Does he even know what he's talking about? One after another this family went. 3 died, 5 became sick, and three months later they want to test family and throw away chickens? Dengue... Which dengue does this? Thank God for honest reporting. We would have just walked away thinking it was just a girl--never knowing that it was a big cluster--and who's to say it ended with 8? Perhaps it kept on going to the clusters we have now?

The South Sulawesi governor and the family of the latest confirmed bird flu victim expressed regret Friday that the Health Ministry did not immediately disclose the cause of the 14-year-old's death.

Health Minister Siti Fadilah Supari said Thursday the government had confirmed the girl, who passed away almost three months ago, was the 48th fatality from the virus.

"Why was the announcement made almost three months later? If we had known earlier, we could have taken the necessary steps immediately," Governor Amin Syam said in Makassar on Friday.

The victim's brother, Alimin, said his family had been shocked to learn the girl died of bird flu.

"It has really shaken us, to hear such news just as we were coming to terms with her death."

Alimin said the family knew that bird flu in humans caused symptoms like the girl's, including a high fever, chest pains and shortness of breath. However, her doctor had diagnosed dengue fever.

"But our suspicion caused us to free all our chickens and pet birds and destroy the coop amd cages immediately after her funeral."

The victim was the 48th person to have died of bird flu in the country, but it was the first case to occur in South Sulawesi.

Laboratory tests conducted by the Health Ministry on fluid samples taken from the victim concluded she died of the H5N1 bird flu virus strain.

The samples were sent by the provincial health office to Jakarta on June 23, the day before the girl died. However, the test results were only announced Thursday.

Runizar Roesin from the National Bird Flu Center said the delay between the discovery of the case and the announcement of the test results was because the samples were not specifically taken from the patient on suspicion of bird flu.

Siti said they would soon carry out poultry culls in the area where the case was found.

The girl's mother and elder sister, both of whom exhibited bird flu symptoms, died before her. After her death, her father, two other siblings, an in-law and a cousin were in turns taken to the hospital with high fevers, but soon recovered.

Alidin said the family had suspected the three died of bird flu but could not confirm the matter since they were all diagnosed as suffering from dengue fever.

The head of the South Sulawesi Health Office, Andi Muhadir, said it was unwise to make assumptions but blood samples would be taken from the victim's 11 family members and her closest neighbors.

The governor has called on related offices to apply standard procedures in dealing with bird flu and asked residents to hand over their poultry to be destroyed.

My guess is the infecting dose was lower. Since it took 3 months to diagnose H5N1 in the confirmed case, I doubt that the relatives were treated with Tamiflu while hospitalized (and the dead were diagnosed with dengue fever).

What a travesty. The truth about the situation in Indonesia is literally coming out of the woodwork, and it's not nice to look at. And if it is spread out over a set of islands thousands of miles wide, do not think for a moment that it can stay isolated to that country for long--the odds that this Indonesian 'human' strain is already widespread in SE Asia are quite high.

There is no mention of other family members in the WHO update.Was there "close and prolonged exposure to a patient during a phase of severe illness" in this newest case or through casual contact ?

The Ministry of Health in Indonesia has confirmed a new case of human infection with the H5N1 avian influenza virus. The case is a 14-year-old female from Makassar, South Sulawesi Province. She developed symptoms on 18 June, was hospitalized on 23 June and died on 24 June. The case had contact with poultry near her home. This case was detected through the Ministry’s routine influenza surveillance system.

The Ministry of Health in Indonesia has confirmed an additional case of human infection with the H5N1 avian influenza virus. The case occurred in a 32-year-old man. He developed symptoms on 15 May and died on 22 May.

The case is part of a family cluster in the Kubu Sembelang village, Karo District, of North Sumatra. The man is the seventh member of an extended family to become infected with the H5N1 virus and the sixth to die. An additional person, who was the first member of the family to fall ill, died of respiratory disease on 4 May. No specimens were taken prior to her burial and the cause of her death cannot be determined. However, as her clinical course was compatible with H5N1 infection, epidemiologists at the outbreak site include this woman as the initial case in the cluster.

The newly confirmed case is a brother of the initial case. Specimens were taken on 21 May and flown the same day to Jakarta. Tests run overnight confirmed his infection. His 10-year-old son died of H5N1 infection on 13 May. The father was closely involved in caring for his son, and this contact is considered a possible source of infection.

Although the investigation is continuing, preliminary findings indicate that three of the confirmed cases spent the night of 29 April in a small room together with the initial case at a time when she was symptomatic and coughing frequently. These cases include the woman’s two sons and a second brother, aged 25 years, who is the sole surviving case among infected members of this family. Other infected family members lived in adjacent homes.

All confirmed cases in the cluster can be directly linked to close and prolonged exposure to a patient during a phase of severe illness. Although human-to-human transmission cannot be ruled out, the search for a possible alternative source of exposure is continuing.

The case had contact with poultry near her home. This case was detected through the Ministry’s routine influenza surveillance system.

It should be rewritten as follows, "The case had contact with her mother and sister, who already died. Subsequently five of her family members were rushed to hospital with similar symptoms but recovered. The Ministry did not even know about it for three months. They're still looking for chickens." The WHO just missed a cluster of 8 people. Oops.

It's because they weren't tested for bird flu but for dengue. In other words, they were looking for low platelet counts, one of the signs of dengue (and bf). The samples became useless by the time that her sample was positive for bf, and now they're again testing their blood. But it's months after the fact, so it's pretty meaningless. They won't find rising titers of IgM either--because it was so long ago, and so they'll happily announce them all to be negative. It's a really pointless exercise. Just think, these people are our front line in the battle to prevent a pandemic...

How could they not recognize it for BF when they had a cluster there less than a month before that. Also this isnt the first time you have seen these two diseases together which in itself is not a good sign.

If they have H5N1 antibodies and have an early negative sample, the patients will have "sero-converted" and be a confirmed H5N1 bird flu case. It is much easier to confirm survivors, as long as samples are tested.

After 1918, patients still had antibodies in 2004!

Thanks for the clarification, Dr. Niman. I was thinking they were only testing for rising titers, not seroconversion.
