Canada Issues Alert on Severe Respiratory Disease in Mexico

    Recombinomics Commentary 02:22
    April 23, 2009

The Public Health Agency of Canada has told quarantine services to be on alert for travellers returning from Mexico after a number of severe respiratory illnesses (SRI) were reported in some regions of the country.

PHAC, in an April 20 report, said Mexican officials informed the Canadian health agency that the "case-fatality rate was relatively high" and that most cases involved healthy adults between the ages of 25 and 44. A number of health-care workers were also affected.

Although no cause has been confirmed, some samples were positive for influenza A and B.

The above comments on an alert issued by Canada offer some insight into the situation in Mexico. The reports out of Mexico are decidedly mixed. Some reports describe an increase in influenza cases which is attributed to a late spike in influenza B, which when combined with influenza A, gives an abnormally high number of cases this late in the season. Other reports discuss revaccinating at risk groups with the current trivalent vaccine.

Samples have been sent to Canada for a comprehensive analysis.

The increased influenza-like illness and fatalities was announced as the CDC issued an MMWR dispatch onswine flu. Two cases have been confirmed in children (9F and 10M) and family members had mild symptoms but were not tested. The precise location of the clusters in southern California has not been released, but one cluster is in San Diego Country, while the other is 100 miles away in Imperial County (see updated map). Additional suspect cases in Imperial County have been noted and it is likely that these cases are near the border with Mexico. Some media reports also note that some contacts have not been interviewed because they were in Mexico.

It remains unclear if these two outbreaks are related. All reported cases in California have been mild, and the two confirmed cases were influenza A positive, but failed to sub-type for seasonal flu. It is unclear if sub-typing failures have led to the confusing reports out of Mexico.

More information on these cases, and results of analysis in Canada, would be useful.

Similarly, a more precise location and number of confirmed and suspect cases in the United States would be useful.

The reason for the alert is the high CFR. They have reported at least 20 deaths in about 130 cases in Mexico City, and these are not the normal type of people (old, young, infirm) who usually die of flu. Also, no clear known etiological agent is being detected. This isn't flu.

Here is today's article from the Diario in Oaxaca.

He noted that respiratory infection, influenza virus, affects the entire world population with seasonal outbreaks. He said that the number of deaths due to influenza will grow in the coming days, because there are serious patients who are tube and mortality in them is high.

At a press conference, authorities announced Cenave no figures for cases of influenza have accrued to date, nor the number of people who are hospitalized for the disease in the country.

This was rejected by Hugo L?pez-Gatell, director adjuto Epidemiology Cenavel, who said that there have been only two deaths related to influenza in this institute and 13 were hospitalized and under observation.

Due to the fear it generated among the public transmission of this severe disease of the respiratory tract, influenza vaccine has begun to exhaust the private medical practices, while in the public sector is no longer available to the public.

?ngel C?rdova Villalobos. Mexico City .- The Ministry of Health launched a national epidemiological alert, even though the country is experiencing an epidemic or pandemic of influenza, despite the 20 deaths recorded in the national territory of the March 18 date, said the head of the agency, Jos? ?ngel C?rdova Villalobos.

"It must be clear and it is important that everybody understands, this is no pandemic influenza, a pandemic is because when several thousand cases, stressed in an interview at the end of the First International Congress Towards Universal Coverage, in the Faculty of Medicine, UNAM.

What we are seeing, he said, are cases that were filed in March and April, which most likely are related to an epidemic that occurred in the United States also led to some deaths and "say it is the tail of the epidemic we are affecting us. "

Therefore, the Secretary of Health, "did the epidemiological alert to take into account this possibility and will pay close attention to people and went to the hospital availability of antiviral to be most affected is the 'oseltamivir' what if he had saved by the pandemic, well, although this is not a pandemic, the virus itself is sensitive to this drug but in some cases like United States, are taking more resistance. "

Furthermore, it was made available to health care medicines, "cephalosporins" high generation because this is complicated with more severe respiratory problems like pneumonia. "And we are seeing the existence of any vaccine that has to begin to distribute mainly for health personnel."

In this sense, C?rdova Villalobos reported that influenza vaccine was used in practically the National Campaign, which used 19 million vaccines, however, because the number of probable cases of seasonal influenza have increased, others will be obtained 400 thousand to apply "for now", which will cost 25 million pesos.

Meanwhile, he said, are increased measures of care between the medical staff to prevent transmission through the use of cubrebocas of gowns, hand washing, in fact "in the INER (Institute of Respiratory Diseases) is directing a flag for these special cases. "

When questioned about if they have cases of influenza in the Federal District, the head of the country's health policy said, "No, there has been a case in Oaxaca, San Luis Potos? we diagnosed eight cases.

In Mexico City, he added, are about 120 cases diagnosed so far, 9 of which were deaths.

Before, Mauricio Hernandez, Undersecretary for Prevention and Health Promotion, said that since March 18 began to observe an unusual increase of the disease and expressed concern about the increase in the number of deaths, which has been presented in a population group that usually "did not see, what we're looking to see what is going on."

He also reported Wednesday that until yesterday had been 13 deaths reported in Mexico City, 4 in San Luis Potosi, Baja California on 2 and one in Oaxaca.

Did not rule out raising the deaths because there are several patients hospitalized in serious situation which could not specify number.

"We know that deaths will increase because there are tube-severe patients, there is high mortality," he said.

On this point, Hugo L?pez-Gatell, director of epidemiology, said that one third of those who died were between 5 and 45 years old, an age group where it is not unusual presents daily deaths.

He indicated that from October 1, 2008, to March 1 this year 237 thousand were detected 7 cases of influenza in the country.

Questioned about the number of cases recorded so far in 2009, stated that the January 1 2009 to last April 17 were 4 thousand 157 cases.

Then, the undersecretary of Health, Mauritius Hernandez announced that 500 thousand to vaccinate workers in this sector, who have contact with patients.

I must be slow or something. As I recall from an old geography class, the US shares a border with Mexico. If that is still true, why is there no US alert? I have heard from reliable sources that there is significant travel between the people of Mexico and the US. And are the Mexican authorities sending samples to Nancy Cox?

Air pollution may be a factor, given that Mexico City is the second or third most polluted city in the world depending on what is being measured. What may be a relatively mild respiratory problem elsewhere could be far worse there, especially for people who spend much active time outdoors.

Perhaps whatever needs to be checked at the border is already in place between the US and Mexico, due to the infamous high traffic. But Canada may not currently have those same checks in place, hence the alert.

I don't understand why no US media source, nor ProMed, nor the CDC has picked up this story yet. The Canadian Health Alert said this disease has a high CFR; it can't be seasonal flu.

I don't see how anyone can say it has a high CFR until they identify the precise pathogen - it's all relative.

I agree. There are two possibilities here. The first is that Canada make have a rough idea what the pathogen is (the post did say that samples were sent to Canada). The other possibility is that these deaths are so closely linked (family members, nurses who treated cases), that most of the people who are getting sick are dying. I still haven't read any posting referring to the recovery of any of these cases.

The official admitted that it is still uncertain what course to take the virus, or if already reached its maximum level of involvement.

"The call to people is that there is no panic, one must be careful if you have a respiratory problem, stay home and see your doctor so they can be cared for," asked the Secretary of Health.

Faced with the possibility that this seasonal outbreak spreads faster in the country and killed more people, he clarified that this is not an epidemic, for it would affect more than five million inhabitants of the national population.

"A pandemic is when it extends to most of the population, we are talking about a five to ten by the way of the population," he said.

However, he acknowledged that it is necessary to be in an epidemiological alert, so this unit will seek to obtain about 400 thousand vaccinations for health care, to prevent an outbreak among doctors and nurses to meet patients without any problem. It also specified that it applied throughout the winter 19 million vaccines, it denied that it is a failure in the vaccination campaign.

"It is an extension of the period of seasonal influenza, which normally ends in February, but we are seeing cases in March and April and most likely is related to an epidemic of United States and also generated some deaths and is the tail of the epidemic which is affecting us, "he said.

In an interview, Dr. James Zuno Echeverria, director of the IMSS medical benefits, said the outbreak of influenza "is not something that leads to a significant widespread alarm."

"We simply need some more of that normally should take these times of year, but no alarm," he said.

The official reported that the epidemiological surveillance system that implemented the special weekend in the IMSS to record probable influenza cases yielded more than 500 throughout the country over the past five days.

At a press conference, Deputy Secretary Mauricio Hernandez said that unlike previous years, the peaks where winter is in November and December, is now having an excess of cases during this April.

"Some cases are severe, that have caused deaths and hospitalizations. These are young adults, mainly because it is the group that did not vaccinate, "he explained.

This is because the people most vulnerable to contracting this disease are children under 36 months and older than 60 years.

On the origin of the outbreak, Hernandez said that the epidemic comes from United States, where it is already out. The threat of an influenza epidemic is alive and in fact it is waiting for ten years.

However, Dr. Fernando Aguilar del Real, pediatrician infectologists, pointed out that this risk should not frighten the public: "It's inevitable, because it carries the virus, the pandemic at some point will be presented.

"But now we have to reassure the people and not put it in panic, because they get into panic if the will to give something very serious, saturarían health services," he added.

Aguilar said that the Royal has already earned the hard way, fear has caused this rise in cases. Yesterday received nearly 40 calls from people who asked him what is happening with the virus.

Is it an epidemic?, Do you have to vaccinate? Were the most frequent questions.

"There is fear and the important thing is to send a message of calm to the company, do not panic, my phone was saturated throughout the day, this is a sign of terror. In the morning I went to the park to run and no one stopped me, just ask for the flu, "the specialist told.

"I could not give consultation in the morning because it was call after call. It must be taken, but do not panic, this is not an epidemic at this point, what is true is that we are seeing more cases than usual, "he said.

The specialist attributed the expansion of the flu season to the seasonal environmental conditions.

The Ministry of Health, assisted by Canadian experts, who still looks strains are circulating and molecular analysis to provide detailed accurate information.

He recalled that the flu is a virus that cause influenza and that manifests itself with acute symptoms: sudden fever starting, sore muscles, colds, headaches, and children will diminish hunger.

On protective measures, of Royal Aguilar recommended to those who are sick not to go to public places.

"The first thing is what we call respiratory protection, and this means that at the time that I have a cold or flu should not I leave my house, that children do not go to school or daycare, and then obviously the control action Most importantly, the vaccine is given every year, "he said.

On the mask as a measure to protect the people who spoke a limited use.

"The mask to get a walk in the street does not guarantee anything, because I can also pass through the eye, so if someone sneezes in my face because I cubrebocas served.

"For what it serves is for me as a patient does not infect anyone else, in fact after a couple of hours to occupy it is moistened with respiratory droplets, and then I issued the viruses and bacteria come out, so they have a mask all day will have little effect, "he explained.

The thing to do is give them the treatment to relieve symptoms and try to avoid contact with others.

"If symptoms are complicated, if you feel shortness of breath and cough worsens, then it may be complicating the picture," explained.

Yesterday, only in the Instituto Mexicano del Seguro Social (IMSS) ten people died from SARS presumably caused by seasonal influenza virus, and six deaths were recorded in the Federal District.
Keep in mind that Mexican authorities are just reacting now to an outbreak that started 3/18 or before. There have likely been many more deaths than reported if one hospital is reporting 10 in one day.
