7 new cases have been admitted to BRD
Medical College Hospital and the other district hospitals of Gorakhpur
and Basti divisions.

According to reports, around 4000 patients have been admitted in the
different hospitals of the State since January [2009].

[Again, these reports indicate that additional cases of encephalitis
are occurring, but without specifying the etiology. Previous reports
have identified Japanese encephalitis virus (JEV) as one of the agents
involved, but the acute encephalitic syndrome several of the previous
reports mention remains of unspecified etiology, but perhaps due to
Nipah virus or enteroviruses.

Gorakhpur (UP), Dec 8 (PTI) Two more children succumbed to encephalitis here, taking the toll in the viral disease in eastern Uttar Pradesh districts to 557 so far this year, a government official said here today.

Six new patients suffering from Japanese Encephalitis and Acute Encephalitis Syndrome have been admitted to district hospitals, he said.

"Of 557 deaths, 391 occurred at BRD Medical College and Hospital this year, while 166 succumbed to the deadly disease in other government hospitals of Gorakhpur and Basti divisions," Additional Director, Health, L P Rawat said.

Sixty four patients are undergoing treatment at BRD M C Hospital, while four are being treated at other government hospitals of the region, he said.

While poking around the National Institute of Virology site I came across this one line;

Acute Encephalitis syndrome in Eastern Uttar Pradesh, Gorakhpur area, occurred with JE as a cause in less than 20% of cases. Enterovirus as causative agent being investigated.

Gorakhpur (UP), Dec 13 (PTI) The toll due to different types of encephalitis has mounted to 560 this year in eastern Uttar Pradesh as one more child succumbed to the brain fever here, an official said today.

Five new patients with symptoms of Acute Encephalitis Syndrome and Japanese Encephalitis have been admitted to BRD Medical College Hospital and government hospitals of the adjoining districts, Additional Director (Health) L P Rawat said.

"Five deaths have occurred during past one week," he said adding, a child patient hailing from Bihar died last night.

Sixty-three patients are undergoing treatment at BRD Medical College while five are being treated at other government hospitals of the region, he said.

ProMed again. Continued speculation about enteroviruses and (unlikely) Nipah. I wonder if Chandipura virus is being considered as well.

[The number of encephalitis cases has increased in this area by 8 since the
5 Dec 2009 report (see ProMED-mail archive no. 20091205.4153). Other than
Japanese encephalitis virus, these reports continue to provide no
information about the etiological agent (presumably a virus) responsible
for the acute encephalitis syndrome mentioned, that may be due to
enteroviruses or Nipah virus.

Gorakhpur (UP), Dec 17 (PTI) The toll due to encephalitis has mounted to 562 this year in the eastern region as two more deaths were reported during past two days, a government official said here today.

As many as 14 new patients with symptoms of Acute Encephalitis Syndrome (AES) and Japanese Encephalitis were admitted to BRD Medical College Hospital and government hospitals of the adjoining districts, additional director health, LP Rawat said.

"One child succumbed today, while one death occurred yesterday," Rawat said.

Currently 65 patients are undergoing treatment at BRD MCH, while two are being treated at other government hospitals of the region, the additional director health said.

One more encephalitis death in UP, toll rises to 567

Gorakhpur (UP), Dec 22 (PTI) With the death of one more person due to encephalitis in Uttar Pradesh today, the statewide toll due to the disease rose to 567, officials said.

The patient died at BRD Medical College Hospital here, they said.

As many as 52 encephalitis patients are undergoing treatment at BRD hospital, while two are being treated at other government hospitals of the region, they said.

"However, the cases of Japanese Encephalitis, caused due to mosquito bites, have substantially dropped due to fall in temperature resulting in low mosquito density," additional director of health LP Rawat said.
