Special report: Global fight against bird flu

HANOI, Aug 3 (Xinhua) -- A 35-year-old man from Vietnam's southern Kien Giang province has been hospitalized after eating dead duck meat and then exhibiting bird flu symptoms, local media reported Thursday.

The man from Giong Rieng district was admitted to the Kien Giang General Hospital on July 30 after having high temperature and respiratory failure and the doctors said his lung has been severely damaged, according to Youth newspaper.

The patient's relatives said he slaughtered and ate a dead duck [[a dead one, you say? not a live one? ]] offered by a neighbor one week before hospitalization. Three days after eating the meat, he had high fever and took medicine at home.

Specimens from the patient are being tested for bird flu viruses.

To prevent bird flu infections among people, Vietnam's National Institute of Hygiene and Epidemiology, has asked for permission from the country's Health Ministry to test bird flu vaccines it made from monkeys' kidneys on volunteers late 2006, the newspaper said.

To prevent potential outbreaks of the disease among fowls and humans, Vietnam is intensifying surveillance, management over transport, trade and import of poultry and related products at border gates, preparation for sufficient supply of relevant medicines and equipment, border quarantine activities, and enhancement of public awareness about the potential relapse of bird flu.

Vietnam has detected 93 bird flu patients since the disease started to hit the country in December 2003, the country's Health Ministry said on August 1, noting that it has seen no new human cases of infections since mid-November 2005. Enditem

Human Bird Flu Cases Suspected in Thailand, Vietnam (Update2)

    http://www.bloomberg.com/apps/news?p...AcU&refer=asia

Aug. 3 (Bloomberg) -- Asia's bird flu threat intensified as suspected human cases were reported in Thailand, Indonesia and Vietnam, indicating a fresh wave of outbreaks of the virus that began spreading in the region almost three years ago.
A patient in Thailand's Chachoengsao province, 50 kilometers (31 miles) east of the capital, Bangkok, may be the second Thai infected with the H5N1 avian influenza strain in eight days, said Thawat Suntrajarn, director general of disease control at the Ministry of Public Health.
``An initial test showed the possibility of infection, and we're awaiting further test results for confirmation,'' Thawat said in a telephone interview today.
Test results are also pending on a man in the southern Vietnamese province of Kien Giang. Indonesian authorities are re-testing six people, including three children in the hospital, after an initial report showed they were negative for H5N1. New cases create chances for H5N1 to mutate into a pandemic form.
The virus is known to have infected 232 people in 10 countries, killing 134 of them. Most infections have occurred in Asia through contact with birds. The disease might kill millions should it become contagious, researchers have said.
    143 Patients
Since 2004, Thailand has reported 23 cases, of which 15 were fatal. The most recent fatality, a 17-year-old man from the northern Thai province of Phichit, was confirmed July 26. Late yesterday, there were 143 patients with avian flu-like symptoms being monitored by the public health ministry.
A 9-year-old girl who died late yesterday in Lop Buri province, 153 kilometers north of Bangkok, may also have had H5N1, said Kumnuan Ungchusak, director of the Bureau of Epidemiology.
``Her symptoms are consistent with avian flu, but we're awaiting laboratory tests,'' Kumnuan said in an interview today. ``She had no history of contacting dead poultry, but we have sent officials to her house to check the poultry in the area.''
A 61-year-old woman from the same province tested positive for avian flu and was being treated in an isolation ward pending the results of confirmatory tests, The Nation newspaper reported today, citing Charn Tantiwaraporn, director of Ban Mi hospital in Lop Buri.
Thawat at the health ministry said the diagnosis hadn't been confirmed and the woman is less likely to be infected with H5N1 than the patient in Chachoengsao.
In Vietnam's Kien Giang province on the Cambodian border in the Mekong Delta, a man was hospitalized with damaged lungs and high fever about a week after eating duck, the Tuoi Tre newspaper reported, citing a local medical official.
    Damaged Lungs
``The patient is in critical condition, his lungs have been seriously damaged and we are using a respirator to support his breathing,'' Pham Van Dom, director of the Kien Giang provincial General Hospital, said in an interview today.
Samples from the man were received yesterday and test results may be reported at the weekend, said Cao Bao Van, head of the molecular biology laboratory at the Pasteur Institute in Ho Chi Minh City.
Ducks from two households in the southern province of Tay Ninh died recently from unknown causes, the Sai Gon Giai Phong newspaper reported today, citing local animal health officials. Test results are pending, the newspaper said. Tay Ninh province is also on the Cambodian-Vietnamese border.
In Laos, which borders Thailand, Vietnam, Cambodia and China, H5N1 last month sickened poultry on three farms owned by a commercial poultry producer near the capital, Vientiane.
    Quarantine Concerns
In Indonesia, a hospital treating three Sumatran children for possible H5N1 infection can't keep them all isolated, highlighting difficulties in preventing the disease spreading if the virus mutates into a contagious form.
The children -- sisters aged 10 and 6 years, and an 18- month-old boy from a neighboring house -- were hospitalized two days ago in Medan, North Sumatra. Initial tests for the H5N1 avian flu strain were negative and more samples are being analyzed to rule out the infection, Health Minister Siti Fadilah Supari told reporters in Jakarta today.
The children had symptoms of cough and high fever, and chickens infected with avian flu were found in their neighborhood, making them ``probable bird flu cases,'' Luhur Soeroso, a doctor treating the children at the Adam Malik Hospital, said in a telephone interview today.
The patients are from the same district where seven members of a family contracted the virus in May, six of them fatally.
The parents of the 18-month-old boy have been reluctant to follow doctors' orders to keep their son in the hospital, Soeroso said. Metro TV today broadcast footage of the infant playing in the garden outside the hospital.
Soeroso said it's possible that at least one of the children being treated in Medan is infected.
``Overall, their condition is relatively good, but we're monitoring the 6-year-old girl closely because her X-ray result doesn't look good,'' he said.

Winter season could mean return of bird flu to Vietnam

A health official in Vietnam?s has warned that although the nation is in the midst of quiescence, bird flu could make its return this winter, in both poultry and in humans.
Deputy Health Minister Trinh Quan Huan said Vietnam is at high risk because the neighboring countries of Laos Thailand and China have all registered bird flu outbreaks.

Recently in Tay Ninh, a Mekong Delta province, a large number of ducks in two flocks have perished and tests have been carried out to find out the cause.

Over the next six months, the seasonal migration of birds in combination with cool weather makes up favorable conditions for the virus to spread, Huan said.

More importantly, recent epidemiological surveys revealed that type-B flu virus could combine with type-A H5N1 flu virus causing human-to-human transmission.

Within the first six months this year, patients infected by type-B flu virus have increased considerably, making up more than 50 percent of flu-infected patients in comparison with just about 10 percent last year.

        End

Dr Niman, can Influenza B reassort with and A type virus? If I remember rightly influenza B is a human only type flu. Could an avian flu + a purely human flu be a dangerous combination? Sorry if that's a dumb question

H5N1 can definitely recombine with influenza B.

He's now in a coma, and the tests are negative. Must have had a subarachnoid hemorrhage. That's what low platelets can cause. I had a patient who almost died of one. Either that or encephalitis.

Comatose man not ailing from bird flu, despite symptoms
              (05-08-2006)
HA NOI — Tests show that a man, 35, who has been in a deep coma for several days, has not been infected with the deadly H5N1 or bird flu virus.
The man was admitted to the Kien Giang general hospital in southern Viet Nam last Sunday with clinical symptoms of bird flu, including acute pneumonia.
But the result from tests done at HCM City’s Pasteur Institute had proved negative, the hospital’s Dermatology Department director Tran Thanh Tung told Viet Nam News yesterday.
The patient, a resident from Thach **** Commune in the border province’s Giong Rieng District, butchered a dead duck for his family to eat before falling ill.
The remainder of his family remained healthy and no sick birds had been found in the neighbourhood, said Tung.
Two people were admitted to Kien Giang General Hospital with bird flu from Cambodia last year.
              Both died.
Deputy Health Minister Trinh Quan Huan warns that Viet Nam "is encircled" by the epidemic.
His ministry had given instructions about the need for the early detection of any outbreak, farms to be kept clean and public vigilance raised, he said.
The HCM City People’s Committee has instructed agricultural, animal health officials and relevant agencies to strengthen bird-flu control.
Its deputy chairman Nguyen Huu Tin said the steering committee for bird flu control in local districts should intensify inspection of the breeding, slaughtering and trading of poultry in markets to prevent renewed outbreaks.
The deputy chairman, who was speaking at a meeting with officials of the Agriculture and Rural Development and the Animal Health Departments on Thursday, emphasised that a high risk of bird flu remained.
The danger stemmed from a lack of public awareness coupled with the impending arrival of cooler weather, he said.
Laos, Cambodia, Thailand, Indonesia and China had all reported new outbreaks, underpinning the importance of keeping the nation on high alert.
The danger was heightened by poultry farmers who were known to ignore veterinary quarantines and safety regulations.
The people’s committee had agreed with animal health officials to pilot a new seal for poultry and poultry products as a means to ensure hygiene for better control.
HCM City Municipal Medicine Preventive Centre deputy director Le Dac Tho reports the start of a campaign to prevent and control any outbreak across the city. — VNS
