Researchers Uncover Key Mutations in Virus That Allow Human Infection

Nov. 16, 2006 ? - "Know thy enemy" -- many would consider that one of the most crucial rules of engagement in any war.


Now those waging war against the deadly H5N1 bird flu virus have come one step closer to knowing their enemy, or at least understanding how this crafty bug makes the leap from birds to humans.


In a letter published in the current issue of Nature, lead researcher Yoshihiro Kawaoka and his colleagues have identified two key changes that took place in some versions of the viral strain that allowed the virus to infect not only chickens and ducks but humans as well.


Kawaoka, a professor of virology at both the University of Wisconsin-Madison and the University of Tokyo, noted in the letter that the findings could help those tracking the virus determine the likelihood that a given strain could spread from birds to humans and from one person to another.


"The findings described by Kawaoka and associates address an important issue related to the process by which an avian or bird influenza virus could acquire the ability to infect humans," says Dr. Steven Hinrichs, director of the Nebraska Public Health Lab and director of the University of Nebraska Center for Biosecurity.


"It is a useful research tool," adds Dr. Gregory A. Poland, chief of the Mayo Vaccine Research Group and associate chair for research at the Mayo Clinic department of medicine in Rochester, Minn. "We can now begin to look at all of the known H5N1 strains and see whether these changes have occurred. If so, we can raise the red flag."


With regard to raising this flag, Hinrichs says that for a bird influenza virus to reach the level of a pandemic and become dangerous to humans, three things must occur. First, the bird virus must be virulent or capable of causing disease. Second, it must be a new virus that can avoid our existing immune system. And third, the virus must be able to spread from human to human.


"At the present time, the current H5N1 virus has only the first two characteristics," he says. "Dr. Kawaoka's research findings add to our ability to detect the basic element of the third characteristic, the ability to pass infection from human to human.

Mutations and Bird Flu Spread

As far as viruses like H5N1 are concerned, a mutation refers to any change that gives the virus a different ability. Like the ability to infect a dog, a cat or a pig, for example. Or the ability to spread among humans.


Poland says these changes can happen on their own, or they can arise through melding and combining with other viruses. The problem is that viruses mutate so quickly; the virus dealt with today may not be the one dealt with tomorrow. Poland says that as H5N1 continues to change and diversify, it poses a moving target for researchers who sprint to keep pace with this flu bug's evolution.


"In some of the viruses they have isolated in humans, these viruses could attach to either avian or human receptors," he says. "The implication of this is that it's as if the virus is slowly adapting and learning how to expand its host range. This is very bad news indeed for humans."
Close to a Pandemic?

Aside from the implications of the research as an early warning tool, some experts say it offers a glimpse at how close we are to a pandemic.


"By analogy, the current H5N1 virus is like an enemy that possesses a nuclear device, has the intention of using the nuclear device, but does not have a delivery missile," Hinrichs says. "When all three are present, the enemy becomes fully capable, and we must increase our readiness to respond."


"We're talking about one or possibly two mutations that would be sufficient to change the ability of the virus to bind to human tissues," says Poland. "So we're one mutation away from human-to-human transmission. Once the virus is able to do that, it's taken another significant step forward -- it's transmittable.

"And once that happens, the pandemic starts."


So far, not a single case of human-to-human transmission of H5N1 has been recorded. (This sentence is incorrect.-MHSC)Also, the United States remains apparently untouched by H5N1, as no human or bird cases have yet been reported in the country.

"The research adds to our knowledge by identifying the location of where changes in the virus could occur that would allow it to pass from human to human."


And in the fight to stem a pandemic, knowledge is a crucial weapon.
"These findings have an indirect implication for development of viral vaccines in that the target sites for the vaccine may incorporate the sites of the new mutations," Hinrichs says.


This means that those working to develop vaccines to fight H5N1 could use the newly discovered mutations to their advantage, exploiting the differences that set the human-infecting viruses apart from the crowd.
But Poland says research cannot keep pace with the accelerating changes in H5N1 and other potentially pandemic strains.


"Take 'if' out of your vocabulary. We absolutely, positively -- I will stake my life on it -- have a pandemic," Poland says. "No one knows when, how severe it will be, or what virus it will be. But what we can say is that the leading candidate right now is the H5N1 virus.


"There is no hope in keeping up with the evolution of this virus."

As long as this man has decided that homologous recombination is a non event, what he says may be true.

As long as we did not have a system to forecast weather, we could not predict it accurately. Once we "invented"/discovered a system, anyone who today says, > research cannot keep pace with the accelerating changes <

We have an information disconnect. Dr. Poland needs to read FT instead of doing anything that he continues to think is important.

I feel at risk... at very great risk. MHSC pointed out the fact that Poland does not even know that Karo exists. SURREAL!!

I agree with you, 4-ABBA, he has my respect for stepping out to warn while taking the risk of prediction (which for academics, is risky and difficult, see "Swine Flu Debacle" for an example of why...)

We're all in this together. Even if we disagree on the way the virus accomlishes its task to jump species, how it will happen- we're still in this together. I personally find the unnecesasary rants getting very wearying, and they are very counter productive to our goal of working together to find solutions and support each other. I dont want to alienate anyone who is working on this, we all need each other. IMHO we dont need more roadblocks caused by disrespectful interaction.

I called and spoke with the assistant. He had never heard of Karo. Obviously, for Polland to say there's no H2H, he had never heard of Karo either. This is not rocket science. It is not about belief. It's about factual knowledge. I also listed off FT, FW and FC as resources as well as recombinomics.com. They were appreciated. I explained and explored the meaning of no evident reassortment, no evident random mutations in long conserved sequences, and that at this time the only option I have seen other than those two is recombination which nobody knows how it works at the molecular level, but which works when its logic is applied to the information being generated and predicted. Niman's projection of S227N was not a lucky guess, and I explained that too.

I'm not solely an attack dog. I educated clearly, with lucidity and with purpose. This man is a public figure, used by the press to present information. He needs information. Others here have information they too want to share, possibliy. So, I posted the numbers.

Instead of this being an observation event I think that the front end of this pandemic needs to be a hardscrabble fight for truth. It's the only thing that has a glimmer of a chance to save humans. This is not fearmongering. This is a close read of the facts.

Todays rapid onset illnesses in kids and their neighbor and a field worker in Central Egypt, which looks like Influenza B with a Twist...that Twist being H5N1, is a slap to our faces. What I say to heighten this "leader's" awareness is pitifully little.

I sincerely think you are missing the mark. This is not a time to attend a garden party. The virus has obviously changed in Egypt big-time. I don't need 100 confirmations for the facts to be as they are.

Your definition of disrespect is a lot different than mine. In the real world of ideas, these are fought over, not ivory tower LaPuta'd. Oliver Swift pointed that out hundreds of years ago. What is deserving of respect is not the job the man holds but what he says. He said there is no H2H.

I respectfully disagree with some posters' comments. If Dr. <st1:country-region w:st="on"><st1:place w:st="on">Poland</st1:place></st1:country-region> had said this:

5, 4, even 2 years ago, it may have sounded courgeous. At this point with all the qualifiers attached, it sounds fairly mainstream. Few people would argue that at some future time, there will be a pandemic.


I absolutely agree with the reporter on this line:

Overall, I thought the article was long on sound bites and short on useful information.

I should have clarified in my previous post. I did a side by side comparision with the intent of making the first post a little easier to read. The article seemed a bit disjointed to me. Let's see if I can remember the differences... Okay, this section is changed. In the version posted above the 2nd paragraph is missing. The 3rd paragraph was attributed as a quote from Dr. Poland. In the current version, the paragraph (#3) is not credited to anyone in particular and does not contain quotes. These are minor changes that may have come from review of notes by the reporter/editor or it could have resulted from a call from Dr. Poland asking the reporter to correct his quotes.

Ranting will do no good. When I call to verify a story I am "low key". Remember - this is an international site. Individuals from many different countries can not log onto all sites everywhere. Does Dr. Poland need persons from the other side of the moon calling him?

Think before you act (or write). Pretend this might be you. How would you like to be treated on an international web site?

As a humanitarian site our policies are different than many other sites. Respect for the individual is paramount, even if he disagrees with the prevailing view here.
