                So here's the issue. Machupo is supposed to have about a 15-30% fatality rate. Three of the four cases that are suspected of having this virus have died, with the fourth still ill. (The fifth case had dengue and has been discharged.) So this illness has a 75-100% CFR in this cluster. There's no evidence that any milder cases have been missed. So might we be dealing with a novel and more severe strain of arenavirus?

                There is some precedent for novel fatal arenavirus discovery. Two new such viruses, Chapare in 2007 and Lujo in 2008, were discovered this century. Lujo in particular is thought to be more severe than some of the other viruses in this family, with four of the five confirmed cases having died. (Lujo is thought to be endemic only in parts of Africa, so is an unlikely suspect here, however.)

                On the other hand, Lassa, which has a stated CFR below that of Machupo (as few as 1% of Lassa infections are assumed fatal, with many of them asymptomatic) has been known to kill entire families or entire medical staffs in West Africa where it is endemic. The explanation given is that these close quarters lead to high viral load infections and worse outcomes than the typical zoonotic exposure. There's also a reporting bias in that a cluster of fatal illnesses is far more likely to attract attention than a cluster with only one or even no fatalities.
