JE cases rise to 12, govt calls meet

NEW DELHI: Delhi government has called an urgent meeting to discuss the steps needed to check the spread of Japanese Encephalitis in the city. The number of cases of the mosquito-borne disease, which was first reported on September 21 from Bawana area in northwest Delhi, has gone up to 12.

He, however, said that all blood samples taken from pigs in the affected areas have tested negative. Health minister A K Walia said he is going to hold a meeting of top health officials in two days. "We will be discussing the preventive steps and ways to identify the source., " he said.

From the week 40 IDSP report. I believe that these are the first reported cases in Haryana this year. This represents the largest outbreak in Haryana since 2007.

Gorakhpur: continue the brunt of encephalitis in Purvanchal. In the last twenty-four hours twenty new patients admitted to BRD Medical College while two others were killed.
The two people died and seven-year-old Arjun and Deoria and Kushinagar them three years to include the ocean. Four of those admitted to Gorakhpur, Kushinagar three, Mhrajagnj, Deoria and Siddharth two - two and Ghazipur are a patient. Still admitted to Nehru Hospital, 236 patients are being treated different wards. BRD Medical College from January 2766 until the last patient arrived for treatment, of which 491 have been killed.

After the blood samples of slaughtered pigs from areas where cases of Japanese Encephalitis (JE) were reported, came out negative, the authorities will go in for wider sampling to detect the origin of the disease. Seven people, including two children, have tested positive for the disease so far and the JE infection has been detected in five people.

?It looks like it is a localised infection, but we have not yet been able to establish the missing link, which will be provided by pigs as they play host to the disease,? said a senior official from the state health department.

?Initially, we took samples of slaughtered pigs from the affected areas, but now we have directed local agencies to collect samples from across the city,? the official added.

?It?s not an easy task to collect samples of slaughtered pigs, as their slaughtering isn?t legal in Delhi. We are taking the help of the veterinary department to trace locations where pigs are slaughtered,? said Dr VK Monga, chairman, public health committee, Municipal Corporation of Delhi (MCD).

In Gorakhpur, thousands have become a burden on their poor families

Lack of rehabilitation facilities for thousands of children, disabled here after a Japanese encephalitis attack, has made life a burden for them. Already reeling under acute poverty, these children are now an economic burden on their families. The monthly allowance given by the Uttar Pradesh government to the disabled in 2005-06 was abruptly discontinued.

?Life is worse than hell for my 13-year-old son Satyendra. Death would have been easier for him as well as the family,? said Krishna Mohan of a remote village in Maharajganj district on the India-Nepal border. His son contracted Japanese encephalitis when he was just one-year-old and has been physically disabled since then.

Had there been a rehabilitation or therapy centre nearby, Satyendra could have improved and led a dignified life, feels Krishna Mohan, whose finances do not permit him to take the boy to a bigger town for treatment.

No equipment, staff
Not that Gorakhpur has any such facility. The Manovikas Kendra at the BRD Medical College Hospital, run by the Social Welfare Department, is virtually non-functional as it has no equipment and trained staff worth mentioning. Built by the Gorakhpur Development Authority and operated by the Orthopaedics department of the hospital, the Kendra claims to have an IQ Testing Unit and physiotherapy facilities but everyone knows it hardly opens, let alone functioning. Also, people do not wish to bring their children to the hospital every day.

Desperately seeking an answer to the Gorakhpur encephalitis mystery, the Union health ministry has now turned to a group of Bangalore-based researchers to find out if their diagnostic kit can finally crack the code.

The indigenous test kit looks for as many as 23 pathogens ? all causing acute encephalitis syndrome (AES) ? in a single sample and identifies the culprit within seven hours.

Notwithstanding the recurring nature of AES outbreaks in Uttar Pradesh in the last seven years, medical investigators still have not been able to identify the causative organism. The ongoing Gorakhpur outbreak has affected close to 3,000 children and killed more than 400.

Even though doctors don?t know what is causing the outbreak, they are treating children for encephalitis symptoms. After reviewing the Gorakhpur scenario on last Friday, the Indian Council of Medical Research decided to send samples to V Ravi of National Institute of Mental Health and Neuroscience who along with B V Ravi Kumar of Xycton Diagnostics Ltd claimed to have developed the detection kit with funding from Council of Scientific and Industrial Research (CSIR). ?We hope to receive the samples within a week. This is the first time we will test it in an outbreak. There is no other diagnostic kit that can search 23 pathogens in a single sample,? Ravi told Deccan Herald.

Even though Japanese Encephalitis was originally the main cause of UP outbreaks, over the years, JE positivity in AES cases has come down to a mere seven percent in 2011 as against 36 per cent in 2005.

At the same time, the proportion of AES cases increased from 64 per cent in 2005 to more than 90 per cent in 2011, making the situation alarming. Scientists at the National Institute of Virology, Pune showed that a large part of AES is due to entero-viruses, transmitted through contaminated water.

Ravi said globally there are 75-100 organisms that cause AES, out of which 22-25 are found in India. The Xycton-NIMHANS diagnostics can detect almost all of them. The multiplex polymerase chain reaction (PCR) kit had been successfully validated with 425 samples at NIMHANS, Kumar said. When multiplex PCR was applied in 127 samples that tested negative in conventional tests, the diagnostics identified 83 of them.

Cerebrospinal fluid of AES patients would be collected in such a manner that the genetic material remains intact. Only a tiny drop of sample (0.5 cc) is required for the elaborate laboratory test that lasts for seven hours. The genetic material from the sample is first isolated and subsequently amplified in a PCR. Then it is put on a specially designed chip that contains probe for all the 23 pathogens, which include a large number of virus and bacteria as well as a solitary fungus and parasite. The test has high specificity and sensitivity.

?We are in the process of filing a patent on this technology,? said a CSIR official.

Two more in grip of Japanese Encephalitis

NEW DELHI: Two more people have been confirmed positive for Japanese Encephalitis in the city, taking the total number of patients suffering from the mosquito-borne disease to 14. According to Dr V K Monga, the MCD health committee chairman, they have not found the source of the infection yet.

"The blood samples collected from pigs in the affected areas and the ones being slaughtered have tested negative. Now, we are going to expand the screening process further and samples will be collected from at least 500 pigs to be sent to the National Research Centre for Equines at Hisar in Haryana for tests," he said. Monga added that pigs are the amplifying host for Japanese Encephalitis.

The deaths of two people two years of Siddharth Ram includes five years of Sarfraz and Bihar. Gorakhpur in the recruitment of those six, four of Kushinagar, Deoria, two, three and Bihar Mhrajagnj, Balrampur and Sntkbirnagr one - including a patient. Still admitted to Nehru Hospital, 242 patients are being treated different wards. Overall in the Medical College since January 2814 has come to treat patients of whom 501 have died.

Killer fever identified

Jalpaiguri, Oct. 28: The mystery fever that has claimed three lives and affected scores in Jalpaiguri district since July has been identified as Japanese encephalitis.

Sources in the district health department said the disease had been cracked at the Calcutta-based National Institute of Cholera and Enteric Diseases where samples had been sent for test. The results of the test show that 11 more persons are down with the mosquito-borne disease.

A five-member team from the Madurai-based Centre for Research in Medical Entomology had visited the affected areas last week and collected blood samples from people suffering from the fever. The Madurai team has not sent the results yet.

The rising number of encephalitis cases in Mumbai are becoming a cause for concern, according to city doctors.

Kasturba hospital, Brihanmumbai Municipal Corporation’s (BMC) only infectious disease hospital, is getting 2-3 cases per week.

A senior doctor from the hospital said: “Encephalitis cases are on the rise since the last one month. We have been getting 2-3 cases every week.”

However, the doctors are unsure whether it is encephalitis or Japanese encephalitis which is also a cause of major concern in parts of North India. “Japanese encephalitis is a kind of encephalitis which spreads through the bite of culex mosquito. We are not sure whether the cases in Kasturba hospital are Japanese encephalitis, but the cases are a cause for concern,” said the doctor. Japanese encephalitis has infected 14 people in Delhi so far this year. Interestingly, the hospital hasn’t sent any samples for testing at the National Institute of Virology (NIV), Pune. “Ideally they should have sent the samples to NIV, but it has not been done so far,” he added.

For nearly four decades, Komal Prasad Kushwaha, a bespectacled and soft-spoken doctor, has seen hundreds of children die under his watch.

As a senior paediatrician at the Baba Raghav Das Medical College, Dr Kushwaha has been on the frontline of a decades-old battle against encephalitis, which has claimed some 6,000 lives at his hospital alone since 1978.

Horror stories are nothing new to Gorakhpur. In one case in 2007, local police picked up a dairy farmer and owner of a blood farm - "a ruthless modern-day vampire", according to one account - who locked up men and extracted blood to sell it in the black market.

But it is encephalitis that has been the leitmotif of Gorakhpur's unending misery.

Encephalitis toll in Eastern UP at 480

Five more children died of deadly disease of Japanese Encephalitis (JE) at B R D Medical colleges, Gorakhpur taking the toll to 551 in eastern parts of Uttar Pradesh during this year. The children who died during the last twenty four hours included two from Deoria district and one each from Maharajaganj, Gorakhour and neighbouring state of Bihar ,the medical college sources said. A total of 3395 patients had been admitted to B R D Medical college since January one last,out of which 551 died this year. During the last 24 hours, 18 new patients were admitted while presently 205 patients are under treatment at medical college and other hospitals of eastern UP. The patients admitted to Gorakhpur medical college included seven from neighbouring country of Nepal and 369 of Bihar. The patients who have so far died of encephalitis included 67 from Bihar and two from Nepal. UNI UP/SB SY BST1833

Gorakhpur medical college doctors on strike

Gorakhpur: Doctors at the BRD Medical College in Uttar Pradesh's Gorakhpur where encephalitis patients are being treated have gone on a strike. One hundred and fifty resident doctors went on strike after a patient's family reportedly roughed up one of the doctors.

The doctors are demanding better security for themselves.

The strike comes at a time when Gorakhpur is reeling from an outbreak of encephalitis.
