After the death of a 30-year-old waitress, crew member of a cross of the MSC, in the dawn of Saturday (18), more ten persons were interned in the Hospital Ana Costa, in Saints, in the coast of Sao Paulo, with respiratory sharp symptoms, like flu, cough and fever.

According to the advisory body of the hospital, it is seven crew members and three passengers who were in the cross MSC Armonia and they were interned soon after the landing, in the Saturday afternoon, in Santos's Oporto. Among the crew members, there is a man of Cheerful Oporto and three foreigners - an Indian and two Thais.

The ship MSC Armonia was inspected by the health surveillance before the disembarking passengers and had authorization from health surveillance to operate again, according to the Press Office of the company. A new cruise departed on Saturday (18) 5:0 pm to Mar del Plata, Argentina, with 2 thousand persons on board. The cruise lasts 8 nights and did not say if there were any withdrawals MSC after the death of the crew.

The rapid discharge of 7 of the 10 (11?) cases likely rules out H5N1, despite the travel from Asia. H1N1 may still be possible.

Comment Seven occupants of the cruise ship MSC Armonia who were admitted to the Hospital Ana Costa, Santos (SP) with acute respiratory problems were discharged on Sunday. According to the hospital, three others - all aboard the ship - remain in hospital, under observation. At dawn yesterday, the crew Fabiana Pasquarelli, 30, died showing the same symptoms. Fabiana was admitted to hospital last Wednesday complaining of severe coughing and fever, with suspected pneumonia. In a statement, MSC Cruises regretted the incident and said it is providing every assistance to family and friends. "The company is overseeing the work of authorities and is awaiting the final reports," says the MSC. "The company strengthens further that ANVISA inspected the ship and the landing is taking place normally. Once you finish this procedure, will start boarding for the next cruise," says MSC Cruises.

[The information, at least for now, are very preliminary and inaccurate.

About death: There were other clinical (sputum, skin manifestations, for example) The time evolution? She had underlying disease? There was contact with similar cases?

Anyway, no chance here, right now, would be mere speculation. Anyway, serious bacterial pneumonias are most often caused by pneumococcus, staphylococcus, _Mycoplasma_, _Streptococcus pyogenes_ ... _Legionella_. Among the viral infections, influenza, RSV ... chickenpox, measles. Again, mere speculation, based on other (additional) information could eventually be included as possible causes other viruses and bacteria and even fungi.

What is the clinical condition of the other five crew members? The similarity of the fatal case, all had respiratory disease? What is the reason for hospitalization: clinical severity? Need for isolation?

Vaccination of the crew? Vaccination lock? Against what? Chickenpox? Meningococcus? Measles? (!?)

We will track results of laboratory investigations and possible developments.

MSC Armonia Crewmember Dies, Passengers Hospitalized

A reader of Cruise Law News in Brazil has informed us that a crewmember of the Armonia cruise ship, operated by MSC Cruises, died yesterday after being admitted to the hospital in serious condition.

A newspaper in Brazil (G1 in Sao Paulo) reports that a local woman, Fabiana Pasquarelli, age 30, was taken from the MSC cruise ship and admitted to the intensive care unit of Hospital Ana Costa.

The deceased crewmember worked as a waitress and was taken to the hospital on Wednesday with a fever and cough and was apparently diagnosed with pneumonia.

There is also speculation that she may have died of swine flu.

Five other crewmembers are also sick and have been under observation in the hospital since Friday.

The cruise line said that passengers are disembarking the cruise ship today and new passengers will board.

Another Brazilian newspaper, A Tribuna, has an account and reports that the diagnosis is unclear although it is the result of an infectious process.

Ms. Pasquarelli died of an acute respiratory failure.

February 19, 2012 Update:

A newspaper in Brazil states that seven people of the ten passengers and crew from the cruise ship were released from the hospital. Three remain hospitalized. The hospital ran tests to determine the type of infection that caused the sickness.

Five crewmembers have contacted us, wishing to remain anonymous, complaining that Ms. Pasquarelli's medical treatment was delayed. Read comments below.

We would like to inform you that our friend Fabiana Pasquarelli is died because of our boss Eugenio Manfredi.
Our bar manager and hospital wanted to give her 1 week at home to solve her sickness (at home) and after embark again, but EUGENIO MANFREDI (boss of our boss) didn´t give her, and menacing her if she desembark she didn´t come back again. She was affraid to desembark that why she become more sick.

Fabiana went to the doctor but he was too busy reading a magazine. The doctor told her: "Come again in crew time, I am busy now!". After si went to bar manager and said: "Boss, I cannot work, I have fever!" and bar manager told her: "Maybe you need a supositorium. May I offer you one?". Fabiana went again to the doctor, he made her an injection and a serious allergia exploded after on her body. The third day, when she was disembarked, she was vomiting blood. They disembarked her like a dog, her tray, her pool cap and the bottle opener were threw on the coridor, like "this one is not good for work anymore". Her luggages were left in the port and the person in charge of us, the crew purser did not even announced the familly and he did not knew to wich hospital she was send. Can you imagine that such a thing is possible in 2012? Yes, unfortunatelly it is. Our friend died efectivelly in her cabin, without nobody to care. This is the life onboard for the crew members. We are almost forced to smile to the passengers by the F&B manager, Eugenio Manfredi, but nobody knows what is in our souls. Pain, anger, tiredness, hours of stress and the idea, the permanent idea that here you are just a number that as long as is good for work...can stay onboard!

Assuming the above report is accurate, rash should be added to the list of symptoms, and might indicate why MMR vaccine was administered. I wonder if the reference to "vomitting blood" above should have been to "coughing up blood" instead.

The result of the examination will have to go out at most up to this Tuesday, when one will know already if the infection is resultant of H1N1 (pig flu), according to the infectologista Luiza Terezinha Souza, director-substitute of Adolfo Lutz. The samples, which were in Adolfo Lutz in Saints, were received in Sao Paulo in the end of the Sunday afternoon. These examinations will allow to an infectologista Evaldo Stanislau Affonso de Ara?jo, chief of the Service of Infectologia of the Hospital Ana Costa, to orientate the presence to the infected persons who are still in Ana Costa. Second Evaldo, four of seven crew members who were in the same cross of Wound presented improvements and were released in this Sunday. Other three are still interned. One still has fever. Three passengers, being two of them mother and daughter, continue in observation. The third case is a 11-year-old boy, with problems gastrointestinais. With the exception of him, all the interned ones presented similar symptoms to that of Wound Pasquarelli: cough and fever.

Examination in dead crew member of ship reveals virus influenza B, says hospital

The Hospital Ana Costa, in Saints, informed in the afternoon of this Monday (20) that the preliminary result of the examination of the respiratory secretion carried out in the waitress Fabiana of Santos Pasquarelli, crew member of the ship MSC Armonia, killed in Friday (17), had how the virus influenza isolated B. Seven hospitalized cruise discharged in Santos

Seven of the 10 persons hospitalized since yesterday [18/fevereiro/2012] Hospital Ana Costa, Santos (SP), acute respiratory symptoms, were discharged today [19/fevereiro/2012]. According to the hospital, patients will be discharged under observation at their homes.

The other three patients, all aboard the MSC Armonia, yet had improved enough to be released.

The group was hospitalized yesterday afternoon [18/fevereiro/2012], shortly after disembarking the ship, with the same symptoms of the crew who worked as a waitress, FPS, 30, who died. She was admitted to the same hospital last Wednesday [15/fevereiro/2012] and died on Friday night [17/fevereiro/2012], after complications in the context of health.

The Instituto Adolfo Lutz, where the clinical examinations were sent for analysis, is preparing to release the result as soon as possible to find the causes of death and hospitalization of crew of 10 others, according to the hospital.

Preliminary examination shows that crew killed in cruise had influenza B

The Hospital Ana Costa, Santos, on the southern coast of S?o Paulo, announced this afternoon [19/fevereiro/2012] preliminary results of the examination of respiratory secretions in the patient performed 30 years, died last Friday, March 17 [fevereiro/2012].

FSP, who worked as a waitress on the ship MSC Armonia, was hospitalized on Wednesday, 15 [fevereiro/2012], acute respiratory symptoms such as flu, cough and fever. According to the hospital, tests showed that F. had isolated the influenza virus B, influenza viruses with lower mortality.

In a statement, the hospital ensures that it is a virus that causes less impact to cause pandemics and therefore "would like to emphasize that people should feel quiet, because the risk of infection by this virus is confined to a restricted physical space ".

Except for F., whose cause of death will be elucidated in an examination obituary still under development, all other patients treated had a favorable outcome, according to the note.

The final reports and all other appropriate measures will be conducted by health authorities and the Ministry of Health, according to the memo, which says that the Hospital Ana Costa stands ready "to serve its customers and other clarifications that may be necessary."

[Although the Norovirus has gained notoriety because of outbreaks of gastroenteritis on cruise ships, influenza outbreaks are also common. The summer cruises along the West Coast, going to Alaska, are the scene of frequent outbreaks.

In Brazil, also in the summer, was the influenza B virus is not a pandemic but also because of severe outbreaks.

Crew members from an Italian-registered cruiseliner were treated for flu-like symptoms here Tuesday after a Brazilian waitress aboard the ship died over the weekend of type B flu, authorities said.

The MSC Armonia arrived in Montevideo from the Brazilian port of Santos and was supposed to depart later in the day to Buenos Aires.

A Uruguayan navy spokesman said five crew members were taken off the ship for treatment at a private clinic.

1. After investigation by the medical service of the vessel and to health services in the cities of Santos and Guaruja, it was found that there were 13 more cases among passengers and crew, compatible with respiratory flu-like illness (fever plus cough or sore throat and other flu-like symptoms), among which 12 were hospitalized, although no respiratory failure, but require observation and completing exams. All 13 crew or passengers who had any symptoms of respiratory secretion samples were collected, and blood samples to determine the etiology of the disease.

3. On February 20, 2012, analysis by PCR of respiratory secretions of the index case and six more suspected cases of flu-like illness, held at Instituto Adolfo Lutz - SES / SP, identified infection with influenza virus type B, concluding in this way, that it was an outbreak of influenza.

4. Influenza or flu commonly is caused by three virus types (A, B, C), and the type A which most commonly cause outbreaks and epidemics such as the recent 2009 pandemic H1N1 pandemic. Type C is less common and usually causes subclinical disease, with no impact on human health. The type B occurs less frequently than type A, circulates throughout the year, can cause more localized epidemics, however, like A, can cause outbreaks.

5. Clinical manifestation of the great majority of cases of both Influenza A and B, evolves uncomplicated, with spontaneous cure in 7 days, however, rare cases can progress to respiratory failure, pneumonia, septicemia and death. The cause of these adverse developments is unknown, but it is known that people with certain comorbid conditions and pregnant women are at higher risk and should therefore be treated differently and priority (see Annex Recommendations).

6. In the period from February 18 until today's date, the vessel is kept information from the daily situation of the cases, reported the occurrence of nine suspected cases on arrival in Montevideo - Uruguay and seven suspected cases on arrival in Argentina. All cases identified in these countries evolved without complications and cases initially admitted to Santos had also been high.

7. 8. These recommendations follow international standards for better control of influenza outbreaks on cruise ships and are mainly aimed at:
a) the treatment of patients, reducing symptoms and frequency of complications;
b) with an institution of chemoprophylaxis: Reducing Influenza B infection of persons who may be at greater risk of complications;
c) inform people who have higher risk of complications and, if not, were vaccinated last year should not be exposed to the risk of infection and
d) eliminate the outbreak on the ship MSC Armonia.
9. Health Surveillance Secretariat, jointly with ANVISA, the Center for Epidemiological Surveillance - SES / SP and Surveillance in the city of Santos ensure the supply of oseltamivir for treatment and chemoprophylaxis necessary and keep the event monitoring and verification of measures to be adopted.
