      Tunisian health worker in Al Ain suspected of having Mers is in good health

      ABU DHABI // A Tunisian health worker in Al Ain who was kept in isolation in hospital last week as a precautionary measure is in good health, an embassy official said on Thursday.

      This follows concerns by a relative who claimed that the man, who works as a paramedic at Al Jimi Hospital, may have contracted the Mers coronavirus...

      ...?We were able to speak to the Tunisian citizen today who confirmed that he?s in good health,? said the Tunisian diplomat in Abu Dhabi. ?He was so surprised to know that he was infected but is confident that he will leave the hospital by tomorrow.?

      The man, whose identity has been withheld, underwent a first test which showed that his immune system was strong enough to fight off the virus.

      ?He is due to take a second test, which confirms that the Health Authority ? Abu Dhabi (Haad) is taking preventive measures,? the embassy official said. ?After having spoken to him today, we are reassured that he?s in stable condition.?...

            ...Mr Balilo said his compatriot went from healthy to ill in a short space of time.

            ?I was told that Abun was very joyful and even performed at the graduation,? said Mr Balilo, referring to a ceremony organised by Face, on April 4, at the Rotana Hotel in Al Ain. ?The function was held at 7pm and ended at midnight. The next day, he suffered from respiratory symptoms.?

            Taken to Al Ain Hospital less than two days later, on Sunday April 6, the paramedic was told he had bronchitis, Mr Balilo said.

            Later that evening, pneumonia was diagnosed, and by the following morning, Esporlas?s kidneys had started to fail. Mers was diagnosed on April 8.

            ?He was declared clinically dead on Tuesday,? Mr Balilo said. ?He was pronounced dead officially early Thursday morning.?

            Vivian Palma, 42, who works at a private school in Al Ain and had known the victim since 2010, said Esporlas had not been his usual self at a friend?s birthday party on April 3.

            ?His face was pale and he was unusually quiet that evening,? she said. ?But, he still attended the graduation ceremony the next day. He?s really nice and was like a big brother to me. I can?t believe he?s gone.?...
            The initial bronchitis diagnosis might have resulted in treatment with corticosteroids. Some studies found those types of drugs used early in pH1N1 infections were associated with worse outcomes:

            Our study provides no evidence of a beneficial effect of corticosteroids in patients with ARDS secondary to influenza pneumonia, but suggests that very early corticosteroid therapy may be harmful.

            Early use of parenteral glucocorticoids therapy for fever reduction and pneumonia prevention increases the risk for critical disease or death from pH1N1 infection. We recommend that guidelines on glucocorticoid use be established and enforced.

              Index Case in Al Ain Cluster

              The Crof posted this on his H5N1 in the last couple of days. It's a link to a story which was written off a recent update on the Al Ain Cluster. It has a very detailed link to the index case (a shop keeper) in the cluster which to my mind is Case #233 (45M) BUT I cannot then find a 45M HCW which would link to Abundio Verzosa Esporlas, 45 (a paramedic from the Philippines).

              Any thoughts most appreciated.

              A total of 37 people in the UAE have contracted MERS, the latest figures show.

              All the cases were found in Abu Dhabi, the World Health Organization (WHO) said, as it announced the potentially fatal virus has now spread to Yemen.

              The patients range in age from four to 73, with a median age of 41, and 70 per cent of them are male.

              More than two‐thirds affected were healthcare workers, including paramedics, but only one became seriously ill.

              Twenty‐eight cases of MERS, short for Middle East Respiratory Syndrome, were identified in a hospital cluster in Al Ain City, the first of which was a 45‐year‐old male shopkeeper who died in UAE on Thursday, 10 April. It was reported that he hadn?t recently travelled or had any contact with animals, and it?s not currently known how he caught it.

              A further 27 cases were healthcare workers or had had contact with infected people; the remaining five cases, among them a mum and daughter and an unrelated four‐year-old child whose mother had recently returned from a pilgrimage to Saudi Arabia.

              Four more cases were reported from Abu Dhabi on Thursday 8 May, but it?s not known whether they?re linked to the cluster of cases in Al Ain City.
