State Reports Single, Possible Case of Bird Flu Virus in Child with Mild Symptoms

SACRAMENTO – California has identified a possible bird flu case in a child in Alameda County who was tested for mild upper respiratory symptoms. The child, who has been treated, is recovering at home. They had no known contact with an infected animal, but public health experts are investigating a possible exposure to wild birds.





The California Department of Public Health (CDPH) is working with local public health officials and the Centers for Disease Control and Prevention (CDC) to investigate the cause of the initial positive test in the child. The positive test showed a low-level detection of the virus, indicating the child was not likely infectious to others. Repeat bird flu testing on the child four days later was negative, and additional testing shows the child was also positive for respiratory viruses that could be the cause of their cold and flu symptoms.





The test specimens are being sent to CDC for confirmatory testing.





Family Members Tested Negative


No person-to-person spread of the virus has been detected or is suspected. Due to mild respiratory symptoms, all close family members of the child were tested – and all are negative for bird flu and not suspected of having the virus at all. Family members were also confirmed to have the same, more common respiratory viruses as the child. They are also being treated with preventive medication.





Additional Contacts Involved, No Indication of Larger Outbreak

Out of an abundance of caution, other individuals who had contact with the child are being notified and offered preventive treatment and testing. Public health officials are aware that the child attended daycare with mild symptoms before the child’s test was reported as a possible bird flu infection. Local public health officials have reached out to potentially exposed caregivers and families to conduct health checks for symptoms and offer connections to preventive treatment and testing if they develop symptoms.





“It’s natural for people to be concerned, and we want to reinforce for parents, caregivers and families that based on the information and data we have, we don’t think the child was infectious – and no human-to-human spread of bird flu has been documented in any country for more than 15 years,” said CDPH Director and State Public Health Officer Dr. Tomás Aragón.





Bird Flu in Humans is Rare


Human infections with bird flu viruses are rare, and no person-to-person spread has been detected in California or the United States. In the rare cases where bird flu has spread from one person to another, it has never spread beyond a few close contacts.





Limited and sporadic human infections with bird flu where there is not clear route of exposure or infection are expected and have occurred in the past and during the outbreak public health officials have been monitoring since 2022. Regardless, public health surveillance and investigations of such cases will continue at the local, state and federal levels to monitor for any concerning changes.





People Working with Infected Animals are at Higher Risk

People who have contact with infected dairy cows, poultry, or wildlife have a greater chance of getting bird flu, which can happen if the virus gets into a person’s eyes, nose, mouth, or is inhaled. As a general precaution, whenever possible people should avoid direct contact with sick or dead wild birds, poultry, and other animals and observe them only from a distance.





Since early October, California has reported 26 confirmed human cases of bird flu, all following direct contact with infected dairy cows. To date, all have reported mild symptoms (primarily eye infections), and none have been hospitalized. Because bird flu viruses can change and gain the ability to spread more easily between people, public health officials have provided preventive measures and are monitoring animal and human infections carefully.





CDPH has been closely monitoring the bird flu situation in both animals and humans since the state's first detection in poultry in 2022. To protect public health, and especially workers who may be exposed to infected animals, CDPH encourages those who work with infected dairy cows, raw milk, or infected poultry to use protective gear. Individuals who have symptoms or a significant exposure (such as raw milk splashing into the eye) should get tested quickly and treatment. In humans, bird flu symptoms include eye redness or discharge, cough, sore throat, runny or stuffy nose, diarrhea, vomiting, muscle or body aches, headaches, fatigue, trouble breathing, and fever.





Public health experts also encourage farm workers to get a flu shot this year. While a seasonal flu shot doesn’t stop bird flu, it protects against severe illness from seasonal flu and reduces the chance of human and bird flu viruses mixing and turning into new viruses that can spread more easily or be more severe.





Pasteurized Milk Products are Safe to Consume


Pasteurized milk and dairy products are safe to consume. Drinking raw milk or eating raw milk products could be dangerous. Reduce your risk by choosing pasteurized milk and dairy foods like cheese and yogurt. Pasteurization, the process of heating milk to specific temperatures for certain times, inactivates the bird flu virus and harmful germs that can be found in raw milk.

​(KRON) — A child from Alameda County has been identified by state health officials as testing positive for bird flu, the California Department of Public Health (CDPH) announced Tuesday. Investigators are working to determine the cause of the initial positive test result.

The East Bay child tested positive while exhibiting mild upper respiratory symptoms, CDPH said. According to the agency, the patient was treated and is recovering at home.

The child, whose age was not released, had no known contact with an infected animal. The Centers for Disease Control and Prevention (CDC), along with local public health officials, are investigating possible exposure to wild birds.

A repeat test for bird flu was conducted four days after the initial positive test. That followup test was negative, according to officials. “Additional testing shows the child was also positive for respiratory viruses that could be the cause of their cold and flu symptoms,” CDPH said.

Family members of the child tested negative for bird flu, and no person-to-person spread has been detected or is suspected by health officials. The family that was tested was confirmed to have “the same, more common respiratory viruses as the child,” CDPH said. Other people who had contact with the child were notified by the department and offered preventative measures.​..

California health officials reported Tuesday that a child in Alameda County tested positive for H5 bird flu last week.

The source of infection is not known — although health officials are looking into possible contact with wild birds — and the child is recovering at home with mild upper respiratory symptoms.

Health officials have confirmed the “H5” part of the virus, not the “N1.” There is no human “H5” flu; it is only associated with birds.

The child was treated with antiviral medication, and the sample was sent to the U.S. Centers for Disease Control and Prevention for confirmatory testing.​..

“The more cases we find that have no known exposure make it difficult to prevent additional” infections, said Jennifer Nuzzo, professor of epidemiology and director of the Brown University School of Public Health’s Pandemic Center. “It worries me greatly that this virus is popping up in more and more places and that we keep being surprised by infections in people whom we wouldn’t think would be at high risk of being exposed to the virus.”…

The child must have been sick enough to be taken to the doctor to get a test. Lots of upper respiratory illnesses going around right now and kids usually don’t get flu tested unless pretty sick. Interesting that the repeat test 4 days later was negative but showed other viruses.

CDC Confirms H5N1 Bird Flu Infection in a Child in California

The Centers for Disease Control and Prevention (CDC) has confirmed a human infection with avian influenza A(H5N1) (H5N1 bird flu) in a child in California. This is the first reported avian influenza H5 virus infection in a child in the United States. Consistent with previously identified human cases in the United States, the child reportedly experienced mild symptoms and received flu antivirals. There were low levels of viral material detected in the initial specimen collected, and follow-up testing of the child several days later was negative for H5 bird flu but was positive for other common respiratory viruses. The child is recovering from their illness. An investigation by the California Department of Public Health (CDPH) into the child’s possible H5N1 exposure source is ongoing.




During CDPH’s investigation, all household members reported having symptoms and specimens were collected from those people. All test results from members of the household were negative for H5 bird flu, and some family members were positive for the same common respiratory viruses as the child. Contact tracing continues, but there is currently no evidence of person-to-person spread of H5N1 bird flu from this child to others. To date, there has been no person-to-person spread identified associated with any of the H5N1 bird flu cases reported in the United States.




This case was detected through influenza testing and reported to CDPH through influenza surveillance. This is the second U.S. case identified through national surveillance. CDC continues to closely monitor available data from influenza surveillance systems, particularly in states affected by outbreaks in animals, including California, where widespread outbreaks of H5N1 bird flu have been detected in wild birds and domestic poultry since 2022 and dairy herds since August 2024 in that state.




Limited and sporadic human infections with avian influenza H5N1 virus, where animal exposure was not identified, are very uncommon but have occurred, primarily in countries other than the United States. These instances underscore the importance of ongoing surveillance and investigations at the local, state, and Federal levels.




Including this most recent case, 55 human cases of H5 bird flu have now been reported in the United States during 2024, with 29 in California.




CDC's risk assessment for the general public is low. However, people with exposure to infected or potentially infected animals, such as birds, dairy cattle, or other animals (including livestock), or to environments contaminated by infected birds or other animals, are at higher risk of infection. CDC recommends avoiding unprotected exposure to sick or dead animals, including wild birds, poultry, other domesticated birds, and other wild or domesticated animals (including cows).

Technical Update: Summary Analysis of the Genetic Sequence of a Highly Pathogenic Avian Influenza A(H5N1) Virus Identified in a Child in California

Although the genetic data generated were insufficient to classify the virus as a specific genotype, the virus gene segments sequenced most closely resemble those segments from recent B3.13 viruses detected in California in humans, dairy cattle and poultry. What to Know

This is a technical summary of an analysis of the genomic sequence data for an influenza A(H5N1) virus from a pediatric case in California that was confirmed by CDC on November 22. This analysis supports the conclusion that the overall risk to the general public associated with the ongoing HPAI A(H5N1) outbreak in U.S. dairy cattle and poultry has not changed and remains low at this time.

CDC Update


December 10, 2024 – As previously reported, CDC, in partnership with the California Department of Public Health (CDPH), confirmed a human infection with avian influenza A(H5N1) virus (H5N1 bird flu) in a child in California on November 22, 2024. The patient was initially detected as a suspected case of A(H5N1) through influenza testing and reported to CDPH through influenza surveillance, with initial subtyping conducted by the Stanford Medicine Clinical Virology Laboratory and presumptive positive test results reported by CDPH on November 19, 2024.

Based on real-time RT-PCR cycle threshold (Ct) values determined during diagnostic testing, the nasopharyngeal specimen from the patient was found to have very low levels of influenza viral RNA. Initial attempts to sequence the viral RNA by both Stanford Medicine and CDC using standard Next Generation Sequencing methodologies produced weak amplicons resulting from the PCR process that were challenging to sequence using both Nanopore and Illumina sequencing methodologies. Additionally, efforts to isolate virus from the specimen were not successful. By employing nucleic acid enrichment techniques, CDC and Stanford Medicine were able to generate the following viral RNA sequence data: full-length neuraminidase (NA) and nucleoprotein (NP) genes and partial hemagglutinin (HA), polymerase basic 2 (PB2), and polymerase basic 1 (PB1) genes. CDC and Stanford Medicine have submitted these data for A/California/192/2024 to both the GISAID and GenBank databases with accession numbers EPI_ISL_19597300 and PQ724471-PQ724473, respectively.

The initial sequence analysis, which was based on short fragments of the HA and NA genes, indicated that the virus was a clade 2.3.4.4b A(H5N1) virus similar to viruses causing outbreaks in dairy cattle and poultry in the United States. Upon obtaining full-length NA and NP genes, CDC was able to perform a comprehensive phylogenetic analysis, which showed that the virus was very similar to viruses detected in both dairy cattle and poultry as well as to A(H5N1) viruses from previous human infections in dairy workers in California. Although the genetic data generated were insufficient to classify the virus as a specific genotype, the NA and NP sequences shared close nucleotide identity and phylogenetic clustering with NA and NP genes from recent B3.13 viruses detected in California in humans, dairy cattle and poultry. Epidemiologic and environmental investigations did not clearly identify a possible source of exposure. Given that additional sequence data from the case could not be generated, it is unlikely that the source of the child's exposure to A(H5N1) virus will be identified, thus completing the investigation into genotype and exposure.

In addition to the phylogenetic analysis of the NA and NP genes, CDC conducted an assessment of the sequences of the virus specimen for molecular changes that might impact infectivity or transmissibility in humans or reduce susceptibility to antivirals, such as oseltamivir. The partial HA sequence, despite lacking portions of the 5' end of the gene, did not have additional changes in predicted antigenic sites compared to available clade 2.3.4.4b candidate vaccine viruses (CVVs). Nor were changes identified in the receptor binding domain of the virus, indicating that the virus retained avian receptor binding properties with no mutations that would impact changes in infectivity or transmissibility in humans. The PB2 and PB1 genes did not have genetic changes associated with mammalian adaptation, nor were there any genetic changes identified in NA sequence data that have been associated with reduced susceptibility to neuraminidase inhibitors, such as oseltamivir. Lastly, the sequence data confirmed that the virus from this case is not closely related to the virus that caused severe illness in a human in British Columbia, Canada. Collectively, these data indicate that the virus detected in this pediatric case is very similar to the majority of other influenza A(H5N1) viruses detected in the United States and bears no signs of concerning mutations.

While the sequence data obtained do not allow for definitive determination of the viral genotype, and no specific animal exposure source has been identified, these findings provide important contextual information and help to inform CDC's overall risk assessment. They also highlight the value of a collaborative approach to this response, including virologic risk assessments based on genetic data.

Follow-up Actions

Sporadic human infections with a novel influenza virus where animal exposure is not identified are not uncommon and have occurred during the current H5N1 bird flu outbreak in U.S. dairy cattle and poultry and in past outbreaks around the world. This case does not change CDC’s assessment of the immediate risk to the general public, which remains low at this time.

There is no evidence of human-to-human transmission of H5N1 virus in California or elsewhere in the United States. However, persons with occupational exposure to infected animals, such as poultry cullers, poultry workers, and dairy farm workers, are at higher risk of influenza A(H5N1) virus infection. Public health surveillance and investigations of all identified cases will continue at the local, state, and federal levels to monitor for concerning changes in the epidemiology of human cases or changes to influenza A(H5N1) viruses that could signal an increased public health threat – potentially including more severe outcomes in those currently at risk of exposure and infection, or increased risk to the general public.

The child in Alameda County with #H5N1 without known exposures was infected with the strain found in dairy cattle and dairy farm workers. This case was picked up through our testing protocol at
@StanfordDeptMed
specifically through the assay developed by the Pinsky Lab. The strategy here was to subtype all Flu A samples regardless of their exposures which the lab has been doing for months. Without doing so, this case would have been missed. Genomic analysis does not show mutations that suggest mammalian adaptation. It is unlikely we will be able to figure out the infection source.
