2 Avian Flu Cases Confirmed in Indonesia
By DONALD G. McNEIL Jr.

A new cluster of avian flu cases has broken out in another remote Indonesian village, according to local reports from Indonesia, some of them confirmed by the World Health Organization yesterday.

Two cases in Cikelet village in the Garut District of West Java Province have been confirmed by laboratories approved by the health organization, becoming the country?s 57th and 58th avian flu cases. One patient died. Indonesia has now had more deaths, a total of 45, from the disease than any other country.

Infection with the A(H5N1) virus is also ?strongly suspected? in a cousin of the villager in the 57th case. The cousin died before samples could be taken, the health organization said.

Beyond those three, reports by local news services and radio stations, translated and posted on Web sites like flutrackers.com, suggest that up to eight villagers have had severe flu symptoms in recent weeks. Some died before the first victim to be tested fell ill on July 26, and one left the local hospital because he could not pay, the reports said.

Dick Thompson, a spokesman for the health agency in Geneva, said he knew of no cases beyond the two confirmed and one suspected. ?But it?s going to take a while to figure out what?s going on,? he added. So far, he said, there is no evidence of human-to-human transmission, ?but it?s the early part of the investigation.?

The case confirmed yesterday, No. 58, involved a 9-year-old girl who became ill on Aug. 1 and died Tuesday.

In Dr. Niman's post at 1;36 pm yesterday, in re: to the 9 year-old girl "developing symptoms on 1 August, being hospitalized on 14 August, and dying on 15 August", it seems like a long time between onset and hospitalization- as compared to times reported for other cases. I guess I am thinking what were the actual reported symptoms for 1 August. And what were they when she was taken to the hospital?

Doesn't the above translation indicate there are two adult females (32 or 35)? It seems that the mother of the confirmed 9F has died, but died in Slamet Hospital in West Java after being treated in Garut.

The other female is alive and in Bandung at Sadikin and with cousin/neighbors (5F and 6F).

Based on names, there seem to also be two 9 year olds. One, Ai Siti, is the confirmed case who died and is a neighbor of the two chldren now hospitalized in Bandung.

The other is Dea, daughter of Euis Lina. Both are also from Jolok and both have died.

Thus, assuming that Dea was not included in the 3 erleir deaths, the cluster would now be 11. Other than the three who died earlier, there are three clusters within the cluster.

Two cousins from Rankassalak (Index case who died and first confirmed, released case)

Confirmed 9F (dead) and 5F and 6F neighbors/cousin (in Bandung) from Jolok

Indonesian woman dies of suspected bird flu

<!-- AN5.0: background end -->
<!-- AN5.0: inline article box end --><SCRIPT language=JavaScript src="/bin/js/article.js"></SCRIPT>JAKARTA, Aug 18 (Reuters) - A 35-year-old woman with bird flu symptoms from an Indonesian village which has seen a number of other confirmed or suspected cases of the disease in humans has died, a hospital official said on Friday.
But a government official denied that the cases in the West Javan village of Cikelet, described by officials as rife with the H5N1 virus, pointed to a possible new cluster case.
"Last night a patient was referred by a doctor in Garut with symptoms of suspected avian influenza such as breathlessness and high temperature, so we isolated her and planned to send her to Bandung hospital," said Yogie Prayogi, spokesman at Dr Slamet Hospital. "But she died before we could bring her".
He said that blood samples had been sent for testing.
A 9-year-old girl from Cikelet, which is in the Garut region, died of bird flu this week according to two local tests, taking Indonesia's confirmed toll from the disease to 45, the highest of any country.
A 17-year-old youth from the remote village, actually a number of hamlets, about 90 km (55 miles) south of Bandung, has also tested positive for H5N1, but he has stayed at home after refusing to be hospitalised.
Another man also died this month with bird flu-like symptoms but samples could not be taken.
Bayu Krisnamurthi, head of the national committee on Avian Influenza Control and Pandemic Preparedness, said authorities were monitoring the 17-year-old at his home and there was no sign of infection among his family members.
He said that a 5-year-old girl and a 6-year-old girl suffering from bird flu symptoms had also been taken to hospital after house-to-house checks in the village.
But he said that there was no evidence of human-to-human transmission.
"All the cases are not clusters because they are from different hamlets," he said by telephone.
The official said that up to Thursday about 3,800 birds had been culled in the village area.
Indonesia has seen a steady increase in human bird flu deaths this year and the virus is endemic in poultry in nearly all of the provinces of the sprawling archipelago.
The country, which has been criticised for not doing enough to stamp out H5N1, has shied away from mass culling of poultry so far citing the expense and the logistical difficulties given the millions of backyard fowl.
Fears that the virus had mutated into a form that could pass easily between humans heightened in May when seven people from an extended family died of the disease in North Sumatra.
It was the biggest cluster the country has recorded.
The virus remains essentially an animal disease but experts fear it could spark a pandemic if it mutates into a form that can pass easily between humans.

I would think that most human beings, regardless of religion, would feel pained and express grief at a loved one passing away; in which case, it's only normal that one would want to console the crying/sobbing aggrieved with hugs and kisses. And such displays of comfort are not limited between family members. Perfect strangers will comfort aggrieved parents/spouses in this way, regardless of religion, not just at a wake, but also in the procession before and after burial. Was there anything more in the behaviour of these sect members that would increase the likelihood of infection? Or were the usual displays of consoling enough to trigger contagion?

Up to now, possible H2H contagion in Indonesia was described as "very close contact" as in mother hugging child for hours, cleaning body wastes, etc. If people attending a funeral in Indonesia should display bird flu symptoms after mere "comforting" contacts, without (again) any animal contacts, then wouldn't this be very bad news indeed?

toggletext frequently switches genders in translation. I think both are girls and there is no evidnece for a 5 or 6 year old boy at this time.

Gender is the least reliable identifier when using toggletext.
