World Health Organization (WHO) 4 report dated 21, Vietnam on April 6 and 9 Days confirmed two cases of human infection with influenza H5N1 highly pathogenic avian influenza (bird flu for short people) cases. The country has been discovered 119 cases of confirmed human cases of bird flu, of which 59 patients died (CFR 49.6%). Apr 21, 2010 (CIDRAP News) ? Cambodia's health ministry today announced that a 27-year-old man died of H5N1 avian influenza, while the World Health Organization (WHO) confirmed two previously reported cases in Vietnam and ruled out human-to-human transmission.

The Cambodian man, whose case is the country's first of the year, lived in the eastern part of the country in Prey Veng province, the Associated Press (AP) reported today. Few other details were available about the case, and the health ministry said it is investigating the man's illness and death. He will be listed as Cambodia's 10th H5N1 case and its 8th death from the disease.

Meanwhile, the WHO today confirmed the H5N1 infections of two Vietnamese patients who are part of a suspected case cluster. The patients, a 22-year-old man and a 2-year-old girl are both from Bac Kan province in the northern part of the country. Their illnesses raise the number of H5N1 cases in Vietnam to 119, of which 59 have been fatal.

Previous news reports said the area where the two live had mass poultry deaths and that at least four other people with flulike symptoms were isolated and treated with antiviral medication. Reports of suspected and confirmed H5N1 infection clusters raise fears that the virus is becoming more transmissible among humans and could become a pandemic flu strain. However, the WHO said an epidemiologic investigation has found no link between the two patients that would suggest human-to-human transmission.

A WHO statement suggests both had similar exposure to the virus. It said an initial investigation revealed sick and dead poultry around both patients' homes and in surrounding areas. It said the girl's family had recently slaughtered sick poultry to eat.

The man is listed in critical condition at National Hospital of Tropical Diseases, and the girl is in stable condition at Cho Moi District Hospital.

The newly confirmed Vietnamese H5N1 cases raise the global H5N1 total to 495 cases, which includes 292 deaths

WHO H5N1 Update For Vietnam

The World Health Organization has posted an avian influenza update (#11) for Vietnam, citing two new cases in Bac Kan province. One is listed in critical condition, the other as stable.



These cases began in late March and early April, and were covered extensively in this blog (here, here, here, and here),on Crofsblog, on Scott McPherson?s blog, and on the flu forums.



Although the two victims lived in close proximity to one another, and came down with the infection within a few days of each other, authorities state they have been unable to establish any epidemiological link suggesting human-to-human (H-2-H)transmission.



The burden of proof for H-2-H transmission of H5N1 has always been high, particularly when other plausible explanations for multiple infections exist.



In this case, sick and dying birds had been reported in and around both homes, and both victims could have been exposed through that route.



The operative words here being `could have?. There is simply no way to know with absolute certainty.



No diagnosis or explanation has been offered for the two dozen others in the village who reported flu-like symptoms, and who reportedly received Tamiflu.



While false negative tests for H5N1 are pretty common, their mass recovery would seem to argue for them having had something other than bird flu, but questions remain.



For now, the good news is that whatever transpired in this village appears to have been brought under control. No new cases have been reported, and most of those treated have apparently recovered.

The initial epidemiological investigations show that there were sick/dead poultry at the patient's home and in the surrounding areas.



The second case is a 2 year old girl residing in Cho Moi district, Bac Kan province. She developed symptoms on 2 April 2010. On 4 April, she was transferred to Cho Moi District Hospital for treatment where she is in a stable condition. Confirmatory test results for influenza A (H5) were obtained on 7 April.



The initial epidemiological investigations show that there were sick/dead poultry at the patient's home and in the surrounding areas. The patient's family slaughtered the sick poultry to eat.



There is no epidemiological link between these two cases that would indicate human-to-human transmission.



Of the 119 cases confirmed to date in Viet Nam, 59 have been fatal.

After reviewing this thread it appears that we were first with the information and we are also the most reliable group who have reported on this event.

Treyfish, Commonground, Dutchy, Shiloh, and others have spent years looking at disease situations in Vietnam. It was apparent to us that the reports of the last 2 days were mentioning the same 2 patients in Bac Kan.

We are aware of what late reporting by a governmental source will do to the news media. They sometimes report what is old news to us, as new. Then different services pick up this old news and report it again.

Yesterday afternoon [14 Apr 2010], at a meeting of the National
Steering Committee for pandemic influenza control and prevention, Dr
Tran Nhu Duong, Deputy Director of the National Institute of Hygiene
and Epidemiology confirmed that influenza A (H5N1) has not been
transmitted from human to human.

The investigation of the Institute on 2 cases of influenza A (H5N1)
in Na Tao village, Nhu Co commune, Cho Moi District, Bac Kan province
did not detect human to human virus transmission.

Reports of two investigation teams of the Institute in Bac Kan showed
that the two patients were living about 100 meters apart, and they
had never been in direct contact with each other.

All 33 people who had contact with the patients, including relatives,
visitors, neighbors, nurses, and physicians, were tested and
monitored for 16 days. There has been no case with symptoms of the
disease. All the test results were negative.

The cumulative number of confirmed human cases of avian influenza A (H5N1) reported to the WHO, dated 9 Apr 2010, is available at <http://www.who.int/csr/disease/avian.../en/index.html>. There have been 117 cases and 59 fatalities reported from Viet Nam to the WHO since 2003, and a total of 5 cases and 2 fatalities have been reported since January 2010. The 2 new confirmed cases from Bac Kan province have yet been added to the WHO report.

I have read in numerous stories of late - that Vietnam has 2 deaths for 2010. That is not true. The WHO did not report one death, although they did confirm it. The Ministry of Health confirmed the death. I would think that should be sufficient. Here is my data:

I went to the Ministry of Health website. While there is a post on the confirmation of the 17 yo, there is not a post declaring they died. The only source is the website I mentioned, saying the MOH confirmed. I will adjust my list. We have 2 deaths to date for Vietnam, in 2010.

Việc ghi nhận 2 bệnh nh&#226;n ở c&#249;ng một th&#244;n mắc c&#250;m A/H5N1 như vậy b&#225;o hiệu sự b&#249;ng ph&#225;t một ch&#249;m ca bệnh, nguy hiểm hơn n&#243; c&#242;n cảnh b&#225;o về sự l&#226;y lan c&#250;m A/H5N1 từ người sang người, điều chưa từng c&#243; ở nước ta.

Trong khi đ&#243;, TS Trần Như Dương, Ph&#243; Viện trưởng Viện Vệ sinh dịch tễ Trung ương cho biết, Viện đ&#227; cử 2 đo&#224;n c&#244;ng t&#225;c xuống điều tra dịch tễ tại th&#244;n n&#224;y. Kết quả cho thấy, cả 2 bệnh nh&#226;n đều c&#243; tiếp x&#250;c với gia cầm bệnh. Hiện chưa thấy c&#243; bằng chứng n&#224;o cho thấy c&#243; sự l&#226;y lan từ người qua người trong 2 ca bệnh n&#224;y.

Li&#234;n quan tới dịch tả, TS Nguyễn Huy Nga cho biết, cả nước hiện ghi nhận 4 địa phương ph&#225;t hiện c&#243; bệnh nh&#226;n tả nhưng nguy cơ b&#249;ng ph&#225;t dịch bệnh nguy hiểm n&#224;y l&#224; rất lớn. Thời tiết nắng n&#243;ng bất thường tại Nam bộ trong những ng&#224;y qua l&#224; điều kiện thuận lợi cho khuẩn tả ph&#225;t triển. Nắng n&#243;ng dẫn đến thiếu nguồn cung cấp nước sạch n&#234;n nhiều người phải sử dụng nguồn nước kh&#244;ng bảo đảm vệ sinh khiến nguy cơ mắc khuẩn phẩy tả tăng cao.

Th&#234;m v&#224;o đ&#243;, h&#224;ng h&#243;a thực phẩm trong điều kiện n&#243;ng ẩm kh&#244;ng được bảo quản tốt đ&#227; l&#224;m vi khuẩn c&#243; hại, đặc biệt l&#224; vi khuẩn tả ph&#225;t sinh. Mặt kh&#225;c, m&#249;a h&#232; l&#224; m&#249;a du lịch n&#234;n nhu cầu đi lại giữa c&#225;c v&#249;ng miền tăng khiến dịch nguy cơ lan rộng.

Để chủ động ph&#242;ng chống bệnh truyền nhiễm trong thời điểm giao m&#249;a, Cục Y tế dự ph&#242;ng v&#224; m&#244;i trường khuyến c&#225;o mạnh mẽ người d&#226;n thực hiện vệ sinh c&#225; nh&#226;n, vệ sinh m&#244;i trường, rửa tay bằng x&#224; ph&#242;ng trước khi ăn v&#224; sau khi đi vệ sinh; đảm bảo ATVSTP, thực hiện ăn ch&#237;n uống s&#244;i, sử dụng nước sạch để ph&#242;ng tr&#225;nh c&#225;c bệnh l&#226;y truyền qua đường ti&#234;u ho&#225;, đặc biệt l&#224; ph&#242;ng tr&#225;nh bệnh tả; trong v&#249;ng c&#243; dịch, c&#225;c gia đ&#236;nh kh&#244;ng n&#234;n tổ chức ăn uống đ&#244;ng người.

I have checked several of the newspapers that carried the original story and they do not have any new patients mentioned.
