Coronavirus: two cases still worrying, the results of tests expected
      11/05/2013 1:51 p.m.

Two people who were in contact with the patient with the novel coronavirus, virus close to SARS are still susceptible to be contaminated: his former roommate at Valenciennes and a member of his family.

The man who shared the patient's room for four days, while the respiratory disease had not been detected, is hospitalized in the infectious disease Lille University Hospital. For him, "further investigations are needed," said the Ministry of Health.

The family member in question, a young man was allowed to remain at home in isolation. All test results should be reported on Saturday afternoon.

Three members of the medical staff - a doctor of Valenciennes, two members of the hospital Douai - whose cases were cause for concern, are out of danger and were able to return home. One hundred and twenty-four people in total have been identified in the environment of the patient, at the University Hospital of Lille, in the ICU and in the hospital where he had previously stayed at Valenciennes and Douai.

The health status of the patient "has stabilized but remains serious," according to Professor Daniel Mathieu, head of the intensive care unit. "It is likely that we will not see a favorable outcome for at least 48 to 72 hours," he said at a press conference.

The Lille University Hospital is the only facility in the region practicing the medical care necessary for his health. The patient needs a cardiopulmonary support to take over its respiratory functions.
      ...
His former roommate at Valenciennes continues to see himself administer "fairly aggressive" antibiotics, according to Professor Benoit Guery, the Infectious Diseases department of the Lille University Hospital.

The Minister of Health, Marisol Touraine, visited Saturday morning both services welcoming patients taking both their new and those caregivers. Before the Board of the former neighbor of hospital beds, which includes an airlock to install a system of protection and prevent communication between air systems, it instructed the nurse to send its greetings recovery and support.

"The need to remain vigilant," then said the minister at a press conference.

Ms. Touraine announced the establishment of an information system at international airports for travelers to the Arabian Peninsula. A leaflet, whose message has been prepared by the Ministries of Foreign Affairs and of Health, will be distributed to airline counters. It is particularly recommended to take precautionary measures, such as washing hands regularly, but especially avoid contact with animals.

Coronavirus: Two suspected cases still being tested

        By Les Echos | 10/05 | 8:38 | updated on 11/05 at 18:25

The results of tests on two persons suspected of being infected are expected by Saturday night - AFP
        ...
Professor Yasdan Yasdanpanah, chief of infectious diseases at the Bichat Hospital in Paris, said on BFM TV Saturday that he "hoped" that these tests are negative, but if that was not the case it would not be totally unexpected or "completely alarming." "If there is a positive case (...), we should not be unduly alarmed," he further said.

For the time being, therefore, only one case of infection with the new coronavirus (nCoV) in France, causing severe respiratory problems, was confirmed on May 7: this is a man 65 years currently hospitalized in intensive care Lille.

French health authorities said early Sunday [12 May2013] that a 2nd person had contracted a deadly new SARS-like virus, after sharing a hospital ward with the 1st victim identified in the country.

The latest victim shared a hospital room with a 65-year-old man who is in intensive care in the northern town of Lille after being diagnosed with the virus that has already killed 18 people, mostly in Saudi Arabia.

"Positive results (for the virus) have been confirmed," for both patients, who are being treated in isolation wards, the health ministry said in a statement.

                ProMED-mail Rapporteur Konihiko Iizuka

[with the confirmation of the 2nd case of severe acute respiratory illness (SARI) associated with infection with the novel coronavirus (nCoV) in France, this brings the total of confirmed cases of infection with this nCoV to 34. It also brings the number of clusters involving person-to-person transmission of this nCoV to 4 - with prior clusters of person-to-person transmission confirmed in Jordan [April 2012], the UK [February 2013], and Saudi Arabia [April/May 2013]. The prior clusters in Jordan and Saudi Arabia have been associated with contact in health care facilities, while the cluster in the UK involved family contacts. Of note is that these current clusters (Saudi Arabia and France) involve individuals who have been admitted to these health care facilities for medical care of other illnesses, suggesting that risk infection with this nCoV may be increased in individuals with pre-existing co-morbidities. Information on the specific co-morbidities involved is not fully available as yet, although some of the earlier cases were reported to have had illnesses with associated immunocompromise.

As stated in the above press releases, as well as prior ProMED-mail posts on this current situation in France, the Ministry of Social Affairs and Health in France is identifying all close contacts of these 2 confirmed cases and continues to monitor for additional cases with placing of suspected infected individuals into isolation while laboratory tests are pending.

As of today, 12 May 2013, the breakdown of total confirmed cases of SARI associated with infection with the nCoV (and deaths) by country of report is:
                - Saudi Arabia: 24 (13 deaths)
                - Jordan: 2 (2 deaths)
                - UK: 4 (one patient from Qatar -- under treatment, 3 patients from UK -- one with history of travel to Saudi Arabia and Pakistan prior to illness; one recovered, 2 deaths)
                - Germany: 2 (one patient from Qatar -- discharged, one patient from the UAE -- one death).
                - France: 2 (one with a history of travel to the UAE prior to illness; 1 health care facility contact of other case)

(note that the above table shows cases by country of report -- of the total 34 cases confirmed at this time, 25 were presumed to be infected in Saudi Arabia, 2 in Jordan, 2 in Qatar, 2 in the UAE, 2 in the UK and 1 in France).

All persons who have been in contact with the two patients with the new coronavirus are subject to close monitoring by health authorities said Sunday Marisol Touraine. However, the Minister of Health believes that we should not be overly concerned.

"We have two cases, two patients, we make sure to have contact with everyone who approached them (...) we are very vigilant," she detailed at a conference presse. But the "professionals, doctors consider that there is no need to be overly concerned."

"Three investigations are being conducted by the Institute of Health on the people of the environment of the first patient, the second victim infected at Lille and also people who participated in the organised trip in the Arabian Peninsula in which the first patient has been infected, "continued the Minister of Health.

People in contact with the two patients followed closely

124 people in contact with the first patient were identified and contacted, said the minister. For the second positive case, 38 people, "mostly around him," have already been identified and contacted by the Institute of Health Surveillance (InVS). Its general director Fran?oise Weber said that they were recommended to follow hygiene measures and wear a mask if they experience flu-like symptoms.

These 38 people are contacted every day to see if they have symptoms. Among this group, there are "a few very close and prolonged close contacts" with the second patient, whose health authorities have recommended to stay at home. In addition, 39 persons of French or Belgian nationality who participated from April 9 to 17 to a trip attended by the first patient have been contacted and are monitored by the InVs.

The Minister assured the "total mobilization of the government...

WHO Novel coronavirus infection - update 12 May 2013

12 May 2013 - The Ministry of Social Affairs and Health in France has informed WHO of an additional laboratory-confirmed case with infection of the novel coronavirus (NCoV).

This patient was identified as part of the epidemiological investigation initiated by the French authorities, following laboratory confirmation of the first case on 7 May 2013. The patient shared a hospital room in Valenciennes with the first laboratory-confirmed patient from 27 to 29 April 2013. The patient is currently hospitalized and isolated in an infectious disease hospital.

Among 120 persons identified as contacts of the first laboratory-confirmed case in France, laboratory tests were conducted by Pasteur Institute in Paris on five suspected cases, of which four tested negative, one (mentioned above) tested positive.

In Saudi Arabia, an investigation is ongoing into an outbreak in a health care facility, where 15 patients, including seven deaths have been confirmed.

From September 2012 to date, WHO has been informed of a global total of 34 laboratory confirmed cases of human infection with nCoV, including 18 deaths.

Based on the current situation and available information, WHO encourages all Member States to continue their surveillance for severe acute respiratory infections (SARI) and to carefully review any unusual patterns.

Health care providers are advised to be vigilant among recent travelers returning from areas affected by the virus who develop severe SARI. Specimens from patients? lower respiratory tracts should be obtained for diagnosis where possible. Clinicians are reminded that nCoV infection should be considered even with atypical signs and symptoms in patients who are significantly immune compromised.

All Member States are reminded to promptly assess and notify WHO of any new case of infection with nCoV, along with information about potential exposures that may have resulted in infection and a description of the clinical course.

WHO does not advise special screening at points of entry with regard to this event nor does it currently recommend the application of any travel or trade restrictions.

WHO continues to closely monitor the situation.

Bernard Baudoux, Mayor of Aulnoye-Aymeries, the town of origin of the second novel coronavirus infected patient, held a press conference early in the afternoon.

He had a thought for the families of the two patients still hospitalized in the intensive care unit of the University Hospital. "I hope that these two people can get out." He said he did not know the identity of the patient from his native town, citing medical confidentiality. "The first time I was asked about this was on May 8, he said. I never had the name and address of the patient. Today, I do not know his name or address, and I do not want to know. "

The first magistrate spoke of the first 38 people of the environment of the patient followed by the Institut de Veille Sanitaire. According to him, they would live for the most in Aulnoye-Aymeries and the group include children. He said none of them, for the moment, had any symptoms of the disease. "Starting Monday, the monitoring will be lifted up. They will be able to resume a normal life, he added. When asked whether these people were confined at home, Bernard Baudoux replied: "They have been advised not to have contact with other people. (...) We do not want them to have contact. "

The mayor added that "no particular action was taken in Aulnoye-Aymeries" following the contamination of one of its inhabitants. "I feel among the people a certain serenity," he reported, noting that the city had not received "hundreds of calls" about it.

"Safety and security don't just happen, they are the result of collective consensus and public investment. We owe our children, the most vulnerable citizens in our society, a life free of violence and fear."

The health of the roommate of the first confirmed case of novel coronavirus in France deteriorated Wednesday, announced Thursday the Lille University Hospital where both hospitalized patients.
"The health status of the second patient deteriorated in the last day. He was placed on ECMO (extracorporeal membrane oxygenation) to take over from his lung function," said the University Hospital in a statement
"His condition has stabilized but remains a serious concern," the statement said.

The first patient with the new coronavirus in France, a man of 65 years, ECMO since May 8, "remains in a stable condition," also indicates the University Hospital in a statement.
"His condition is very serious and doctors are very reserved about his prognosis," the source said.
Both patients are still treated in isolation in a vacuum chamber, in the ICU, the statement said.
