  After the adjustment of the epidemic prevention policy, as the number of people infected with the new crown continues to increase, the fever wards of various medical institutions have received a large number of patients seeking antipyretics, and the emergency department has become the place where patients with serious illnesses seek help.

  In the past three weeks, the number of visits to the emergency department of the hospital has continued to "climb". On weekdays, the number of daily consultations is about 150, and on December 25 it exceeded 450, and it seems that it has not yet reached its peak.

  Unlike fever clinics, where the number of visits is gradually falling, the emergency department is becoming more and more "crowded", and the proportion of critically ill patients is increasing. According to Qin Yuhong's analysis, the first round of large-scale infections were mainly young people who went out for more activities, and then it would spread to the elderly, a vulnerable group. When the condition is mild, many elderly people choose to carry it, and do not enter the hospital until it develops into a severe or critical condition. The changes that have taken place in the emergency department reflect this process.

  "No matter how many patients there are, we can still receive them. The key lies in the limited ability to admit and treat severe and critical patients. This is the biggest challenge that emergency departments are facing now." There are not many people in the hospital, and the hospital has taken maximum measures to concentrate the strength of ICU, CCU, RICU and other departments with critical care capabilities in the emergency department; the emergency rescue room, observation ward, and places where oxygen ventilators can be connected are all used for emergency treatment. For the rescue of critically ill patients, all hospital beds have been changed to intensive care beds, and the total number of beds has doubled; on weekdays, one doctor is in charge of two or three critically ill patients, but now "one doctor manages ten", obviously in an overloaded state.

  The only gratifying thing is that the medical staff had experience in supporting the epidemic in Hubei in the early stage, and had experience in the use of ventilators, condition assessment, and hormone application for patients with new crowns.   Tracking changes in the epidemic and dynamically adjusting drug inventory

  For Duan Jingli, director of the pharmacy department, the supply of medicines reached its peak quickly after the introduction of the "New Ten Rules".

  After the release of the "Twenty Measures", with the increase in the number of people with fever, the hospital increased the storage of antipyretic drugs, from the previous two-week dosage to the nearly two-month dosage. However, in the "New Ten Regulations" After the introduction, these preparations were all sold out within a day or two. The hospital's daily prescription volume was originally lower than the number of outpatient visits, but now it surpassed the number of outpatient visits. The pharmacy became extremely busy, and pharmacists from other departments had to be transferred to outpatient clinics.

  The rear tried its best to fill the gap. Affected by the infection of personnel, some medicines that can be contacted, no one can deliver them. In order to get the medicines into the warehouse as soon as possible, the pharmacist "guest role" played the courier brother, and went to the health and health commission and the company to pull medicines; due to a large number of medical institutions at the same time " Seeking medicine”, the pharmaceutical company’s inventory is tight, so they have to turn to foreign pharmaceutical factories for help.   As the epidemic changes, the demand for medicines is also changing. Compared with antipyretic drugs, the demand for sore throat and cough medicines is increasing; due to the increase in critically ill patients, antiviral drugs and clinical rescue drugs need to be prepared in advance. Duan Jingli said that the medication can basically meet the clinical needs, and even if there are occasional short-term supply cuts, alternative drugs can still be selected without delaying treatment. According to the amount of patients used, changes in wards, and the supply situation of pharmaceutical companies, the inventory of hospital pharmacies will continue to be dynamically adjusted.

  For fever clinics, a "tough battle" has just ended. Normally, there are only 30 to 40 visits to the fever clinic a day, but it is currently at its peak, with nearly 200 visits a day, and the waiting time in line is as long as 3-4 hours.

  At present, many medical staff in the hospital have returned to work after "overcoming illness", and the shortage of manpower has gradually improved.

  According to Rong Yihui, the next key work is the treatment of severe and critically ill patients: the elderly should be alert to silent hypoxia, young people should pay attention to myocarditis, and vulnerable groups such as pregnant women should also pay attention. The hospital's fever clinic and emergency department have established a linkage mechanism to treat patients in different grades. If the patient's condition is stable and the image of lung inflammation does not exceed 30%, he can take oral medicine and observe at home; if it is serious, he must be admitted to the emergency department for further treatment.

  Qin Yuhong told reporters that there are currently more than ten intubated patients in the emergency department. Once the patient's condition is stable, they can be transferred to another ward for continued treatment, thereby freeing up intensive care beds to accept new patients. He predicted that in another 2-3 weeks, the first wave of shock is expected to pass.
